不同根充系统对牙根抗折性能影响的体外研究

In vitro Influence of Different Root Canal Filling Systems on the Fracture Resistance of Roots

摘要: 【摘要】目的 探讨不同根管充填系统对牙根抗折性能的影响。方法 将68颗单根管前磨牙由釉牙骨质界处截冠,随机分为4组。A、B、C组手动Protaper根管锉冠向下预备法预备根管至F3号及对照组。A组Cortisomol糊剂和牙胶尖冷侧压法充填根管;B组Cortisomol糊剂和System B & ObturaⅡ热牙胶垂直加压法充填根管;C组RealSeal根充系统冷侧压法充填根管。对照组不予处理。将所得标本制成试件,置于万能材料试验机上加压至牙根折裂,记录牙根折裂时的瞬间压力值。体视显微镜下观察A、B、C 3组标本断裂界面的断裂模式。结果 对照组压力值(380.65±20.83) N明显高于A组(231.36±11.12)N、B组(236.88±12.87)N及C组(275.96±21.51)N (P < 0.05),C组压力平均值明显高于A、B 组(P < 0.05)。A、B组标本的断裂模式以封闭剂与牙本质界面的断裂为主。C组标本的断裂模式以混合型断裂为主,差异无统计学意义(P > 0.05)。结论 与其他两种根充系统相比,RealSeal根充系统能增强牙根抗折能力。

Abstract: Abstract Objective  To evaluate the influence of different root canal filling systems on the fracture resistance of roots. Methods    68 single canal premolars were selected. The crowns were removed at the cementoenamel junction. And then they were randomly divided into four groups.The root canals of groupA, B and C were instrumented to F3 with rotary Protaper system using crown-down technique. Group A root canals were obturated with master gutta-percha cone and Cortisomol sealer by using cold lateral compaction.Group B root canals were obturated with gutta-percha and Cortisomol sealer by using warm vertical compaction. Group C root canals were obturated with RealSeal by using cold lateral compaction.Group D root canals were not instrumented or obturated. Then placed all the specimen in a universal testing machine and loaded each specimen to fracture. For each specimen,we recorded the force to fracture and the fracture mode of the teeth specimen using an optical stereomicroscope. Results  The force required to fracture the roots of Group D was significantly higher than that required to fracture the roots of all other groups. The force required to fracture the roots of Group C was significantly higher than that required to fracture the roots of Group A and B. In the Group A and B,the ruptures were mainly occurred in the dentine and sealer interface. In the Group C, there was a predominance of mixed failures.Conclusion   Teeth obturated with RealSeal were more resistant to fracture than those obturated with the other two systems.
