严重烧伤患者必需脂肪酸营养状况的分析

摘要: 严重烧伤患者必需脂肪酸营养状况的分析 张明谏,王其芳,李小兵,张静琦,刘光晶,李刚 天津市第一中心医院烧伤整形科,300192,天津 摘要 目的: 了解严重烧伤患者伤后两周必需脂肪酸的营养状况。方法: 调查分析30例严重烧伤患者伤后3-7天、8-14天平均每日经膳食和肠内肠外营养途径摄入能量、蛋白质、脂肪、多不饱和脂肪酸量。于伤后1-3天,6-8天和13-15天,用气相色谱仪测定血清多不饱和脂肪酸(C16:1、C18:2、C20:3和C20:4)相对浓度。结果: 严重烧伤患者伤后3-7天和8-14天平均每日摄入能量分别是: 2272±319 Kcal 和 2575±324 Kcal, 多不饱和脂肪酸平均每日摄入量分别占总能量的7.3%和8.1%。 伤后1-3天、6-8天和13-15天血清多不饱和脂肪酸C18:2、C20:3、C20:4相对浓度低于健康人水平(P<0.05); 伤后6-8天和13-15天血清相对脂肪酸浓度C16:1/C18:2 和 C20:3/C20:4比值均高于健康人比值。 多不饱和脂肪酸摄入量与血清多不饱和脂肪酸C18:2(r=0.487)、C20:3(r=0.505)、C20:4(r=0.495)相对浓度呈正相关。结 论: 严重烧伤患者伤后两周机体有必需脂肪酸缺乏的趋势。 关键词 烧伤 营养状况 必需脂肪酸 多不饱和脂肪酸

Abstract: Analysis of essential fatty acids status in severely burned patients ZHANG Mingjian, WANG Qifang, LI Xiaobing, ZHANG Jingqi, LIU Guangjing, LI Gang. Department of Burns and Plastic Surgery, the First Central Hospital of Tianjin,300192, P. R. China Abstract Objective: To investigate essential fatty acids status in severely burned patients. Methods: The average daily intake amounts of calorie, protein, fat, and polyunsaturated fatty acids via diet, enteral and parenteral nutrition in 30 severely burned patients were analyzed from days 3-7 and days 8-14 post-burn. Gas chromatography procedure was used to measure the relative concentrations of serum C16:1、 C18:2、C 20:3 and C20:4 from days 1-3 , days 6-8 and days 13-15 postburn . Results: The average daily intake amounts of calorie from days 3-7 and days 8-14 postburn were separately 2272 ± 319 Kcal, 2575 ±324Kcal. The average daily calories from polyunsaturated fatty acids constituted 7.3%-8.1% of total calories. The relative concentrations of serum C18:2, C20:3 and C20:4 in burn patients were lower than those in healthy controls from days 1-3, days 6-8 and days 13-15 postburn(P<0.05 ). The ratios of serum C16:1/ C18:2 and C20:3/ C20:4 in severely burned patients from days 6-8 and days 13-15 postburn were higher than those in healthy control; There was a positive correlation between the average daily intake amounts of polyunsaturated fatty acids and the relative concentrations serum C18:2, C20:3and C20:4 . Conclusion : There was a tendency towards deficiency of polyunsaturated fatty acids in severely burned patients from two weeks postburn.
