肾移植术后动态监测血清胱抑素C变化的临床意义

Study on the change of serum Cystatin C in the post kidney transplanted patients

摘要: [摘要]目的 通过动态监测血清胱抑素C(Cystatin c ,Cys C)以及血清肌酐(Scr)、血清尿素氮(BUN)在肾移植患者术后变化, 研究Cys C在肾移植反应中的诊断价值。方法 29例肾移植术后无明显并发症的患者,动态检测术前、术后1、3、5、7天血清Cys C以及血清肌酐(sCr)、尿素氮(BUN)浓度,以99mTc-DTPA血浆清除率(双血浆法)测定肾移植患者术后肾小球滤过率(glomerular filtration rate, GFR), 分别比较Cys C、Scr、BUN与GFR的相关性,同时选用20例健康体检者作为正常对照组。结果 肾移植术前Cys C、sCr、BUN均显著高于正常对照组(P<0.01);Cys C、sCr、BUN从术后1d起低于术前水平(P<0.01),下降幅度分别为47.6%, 37.4% ,33.0%,Cys C下降最为显著;Cys C、sCr、BUN与GFR均呈负相关(P<0.01),相关系数(r)分别为-0.892,-0.811,-0.772。结论 Cystatin C是一较其余指标更能早期、灵敏判断肾移植术后反应的血清标志物。 关键词 血清胱抑素C 肾移植 肌酐 尿素氮 肾小球滤过率

Abstract: Study on the change of serum Cystatin C in the post kidney transplanted patients Xie lili, Liu juan, Gao qiang, Sun ping. Tianjin NO.1 centre hospital(300192) Abstract: Object To investigate diagnostic value of serum Cystatin C(Cys C) in the post kidney transplanted patients by dynamic monitoring the change and relationship among Cys C, sCr, BUN and Glomerular filtration rate (GFR). Method The serum Cys C, sCr and BUN were continuing measured in 29 patients during one day before operation and 1,3,5,7 day after operation compared with 20 controls. Particle-enhance turbidmetric immunoassay(PENIA) was used to detect the Cys C; sCr and BUN was measured by routine biochemistry assay; GFR was measured at by double plasma method. Result The value of the Cys C、sCr、BUN in the transplant group were higher than that in the control before operation, and all of them decreased at 1 day after kidney transplant operation, 47.6%, 37.4% ,33.0% respective, the level of GFR was negative correlated with Cys C(r=-0.892 P<0.01), sCr(r=-0.811 P<0.01) and BUN (r=-0.772 P<0.01) .Conclusion The serum Cys C is a rapid, sensitive mark that could judge the kidney function after transplant operation.
