子宫内膜癌与胰岛素/Ras/MAPK信号通路的相关性研究

摘要: 目的:探讨胰岛素/Ras/MAPK通路中主要调节蛋白生长因子受体结合蛋白2 (growth factor receptor binding protein 2,Grb-2)、Ras、P-ERK、细胞周期素D1(CyclinDl,CD-1)在子宫内膜癌中的表达及意义;方法:采用免疫组化方法检测48例正常子宫内膜,10例不典型增生子宫内膜,51例子宫内膜癌组织中Grb-2、Ras、P-ERK及CD-1的表达情况,放免法检测内膜癌患者空腹血清胰岛素水平;结果:Grb-2、Ras、P-ERK、CD-1在正常子宫内膜组织、不典型增生子宫内膜、子宫内膜癌组织中阳性表达率依次升高,其中Grb2、P-ERK、CD-1差别有统计学意义。Grb-2、P-ERK在子宫内膜癌中的表达与肿瘤的手术-病理分期呈正相关(P<0.05),子宫内膜癌中Grb-2与Ras、Grb-2与P-ERK表达呈正相关,差异有统计学意义(P<0.05) ,Grb-2与淋巴结转移呈正相关(P<0.05)。循环胰岛素水平与Grb-2、P-ERK表达呈正相关(P<0.05) 结论:胰岛素/ Ras / MAPK信号传导通路在子宫内膜癌的发生发展过程中可能起重要作用。

Abstract: Objective: To investigate the expression and correlation of Grb-2、Ras、P-ERK、CD-1,which are the main members of insulin/Ras/MAPK signal pathway in endometrial carcinoma. Methods:The expressions of Grb-2、Ras、P-ERK、CD-1 protein in tissues from 51 cases of endometrial carcinoma, 10 endometrial atypical hyperplasia, and 48 normal endometrium were detected by using immunohistochemistry Elivison method and image analysis system. The serum insulin concentration was measured by radioimmunoscintigraphy in endometrial carcinoma.Results:The positive expression rate of Grb2、Ras、P-ERK、CD-1 in normal endometrium、endometrial atypical hyperplasia and endometrial carcinoma was gradually increased. The expression rate of Grb2、Ras、P-ERK、CD-1 in endometrial carcinoma was higher than that in normal endometrium and endometrial atypical hyperplasia. In endometrial carcinoma there were positive correlation between Grb-2 and FIGO stage,either between P-ERK and FIGO stage (P<0.05). Also,there were positive correlation between Grb-2 and lymphonode metastasis(P<0.05). There were positive correlations among the expression rates of Grb-2-Ras、Grb-2-P-ERK in endometrial carcinoma. The expression of Grb-2 and P-ERK was correlate with fasting serum insulin level in endometrial carcinoma(P<0.05).Conclusion:Insulin/Ras/MAPK pathway may play an important role in the carcinogenesis and development of endometrial cancer.
