精索静脉曲张患者精浆丙二醛水平和精子DNA碎片率检测及相关性分析

Detection and correlation analysis of sperm DNA fragmentation and level of MDA in seminal plasma among infertile males with varicocele

摘要: 摘要: 目的 检测精索静脉曲张所致的不育患者精子DNA碎片率和精浆丙二醛水平并分析其相关性。方法 采用染色质扩散实验(sperm chromatin dispersion, SCD)对52例精索静脉曲张(VC)所致男性不育患者和24例健康生育男性进行精子DNA碎片率(DNA fragmentation index, DFI)检测;采用硫代巴比妥酸法(TBA)测定精浆丙二醛(MDA)的含量,采用计算机辅助精液分析仪按照WHO标准进行精液常规分析;采用苏木素-伊红染色, Kruger评分标准进行精子形态学分析;比较VC组精液部分常规参数、精子DNA碎片率和精浆MDA的含量与对照组的差异并分析两组精子DNA碎片率和精浆MDA水平的相关性。结果 与正常生育力男性相比,VC组的精液量、精子密度、前向精子活动百分率和正常精子形态百分率下降,而精子DNA碎片率和精浆MDA的含量显著升高,且差别有统计学意义(P<0.05);VC组的精浆MDA水平与精子DNA碎片率显著正相关(r=0.59, P<0.05)。结论 精浆MDA水平升高和精子DNA碎片率的增加可能与VC患者的不育相关。而且精浆MDA水平升高可能是导致精子DNA碎片率增加的重要因素之一。

Abstract: Abstract: Objective To detect the sperm DNA fragmentation and level of MDA in seminal plasma and explore their correlation among infertile males with varicocele. Methods The sperm chromatin dispersion test was employed to detect the sperm DNA fragmentation in 52 infertile men with varicocele and 24 healthy fertile male; Content of malonddialdehyde in seminal plasma was measured by reaction of thiobarbiuric acid; At the same time, the classical semen parameters,their including sperm concentration, progressive motility and sperm morphology were evaluated by computer assisted sperm analysis. The differences in seminal parameters between infertile groups and control group were compared, and the correlation of the sperm DNA fragmentation and level of MDA in seminal plasma was explored in varicocele group. Results Men with varicocele had lower ejaculate volume, sperm concentration,progressive motility and morphology than men in the control group. Sperm DNA fragmentation and level of MDA in seminal plasma among infertile males with varicocel were significantly higher than those in control group.The perm DNA fragmentation was positively correlated with the level of MDA in varicocele group. Conclusion The increased sperm DNA fragmentation and level of MDA in seminal plasma may be related to the infertile males with varicocele. The increased level of MDA in seminal plasma may be one of the causes of increased sperm DNA fragmentation in infertile males with varicocele.
