2型糖尿病周围神经病变与代谢综合征相关性探讨

A Study on Relationship of Metabolic Syndrome with Diabetic Peripheral Neuropathy in Type 2 Diabetes Patients

摘要: 目的:探讨2型糖尿病合并糖尿病周围神经病变与代谢综合征(MS)的相关性。方法:对302例2型糖尿病患者完成Michigan糖尿病神经病变症状评分和Michigan体征评分及神经电生理检查,同时进行代谢综合征组分(腰围、血糖、血脂、血压)测定,比较上述神经病变指标在MS组与非MS组间差异性。结果:糖尿病合并MS组周围神经患病率(57.5%)、神经症状积分(4.12±1.89)和体征积分(2.15±1.60)较非MS组(44.3%,1.69±1.54,1.56±1.01)增高,差异有统计学意义(P<0.05),腓总感觉神经(左侧43.7±4.6m/s,右侧40.0±4.9m/s)及左正中感觉神经传导速度(51.7±4.2m/s)较非MS组(左腓神经46.1±4.0m/s,右腓神经45.9±4.5m/s,左正中神经55.5±4.3m/s)减慢,差异有统计学意义(P<0.05)。多因素回归分析显示,在校正了性别、年龄、病程、血糖、血压等因素后,代谢综合征仍与神经病变发生独立相关,OR值为1.195,P<0.05。结论:代谢综合征与2型糖尿病患者周围神经病变的发生密切相关。

Abstract: Objective: To research the relationship between peripheral neuropathy and Metabolic Syndrome (MS) in 2 type diabetes. Methods: 302 type 2 diabetes patients Were examined by Michigan diabetic neuropathy score and nerve electrophysiological test. Patients were divided into 2 groups according to their blood pressure, glucose levels, blood lipids and waistlines. Results: In the metabolic syndrome group, the prevalence of peripheral neuropathy (57.5%)and Michigan diabetic neuropathy symptom score(4.12±1.89) and sign score(2.15±1.60) were higher than those in non-MS group(44.3%,1.69±1.54,1.56±1.01)(all P<0.05). Peroneal sensory nerve conduction velocity (43.7±4.6m/s in left, 40.0±4.9m/s in right) in MS group were lower than those in non-MS group(46.1±4.0m/s in left, 45.9±4.5m/s in right).The left median sensory nerve conduction velocity(51.7±4.2m/s) in MS group was lower than that of non-MS group(55.5±4.3m/s) (all P<0.05).Multiple regression analysis show that MS was independently related to peripheral neuropathy after adjustment for sex, age, disease duration, blood pressure and blood glucose. OR value was 1.195, P<0.05. Conclusion: Metablic syndrome may be related to the progress of diabetic peripheral neuropathy in type 2 diabetes.
