鼻髓外浆细胞瘤六例临床观察

摘要： [摘要] 目的：探讨鼻髓外浆细胞瘤( Extramedullary Plasmacytoma , EMP) 的临床特征、诊断及治疗。方法:回顾性分析我科12年来收治的6例鼻EMP的临床资料。发生于鼻腔3例、上颌窦2例、鼻咽部1例。结果:6例均经病理学确诊为EMP。3例鼻腔EMP病人单纯手术治疗,1例鼻咽部EMP病人行单纯放疗,2例上颌窦EMP病人手术加放疗。随访2～12年。3例无瘤生存者分别已超过2年、6年、12年。1例3年后死于多发性骨髓瘤,1例5年后死于心脏病，失访1例。结论:鼻EMP属于低度恶性肿瘤,依据临床和组织学特点,结合免疫组化可明确诊断,手术和放疗是其主要治疗手段。

Abstract: [Abstract] 　Objective: 　To discuss the clinical features,diagnosis and treatment of extramedullary plasmacytoma ( EMP) in nose. Method :Clinical data of 6 patients with EMPs in nose, who treated in our hospital from Jan. 1996 to Dec. 2008 , were reviewed. Of 6 cases , 3 cases with the tumors occurred in nasal cavities , 2 in maxillary sinus , 1 in nasopharynx . Result :Six patients who were all pathologic confirmed EMP . 　Of six cases , 3 cases with nasal cavity EMPs were treated by surgery only , 1 with nasopharyngeal EMP received radiotherapy , and 2 with maxillary sinus EMPs treated with combination of surgery and post-operative radiotherapy. All patients were followed-up for 2 to 12 years. Three cases were still alive disease-free for more than 2, 6 , and 12 years after treatments , respectively. 1 died of multiple myeloma in 3 year , and 1 died of heart attack in 5 years after treatments , respectively. One lost follow-up. Conclusion :EMPs in nose are low potential malignancy tumors.The diagnosis of which is based on its clinical characteristics , histological features and immunohistochemistry analysis. Radiotherapy and surgery are the main approaches for treatment of this disease.
