腹股沟直疝及斜疝患者胶原、MMPs及TIMPs表达的研究*

【摘要】目的 比较胶原、基质金属蛋白酶(MMP)-2、MMP-9及金属蛋白酶组织抑制因子(TIMP)-1、TIMP-2在腹股沟直疝及斜疝患者腹横筋膜与血清中的表达,探讨腹股沟直疝的发病机制。方法 用Masson法检测在我院行疝修补术患者(直疝19例,斜疝27例)腹横筋膜中胶原的表达,用Western blot法对腹横筋膜MMPs及TIMPs的表达进行比较,同时应用ELISA法对血清MMPs及TIMPs进行测定。结果 直疝患者腹横筋膜胶原排列紊乱。斜疝患者腹横筋膜胶原和直疝相比排列较整齐,呈连续分布。直疝患者腹横筋膜胶原含量显著低于斜疝患者(P < 0.05),腹横筋膜及血清中MMP-2含量显著高于斜疝患者((P < 0.05),TIMP-2与TIMP-1含量显著低于斜疝患者(P < 0.05)。结论  MMP-2与TIMP-2、TIMP-1的异常变化导致其细胞外基质成分的改变可能是发生腹股沟疝不同类型的原因之一。

Abstract:

[Abstract] Objective  To investigate the expressions of collagen, MMP-2, MMP-9, TIMP-2 and TIMP-1 in patients with direct and indirect inguinal hernia, and explore the pathogenesis of the inguinal hernia. Methods Herniorrhaphy was performed in 46 patients with inguinal hernia (19 direct and 27 indirect), and the expressions of collagen, MMP-2 and TIMP-2 in the transversalis fascia were examined by using Masson stain and Western blot. The serum levels of MMP-2, MMP-9, TIMP-2 and TIMP-1 were examined by using ELISA method. Results  The irregular and disorder arrangement of collagen were found in patients with direct hernia, and the regular arrangement of collagen was found in patients with indirect hernia. There was a significantly lower level of collagen in the transversalis fascia in patients with direct hernia than that of patients with indirect hernia(P < 0.05). There was a significantly higher serum level of MMP-2 and the higher level in the transversalis fascia in patients with direct hernia than that of patients with indirect hernia(P < 0.05). The levels of TIMP-2 and TIMP-1 were significantly lower in patients with direct hernia than that of patients with indirect hernia(P < 0.05). Con clusion  The disordered expressions of collagen, MMP-2, TIMP-2 and TIMP-1 might be associated with the development of direct inguinal hernia.
