人工气道建立并发鼻窦炎的转归

Outcome of sinusitis in patients with artificial airway

摘要: 探讨人工气道建立并发的鼻窦炎在人工气道解除后近期的转归。方法 对于经鼻、经口气管插管及气管切开患者在拔管前行鼻窦CT扫描,确诊为鼻窦炎者,拔管第7天复查鼻窦CT。结果 56例拔管前确诊为鼻窦炎者,放射性鼻窦炎44例,感染性鼻窦炎12例。拔除气管导管后第7天复查鼻窦CT,23例痊愈,占41.07%(23/56);22例好转,占39.29%(22/56)。其中放射性鼻窦炎痊愈22例,为50.00%(22/44);好转16例,为36.36%(16/44)。感染性鼻窦炎痊愈2例,为16.67%(2/12);好转5例,为41.67%(5/12)。经鼻气管插管并发鼻窦炎18例痊愈,为47.37%(18/38);14例好转,为36.84%(14/38)。经口气管插管并发鼻窦炎痊愈3例,为27.27%(3/11);好转5例,为45.45%(5/11)。气管切开并发鼻窦炎痊愈2例,为28.57%(2/7);好转3例,为42.86%(3/7)。结论 作为人工气道建立的并发症,以放射性鼻窦炎为主,感染性鼻窦炎发生率低。人工气道解除后,大多数放射性鼻窦炎痊愈或好转,感染性鼻窦炎也多数好转,无需局部处理。

Abstract: Objective We performed this study to investigate the outcome of sinusitis in patients who had removed artificial airway in the near future. Methods Patients who had undergone nasotracheal, orotracheal intubation and incision of trachea performed sinus computed tomography (CT) scans before removing intubation. And the patients who were diagnosed sinusitis rechecked sinus CT scans seven days postoperatively. Results 44 cases of radiologic sinusitis and 12 cases of infectious sinusitis were diagnosed before removing intubation. 22 of the 44 (50.00%) patients were recovered and 16 of the 44 (36.36%) patients were improved after rechecking with radiologic sinusitis. 2 of the 12 (16.67%) patients were recovered and 5 of the 12 (41.67%) patients were improved after rechecking with infectious sinusitis. 18 of the 38 (47.37%) patients were recovered and 14 of the 38 (36.84%) patients were improved with nasotracheal intubation. 3 of the 11 (27.27%) patients were recovered and 5 of the 11 (45.45%) patients were improved with orotracheal intubation. 2 of the 7 (28.57%) patients were recovered and 3 of the 7 (42.86%) patients were improved with incision of trachea. Conclusion With the complication of Artificial airway, infectious sinusitis is less frequent than infectious sinusitis. Most patients who were diagnosed sinusitis recovered or improved after removing intubation without treatment.
