脑钠肽在晕厥患者鉴别诊断中的临床研究

摘要: 目的:探讨血浆脑钠肽(BNP)在心源性晕厥和非心源性晕厥鉴别诊断中的临床价值。方法:选取晕厥的住院患者85例,通过详细追问病史、体格检查,结合动态心电图、心脏彩超、冠状动脉造影、倾斜试验等相关检查,分为心源性晕厥组32例和非心源性晕厥组53例,并检测两组患者血浆BNP的浓度;结果:心源性晕厥组脑钠肽浓度342.8±208.5pg/ml,非心源性晕厥组脑钠肽52.3±34.4pg/ml,两组比较差异有显著统计学意义(P<0.01)。BNP以150ng/pg为分界点,其诊断心源性晕厥的敏感度为87.5%,特异度为66.0%,阴性预测值为89.7%。结论:血浆BNP水平测定在心源性晕厥和非心源性晕厥鉴别诊断中有着重要的临床意义。

Abstract: Objective: To discuss the clinic value of BNP for differential diagnosis in patients with syncope. Method: Eighty-five patients with syncope were divided into cardiac syncope group and non-cardiac syncope group according to many examine. BNP were mensurated for two groups. Result: BNP in cardiac syncope group was 342.8±208.5pg/ml and BNP in non-cardiac syncope group was 52.3±34.4pg/ml. BNP levels had significative statistics difference in two groups. At a cutoff of 150 pg/ml, BNP showed a sensitivity of 87.5%, a specificity of 66.0% and a negative predictive value of 89.7% for the diagnosis of cardiac syncope. Conclusion: BNP levels had important clinic significance for the differential diagnosis of cardiac syncope and non-cardiac syncope.
