T-SPOT.TB在肺结核早期抗痨治疗有效过程中的 变化趋势研究

Trend of T-SPOT.TB in the effective process of early antituberculosis treatment of tuberculosis

摘要: 摘要:目的 研究结核感染T细胞试验(T-SPOT.TB)在肺结核早期抗痨治疗有效过程中的变化趋势。方法 选 择2016年6月—2019年2月于我院结核科进行正规抗痨治疗的50例确诊初治活动性肺结核患者为研究对象,分析 其抗痨治疗前、治疗后(30 d、1个月、3个月)这4个时间点T-SPOT.TB结果动态变化趋势,及治疗前、后机体对结核分 枝杆菌特异性抗原[早期分泌靶向抗原6(ESAT-6)、培养滤液蛋白10(CFP-10)]刺激应答水平变化。结果 在为期 3 个月有效抗痨治疗期间,随着治疗时间的延长,T-SPOT.TB 结果大体呈逐步下降趋势,差异有统计学意义(P< 0.05)。治疗3个月后机体对ESAT-6、CFP-10刺激免疫应答变化一致。结论 T-SPOT.TB可能成为监测肺结核疗效 的良好指标;抗痨治疗有效后(第3个月时)机体对ESAT-6、CFP-10刺激应答水平变化一致。

Abstract: Abstract:Objective To study the change trend of T-cell spot of Tuberculosis (T-SPOT.TB) in the effective process of early anti-tuberculosis treatment of tuberculosis. Methods Fifty newly diagnosed patients with active pulmonary tuberculosis who received regular anti-tuberculosis treatment in tuberculosis department of our hospital from June 2016 to February 2019 were selected as subjects of the study. The dynamic change trend of T-SPOT.TB results and the changes of body's response to tuberculosis specific antigen [early secreting antigen target-6 (ESAT-6), culture filtrate protein-10 (CFP- 10)] stimulation before and after anti-tuberculosis treatment (30 days, 1 month and 3 months) were analyzed. Results During the period of effective anti-tuberculosis treatment of three months, the results of T-SPOT.TB decreased gradually with the prolongation of treatment time, and the difference was statistically significant (P<0.05). The changes of immune response to ESAT-6 and CFP-10 were consistent after treatment for three months. Conclusion T-SPOT.TB may be a good indicator for monitoring the efficacy of tuberculosis. The changes of body response to ESAT-6 and CFP-10 are consistent after anti-tuberculosis treatment (at the 3rd month).
