右旋美托咪啶对腹部手术患者全麻辅助作用的研究

The Study on Complementary Role of Dexmedetomidine in Abdominal Surgery Patients undergoing General Anesthesia

摘要: 摘要 目的:观察右旋美托咪啶(DEX)对腹部手术患者全麻诱导期血流动力学的影响及对麻醉药的节俭作用。方法:前瞻性、随机、双盲、安慰剂对照、平行分组研究。择期全麻下开腹手术患者48例随机分为2组:DEX组(D组)和安慰剂组(C组)。麻醉诱导前D组在10min内静脉注射DEX 1 μg/kg,C组静脉注射同体积生理盐水。分别观察给药前后及麻醉诱导期间血流动力学变化(血压、心率),同时记录全麻药用量。 结果:D组给药后心率明显低于C组(P<0.01),气管插管前血压最低值明显高于C组(P<0.01),气管插管前心率最低值明显低于C组(P<0.01),气管插管后心率最高值明显低于C组(P<0.01)。D组全麻药用量明显少于C组(P<0.05)。结论:DEX可增强麻醉诱导期心血管稳定性,减少静脉麻醉药丙泊酚、芬太尼及吸入麻醉药七氟烷的用量,具有很好的麻醉辅助作用。

Abstract: Abstract Objective: To evaluate the effect of haemodynamic changes during induction of general anesthesia and the dexmedetomidine-sparing effect of anesthetics in abdominal surgery patients undergoing general anaesthesia. Methods: This was a prospective, randomized, double-blind, placebo-controlled study. 48 patients (ASA I~II) aged 18 to 65 years old undergoing abdominal operation under general anesthesia were randomly divided into group D(intravenous 1μg ?kg-1 Dex for 10 min before induction of general anesthesia) and group C (0.9% normal saline was given as control). The haemodynamic changes including blood pressure and heart rate during induction of general anesthesia was observed, and the consumption of anesthetics was recorded. Results: The heart rate was significantly decreased in group D than that in group C after DEX administration(P<0.01). The minimum of blood pressure was significantly increased in group D than that in group C before tracheal intubation(P<0.01), while the minimum of heart rate was contrary. The maximum of heart rate was significantly decreased in group D than that in group C after tracheal intubation(P<0.01).The consumption of anesthetics was significantly less in group D than that in group C(P<0.05). Conclusion: Dexmedetomidine can attenuate hemodynamic responses and reduce requirements of propofol, fentanyl and sevoflurane. Dexmedetomidine play a very good complementary role in general anesthesia.
