胸腔气体分析对引流管堵塞气胸患者的治疗价值

The Value of Gas Partial Pressure Evaluate in Pleural Cavity to Pneumothorax Patients with Blocked Drainage Tube

摘要: 摘要 目的: 探讨两次胸腔内气体分压测定结果结合动脉血气分析对引流管堵塞气胸患者进一步治疗的指导价值。方法: 对于已行经皮肋间插管水封瓶引流术,但引流管堵塞患肺仍未复张的患者,抽取胸腔内气体。先后两次行气体分压测定,同时抽取桡动脉血行动脉血气分析。按照[Pp(CO2)],[Pp(O2)] ,[Pp(CO2)]/ [Pp(O2)],[Pp(CO2)]与[p(CO2)]的对比,判定患者引流管堵塞后气胸的类型,采取相应的治疗。结果: 49例患者中,判定为闭合性气胸30例,交通性气胸16例,张力性气胸3例;符合率为97.96%(48/49),其中张力性气胸,交通性气胸均为100%,闭合性气胸为96.67%(29/30),对97.96%(48/49)的患者采取了科学、针对性的治疗措施。结论: 以胸腔内气体分压测定结果为依据,较准确判定患者的气胸类型,采取科学有效的排气措施。

Abstract: Abstract Objective:To study the guidance value of gas in pleural cavity partial pressure evaluate twice of pneumothorax and arterial blood gas analysis in patients with blocked drainage tube for further treatment. Methods: The patients with intercostal tube and water seal bottle percutaneous drainage, but drainage tube blocking and ipsilateral lung is not reexpansion. Extracted gas in the pleural cavity twice, do gas partial pressure evaluate and arterial blood gas analysis of radial artery. In accordance with [Pp(CO2)], [Pp(O2)], [Pp(CO2)] / [Pp(O2)],[Pp(CO2)]and[p(CO2)judge different type of pneumothorax in patients with the blocked drainage tube, choose the way to treatment. Result: The group of 49 patients, 30 cases judged closed pneumothorax, 16 cases of traffic pneumothorax , 3 cases of tension pneumothorax; coincidence rate is 97.96%(48/49). Including tension pneumothorax and traffic pneumothorax are 100%,closed pneumothorax is 96.67%(29/30),97.96%(48/49)patients get a scientific treatment. Conclusion: To extract the gas of pleural cavity and evaluated twice, according to the results determine the type of patients with pneumothorax is more accurately, the exhaust in order to take a scientific and effective measures.
