慢性阻塞性肺病患者夜间低氧血症与室性心律失常的关系研究

The relationship between nocturnal hypoxemia and ventricular arrhythmia in patients with chronic obstructive pulmonary disease

摘要: 目的:探讨慢性阻塞性肺病(COPD)患者夜间室性心律失常与夜间低氧血症的关系。方法: COPD组患者36例和同期体检健康者36例(对照组),进行多导睡眠图(PSG)、24 h动态心电图(Holter)、血气分析、肺功能检测。结果:(1)COPD患者夜间低氧血症的发生19例,对照组均未发生;夜间室性心律失常(VA)的发生率明显高于对照组(58.3%vs5.6%),差异有统计学意义 (P<0.01)。(2)COPD患者夜间低氧血症与VA的发生无相关性(r=-0.077,P=0.646)。(3)COPD患者在快速动眼期(REM)和非快速动眼期(NREM)发生VA的频次[(5.51±0.53)次/h vs(5.58±0.32)次/h]差异无统计学意义(P>0.05)。结论:COPD患者出现夜间低氧血症和VA的比率明显要高于正常人,但COPD患者夜间低氧血症与室性心律失常的发生无相关性。

Abstract: Objective: The purpose of this study was to investigate the relationship between nocturnal hypoxemia and risk of ventricular arrhythmia (VA) during sleep in patients with chronic obstructive pulmonary disease (COPD). Method: 36 patients with COPD and 36 healthy subjects were selected . All the patients accepted Polysomnography, 24-h Holter electrocardiographic recording, Arterial blood gas analysis and pulmonary function tests. Results : Nocturnal hypoxemia episodes were found in nineteen patients with COPD during sleep but none of the control subjects . The prevalence of VA in patients with COPD was also significantly higher than that observed in healthy subjects( 58.3% vs 5.6% ,P<0.01). No associations were found between VA and nocturnal hypoxemia in patients with COPD (rho=-0.077,P=0.646). No differences were found between during REM and non-REM sleep for the frequency of VA[(5.51±0.53)times/h vs(5.58±0.32)times/h] . Conclusion:Patients with COPD have a higher frequency of VA and nocturnal hypoxemia episodes than health subjects. However, no associations were found between VA and nocturnal hypoxemia in patients with COPD.
