血清氧化低密度脂蛋白升高对早发冠心病的预测价值

【摘要】 目的   探讨早发冠心病(PCAD)患者血清氧化低密度脂蛋白(ox-LDL)及传统心血管危险因子的特征。方法  连续选取经冠状动脉造影术确诊的冠心病(CAD)患者66例选入PCAD组,冠脉血管正常的患者56例作为对照组。比较2组患者血清ox-LDL、高密度脂蛋白胆固醇(HDL-C)、三酰甘油(TG)、血肌酐(SCr)、尿酸(UA)等生化指标水平及性别、冠心病家族史、体质指数、高血压史、糖尿病史、吸烟史等的差异。结果 (1)PCAD组血清ox-LDL、TG、SCr、UA、空腹血糖(FBG)水平及男性比例高于对照组(P<0.05),HDL-C水平低于对照组(P<0.05),其他指标2组间差异无统计学意义。(2)ox-LDL水平与UA、SCr水平呈正相关,rs分别为0.243(P=0.006)、0.186(P=0.034)。(3)Logistic回归分析显示:ox-LDL、HDL-C和SCr是PCAD的影响因素,OR值分别为1.155、0.143、1.052。结论  ox-LDL、HDL-C和SCr异常是PCAD的独立预测因素。

Abstract: Objective    To investigate the characteristics of serum oxidized low density lipoprotein(oxLDL) and traditional cardioascular risk factors of premature coronary atherosclerotic heat disease(PCAD)patients. Method    We continuously selected 70 PCAD patients who have accepted coronary artery interventional therapy and 60 patients with normal coronary artery confirmed by angiography to be control group, male patients in two groups were 31-55 years old and female patients were 42-65 years old. Compareted the levels of ox-LDL, high density lipoprotein cholesterol(HDL-C), triglyceride(TG), serum creatinine(SCr), uric acid(UA), C-reactive protein(CRP) and other serum biochemical indicators and the differences of general clinical materials in the two groups. Results     The PCAD group had significantly raised plasma levels at ox-LDL, TG, SCr,UA and percentage of male as compared with the contral group(P<0.03), but significantly declined plasma level at HDL-C(P=0.03) and there was no difference at the others indicators. There were positive correlations between UA,SCr and ox-LDL. The regression analysis showed: ox-LDL, HDL-C and SCr had significantly predictive value to PCAD, OR is 1.172、0.144、1.055 respectively. Conclusion    High levels of ox-LDL and SCr, and low levels of HDL-C are the independent predictors of PCAD.
