跌倒患者的下肢肌肉力量研究

摘要: 摘要 目的: 研究跌倒者下肢力量的变化,为跌倒风险评估和预防提供依据。方法: 72位老年人根据在过去一年内有无跌倒现象,分为跌倒组(14例)和对照组(58例),分别进行利侧的下肢力量测试,下肢力量测试采用美国Biodex公司生产的等速力量测试训练系统,由内设电脑自动记录下60°/s和120°/s的角速度下的膝关节和踝关节的最大转矩、平均功率。结果 两组间膝关节的伸膝肌和屈膝肌在60°/s和120°/s的角速度下的最大转矩、平均功率的比较差异无统计学意义(p>0.05);两组间踝关节的背伸肌在120°/s的角速度下的最大转矩、平均功率的比较,跌倒组明显降低,差异有统计学意义(p <0.05);两组间踝关节跖屈肌在60°/s角速度下的最大转矩的比较,跌倒组明显降低,差异有统计学意义(p <0.05)。结论 下肢力量测试是跌倒风险评估的重要参考数据,同时加强下肢力量训练也是防跌倒的重要内容。

Abstract: Abstract Objective : To study lower limb strength changes of fallers in further to analyze fall related factors and prevent fall. Method : Isokinetic muscle testing was performed in 14 subjects with a history of one or more falls in the prior year (i.e. fallers) as well as a control group of 58 non-fallers. Peak torque (PT) and power (POW) were recorded at two velocities (60 degree/s and 120 degree/s) on a Biodex Isokinetic Dynamometer for four muscle groups which include knee extensors, knee flexors, ankle plantar flexors and ankle dorsiflexors. All parameters of fall group were compared with control group. Results: No significant difference was observed in PT and POW of knee extensors and flexors in two groups (p>0.05). PT and POW of ankle dorsiflexors in fall group was significant smaller than non- fall group at 120 degree/s (p<0.05). PT of ankle plantar flexors in fall group was significant weakness than non- fall group at 60 degree/s (p<0.05). Conclusion : Isokinetic muscle testing can be applied to older people fall risk protocol. It can provide referencing data for fall risk evaluation as well as the major training item.
