COPD患者不同机械通气参数对腹腔压力的影响

Effects of Different Mechanical Ventilation Parameters on Intra-Abdominal Pressure in COPD Patients

【摘要】目的 探讨机械通气状态下不同参数变化对腹腔压力的影响。方法 选取慢性阻塞性肺疾病(COPD)伴呼吸衰竭行机械通气的42例患者,当患者病情稳定后,在同步间歇指令通气(SIMV)+压力支持通气(PSV)的通气模式下,观察吸气平台时间(Tplat)、吸气时间(Ti)、呼气末正压(PEEP)的变化对腹腔压力的影响。结果在 SIMV+PSV通气模式下,Tplat、Ti的变化对腹腔压力无明显影响(P>0.05)。SIMV+PEEP0与基线值比较差异无统计学意义(P>0.05),随着PEEP的增加,腹腔压力随之升高(P<0.05)。结论机械通气状态下,呼气末正压的变化将会对腹腔压力产生显著影响。选择合理的机械通气参数至关重要。

Abstract: [Abstract] Objective To discuss the effect on intra-abdominal pressure by different mechanical ventilation parameters. Methods 42 cases with mechanical ventilation suffering from chronic obstructive pulmonary diseases and respiratory failure were selected for observing the effects on intra-abdominal pressure by changing Tplat, Tis and peep. Results Compared with Tplat and Tis, the change of peep has much more effect on intra-abdominal pressure, both in SIMV and PSV. Conclusion The alteration of peep has a great influence on intra-abdominal pressure ,which increases the importance of selecting proper mechanical ventilation parameters.
