血清POSTN、SDC4水平和24 h尿蛋白定量与紫癜性肾炎 患儿肾脏病理分级的相关性

Correlation between serum levels of periostin and syndecan-4 and 24-hour urinary protein and renal pathological grade in children with Henoch-schönlein purpura nephritis

摘要: 摘要:目的 探讨血清骨膜蛋白(POSTN)、多配体蛋白聚糖-4(SDC4)水平及 24 h 尿蛋白定量与紫癜性肾炎 (HSPN)患儿肾脏病理分级的相关性。方法 选择2015年1月—2018年12月在南华大学附属第二医院儿科住院的 58例经肾活检确诊为HSPN的患儿为研究对象,根据肾脏病理分级分为≤Ⅱa级15例、Ⅱb级19例、Ⅲa级15例、≥Ⅲb 级9例。采用酶联免疫吸附法(ELISA)测定血清POSTN、SDC4水平,采用终点法测定24 h尿蛋白定量;比较各检测指 标水平在不同肾脏病理分级之间的差异,并分析各指标之间及各指标与肾脏病理分级之间的相关性。结果 不同 肾脏病理分级患儿的血清POSTN、SDC4水平及24 h尿蛋白定量比较差异均有统计学意义,且各指标水平均随肾脏 病理分级升高而递增(P<0.05)。58例HSPN患儿血清POSTN、SDC4水平与24 h尿蛋白定量均呈正相关(r分别为 0.616、0.651,均P<0.05),血清POSTN与SDC4水平呈正相关(r=0.512,P<0.05);血清POSTN、SDC4水平和24 h尿蛋 白定量与肾脏病理分级均呈正相关(rs分别为0.705、0.589、0.661,均P<0.05)。结论 测定血清POSTN、SDC4水平 和24 h尿蛋白定量有助于评估HSPN患儿肾脏损伤的严重程度。

Abstract: Abstract:Objective To investigate the correlation between serum levels of periostin (POSTN) and syndecan-4 (SDC4) and 24-hour urinary protein and renal pathological grade in children with Henoch-schönlein purpura nephritis (HSPN). Methods A total of 58 children who were diagnosed as HSPN by renal biopsy in the Second Affiliated Hospital of University of South China from January 2015 to December 2018 were selected in this study. They were divided into four groups according to renal pathology results: grade ≤Ⅱa (n=15), grade Ⅱb (n=19), grade Ⅲa (n=15) and grade ≥Ⅲb (n=9). The serum levels of POSTN and SDC4 were measured by ELISA, and 24-hour urine protein was quantified by end-point method. The difference of each index in different pathological grades was compared and the correlation between each index and renal pathological grade was analyzed. Results There were significantly differences in serum levels of POSTN, SDC4 and 24-hour urine protein between children with different pathological grades (P<0.05). The level of each index was increased accompanied with the increase of pathological grade (P<0.05). There were positive correlations between the serum levels of POSTN, SDC4 and 24-hour urine protein in 58 children with HSPN (r = 0.616, 0.651, respectively, all P< 0.05). The serum level of POSTN was positively correlated with the serum level of SDC4 (r = 0.512, P<0.05). The serum levels of POSTN, SDC4 and 24-hour urine protein were all positively correlated to the renal pathological grade (rs = 0.705, 0.589, 0.661, respectively, all P<0.05). Conclusion The determination of the serum levels of POSTN, SDC4 and 24-hour urine protein may be used to assess the severity of renal injury in children with HSPN.
