颈动脉内膜中层厚度与心脏瓣膜钙化的相关性及其临床意义

The Clinical Significance of the Relationship of the Heart Valve Calcification and Carotid Intimamedia Thickness and Plaque Integral

摘要: 目的:研究心脏瓣膜钙化程度与颈动脉内膜中层厚度、斑块积分的相关性,并分析其临床意义。方法:用经胸多普勒超声显像仪评估206例患者心脏瓣膜钙化情况,根据瓣膜钙化诊断标准将所有患者分为4组:无钙化组、轻度钙化组、中度钙化组和重度钙化组。比较4组间一般资料、病史、血压、血脂,同时测量4组患者颈动脉内膜中层厚度、软斑块积分、硬斑块积分及总斑块积分。结果:4组间的年龄、性别、体质量指数、吸烟情况、总胆固醇、三酰甘油、高密度脂蛋白、低密度脂蛋白差异无统计学意义;瓣膜钙化程度与冠心病患病率、收缩压、脉压差值、颈动脉内膜中层厚度、总斑块积分、硬斑块积分呈正相关(r分别为0.546、0. 628、0. 512、0.802、0. 628、0.544,均P<0.001),与舒张压、软斑块积分无相关性。结论:心脏瓣膜钙化程度可作为推测颈动脉内膜中层厚度、总斑块积分、硬斑块积分的一个重要工具。

Abstract: Objective: To Research the Relationship of the degree of the heart valve calcification and carotid intimamedia thickness and plaque integral, and analyze its clinical significance. Methods : With transthoracic Doppler ultrasound imaging device ,the valve calcification of 206 patients were assessed, and all the patients were divided into four groups: no calcification group, mild calcification group, moderate group and severe calcification calcification group according to diagnostic criteria. The general information, medical history, blood pressure , blood lipids were Compared, simultaneous measured the intima-media thickness, soft plaque score, hard plaque score and the total plaque score of carotid artery of all the patients in the four groups. Results: The difference of age, gender, body mass index, smoking status, total cholesterol, triglycerides, high density lipoprotein, low density lipoprotein of the four groups was not statistically significant. The degree of valve calcification was positively correlated with coronary heart disease rate, systolic blood pressure , pulse pressure, carotid intima-media thickness, total plaque score and hard plaque score (r =0.546,0. 628,0. 512,0.802,0. 628,0.544, P<0.001) , and had no correlation with diastolic blood pressure and soft plaque score. Conclusion: Heart valve calcification can be a tool to speculate the carotid intima-media thickness, total plaque score and hard plaque score.
