摘要:

局部麻醉药在临床麻醉和疼痛管理中占据重要地位,然而传统局部麻醉药的作用时间有限,限制了其在术后镇痛领域的应用效果。布比卡因脂质体作为一种新型长效局部麻醉药,通过独特的DepoFoam技术显著延长了药物作用时间,为疼痛管理带来了新的解决方案。自2011年美国食品药品监督管理局首次批准其用于伤口浸润以控制术后疼痛以来,布比卡因脂质体在全球范围内逐渐受到关注,相关研究不断涌现。该文系统阐述布比卡因脂质体的药代动力学及药效学等药理特性,并全面梳理其在局部浸润麻醉、神经阻滞麻醉及特殊人群中的临床研究进展,旨在为临床合理规范使用布比卡因脂质体及优化围手术期镇痛方案提供循证依据。

Abstract:

Local anesthetics play a significant role in clinical anesthesia and pain management. However, the duration of action of traditional local anesthetics is limited, which restricts their application in the field of postoperative analgesia. Liposomal bupivacaine, as a novel long-acting local anesthetic, significantly prolongs the duration of drug action through its unique DepoFoam technology, offering a new solution for pain management. Since the U.S. Food and Drug Administration (FDA) first approved bupivacaine liposome for wound infiltration to control postoperative pain in 2011, it has gradually attracted global attention, and related studies have emerged continuously. This article will elaborate the pharmacological properties of liposomal bupivacaine including pharmacokinetics and pharmacodynamics, and comprehensively review its clinical research progress in local infiltration anesthesia, nerve block anesthesia and application in special populations, so as to provide evidence-based references for the rational and standardized clinical application of liposomal bupivacaine and the optimization of perioperative analgesia regimens.
