摘要:

随着宫颈癌发病机制研究的深入及宫颈癌多种筛查手段的发展,人乳头瘤病毒(HPV)E6/E7 mRNA检测通过靶向病毒癌基因的转录活性,直接反映HPV的致癌潜能与病毒整合状态,推动了宫颈癌筛查策略从单纯“发现感染”向精准“评估风险”的范式转型。该文系统综述了HPV E6/E7 mRNA检测的分子基础、技术特征、诊断效能及筛查分流应用,探讨其动态监测在癌前病变分层管理中的价值及面临的临床挑战,为宫颈癌的精准防控与医疗资源优化提供参考。

Abstract:

With the deepening understanding of the pathogenesis of cervical cancer and the development of diverse screening modalities for cervical cancer, the detection of human papillomavirus (HPV) E6/E7 mRNA targets the transcriptional activity of viral oncogenes, directly reflects the carcinogenic potential and viral integration status of HPV, driving a paradigm shift in cervical cancer screening strategies from merely "detecting infection" to precisely "assessing risk". This article systematically reviews the molecular basis, technical characteristics, diagnostic performance and triage application of HPV E6/E7 mRNA testing, explores its value in dynamic monitoring for risk-stratified management of precancerous lesions and the clinical challenges encountered, and provides insights for precision prevention and control of cervical cancer and optimization of healthcare resources.
