血浆 PDGF-BB 水平与冠心病及冠状动脉狭窄程度的相关性研究

The relationship between plasma PDGF-BB level and coronary heart disease and coronary artery stenosis

摘要: 摘要:目的 探讨血小板衍生生长因子-BB(PDGF-BB)水平与冠心病及冠状动脉狭窄程度的关系。方法 收 集天津市胸科医院心内科住院患者共 262 例,根据病史、症状、实验室检查及冠状动脉造影结果分为稳定型心绞痛 (SAP)组 57 例,急性冠脉综合征(ACS)组 119 例,冠脉正常组 86 例。ACS 组中冠脉单支病变 38 例,双支病变 35 例,多支病变 46 例。比较 SAP 组、ACS 组和对照组间一般临床资料、生化指标及血浆 PDGF-BB 水平的差异; Spearman 相关分析血浆 PDGF-BB 水平与超敏 C 反应蛋白(hs-CRP)及 Gensini 评分的相关性;Logistic 回归分析血 浆 PDGF-BB 水平及其他指标与冠心病发病的关系。结果 (1)ACS 组 hs-CRP 及血浆 PDGF-BB 水平高于对照组、 SAP 组(P<0.05)。(2)Spearman 相关性分析显示血浆 PDGF-BB 水平与 hs-CRP 及 Gensini 评分相均无线性相关性 (均 P > 0.05)。(3)ACS 组中单支、双支及多支病变患者间血浆 PDGF-BB 水平差异无统计学意义(P>0.05)。(4) Logistic 回归分析显示血浆高 PDGF-BB 水平是冠心病发病的危险因素。结论 血浆高 PDGF-BB 水平与冠心病发 病有关,一定程度上可能反映冠状动脉斑块的不稳定性,但尚不能作为评估冠脉病变严重程度的指标。

Abstract: Abstract: Objective To investigate the relationship between plasma level of platelet derived growth factor- BB (PDGF-BB), coronary heart disease (CHD) and the severity of coronary artery stenosis. Methods A total of 262 patients hospitalized in Department of Cardiology, Tianjin Chest Hospital were collected in this study. According to the medical history, symptoms, laboratory examination and the results of coronary angiography, patients were divided into stable angina pectoris (SAP) group (n=57), acute coronary syndrome (ACS) group (n=119) and normal control group (n=86). The ACS group was divided into three subgroups: single vessel group (n=38), double vessel group (n=35) and multiple vessel group (n=46). The general clinical data, biochemical parameters and plasma PDGF-BB levels were compared between SAP group, ACS group and control group. Spearman correlation analysis was used to analyze the relationship between PDGF-BB level, highsensitivity C-reactive protein (hs-CRP) and Gensini scores. Logistic regression analysis was used to analyze the risk factors of coronary heart disease. Results (1) The plasma levels of hs-CRP and PDGF-BB were significantly higher in ACS group than those in control group and SAP group (P < 0.05). (2) Spearman correlation analysis showed that there was no correlation between plasma levels of PDGF-BB and hs-CRP and Gensini score (P > 0.05). (3) There was no significant difference in plasma level of PDGF- BB between single vessel group, double vessel group and multiple vessel group (P > 0.05). (4) Logistic regression analysis showed that high plasma level of PDGF- BB was the risk factor for coronary heart disease. Conclusion PDGF-BB plasma level is associated with the pathogenesis of coronary heart disease, which may reflect the instability of coronary atherosclerotic plaques, but it is not an index to evaluate the severity of coronary stenosis.
