转甲状腺素蛋白的化学修饰物在肺感染性疾病诊断中的作用

Investigation The Modification of Transthyretin For Diagnosis of Infectious Lung Disease

摘要: 目的:探讨胸水和血清中转甲状腺素蛋白(TTR)的含量和其化学修饰类型在诊断肺感染性疾病中的作用。方法:(1)测定血清和胸水中总蛋白(TP)、白蛋白(ALB)、甘油三酯(TG)、胆固醇(Cho)、载脂蛋白A(Apo A)、载脂蛋白B(Apo B)、乳酸脱氢酶(LDH) 、腺苷脱氨酶(ADA)、葡萄糖(GLU)、和转甲状腺素蛋白(TTR)等指标水平。(2)基质辅助激光电离飞行时间质谱(MALDI-TOF-MS)分析TTR蛋白的化学修饰类型及比例。结果:(1)胸水中有8项指标比血清明显减低,按减低程度明显的顺序,依次是:TG、Cho、TTR、ApoA、ApoB、TP、Alb和GLU,而增高的仅ADA指标,LDH无统计学差异。(2)在蛋白指标中TTR减低最明显。(3)肺感染患者胸水和血清中只发现两种修饰类型,与健康对照组相比无cysgly-TTR修饰型。结论:肺感染患者血清及胸水中cysgly-TTR化学修饰型的消失可能作为肺部感染诊断的参考指标。

Abstract: Objective: To investigated the role of transthyretin (TTR) level and its modification in serum and pleural fluid (PF) for diagnosis of infectious lung disease. Methods: (1) The level of Total protein (TP), albumin (ALB), triglycerides (TG), cholesterol (Cho), apolipoprotein A (Apo A), apolipoprotein B (Apo B ), lactate dehydrogenase (LDH), adenosine deaminase (ADA), glucose (GLU), and transthyretin protein (TTR) etc. were detected in serum and PF samples.(2) Chemically modified type and proportion of TTR proteins was analyzed by matrix-assisted laser ionization time of flight mass spectrometry (MALDI-TOF-MS). Results: (1) 8 components were significantly lower in PF than in serum, reduced order as follow: TG, Cho, TTR, ApoA, ApoB, TP, Alb and GLU , while the ADA was increased only, and LDH was no statistical differences; (2) Among protein indexs, TTR is the most significantly reduced one; (3) 2 types of modified TTR were detected in PF and serum of patients with infectious lung disease, while contrast with the healthy control group ,no cysgly-TTR was present. Conclusions: The cysgly-TTR was disappeared in both PF and serum, indicating that TTR modification may be an assistant for lung infection diagnosis.
