HLA-B27与幼年型脊柱关节病的回顾性分析

摘要： 目的：探讨幼年性脊柱关节病患者与人类白细胞抗原HLAB-27阳性与阴性的临床表现、化验指标等之间的差异，为临床诊疗提供借鉴。方法：将临床诊断为JSpA患儿根据HLA-B27 DNA是否阳性分为两组，阳性组94例，阴性组80例；分析两组患儿的临床表现、相关实验室检查结果等来比较两组之间的差异。结果：男女比例，发病年龄，骶髂关节受累情况，影像学检查及IgA，ESR，ANA，蛋白电泳β等两组比较均有统计学意义（p<0.05）。而外周关节受累，其他免疫学指标及CRP等，差异无统计学意义（p>0.05）。结论：HLA-B27阳性组JSpA患者发病年龄较晚，且临床症状比较重，更易发展为不可逆性病变。

Abstract: Objective：To investigate the relation of HLA-B27 in the progression and prognosis of patients with juvenile-onsetspondyloarthropathies（JSpA），we compare the clinical and laboratory examination manifestation between the HLA-B27 antigen positive and negative groups。Methods：The clinical and laboratory manifestations of 94 JSpA patients with positive HLA-B27 were analyzed and compared with those of 80 patients with negative HLA-B27.Results：There was significance difference in the gender，age of disease onset，the involved of articulatio sacroiliaca，the imageology and the IgA，erythrocyte sedimentation rate（ESR），antinuclear antibody，protein electrophoresisβmanifestation between the two groups（p<0.05），in contrast to the peripheral joint involvement，the other immune index，C-reactive protein(CRP) and so on (p>0.05)。Conclusion: HLA-B27 (+) JSpA patients have an later onset of age,severer symptoms and are tend to develop irreversible lesions.
