肿瘤晚期患者癌性疲乏与影响因素调查

Influential Factors of Cancer-Related Fatigue in Advanced Cancer Patients

摘要: 【摘要】目的  调查晚期肿瘤患者癌性疲乏的发生情况,分析癌性疲乏的相关影响因素。方法 采用病例回顾与问卷调查相结合的办法,调查我科95例肿瘤晚期患者,所用量表为多维疲乏症状量表简表、数字评估量表、医院焦虑抑郁量表、社会支持评定量表。结果  回收有效问卷95份,其中83例(87.37%)存在疲乏;疲乏程度人口统计学特征之间无明显差异(P>0.05);血红蛋白、白细胞、血清人血白蛋白、KPS评分、疼痛VAS评分、社会支持总分、主观支持、社会支持利用度、焦虑、抑郁与疲乏总分显著相关(均P < 0.05);多因素分析显示疼痛、贫血、抑郁及主观支持评分低是影响晚期肿瘤患者癌性疲乏的主要危险因素。结论  晚期肿瘤患者中疲乏的发生率高,程度较重,改善患者的合并症如疼痛、抑郁、贫血,增加主观支持,可能在一定程度上会降低疲乏的程度,提高患者生活质量。

Abstract:

[Abstract]  Objective  To investigate the incidence and influential factors of cancer-related fatigue in advanced cancer patients. Methods   The clinical data of 95 advanced cancer patients were retrospectively surveyed and the questionnaire were used together in the study including Multidimensional Fatigue Symptom Inventory-Short Form,Visual Analogue Scale,Hospital Anxiety and Depression Scale and Social Support Rating Scale. Results    The total of 83 cases (87.37%) experienced fatigue. There was no significant difference in the fatigue between demographic characteristics (P > 0.05). The levels of hemoglobin, white blood cells, serum albumin, Karnofsky performance scale (KPS), VAS pain score, the total social support score, subjective support, support availability, anxiety and depression were significantly related to the fatigue (P < 0.05). Multiple regression analysis indicated that pain,anemia,depression and subjective supports were major risk factors of cancer-related fatigue.Conclusion   There is a high incidence of severe fatigue in advanced cancer patients. The improvement of complications, such as pain, depression, anemia, and increase of subjective support may alleviate the fatigue and improve the quality of life in advanced cancer patients.
