Endoscopic diagnosis and treatment of a case of rectal submucosal angiomyolipoma

Abstract

Abstract:

Submucosal angiomyolipoma (AML) of rectum is clinically rare. This article reports the endoscopic diagnosis and treatment process of a case of rectal AML. By reviewing the clinical data of this patient and combining with literature review, we aim to deepen the current understanding of AML in the digestive tract and avoid misdiagnosis and mistreatment in clinical practice.

直肠AML	中低回声为主，回声混杂，内可见高回声影，多位于黏膜下层，回声不均匀，边界欠规整	平滑肌、血管、脂肪混合成分	平滑肌细胞标志物（SMA、Desmin），黑色素细胞标志物（HMB-45）、血管源性标志物（CD34）、间叶组织标志物（CD117、Vimentin）阳性	SMA、Desmin阳性、CD34、CD117散在阳性，结合镜下平滑肌、血管、脂肪混合成分特征性结构，符合AML表现
直肠GIST	多位于固有肌层，呈低回声，形态规则，高低混杂回声，可通过EUS评估其恶性潜能	梭形细胞最常见，其次为上皮样细胞，无脂肪成分	多为CD117、CD34阳性，Dog-1阳性，SMA阴性	Dog-1阴性、SMA阳性
直肠NET	位于黏膜下层，呈低回声，回声前均匀	巢状排列的神经内分泌细胞，无脂肪成分	Syn、CgA、CK阳性	CK阴性
直肠平滑肌瘤	位于固有肌层或黏膜肌层，呈均匀低回声，边界清晰	梭形平滑肌细胞，无脂肪成分	SMA、Desmin阳性、 S100阴性	病理可见脂肪成分，S100阳性
直肠脂肪瘤	位于黏膜下层，呈均匀高回声，边界清晰	成熟脂肪细胞，无平滑肌、血管成分	S100阳性、SMA阴性	SMA阳性，EUS 非均匀高回声
