毫米波辅助治疗膝骨关节炎的疗效观察

Observation on the efficacy of millimeter wave adjuvant therapy for knee osteoarthritis

摘要:

目的 评估毫米波辅助治疗对膝骨关节炎(KOA)的疗效及外周血辅助性T淋巴细胞1型/辅助性T淋巴细胞2型(Th1/Th2)细胞因子平衡的影响。方法 选取90例KOA患者,随机分配至对照组(常规综合治疗,45例)和观察组(常规综合治疗+毫米波辅助治疗,45例)。比较2组患者临床疗效、疼痛视觉模拟量表(VAS)评分、西安大略和麦克马斯特大学骨关节炎指数(WOMAC)及膝关节活动度(ROM)、外周血Th1/Th2细胞因子水平、不良反应情况。结果 治疗12周后,观察组总有效率高于对照组(P<0.05);观察组治疗4、8、12周的疼痛VAS评分均低于对照组(P<0.05);治疗12周后,观察组WOMAC评分、白细胞介素(IL)-2、干扰素(IFN)-γ水平低于对照组,ROM、IL-4、IL-10水平高于对照组(P<0.05);2组不良反应发生率比较差异无统计学意义(P>0.05)。结论 毫米波辅助治疗能够调节外周血Th1/Th2细胞因子平衡,有效缓解KOA患者关节疼痛,改善膝关节功能,且安全性良好。

Abstract:

Objective To evaluate the efficacy of millimeter wave adjuvant therapy on knee osteoarthritis (KOA) and the influence on peripheral blood T helper type 1/T helper type 2 (Th1/Th2) cytokine balance. Methods A total of 90 patients with KOA were selected and randomly assigned into the control group (conventional comprehensive therapy, 45 cases) and the observation group (conventional comprehensive therapy plus millimeter wave adjuvant therapy, 45 cases). The clinical efficacy, pain visual analogue scale (VAS) score, Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), knee range of motion (ROM), levels of peripheral blood Th1/Th2 cytokines and adverse reactions were compared between the two groups of patients. Results After 12 weeks of treatment, the total effective rate was higher in the observation group than that in the control group (P<0.05). Pain VAS scores after 4, 8 and 12 weeks of treatment were lower in the observation group compared to those of the control group (P<0.05). After 12 weeks of treatment, WOMAC scores, IL-2 and IFN-γ levels were lower in the observation group compared to those of the control group, and ROM, IL-4 and IL-10 were higher (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). Conclusion Millimeter wave adjuvant therapy can regulate the peripheral blood Th1/Th2 cytokine balance, effectively relieve the joint pain and improve the knee function in KOA patients, with good safety.
