Abstract: Objective: To evaluate the predictive value of heart -type fatty acid-binding protein (H-FABP) for the acute pulmonary embolism patients in recent prognosis . Methods: From August 2009 to July 2011, 87 patients with acute pulmonary embolism were enrolled in people's hospital of Xingtai . After be in hospital the H-FABP level of these patients were determined. According to the blood H-FABP determination values ,87 cases were divided into two groups. Positive group: H-FABP ≥10 ng/mL (n = 21), negative group: H-FABP < 10 ng/mL (n = 66). And we have compared the different rates of adverse events between the two groups in syncope, shock and death .At the same time we have respectively compared the different blood H-FABP levels between the two groups when these clinical adverse events happen or not. Result: In the positive group it has higher incidence in Syncope, shock ,right heart dysfunction, fibrinolytic therapy, and mechanical ventilation. These all have statistically significant (P < 0.05). Total 2 deaths had occurred in H-FABP positive group during the 30 days. H-FABP level in the group with clinical adverse events(15.23 ng/mL±2.92 ng/mL)is significantly higher than the group without adverse events(5.49 ng/mL±3.36 ng/mL).The result has statistically significant differences (P < 0.01). Conclusion: H-FABP increase is the risk factor in the recent death of acute pulmonary embolism patients and indicates poor prognosis . It provides effective risk stratification for the acute pulmonary embolism patients.
