    Effects of overexpression of CTCF on apoptosis factors Bax and Bcl-2 in breast cancer cells

    Objective To investigate the effect of the over- expressed CTCF on apoptosis factors Bax and Bcl- 2 in human breast cancer cell line MDA-MB-231. Methods Reverse transcription-polymerase chain reaction (RT-PCR) were used to detect the expressions of CTCF,Bax and Bcl-2 in MDA-MB-231. The overexpression vector of CTCF/pEGFP-N1 was constructed. The overexpression plasmid CTCF/pEGFP-N1 and the empty vector plasmid pEGFP-N1 were transfected into breast cancer cell line MDA-MB-231 by lentivirus transfection, and the MDA-MB-231 cells were divided into CTCF group and control group. After successfully transfection of MDA-MB-231 identified by RT-PCR, real time quantitative PCR (Q-PCR) was used to detect the mRNA levels of Bax and Bcl-2 in MDA-MB-231 of the CTCF group and the control group. The protein levels of Bax and Bcl-2 were detected by Western blot assay and enzyme-linked immunosorbent assay (ELISA). Results The expression of CTCF was not found in MDA-MB-231, and expressions of Bax and Bcl-2 were found in MDAMB-231. Results of Q-PCR showed that the mRNA levels of Bax were 4.63±1.08 and 2.27±0.16 in CTCF group and control group, respectively, and they were statistically significant (t=27.50, P<0.05). The mRNA levels of Bcl-2 were 1.39±0.14 and 3.56 ± 0.97 in CTCF group and control group, and there was significant difference between two groups(t=39.00, P<0.05). Results of Western blot assay showed that the protein level of Bax was higher in CTCF group compared with that of control group. The protein level of Bcl-2 was lower in CTCF group compared with that of control group. Results of ELISA showed that the protein levels of Bax were 15.25±2.17 and 6.24±1.78 in CTCF group and control group, respectively, and there was significant difference between the two groups (t=26.84, P<0.05). The protein levels of Bcl- 2 were 4.59±0.97 and 10.68± 1.93, and there was significant difference between the two groups (t=21.72, P<0.05). Conclusion The over- expressed CTCF can promote the expression of apoptotic factors and inhibit the expression of anti-apoptotic factors in breast cancer cells.

    microRNA-155 inhibits immuno-inflammatory reaction of foam cells by targeting MyD88

    Objective To investigate the effects of microRNA- 155 (miR-155) on immuno-inflammatory reaction of foam cells by targeting MyD88 and the possible mechanism. Methods RAW264.7 macrophages were cultured in vitro and transfected with miR-155 mimics, miR-155 inhibitor, MyD88 siRNA and their negative control respectively, then ox-LDL stimulation was given to build foam cell model. The expression of MyD88 in foam cells was detected by RT- qPCR and Western blot assay. Moreover, the expression levels of interleukin (IL)- 10, TGF- β1 and MCP- 1 in supernatant were determined by ELISA. Results After being transfected with miR- 155 mimics, the mRNA level of MyD88 remained unchanged compared with that of control group (P>0.05). The protein level of MyD88 decreased significantly (P<0.05), and the expression levels of IL-10, TGF-β1 and MCP-1 in supernatant also decreased significantly (P<0.05). After being transfected with miR- 155 inhibitor, the mRNA level of MyD88 remained unchanged compared with that of control group (P>0.05). The expression levels of MyD88 protein and inflammatory cytokines increased significantly (P<0.05). After being transfected with MyD88 siRNA, the expression levels of MyD88 mRNA and protein decreased significantly, and the expression levels of inflammatory cytokines also decreased significantly (P<0.05). Conclusion miR-155 can negatively regulate inflammation by targeting MyD88 through the inhibition of translation.

    PEGylated doxorubicin nanoparticles mediated by HN-1 peptide for targeting transplanted oral squamous cell carcinoma

    Objective To preliminary study on the physical properties of HN- 1 modified adriamycin (DOX) nanoparticle system HPD and to prove its transplanted oral squamous cell carcinoma (OSCC)- targeting capability. Methods Firstly, the targeting capability of HN-1 for OSCC cells was verified. PEGylated DOX (PD) nanoparticles were synthesized by self-assembly in aqueous media. HN-1 was then chemically grafted onto the surface of PD nanoparticles to form HPD nanoparticles. The HPD nanoparticles were stored in H2O, PBS and PBS containing 10% serum for different time periods. The sizes and distribution diagrams of nanoparticles were evaluated at 1, 3, 5 and 7 days. The serum levels of DOX mediated by HPD nanoparticles were measured. The stability of HPD nanoparticles in vivo was studied. The tumor bearing mice were prepared by inoculating 2.0×106 SCC-25 cells into BALB/C nude mice. Then, mice were randomly divided into four groups (n=2 for each group), normal saline group (control), free DOX group, PD group and HPD nanoparticle group. A dose of 8.0 mg/kg body weight on a DOX basis was injected intravenously though tail vain. The mice were sacrificed at 8 h and 24 h after injection, and the major organs (heart, liver, spleen, lung and kidney) and tumor were excised. The ex vivo DOX fluorescence imaging was obtained using the IVIS. Results HN-1 showed strong capability of targeting OSCC cells. HPD nanoparticles showed an uniform spherical shape and a small size of 150 nm, which also showed strong stability. In the nude mice bearing OSCC tumor, HPD nanoparticles displayed remarkably enhanced tumor- targeting and penetrating efficiency compared with those of PD nanoparticles. Conclusion This study demonstrates that HPD nanoparticles mediated by HN-1 can efficiently target transplanted OSCC, and have potential application for the OSCC-targeting treatment.

    Antitumor effect of DC-GPC3 cocultured with CIK on hepatocellular carcinoma in vivo

    Objective To investigate whether immunotherapy with dendritic cells (DC) transduced with glypican 3 (GPC3) (DC- GPC3) and cocultured with cytokine- induced killer cells (CIK) (DCIK- GPC3) is capable of inhibiting hepatocellular carcinoma (HCC) in vivo. Methods The hepatocarcinoma cell-bearing mouse models were established and divided randomly into four groups (A, B, C, D) for the injection of normal saline (NS), CIK, DC-CIK, and DCIK-GPC3, respectively. Changes of tumor size, tumor inhibitory rate and cell apoptosis were compared between four groups. Results After treatment, the tumor volumes and weights were significantly decreased in D group compared with those of A, B and C groups (P<0.01). The inhibition rate was significantly increased in D group compared with that of B group and C group (P< 0.01). The apoptotic rate was significantly increased in D group compared with that of A group, B group and C group (P< 0.01). Conclusion DCIK-GPC3 can significantly inhibit tumor growth in vivo.

    Objective To study the influences of hemolysis, fatty blood, storage temperature and storage time on blood HIV RNA and HBV DNA. Methods The HBV DNA and HIV RNA samples (the concentration was 12-30 times of limit of detection of reagent), which were stored for 4 h, 1 d, 3 d,1 w and 4 w under the conditions of 4 ℃, 25 ℃, 37 ℃ and -30 ℃, were detected using Roche MPX V2.0 kit. The HBV DNA and HIV RNA samples (the concentration was 2-5 times of limit of detection of reagent) were detected by the 6 groups of hemolytic samples (the concentrations of hemoglobin were 97 g/L, 34 g/L,17 g/L, 8 g/L, 5 g/L and 3 g/L, respectively). NAT test was performed at 5-group lipid samples (the concentrations of triglyceride were 7.93 mmol/L, 3.80 mmol/L, 2.63 mmol/L, 1.83 mmol/L and 1.49 mmol/L, respectively) and the control group samples (the concentrations of hemoglobin and triglyceride were 0 g/L and 0.95 mmol/L respectively). Results There were no significant differences in Ct values of HIV RNA or HBV DNA between 4 h to 4 w at 25 ℃ (P>0.05). There were significant differences in Ct values of HBV DNA after preservation for 3 d and 1 w under 37 ℃ compared with those of preservation for 4 h,1 d and 2 d (P<0.005). When Hb concentration was reached to 97 g/L, the results of HBV DNA and HIV RNA were negative. When the concentration of Hb was less than 34 g/L, compared with the control group, there were no significant differences in Ct values of HIV RNA and HBV DNA (P>0.05). When the TG concentration was ≤ 7.93 mmol/L, there were no significant differences in Ct values between the control group and the TG group (P>0.05). Conclusion The samples of HIV RNA and HBV DNA detected by Roche MPX V2.0 kit can be stored at room temperature (25 ℃) for four weeks. When the concentrations of TG and Hb are less than 7.93 mmol/L and 34 g/L respectively, there are no effects on HIV RNA or HBV DNA samples detected by Roche MPX V2.0 kit.

    The effects of dynamic pressure on expression of Sox9 mRNA and protein in metaphyseal chondrocytes of rats

    Objective To study the effect of dynamic stress stimulation on the expression of Sox9 mRNA and protein in metaphyseal chondrocytes in vitro, and to explore the specific mechanism of mechanical signal transduction. Methods The rat metaphyseal chondrocytes separated and cultured for the 3rd generation in vitro were randomly divided into four groups: control group (all interventions were not applied), simple dynamic pressure group (a dynamic pressure stimulus with a size of 90 mmHg and a frequency of 0.1 Hz was applied using an open pressure control culture system), simple calcium antagonist group (the concentration of 10 μmol/L nifedipine was given) and dynamic pressure + calcium antagonist group (a dynamic pressure stimulus with a size of 90 mmHg, frequency of 0.1 Hz and concentration of 10 μmol/L nifedipine were given at the same time). The expression of Sox9 mRNA was detected after 24 h intervention by real- time quantitative polymerase chain reaction (RT-PCR) in four groups. The expression of Sox9 protein was detected by Western blot assay. The intracellular free Ca2 + in metaphyseal chondrocytes was labeled with Fluo- 3/AM, and the average fluorescence intensity detected by laser scanning confocal scanning microscopy was compared between four groups. Results The expression of Sox9 mRNA was 3.81 times higher in dynamic stress group than that in the control group, and the protein expression level was 2.33 times higher than that of the control group (P<0.05). There were no significant differences in the expression of Sox9 mRNA and protein between the calcium antagonist group and the control group. The expressions of Sox9 mRNA and protein were lower in dynamic pressure + calcium antagonist group than those in the dynamic stress group, but which were higher than those of control group(P<0.05). The results of average fluorescence intensity showed that there was no significant difference in the intracellular free Ca2 + concentration between four groups (P > 0.05). Conclusion Dynamic stress stimulation can increase the expression of Sox9 mRNA and protein in rat metaphyseal chondrocytes. There is calcium channel involvement in the mechanical signal transduction.

    A study on memory disorder after marginal division of the striatum stroke

    Objective To explore the effects of marginal division of the striatum (MrD) stroke on associative learning and memory. Methods A total of 21 cases with marginal division of the striatum stroke were used as stroke group. The control group consisted of 21 healthy volunteers matched with the stroke group for sex, age and education. The clinical memory scale edited by Psychological research institute, Chinese academy of sciences was used to evaluate data of learning and memory. Results The score was significantly lower in stroke group compared with that of control group (17.71±5.59 vs. 24.80±5.05, P<0.01). The score was significantly lower in patients with lesions on left than that in patients with lesions on right (11.78±1.48 vs. 22.17±2.17, P<0.01). There was no significant difference in the score between hemorrhagic stroke group (16.50±6.59) and ischemic stroke group (18.46±5.01,P>0.05). Conclusion Marginal division of the striatum stroke can cause the associative learning and memory disorder, which is more serious in patients with lesions on left than that in patients with lesions on right.

    Observation of IL-17 levels of perioperative period in congenital heart disease patients with pulmonary hypertension

    Objective To discuss the changes and significance of interleukin- 17 (IL-17) in perioperative period of congenital heart disease patients with pulmonary hypertension. Methods A total of forty patients with congenital heart disease underwent cardiopulmonary bypass (CPB) were included in this study. According to the pulmonary artery systolic pressure (PASP), patients were divided into non- pulmonary hypertension group (group Ⅰ, PASP < 30 mmHg) and pulmonary hypertension group (group Ⅱ, PASP≥30 mmHg). Blood samples were taken before anesthesia (T1), start CPB (T2), 30 min after CPB (T3), 6 h (T4), 24 h (T5) and 7 d (T6) after operation. The concentration of IL-17 was detected by ELISA. Arterial oxygen partial pressure [p(O2)] and arterial carbon dioxide partial pressure [p(CO2)] during the first five time points were recorded. Oxygen index (OI) and alveolar arterial oxygen tension difference (AaDO2) were calculated. Results The plasma IL-17 levels in perioperative period were significantly higher in group Ⅱ than those of group Ⅰ (P < 0.05). The highest concentration of IL-17 emerged at T3, then decreased gradually in both groups. At this time point, the OI decreased, and AaDO2 increased significantly in both groups. Compared with groupⅠ, the OI decreased, while AaDO2 increased at T5 in groupⅡ (P <0.05). Conclusion The high level of IL-17 promotes the formation of pulmonary hypertension in congenital heart disease and leads to the lung injury during CPB, which can be used as a clinical monitoring indicator of evaluating severity.

    Objective To explore the therapeutic effects of endovenous laser ablation (EVLA) combined with percutaneous continuous circumsuture (PCCS) and EVLA in treating severe great saphena varicose. Methods A total of 60 patients with unilateral great saphenous varicose level C5 - C6 were randomly divided into control group and experimental group according to the CEAP system. Control group was given EVLA surgery while experimental group was given EVLA+ PCCS surgery. Data of operation time, hospital stay, intraoperative blood loss, the rate of ulcer healing, variceal recurrence rate and postoperative complication rate within 6 months after operation were compared between two groups. Results The mean operative time and intraoperative blood loss were lower in the experimental group than those in the control group (P< 0.05). There were no significant differences in hospital stay, ulcer healing rate and recurrence rate between two groups (P> 0.05). No deep venous thrombosis was found after treatment in two groups. The occurrence rates of skin burns and subcutaneous ecchymosis were significantly lower in the experimental group than those in the control group (P<0.05). There were no significant differences in the incidence rates of other complications between two groups (P>0.05). Conclusion EVLA combined with PCCS in the treatment of severe saphenous varicose veins can significantly shorten the operation time, reduce the amount of bleeding, reduce the incidence rates of skin burns and subcutaneous ecchymosis on the premise of promising cure rate and recurrence rate. Overall, the combination therapy is superior than monotherapy.

    Clinical study of DSA combined with dual - chamber Fogarty catheter in venous crisis

    Objective To observe the clinical effect of digital subtraction angiography (DSA) combined with doublechamber Fogarty catheter in venous crisis after replantation of severed extremities. Methods A total of 12 cases with venous crisis after replantation of severed extremities treated by DSA combined with double-chamber Fogarty catheter from June 2011 to January 2016 were included in this study. Results Eleven cases survived after operation. Healing status of surgical incisions was stage Ⅰ for all patients. Of which osteofascial compartment syndrome was found in 1 case, and it was cured by surgical incision treatment; liver damage was found in another case, and acute liver damage was treated by comprehensive treatment of internal medicine. The postoperative venous thrombosis was found again in one case. The patient underwent exploratory surgery, and venous thrombosis was found again, limb partial necrosis was found, amputation was perform, and the incision was healed in Ⅰ stage. Complications including vascular rupture, air embolism, infection and sepsis were not found in all patients. Thrombosis was presented in the vein, the length of 0.6-4.2 cm. A total of 11 patients were treated with Fogarty catheters and followed up for 11 months (7 months to 29 months). The limb shape of the patient was satisfactory, blood supply of limb was improved, and capillary filling time was 1.5-3.3 s, swelling degree was improved, skin temperature was normal or lower than the limb 0.6-1.5 ℃, the skin color was normal, the activity improved, the feeling of recovery S0- S4 levels, the average in the S3 + level, two- point discrimination was 3- 8 mm with an average of 4.5 mm. According to the evaluation criteria of the replantation function of the limb replantation of the Chinese Medical Association, 5 cases were excellent, 4 cases were good and 2 cases were poor. Conclusion The application of DSA combined with double- chamber Fogarty balloon catheter for the treatment of venous crisis shows precise localization of thrombosis, increased targeting venous branch in blood vessels, minimally invasion, quick, and satisfactory clinical results.

    CT imaging analysis of superior mesenteric artery calcification in elderly patients

    Objective To retrospectively analyze the relationship between the atherosclerosis plaques in abdominal aorta and superial mesenterial artery (SMA) and the development of ischemia bowel disease (IBD) in elderly patients. Methods Elderly patients diagnosed as IBD (n=20) and non-IBD elderly patients with coronary heart disease (n=20) were selected in our hospital from January 2010 to December 2015. Data of CT imaging of abdominal aorta and SMA were evaluated by Syngo.Via software in two groups. Results The calcified plaques were dominated by dots in control group, while they were the annular lesions in IBD group, according to the CT imaging data. The mean sum of calcification in SMA was significantly increased in IBD group than that in control group (χ2=5.010,P=0.025). The stenosis of SMA was more significant in IBD group compared to that of control group (Z=3.370,P=0.001). The degree of SMA lesion was positively correlated with its opening stenosis in the IBD group (rs=0.650,P=0.002). Conclusion The basic vascular lesion is dot calcification in elderly patients with coronary heart disease, and the opening stenosis in SMA induced by mass calcification is the main cause of atherosclerosis-induced ischemic intestinal disease in elderly people.

    The clinical observation on the treatment of acute cerebral infarction by combining Solitaire stent arterial embolectomy with multi-mode vascular recanalization

    Objective To explore the efficacy and safety of Solitaire stents and the multi-mode vascular recanalization in the treatment of acute cerebral infarction. Methods Twenty- two patients with acute cerebral infarction, who were treated by Solitaire stents and the multi-mode vascular recanalization (research group) in our hospital from November 2014 to February 2017, were included in this study. Among them, 16 cases were combined with balloon dilation after arterial thrombosis, 4 cases were given stent implantation (3 cases were given Solitair stent and 1 case was given Apollo stent), and 2 cases were given arterial catheter directed thrombolysis. Eighteen patients with acute cerebral infarction who were treated only by Solitaire stent artery occlusion from October 2011 to October 2014 were used as control group. Data of the onset to the vagina vasorum time, the onset to the recanalization time, the revascularization of interventional therapy, the NIHSS scores at admission and discharge, mRS score after 90- day treatment, incidence rate and the mortality were compared between two groups. Results There were no significant differences in the durations from onset to the vagina vasorum and from the onset to the recanalization between the two groups. The recanalization was better in research group than that of control group (P<0.05). There were no significant differences in scores of NIHSS at hospital discharge and admission between two groups. The near-term treatment efficacy was similar in two groups. However, mRS score was significantly lower in the research group than that in control group after 90-day treatment (P < 0.05). There were no significant differences in the symptomatic intracranial hemorrhage, high perfusion encephalopathy, the incidence rate and the mortality rate of the complications related to the operation between two groups of patients. Conclusion Solitaire stents and the multi-mode vascular recanalization can significantly improve the revascularization, the further clinical prognosis and the quality of survival in patients with acute cerebral infarction, which are safe and efficacy without increasing incidence rate and mortality rate of complications.

    Analysis of the clinical effect of vaginal ovarian cystectomy on ovarian reserve

    Objective To compare the clinical efficacy between the vaginal ovarian cystectomy and the laparoscopic ovarian cystectomy. Methods A total of 104 patients with ovarian cyst hospitalized in our hospital during June 2013-June 2016 were selected and divided into observation group (vaginal ovarian cystectomy, n=55) and control group (laparoscopic ovarian cystectomy, n=49). There were no significant differences in mean age, body mass index (BMI), abdominal surgical history, cyst diameter and cyst type between the two groups. The operation time, blood loss in operation, hospitalization expenses and average hospital stay were observed in the two groups. The ovarian reserve function before and after the operation was also observed. Results The operation time [(46.9±18.4 min)], hospitalization cost [(8 165.3±951.8) yuan] and the average length of hospital stay (4.5±1.9 days) were significantly less in the observation group than those of the control group [(62.3±26.5) min], [(12 543.9±1 464.4) yuan] and [(6.8±2.3) days], (P<0.05). Before the operation, there were no significant differences in estradiol (E2), follicle stimulating hormone (FSH) / luteinizing (LH), FSH and antral follicle count (AFC) between the two groups. After the operation, the values of FSH/LH (1.4±0.4 vs. 1.8±0.6) and FSH [(9.2±2.9) U/L vs. (13.6±4.3) U/L] were significantly decreased in the observation group than those of the control group. The AFC (7.2±1.6 vs. 6.0±1.3) was significantly more in the observation group than that of the control group (P<0.05). Conclusion The vaginal ovarian cystectomy is of little damage to ovarian reserve function, which is suitable for women of child- bearing age and conforms to the requirement of "value medicine".

    A clinical comparative study of methotrexate intrauterine injection in the treatment for cesarean scar pregnancy

    Objective To analyse the clinical efficacy of methotrexate (MTX) combined with intrauterine embryo sac garrotte injection in the treatment of cesarean scar pregnancy (CSP), and discuss its clinical significance. Methods A total of 77 patients with CSP treated in our hospital during June 2013 to December 2016 were selected in this study. Forty patients treated with embryo sac destruction and methotrexate injection were included in the observation group, while 37 cases treated by uterine artery embolization combined with curettage were used as the control group. The time of vaginal bleeding, the time of postoperative blood level of human chorionic gonadotropin (HCG) returned to the normal level, average hospitalization cost and the curative rate were recorded in two groups. All patients were followed up by the outpatient visit. Results In the observation group, the vaginal bleeding time [(22.1±6.7) days vs. (29.5±10.8) days] and treatment cost [(8 774.2±714.5) yuan vs. (15 258.3±1 084.2) yuan] were less than those of the control group (P<0.001). There were no significant differences in the recovery time of HCG [(26.4±9.0) days vs. (25.1±10.4) days] and treatment success rate (87.5% vs. 91.9%) between the two groups (P>0.05). No bleeding or threatened rupture of scar were found in two groups of patients. Conclusion In this study, we take the embryo sac puncture combined with methotrexate injection in the treatment of scar pregnancy. This method has the advantages of low operative difficulty, definite clinical curative effect and low cost

    Analysis of risk factors of postpartum hemorrhage and application value of predictive score of postpartum hemorrhage

    Objective To explore the risk factors of postpartum hemorrhage and the application value of predicting score of postpartum hemorrhage. Methods A total of 126 cases of postpartum hemorrhage hospitalized in our hospital from January 2013 to December 2016 were included in observation group, and 674 puerperae without postpartum hemorrhage were used as the control group. Data of the gestational age, delivery time, delivery mode and fetal weight were investigated, and the predictive scores of postpartum hemorrhage were compared between the two groups. Logistic regression analysis was carried out on influencing factors of postpartum hemorrhage. The value of postpartum hemorrhage prediction score in diagnosing postpartum hemorrhage was evaluated by ROC curve. Results The proportions of patients ≥35 years old, BMI ≥ 24 kg/m2, the number of abortion ≥ 2 times, production time ≥ 2 times, cesarean section, fetal weight ≥4 000 g and pregnancy rates were 44.44%, 47.62%, 40.48%, 44.44%, 64.29%, 43.65% and 66.67% in observation group, which were significantly higher than those of control group (P<0.05). The predictive score of postpartum hemorrhage was higher in observation group than that in control group[(6.83 ± 1.25) points vs. (2.01 ± 0.98) points, t=48.356, P<0.01]. Logistic regression analysis showed that the fetal weight ≥ 4 000 g, cesarean section, patients with pregnancy complications and abortion ≥ 2 times were risk factors of postpartum hemorrhage (OR=4.195, 7.553, 5.596 and 7.192, P<0.05). Prediction of postpartum hemorrhage ROC curve AUC was 0.657 evaluated by postpartum prediction scale, and when the total score ≥4 points, the sensitivity of predicting postpartum hemorrhage was 81.25%, specificity was 65.55%. Conclusion Postpartum hemorrhage is related to fetal birth weight, cesarean section, pregnancy complications and the number of abortions. The predictive score of postpartum hemorrhage is expected to be an effective tool for predicting postpartum hemorrhage in clinical practice.

    Investigation of the value of stable micro bubble test for early diagnosis of hyaline membrane disease in premature infants

    Objective To explore the value of stable micro bubble test (SMT) for early diagnosis of hyaline membrane disease (HMD), and to provide early intervention for clinical diagnosis method of hyaline membrane disease in premature infants. Methods A total of 288 premature infants admitted to neonatal intensive care unit (NICU) of Ankang City Central Hospital from January 1st, 2013 to January 1st, 2016 were involved into the study. The primary gastric juice 1-2 mL was extracted for SMT test within 30 minutes after the birth. The number of the stable micro bubble with diameter < 15 μm was counted by low magnification. The clinical symptoms and X-ray results within 6 h after the birth were used as the gold standard of diagnosis of HMD. The premature infants were divided into the case group and control group. The general information was compared between two groups. The optimal critical stable micro bubble value was determined by the ROC curve. The number of micro bubbles <5/mm2, <10/mm2, <15/mm2 and <20/mm2 were used as the threshold, and the sensitivity, specificity, positive and negative predictive values were calculated for HMD diagnosis. Results Among the 288 cases of premature infants, 91 cases (31.6%) were diagnosed as HMD, 197 cases (68.4%) were diagnosed as non HMD. There were lower gestational age, lower birth weight and more proportion of male babies in the case group compared with those of control group (P<0.05). There were no significant differences in prenatal usage of hormones, premature rupture of membranes, the proportion of cesarean section and 5-min Apgar scores between two groups. The number of micro bubbles of 15/mm2 was the optimal threshold for diagnosis of HMD, and AUC was 0.80 (0.73-0.84). When the number of micro bubbles of 5/mm2 was used for the diagnosis, 38 cases were in line with the diagnosed as HMD. When the number of micro bubbles of 10-15/mm2 was used for the diagnosis, 16 cases were in line with diagnosed as HMD. The number of micro bubbles of <15/ mm 2 was used as boundary value of HMD. The sensitivity, specificity, positive predictive value, negative predictive value and Youden index for SMT prediction and early diagnosis of HMD were 89.0%, 91.4%, 82.7%, 94.7% and 80.4%, which can be used as predictive indicators of HMD. Conclusion SMT test is simple, rapid, economic, high sensitivity and good specificity, which can effectively predict and early diagnose HMD, and have high clinical value.

    Objective To investigate the survival status and pathogens in adult patients with severe communityacquired pneumonia (SCAP) for the past ten years in our hospital. Methods A total of 159 adult patients with SCAP were enrolled for this study from January 2007 to December 2016 in our hospital. Patients were divided into early stage group (from January 2007 to December 2011, n=71) and late stage group (from January 2012 to December 2016, n=88). The clinical data, pathogen distribution and prognosis were compared between two groups. Results (1) The proportion of patients with blood urea nitrogen (BUN) > 7.1 mmol/L was significantly higher in late stage group than that of early stage group (P<0.05). There were no significant differences in baseline data, vital signs, imaging findings and other laboratory examinations between the two groups (P>0.05). (2) For distribution of pathogens, 42 and 49 cases were detected pathogens in early stage group and late stage group. The detection rate of Legionella pneumophila was significantly higher in late stage group than that in early stage group (26.5% vs. 7.1%, P<0.05). (3) For complications, the incidence of septic shock was significantly higher in late stage group than that in the early stage group (22.7% vs. 9.9%, P<0.05). The fatality rate within 30 d after admission was significantly higher in late stage group than that of early stage group (43.2% vs. 25.4%, P < 0.05). Results of multi-factor Cox proportional hazard regression analysis showed that septic shock and respiratory acidosis were independent risk factors of mortality in early stage group (P<0.05) while septic shock, chronic obstructive pulmonary disease (COPD) and multi- lobar infiltrates were independent risk factors of mortality in late stage group (P<0.05). Conclusion In the past ten years, basic clinical characteristics of adult patients with SCAP in our hospital have changed little, but detection rate of legionella has showed increasing trend, and the short-term mortality rate has increased. Whether or not patients are combined with septic shock is a key factor affecting the prognosis.

    Objective To investigate the standard prescription in patients with stable chronic obstructive pulmonary disease (COPD) and to provide a reference for rational drug use. Methods A total of 157 patients with COPD in our hospital were included into the study from June 2016 to January 2017. Of patients, there were 3 patients with mild airflow limitation, 18 patients with moderate airflow limitation, 39 patients with severe airflow limitation and 97 patients with extremely severe airflow limitation. The self-made questionnaire was used to understand the basic situation, complications and irrational drug use. Results Most severe and extremely severe COPD patients were male, and most of them combined with high blood pressure and cardiovascular disease. Of the 157 patients, 12 cases with moderate airflow limitation used inhaled therapy of corticosteroids/bronchodilator medications, 45 cases combined with pneumocardial disease used compound methoxyphenamine, 26 cases used leukotriene receptor antagonist, 14 cases used antibiotics for no obvious infection symptom and 11 cases used systemic corticosteroids in stable COPD. All of them belong to rational drug use. Conclusion There are some unreasonable situations of drug using in patients with stable COPD. The medical staff should strictly control indications, strengthen the education for drug use, guide rational drug use and avoid the occurrence of adverse consequences.

    The evaluation of different stages of TNM in prognosis of appendiceal neuroendocrine tumors

    Objective To investigate the prognostic value of different stages of TNM in patients with appendiceal neuroendocrine tumors (NETs). Methods The clinical data of 40 cases of NETs were analyzed, and follow-up data were staged by the ENETS and AJCC TNM staging system. Kaplan- Miere survival curves were used to compare survival differences in patients with different stages of TNM. Results Of the 40 patients, no one was diagnosed as appendiceal NETs before surgery. Twenty-one patients were diagnosed as acute appendicitis, 16 were diagnosed as chronic appendicitis and 3 were suspected as appendiceal tumors due to thickening of the lumen before surgery. No clinical manifestations were found in 40 patients before surgery including diarrhea, abdominal pain, facial flushing, asthma- like seizures and other carcinoid syndrome performance. All patients underwent appendectomy, and appendix NETs were diagnosed by pathology after surgery. In 40 appendical NETs patients, 15 cases were classified as stage Ⅰ(37.5%), 10 cases were classified as stage ⅡA (25%), 2 cases were classified as stage ⅢA (5%), 13 cases were classified at stage ⅢB (32.5%), and no patients were classified as stage Ⅳ. The median survival times (months) were 27, 22, 21 and 18 for patients with different stages, and there were no significant differences in survival times between four stages (χ2=0.898 8,P>0.05). Conclusion The clinical features of appendical NETs are nonspecific. The diagnosis of appendical NETs is based on pathological examination and immunohistochemistry. There are no correlation between different TNM stages and prognosis of patients.

    Accuracy of pioneer drilling guiding and full navigation guiding plates in the flapless immediate implantation

    Objective To compare the operative precision between pioneer drilling guiding and full navigation guiding plates in the flapless immediate implantation. Methods Data of 50 immediate implants, which were implanted from 47 outpatients (50 implants) of Tianjin Stomatological Hospital from 2014 to 2016 were retrospectively analyzed. The patients were conducted on the pioneer drilling guiding system (22 implants) and full navigation guiding sytem (28 implants). The cone beam CT (CBCT) was preliminarily taken and modeled for each patient. All of the patients underwent implant surgery after making guiding plates by three-dimensional reconstruction, simulation planting design and the application of digital rapid prototyping technology. Cavity preparations were made by pioneer drilling and full navigation assisted with planting drills respectively to finish implantation. The post- operative CBCT was taken and performed the three- dimensional reconstruction. The implantation precision guiding by two systems were compared. Results All of the 50 immediate implants were successfully completed with the guiding plates. Implant mucositis appeared in one case after 3 months of temporary repair, and after the targeted therapy, the final repair was finished. The rest cases were finally completed. No implant was loose or fell off. The deviation was less in neck apex and root apex angle in full navigation group than that of pioneer drilling guiding group (P < 0.01). But there was no significant difference in axial angle between the two groups after navigation and implantation (P > 0.05). Conclusion The digital surgical guiding plates provide nice technical support for the application of the flapless immediate implantation. And the full navigation system has higher precision than that of the pioneer drilling navigation system.

    Objective To analyse the retrobulbar haemodynamic changes after vitrectomy in patients with rhegmatogenous retinal detachment (RRD). Methods Color Doppler flow imaging (CDFI) was used for measurement of blood flow velocities including peak systolic velocity (PSV), end diastolic velocity (EVD) and resistive indexes (RI) of the ophthalmic artery (OA), posterior ciliary arteries (sPCA) and central retinal artery (CRA) in 50 eyes of 50 patients with RRD. In them 22 eyes were filled with silicone oil, 28 eyes were filled with 12%C3F8 and 22 eyes were operated to remove silicone oil after filled with silicone oil for 2-4 months, and then CDFI parameters were obtained. The contralateral eyes were used as control eyes before and after the operation. Results There were no significant differences in CRA and sPCA, and PSV, EDV and RI before treatment between RD and OA eyes and control eyes (P>0.05). PSV and EVD of CRA were significantly increased 3 months after surgery, RI were decreased significantly (P<0.05). There were no significant differences in blood flow parameters of OA and sPCA before and after surgeries (P>0.05). No changes were found in control eyes 3 months after surgery. Conclusion VRS might increase the velocity of CRA, decrease RI and improve ocular blood supply postoperatively.

    To report a case with primary lung mucosa-associated lymphoid tissue type marginal zone B-cell lymphoma (PMZL-MALT). The patient was admitted in our hospital mainly due to intermittent cough and expectoration for 4 years, and aggravating for 1 year. The anti-infective treatment was given to this patient, and the efficacy was not good. Laboratory tests showed no specific indicators. Chest enhancement CT examination showed pulmonary consolidation lesions. After taking biopsy by tracheoscopy and pathological examination, the diagnosis of mucosa- associated lymphoid tissue type marginal zone B- cell lymphoma was confirmed. After receiving chemotherapy, the patient remains under treatment in a stable condition and regular reexamination.

    Analysis of the death epidemiology of malignant tumor from 2010 to 2015 in Xiqing District of Tianjin

    Objective To analyze the epidemiological characteristics and the change trends of malignant tumor death in Xiqing District of Tianjin in 2010- 2015, and to provide reference for prevention and treatment of malignant tumor. Methods The death monitoring data in 2010- 2015 of Xiqing District were collected, and which were classified by international classification of diseases ICD-10 encoding. The mortality rate, standardized mortality rate of malignant tumor and tumor death in different gender and age groups were analyzed. Results The average annual crude mortality rate of malignant tumors was 142.90/100 000 in Xiqing District of Tianjin from 2010 to 2015, and the standardized mortality rate was 98.95/100 000. The average crude mortality rate of male was 165.25/100 000, the standardized mortality rate was 110.21/ 100 000. The average crude mortality rate of female was 121.23/100 000, and the standardized mortality rate was 87.54/100 000. Among the leading causes of death in malignant tumors, the top three leading cancer types were lung cancer (62.42/100 000), liver cancer (18.82/100 000) and gastric cancer (13.22/100 000). The mortality rates of lung cancer, liver cancer, gastric cancer, colorectal cancer, leukemia, esophageal cancer, bladder cancer and nasopharyngeal carcinoma were significantly higher in males than those in females (P < 0.05). There were death cases in different age groups, and the high mortality rate was concentrated in the age group of 0-1 and age group of more than 85. Conclusion The lung cancer, liver cancer, gastric cancer and other malignant tumors pose a serious threat to the health of Xiqing district residents. It is important to improve the monitoring system and promote the prevention and control of risk factors, continue to strengthen tobacco control and air pollution control, advocate a reasonable diet, moderate exercise and healthy lifestyle, take the comprehensive intervention measures, and reduce the incidence of malignant tumors.

    Systematic review of mindfulness intervention in improving physical and mental symptoms in diabetic patients

    Objective To evaluate the effect of mindfulness intervention (MBI) on physical and mental symptoms in patients with diabetes mellitus. Methods Data of the relevant MBI in the physical and psychological symptoms were reviewed in patients with diabetes. The quasi- experimental studies or randomized controlled trials were used to retrieval from May 2016, and back into the study of references. According to the inclusion and exclusion criteria, the literature was screened and the quality was evaluated to extract the data. Review Manager 5.3 software was used for Meta- analysis. Results The total of 9 studies and 474 patients were included in this study. Patients were divided into MBI group (n=251) and control group (n=223). Meta- analysis showed that mindfulness therapy was effective in relieving depression, anxiety, stress and diabetes distress symptoms in diabetic patients compared with those of controls [MD=-5.35, 95%CI:-7.11- -3.59; MD=-1.91, 95%CI:-3.04- -0.79; MD=-4.65, 95%CI:-6.68- -2.44; MD=-6.37, 95%CI:-10.84- -1.90], the differences were statistical significance (P < 0.05). The improvement of the negative emotions was sustained long- term effect, and patients can benefit from it. The effects of MBI on the quality of life, whether short-term or long-term, were inconsistent and required further verification. The effects of glycemic control were generally better in MBI group than those of the control group. Conclusion Mindfulness intervention shows beneficial non-drug adjuvant therapy in reducing depression, anxiety, stress and diabetes related distress in patients with type 2 diabetes. The effects of MBI on improving the quality of life, blood glucose control need further verification.

    The peritoneal dialysis efficacy of icodextrin-based solution: a Meta-analysis

    Objective To compare the efficacy of icodextrin-based solution (ico) and glucose-based solution (GLU) in peritoneal dialysis patients. Methods Pubmed (1996-2016.12), MEDLINE (1996-2016.12), Embase (1974-2016.12) and Cochrane library were searched by two independent investigators who conducted quality assessment and data mining and performed Meta-analysis using RevMan 5.2. Results Ten randomized controlled trials with 825 participants were included in this study, and 661 patients completed the trials at last. The Meta- analysis showed that there were no significant differences in body weight (WMD=-1.88, 95%CI:-4.68-0.93, P=0.19), fasting plasma glucose (WMD=-0.76, 95%CI:-1.79- 0.28, P=0.15), plasma triglycerides (WMD=-0.56, 95%CI:-1.18-0.06, P=0.08), plasma total cholesterol (WMD=-0.17, 95% CI:-0.63-0.29, P=0.47) and adverse events (RR=1.06, 95%CI:0.86-1.29, P=0.59) between ICO group and GLU group. The peritoneal creatinine clearance (WMD=0.48, 95%CI:0.27-0.68,P<0.001) and peritoneal urea clearance (WMD=0.44, 95% CI:0.23- 0.66, P<0.001) were better in ICO group than those of GLU group. Conclusion ICO can provide a better peritoneal creatinine clearance and peritoneal urea clearance, and which has the same safety compared with GLU.

    Research progress in correlation of miR-205 and breast cancer

    As evolutionarily conservative small non- coding regulatory RNAs, microRNAs (miRNAs) are capable of silencing gene expression by translational repression or mRNA degradation through complementary base pairing with the mRNA of target genes. Accumulating evidence indicates that deregulation of miRNAs is often associated with human malignancies because miRNAs can function as oncogenes or tumor suppressors. Among them, miR- 205 is significantly underexpressed in breast tumors. Overexpression of miR-205 in breast cancer cells significantly inhibits cell proliferation, invasion and metastasis through repressing the expression of human epidermal growth factor receptor 3 (ERBB3), zinc finger E- box binding homeobox 1 (ZEB1), zinc finger E- box binding homeobox 2 (ZEB2), vascular endothelial growth factor A (VEGFA) and other target genes, which affects therapeutic sensitivity and prognosis of breast cancer patients. This article reviews the role and regulation of miR- 205 in breast cancer, the value of miR- 205 in clinical application and miR- 205 related research progress, which is expected to provide new strategy and therapeutic target for breast cancer diagnosis, treatment and prognosis.

    Recent advances in the application of growth factors in spinal cord injury

    The incidence of spinal cord injury is increasing year by year, and more and more attentions have been paid. Growth factors can promote the regeneration of nerve fibers and synapses, and they also play an important role in the treatment of spinal cord injury and the recovery of neurological function. The growth factor itself has a short half-life, so it is necessary to use growth factor gene vectors to transfect stem cells and nanoparticles to deliver growth factors or biocompatible scaffolds to support growth factors to treat spinal cord injuries. With the development of the research on growth factors, the application of single growth factor is difficult to meet the need of treatment to spinal cord injury. To explore the synergistic effect of various growth factors in order to achieve a better therapeutic effect is a promising research direction in the future. The purpose of this review is to summarize the therapeutic effects of growth factors on spinal cord injury including brain-derived neurotrophic factor, neurotrophic factor 3, nerve growth factor, basic fibroblast growth factor and synergy therapy of growth factors.
