    The protective effect of IGF-1 on 6-OHDA induced oxidative damage in neurons

    Objective To investigate the protective effect of insulin-like growth factor-1 (IGF-1) on 6-hydroxy dopamine (6-OHDA) induced oxidative damage in PC-12 cells. Methods PC12 cells were treated with 6-OHDA (concentrations of 25, 50, 100, 150 and 200 μmol/L). In order to select the optimal experimental concentration and treatment time, the activity of PC12 cells was detected by MTT at different time points of 12 h, 24 h and 48 h after treatment. PC12 cells were divided into three groups: control group, 6-OHDA group and IGF-1+6-OHDA group. The activity of PC12 cells was detected by MTT assay. Reactive oxygen species (ROS) level of PC12 cells was detected by immunofluorescence staining, and apoptosis was detected by Hoechst33342 / PI double staining method. Results With the increased concentration and the prolongation of the action time of 6-OHDA, the activity of PC12 cells decreased gradually. The concentration of 150 μmol/L and action time of 24 h of 6-OHDA were selected as the optimal experimental concentration and observation time for this study. Compared with 6-OHDA group, the activity of PC12 cells increased, the expression level of ROS and the apoptosis decreased in IGF-1+ 6-OHDA group. Conclusion IGF-1 pretreatment can reduce 6-OHDA induced oxidative damage and apoptosis of PC12 cells, also can increase cell activity, which can provide a potential strategy for the prevention and treatment of Parkinson’s disease.

    Objective To investigate the inhibitory effect and molecular mechanism of delphinidin (Dp) on HER-2 positive breast cancer. Methods The HER-2 positive breast cancer cells (MDA-MB-453 and BT-474) were treated with different concentrations of delphinidin (10,20,40,80 and 160 μmol/L) for 48 hours. The same concentration of DMSO was used as the solvent control. CCK-8 method was used to measure the effect of Dp on cell activity, and half maximal inhibitory concentration (IC50) was calculated to determine the concentration of subsequent experiments. The cells were treated with different concentrations of Dp (20, 40 and 80 μmol/L) for 48 hours. HE staining was used to observe the cell morphological changes. Flow cytometry was used to analyze the cell cycle. Cell apoptosis rate was detected by TUNEL assay. The phosphorylation levels of NF - κB signaling pathway related proteins were determined by Western blot assay. Results Delphinidin inhibited the proliferation of MDA-MB-453 and BT-474 in the concentration ranges of 10,20,40,80 and 160 μmol/L (IC50: 41.02 and 60.97 μmol/L). In the concentrations of 20,40 and 80 μmol/L, compared with the control group, it was found that some cells were detached, floated and lysed, and the cell volume was decreased, the proportion of cells in G2/M phase and the apoptosis rate were increased in DP treatment groups. Compared with the control group, the expressionlevels of p-NF - κB / p65, p-IκBα, p-IKKα/β and p-PKCα were significantly decreased in the 40 and 80 μmol / L Dp treatment groups, while the expression levels of IκBα, IKKα, IKKβ and PKCα were increased in the Dp treatment groups (P<0.05). Conclusion Delphinine can inhibit the proliferation of breast cancer cells by blocking the NF- κB signaling pathway and inducing the G2/M cycle arrest and apoptosis of MDA-MB-453 and BT-474 cells

    Time-series analysis of expression profile of adrenocortical carcinoma in children for screening related targets

    Objective To explore the potential pathogenic mechanism of adrenocortical carcinoma (ACT), and screen out genes that may be related to biological targets. Methods In this study, the gene expression datasets of ACT were obtained from the Gene Expression Omnibus (GEO) with the accession number of GSE75415. Through R programming software, the microarray preprocessing and differential expression analysis of 18 ACT tissue samples (experimental group)and 7 normal adrenocortical tissue samples (control group) were conducted to identify potential biomarkers for ACT in different stages. Besides, through functional enrichment and Kaplan-Meier analysis, several more reliable biomarkers for ACT were identified. At the same time, the two generation sequencing data of the TCGA database, including 79 ACT samples were analyzed, and the genes that can affect the survival of ACT patients were screened. Results There were 248, 334, 315 and 561 differentially expressed genes in stage1-4 respectively. There were 73 overlapping genes (OLDEGs) among the different grading samples. Central genes HSPA13, GARS, STXBP1, AKIRIN1 and TUBB3, were up-regulated in all of stages of ACT samples compared with those of normal samples, while, central genes ADH1B, DCN, RASSF2, PDGFRA, PLAT, C3 and FOS were down-regulated in ACT samples. They were found to be significantly associated with pathways of immune response, cell cycle, phosphorylation and cruor, which were all closely related with ACT progression. Besides, Kaplan-Meier analysis of 73 OLDEGs in 79 ACT samples from TCGA database identified several genes, including XPO1, RACGAP1,PDGFD, NR4A2, MXRA5, VPS51, TMED3, NDFIP1 and CDKN1C, which were significantly associated with ACT overall survival. Conclusion Differentially expressed genes and survival related genes in all of stages can serve as new targets for ACT therapy, and which should be helpful for the understanding of its pathogenesis and prognosis.

    The effect of DUSP2 on proliferation and apoptosis of gastric cancer cells and its mechanism

    Objective To investigate the effect of dual-specificity phosphatase-2 (DUSP2) on the cell proliferation and apoptosis in the gastric cancer and its mechanisms. Methods Firstly, the effects of different expressions of DUSP2 on the overall survival of 876 gastric cancer patients were analyzed by online analysis tool KM plotter, and the expressions of DUSP2 in various gastric cancer cell lines (MKN-45, SGC-7901, HGC-27 and N-87) were verified. Secondly, DUSP2 overexpressed lentiviral vector was constructed, and MKN-45 was transfected by packaged virus. DUSP2-overexpression gastric cancer cell line was gained by drug screening. Meanwhile, gastric cancer cells infected with empty vector virus were used as control. Then the effect of DUSP2 upregulation on the proliferation ability of gastric cancer cells was evaluated by MTS cell proliferation assay, and the apoptosis was determined by Annexin V-FITC / PI double staining. The protein expressions of DUSP2, ERK, p-ERK (Thr202/Tyr204), P38 and p-P38 were tested by the Western blot analysis. Results Gastric cancer patients with high DUSP2 expression showed a significant survival advantage compared with those with low DUSP2 expression, and DUSP2 levels were decreased in several gastric cancer cell lines. The Western blot analysis revealed that the expression of DUSP2 markedly increased in overexpressed DUSP2 group (experimental group) compared with that of control group. The MTS experiment showed that the cell viability was significantly decreased in experimental group than that of the control group. Correspondingly, the cell apoptosis test showed that the cell apoptosis rate was obviously higher in the experimental group than that of the control group. The results of Western blot assay indicated that p-ERK (Thr202/Tyr204) and p-38 were significantly down-regulated in the experimental group compared with those of control group. Conclusion The over-expression of DUSP2 can efficiently inhibit cell proliferation and promote its apoptosis in gastric cancer cells, and the mechanism is related to DUSP2 inhibiting the phosphorylation levels of ERK and P38.

    Objective To investigate the dynamics of cortical granules exocytosis (CGE), and the role of Synaptotagmin1 (Syt1) in mouse fertilization. Methods The dynamics of mouse CGE were filmed on Perkin Elmer precisely Ulta VIEW VOX confocal Imaging System. The Syt1 functions on mouse fertilization and expression in zygotes were analyzed by immunofluorescence, Western blot assy and qRT-PCR. Results The dynamic process of mouse CGE was observed after oocyte activation, and the exact pot seems to be close to the polar body. The Syt1 expression was gradually increased after fertilization. The mouse fertilization rate after Syt1 knockdown was significantly lower than that of the control group (P<0.05). Conclusion The dynamic process of CGE is studied. It is found that Syt1 is involved in mouse fertilization process.

    Objective To observe the effect of Xuebijing on immunosuppressed sepsis of model mice. Methods The 152 mice were randomly divided into control group (Control), immunosuppression group (IM), immunosuppressed sepsis model group (ISM) and Xuebijing treatment group (XT). There were thirty – eight mice for each group. For group IM,cyclosporine A was injected intraperitoneally along the median line of the lower abdomen for immunosuppression, 25 mg/kg,1 time every other day for a total of 3 times. For group ISM, after immunosuppression, 300 μL escherichia coli 44102 with a concentration of 1×109 CFU/mL was injected intraperitoneally along the midline of the lower abdomen. The 30 minutes after the establishment of the immunosuppressive sepsis model, Xuebijing 4 mL/kg was intraperitoneally injected along the midline of the lower abdomen in the ISM group. After 30 minutes, the same dose of Xuebijing injection was repeated once. Control group was injected intraperitoneally with the same amount of normal saline. (1) After 8 h, 4 mice in each group were taken for blood bacterial culture. (2) After 12 h, 10 mice in each group were taken for detecting CD3+CD4+ and CD3+CD8+ in peripheral blood using flow cytometry. (3) After 12 h, 10 mice in each group were taken for detecting blood levels of tumor necrosis factor-α (TNF-α) and interleukin-6 (IL-6) by enzyme-linked immunosorbent assay (ELISA). (4) After 12 h, 4 mice in each group were taken for detecting high-mobility group protein (HMGB1) by Western blot assay. (5) After 12 h, 10 mice in each group were taken for detecting alanine aminotransferase (ALT), aspartate aminotransferase (AST), urea nitrogen(BUN) and serum creatinine (CR) levels by automatic biochemical analyzer. Results Compared with the ISM group, the ratio of CD3+CD4+/CD3+CD8+ was significantly increased, the number of blood bacteria culture decreased obviously in the XT group. Liver and kidney function indicators ALT, AST, CR, BUN and inflammatory factor indicators TNF-α, IL-6 and HMGB1 were also significantly decreased in the XT group. Conclusion Xuebijing can obviously modulate the immunosuppression, against bacteria, inhibit the inflammatory reaction, and protect the vital organs.

    The significance of high-mobility group alarmins expression in the prognosis of NSCLC patients

    Objective To explore the relationship between expressions of high mobility group B1 and N1(HMGB1 and HMGN1) and clinical pathological parameters and prognosis in patients with non-small cell lung cancer (NSCLC). Methods Ninety-one postoperative tumor tissue specimens from the patients with NSCLC were collected in Tianjin Medical University Cancer Institute and Hospital from January 2004 to May 2011. Immune histochemical assay was used to detect the expressions of HMGB1 and HMGN1. According to the staining intensity, expressions of HMGB1 and HMGN1 were divided into positive and negative groups. Kaplan-Meier was used to analyze the correlation between the expressions of HMGB1/HMGN1 and clinical pathological parameters/prognosis. Results The cytoplasmic expressions of HMGB1 (40%,36/91) was positively correlated with cytoplasmic expression of HMGN1 (31%, 28/91, rs=0.319,P<0.001). The expression level of HMGN1 was significantly higher in patients with late stage of NSCLC (Ⅲ~Ⅳ) than that in patients with early stage of NSCLC (Ⅰ~Ⅱ, P<0.05). It was also found that the expression level of HMGN1 was significantly higher in patients with lymph node metastasis than that in patients without lymph node metastasis (P<0.05). The poor prognosis of NSCLC was slightly lower in patients with high expression of HMGN1 than that in patients with low expression of HMGN1, but the difference was not statistically significant. Conclusion The HMGN1 can be used as a promising prognostic biomarker for predicting the prognosis of NSCLC patients.

    BHMT and RFC1 polymorphisms and susceptibility to neural tube defects in children of Han population in northern China

    Objective To investigate the relationship between single nucleotide polymorphisms (SNPs) of betaine homocysteine methyltransferase (BHMT) and reduced folate carrier 1 (RFC1, SLC19A1) genes and susceptibility to neural tube defects (NTDs) in children of Han population in northern China. Methods A total of 152 NTDs patients, and 169 controls were used in this study. Sequenom Mass-ARRAY was used to genotype 9 SNPs in 2 genes, and Haploview 4.2 software was used for Haplotype analysis. Results The allele frequency of rs3733890 in BHMT was significantly correlated with the incidence of NTDs, and the children with A allele were at higher risk of NTDs compared with those with G allele (OR=1.408,95%CI: 1.013-1.956). The distribution of allele and genotype frequencies of rs1051266 in RFC1 was significantly associated with the incidence of NTDs, and the children with G allele and GG genotype were associated with higher risk of NTDs compared with those with A allele and AA genotype (OR=1.492, 95%CI: 1.089-2.044; OR=2.020, 95%CI: 1.081-3.780).The allele frequency of rs3788200 in RFC1 was significantly correlated with the incidence of NTDs, and the children with G allele were at higher risk of NTDs than those with A allele (OR=1.368, 95%CI: 1.002-1.868). Meanwhile, Haplotype analysis showed that C-A-A-A haplotype (rs567754-rs3733890-rs558133-rs585800) of BHMT and G-G-G-T haplotype(rs1051296-rs3788200-rs1051266-rs4819130) of RFC1 were associated with the increased risk of NTDs, but C-G-A-A haplotype of BHMT gene was associated with the decreased risk of NTDs. Conclusion The rs3733890 of BHMT gene,rs1051266 and rs3788200 of RFC1 gene are associated with susceptibility to NTDs in children of Han population in northern China.

    The comparison of clinical outcomes of the first and the second-generation drug-eluting stents implanted in vein grafts after coronary artery bypass graft

    Objective To compare the clinical outcomes of the first and the second-generation drug-eluting stents (DES) implanted in saphenous vein grafts (SVG) in patients after coronary artery bypass graft (CABG). Methods A total of 108 patients with coronary angiography and DES implanting in SVG due to ischemia symptoms after CABG were collected in this study, including 69 patients with the first-generation of DES (drug-eluting: sirolimus) and 39 patients with the secondgeneration of DES (drug-eluting: zotarolimus or everolimus). The success rate of stents and mortality in hospital were compared between two groups of patients. The major adverse cardiac events (MACE), such as all-cause death, target vessel revascularization (TVR) and acute myocardial infarction (AMI) in 2-year follow-up were also compared between the two groups of patients. The survival curve was drawn by Kaplan-Meier method, and the MACE free survival rates of two groups of patients were compared. Cox regression analysis was used to evaluate the risk factors for MACE in patients with SVG stent implantation. Results There were no significant differences in the success rate of stents and mortality in hospital between the two groups. In average 2-year follow-up, a total of 37 cases of MACE were performed. There was no statistical difference in the incidence of MACE between the two groups (34.8% vs. 33.3%, P>0.05). The proportion of TVR was significantly lower in the second-generation group than that of the first-generation group (13.0% vs. 2.6%, P<0.05). Kaplan-Meier survival analysis showed that there were no statistically differences in the survival rates of no-cumulative events between the two groups (81.2% vs. 79.5%, Log-rank χ2=0.029, P>0.05). COX regression analysis showed that diabetes (HR=2.530, 95%CI: 1.008-6.345, P=0.041) and stent diameter (HR=1.143, 95% CI: 1.043-1.253, P=0.004) were independent predictors for the MACE in patients implanted stents in SVG. Conclusion There are no significant differences in mortality in hospital and the MACE in 2-year follow-up between the patients of two generations of DES implanting in the SVG after coronary artery bypass grafting. The proportion of TVR is lower in the second-generation DES group. Patients with diabetes and large diameter stents have a poor prognosis.

    The pathogen distribution and its influence on inflammatory factors in old patients with heart failure complicated with pulmonary infection

    Objective To explore the pathogen distribution, changes of cardiorespiratory function and changes of serum inflammatory factors in old patients with cardiac failure complicated with pulmonary infection. Methods A total of 153 cardiac failure patients hospitalized in our hospital from March 2014 to September 2017 were retrospectively analyzed.The patients were divided into pulmonary infected group (n=76) and non-infected group (n=77). Another 82 healthy subjects were served as control group. The respiratory secretions were collected to detect pathogen distribution in infected group. The changes of cardiac and pulmonary functions and peripheral blood inflammatory factors were compared between the three groups. Results A total of 110 pathogens were isolated from infected group, which contained 81 (73.63%) gram negative strains, 27 (24.55%) gram positive strains and 2 (1.82%) fungus. Compared with the control group and non-infected group, the lung function index and pulmonary function decreased significantly in the infected group, containing forced expiratory volume in one second (FEV1), forced vital capacity (FVC), FEV1/FVC, carbon monoxide diffusing capacity (DLCO), maximum mid expiratory flow (MMEF), left ventricular end-diastolic dimension (LVEDD) and left ventricular end-systolic dimension (LVESD). Meanwhile, the serum levels of tumor necrosis factor (TNF) –α, interleukin (IL)-6 and procalcitonin (PCT) weresignificantly higher in the infected group than those in the non-infected group and the control group (P<0.05).Conclusion Gram negative strains dominate in pulmonary infection of old patients with cardiac failure, and the cardiopulmonary function is significantly decreased due to the infection, whereas the serum levels of inflammatory factors are dramatically increased, which has an auxiliary value in the evaluation of pulmonary infection.

    Study on the efficacy evaluation of real-time fluorescence assay of RNA isothermal amplification in the treatment of patients with positive pulmonary tuberculosis

    Objective To investigate the value of real-time fluorescence detection technique of RNA (SAT constantamplification in monitoring the effect of chemotherapy on patients with sputum positive pulmonary tuberculosis. Methods Sixty-two patients were selected, who were clinically diagnosed as the first-time retreatment for sputum smear-positive pulmonary tuberculosis and were hospitalized in our department from June 2015 to December 2016. After two-month standard anti-tuberculosis treatment, sputum samples were detected by Peng’s vessel acid-fast staining, BACTEC MGIT-960 culture and strain identification, SAT detection. The BACTEC MGIT-960 culture and strain identification were used as gold standards, the value of SAT during the monitoring the therapeutic effect of the anti-tuberculosis drugs was assessed.Results After the treatment, 49 cases out of 62 showed positive results in mycobacterium tuberculosis culture test, among them 42 patients were diagnosed as human type mycobacterium tuberculosis, 7 patients were diagnosed as nontuberculous mycobacteria infection, and 13 cases showed negative results in mycobacterium tuberculosis culture. Thirty-two cases showed positive results in sputum Peng’s vessel acid-fast staining test, and 30 cases showed negative results. Forty-one cases showed positive results in SAT test and 21 cases showed negative results in SAT. SAT results were well concordant with sputum culture results (Kappa value=0.964), and the sensitivity, the specificity, the positive predictive value and the negative predictive value of SAT were 97.62%, 100%, 100% and 95.24% respectively. Peng’s vessel acid-fast staining results were badly concordant with MGIT-960 culture results (Kappa value=0.086), and the sensitivity, the specificity, the positive predictive value and the negative predictive value of Peng’s vessel acid-fast staining were 54.76%, 55.00%,71.88% and 36.67% respectively. Conclusion SAT results can be used as good indices during the monitoring therapeutic effects of drugs used for the first-time retreatment in patients with sputum smear-positive pulmonary tuberculosis, which is worth promoting.

    The relationship between subclinical hypothyroidism and visceral adipose tissue

    Objective To investigate the relationship between subclinical hypothyroidism (SCH) and visceral adipose tissue. Methods A total of 68 SCH subjects from medical examination center and inpatient department of our hospital and 100 healthy controls matched for age and sex were enrolled in this study. The patient gender, blood pressure (SBP/DBP),anthropometric measurements, lipid profile, lipid accumulation product (LAP) and visceral adipose area (VA) were compared between two groups. The Sperman’s correlation was used to analyze the correlation between thyroid stimulating hormone(TSH) and variables, and the Binary logistic regression analysis was used to analyze the impact of variables on SCH.Results The waist circumference (WC), waist to hip ratio (WHR), non high density lipoprotein cholesterol (nonHDL-C),three acylglycerol (TG), TSH, LAP and VA were higher in SCH subjects than that in the healthy controls (P<0.05), while HDL-C was lower in SCH subjects (P<0.05). There were no significant differences in sex, age, body mass index (BMI), SBP,DBP, total cholesterol (TC) and low density lipoprotein cholesterol (LDL-C) between two groups (P<0.05). Correlation analysis showed that TSH was positively correlated with WC, TG, nonHDL-C, LAP and VA, and negatively correlated with HDL-C (P<0.05). There was no significant correlation between TSH and age, BMI, WHR, TC and LDL-C (P> 0.05). Binary logistic regression analysis showed that female and low level of HDL-C were risk factors of SCH, while the effects of VA and LAP on SCH were not statistically significant. Conclusion Our findings show that the dyslipidemia and VA increase are more serious in subjects with SCH than those with normal thyroid function and healthy controls, which suggest that SCH is closely related to abdominal obesity.

    Objective To investigate the association between neutrophil to lymphocyte ratio (NLR) and the severity of coronary artery disease in patients with acute coronary syndrome (ACS), and analyze the value of NLR in the diagnosis of acute myocardial infarction. Methods The clinical data of 259 ACS patients with coronary angiography at our hospital from November 2016 to March 2017 were retrospectively analyzed. Among them, there were 148 cases of unstable angina (UA group), 46 cases of acute non-ST elevation myocardial infarction (NSTEMI group), and 65 cases of acute ST-elevation myocardial infarction (STEMI group). The differences of white blood cell count (WBC), neutrophil counts (N), lymphocyte count (L), NLR, hypersensitive c-reactive protein (hs-CRP), myocardial enzyme spectrum, blood lipids, blood glucose, and liver function were compared between the three groups. According to the results of coronary angiography, patients were divided into single-vessel disease group (n=70), double-vessel disease group (n=70) and three-vessel disease group (n=119) in the basis of the number of coronary lesions, and the differences in inflammatory cells were compared between the different-number vessel lesion groups. At the same time, the receiver operating characteristic (ROC) curves of NLR were drawn to evaluate the predictive values of NLR for acute myocardial infarction. Results The levels of WBC, N, NLR, hsCRP, cTnT, CK and CK-MB were significantly higher in turn in UA group, NSTEMI group and STEMI group (P<0.05).NLR increased gradually with the increased numbers of stenosed coronary artery. The ROC curve showed that the area under the curve of NLR was 0.865 (95%CI: 0.814-0.916). In addition, the optimal cutoff value of NLR in predicting acute myocardial infarction was 4.22 with sensitivity of 72.1% and specificity of 95.3%. Conclusion NLR levels are positively correlated with the severity of ACS, and which have high value for the diagnosis of acute myocardial infarction, and can be used as a predictor of the condition of ACS patients.

    Objective To explore the clinical efficacy of transjugular intrahepatic portosystemic shunt (TIPS) in the treatment of upper gastrointestinal bleeding in patients with hepatocirrhosis. Methods Totally 75 hepatocirrhosis patients with upper gastrointestinal bleeding accepted TIPS treatment in our institution from March 2012-March 2017 were collected retrospectively in this study. Patients were divided into two groups, the control group (n=35) underwent conservative treatment, and the TIPS group (n=75) received TIPS treatment on the basis of conservative therapy. Indexes of portal venous pressure, portal vein diameter, portal velocity, platelet count (PLT), erythrocyte count (RBC), albumin (ALB), total bilirubin (TBIL), glutamic-pyruvic transaminase (ALT), and glutamic-oxalacetic transaminase (AST) before and after operation were compared between two groups. Data of survival rate, rehaemorrhage rate and living quality after the operation were followed up, and which were compared between the two groups. Results Totally 75 patients received TIPS and were all successful,with the technical success rate 100%. There were no patients died during the perioperative period. No serious complications such as cardiac tamponade, hematocelia and biliary fistula were found. Compared with pre-operation, portal venous pressure and portal vein diameter were significantly decreased, and portal velocity was increased after operation (P<0.05). Totally 11 patients were dead during the follow-up period, with fatality rate 14.7%. Original unobstructed rate of TIPS runner at 3, 6,12, 24 and 36 months were 95.6%, 90.1%, 85.7%, 79.0%, and 74.2% respectively. Compared with pre-operation, indexes of PLT, RBC and ALB at 6 months after surgery were improved (P<0.05). Whereas there were no significant differences in TBIL, ALT and AST before and after operation (P>0.05). There were no significant differences in accumulate survival rates at 3, 6, 12, 24 and 36 months between control group (94.3%, 91.3%, 75.8%, 71.0%, and 59.2% respectively) and TIPS group(98.7%, 97.3%, 95.7%, 83.4%, and 70.6% respectively, Log-rank χ2=2.743, P=0.098). Accumulate rehaemorrhage rates at 3, 6, 12, 24, and 36 months were 5.7%, 17.1%, 23.5%, 31.9%, and 43.2% respectively in the control group, and they were 2.7%, 4.0%, 8.3%, 14.4%, and 24.0% respectively in the TIPS group. There were significant differences in accumulate rehaemorrhage rates between the two groups (Log-rank χ2=4.608, P=0.032). Compared with control group, indicators of living quality were improved at 6 months after surgery in the TIPS group (P<0.05). Conclusion TIPS is safe and effective for cirrhosis patients with upper gastrointestinal bleeding. Compared with conservative treatment, TIPS can decrease rehaemorrhage rate and improve living quality of patients.

    Objective To discuss the etiology, clinical features, imaging manifestations, treatment principles and prognosis of the orbital infectious diseases. Methods Thirty-four patients with orbital infectious diseases hospitalized in Tianjin Medical University Eye Hospital from August 2011 to October 2017 were collected and retrospectively analyzed. We analyzed clinical data of patients including the etiologies, types of the infection, imaging manifestations, treatments and prognoses. Relsults There were 21 patients with orbital abscesses, 13 patients with orbital cellulitis only, no abscess formation. The main cause of the orbital infectious patients disease was related to paranasal sinusitis, others included furuncles of eyelid skin, dacryocystitis, trauma, entophthalmia, odontogenic infection, nosocomial infection and systemic diseases. Twenty-two cases were operated, and 12 cases were treated conservatively. Thirty (30 / 34) cases acquired well prognoses without serious complications, except 1 occurred central retinal artery and vein occlusion, and 3 were treated by ophthalmectomy and lost their visual function permanently. The CT imaging of orbital infectious diseases showed the swelling, increased density, diffuse inflammation and unclear boundaries of the involved tissues. The imaging of orbital abscesses showed that in addition to inflammation, the abscesses and the masses were obvious in the orbit. MR generally showed long T1 and long T2 signals. Conclusion There are many causes of orbital infectious diseases, and the course progresses rapidly. With the help of imaging examination, it can accurately locates the infection, and shows the type and range, and comprehensively evaluates the disease in combination with clinical features. Anti-infection and etiological therapy are crucial factors, if necessary, surgery and multiple department treatment should be combined to avoid serious complications.

    Objective To compare the therapeutic effect of epithelial flap removed laser subepithelial keratomileusis (LASEK) and femtosecond laser assisted LASIK (FS-LASIK) in the treatment of moderate and low myopia. Methods A total of 103 patients (103 eyes) of low and moderate myopia were selected, 51 patients (51 eyes) were treated with LASEK and 52 patients (52 eyes) were treated with FS-LASIK. Data of uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), residual equivalent spherical degree (SE), total high-order aberrations (HOAs), spherical aberration, coma and trefoil were examined before operation and 1month and 3 months after operation, and compared between two groups of patients. Results There were no significant differences in UCVA and BCVA 1 month and 3 months postoperatively between the two groups. SE shifted to myopia direction in LASEK group compared with that of FS-LASIK group 1 month postoperatively (P<0.05), but reached a similar level 3 months after surgery. The HOAs and spherical aberration were significantly increased 1 month and 3 months postoperatively compared with those before operation in LASEK and FSLASIK groups (P<0.05). Coma was significantly increased 1 month postoperatively in FS- LASIK group and returned to the preoperative level 3 months postoperatively, while it remained at baseline 1 month and 3 months postoperatively in the LASEK group. The HOAs and coma were significantly higher 1 month postoperatively in FS-LASIK group compared to those of LASEK group (P<0.05), while they reached a similar level 3 months postoperatively. Conclusion The therapeutic effect of epithelial flap removed LASEK can reach a level equal to FS-LASIK at the early stage after surgery. It is a good selectable choice for patients with low to moderate myopia.

    Objective To summarize the experience of combined application of low and high frequency ultrasound in the diagnosis of acute primary epiploic appendagitis. Methods The clinical data of 130 patients with acute primary epiploic appendagitis confirmed by operation or other clinical examinations were retrospectively analyzed. Results All the 130 patients were found homogenous or heterogeneous solid block in the area of the abdominal pressure point. Four patients were treated by operation and 126 patients were cured by symptomatic supportive treatment. There were no serious complications in all patients. Conclusion Ultrasonography plays an important role in the diagnosis and differential diagnosis of acute primary epiploic appendagitis.

    Objective To improve diagnostic and therapeutic levels of pneumocystis pneumonia (PCP) in non-HIVinfected patients. Methods The clinical data (including clinical manifestation,laboratory examination, imaging features,diagnosis, treatment and prognosis) of sixteen non-HIV-infected patients with acute respiratory failure caused by PCP who were diagnosed in our intensive care units (ICU) during January 2015 to December 2017, were summarized and discussed.Results Almost all patients were immunocompromised by hormone or immunosuppresant before PCP (15/16). Fever, dry cough, and dyspnea were the first symptoms in all patients. The development of PCP was rapid. All patients showed acute respiratory failure [oxygen index: (85.8±16.8) mmHg]. Invasive ventilator support was required in 11 patients (68.8%), high flow oxygen therapy was used in 5 patients (31.2%). The chest CT characteristics indicated diffuse ground glass opacity. Two patients were found methenamine silver-stained positive in bronchoalveolar lavage fluid. Sixteen cases were positive for pneumocystis detected by PCR. Sixteen patients were treated with compound sulfonamide (SMZco),caspofungin and corticosteroid. Eight patients were cured, 7 died and 1 case was given up and then died. The ICU length of stay was 3-29 days, average days (13.0±7.1). Conclusion PCP is a kind of opportunistic infection among patients who are treated with immunosuppressive agents. SMZco combined with caspofungin and corticosteroid is effective in the treatment of severe PCP patients.

    Effects of dezocine combined with dexmedetomidine on reducing adverse reaction of hyperthermic intraperitoneal chemotherapy after intestinal operation

    Objective To observe the effect of dezocine combined with dexmedetomidine on reducing adverse reaction of hyperthermic intraperitoneal chemotherapy (HIPEC) after intestinal operation, and provide reference for clinical application. Methods A total of 80 patients with HIPEC were divided into intervention group and routine group according to different methods of sedation and analgesia. On the basis of routine sedation and analgesia,the 5 mg muscle injection of dizocine was given for patients in intervention group, and normal saline 48 mL+ dexmedetomidine 200 μg were continuously pumped at 0.3 μg/(kg·h) until the end of the HIPEC. The occurrence of adverse reactions in HIPEC was compared between the two groups of patients. Results Results showed that the heart rates [(83.34±8.25) times/min vs. (90.11±10.69) times/min] and body temperature [(37.33±0.42)℃ vs. (37.95±0.51)℃] were significantly lower in the intervention group than those of routine group (P<0.01). The incidence rates of nausea, vomiting, pain and irritability were significantly lower in the intervention group than those of routine group (P<0.05). Conclusion Dezocine combined with dexmedetomidine can significantly reduce the incidence of adverse reactions in patients undergoing HIPEC after intestinal surgery.

    Analysis of the influencing factors of atrial fibrillation after pulmonary lobectomy

    Objective To analyze the influencing factors of atrial fibrillation after pulmonary lobectomy. Methods The clinical data of 328 patients with pulmonary lobectomy in our hospital from February 2013 to February 2017 were retrospectively analyzed. The patients were divided into atrial fibrillation group (132 cases) and control group (196 cases)according to whether atrial fibrillation occurred after pulmonary lobectomy, and the influencing factors of atrial fibrillation after pulmonary lobectomy were analyzed. Results The univariate factor analysis showed that the occurrence of atrial fibrillation after pulmonary lobectomy was related to the patient's age, gender, smoking history, preoperative forced expiratory volume in 1 second vs. forced vital capacity (FEV1%), preoperative lung infection, and postoperative oxygen partial pressure (P<0.05). Multivariate Logistic regression analysis showed that age > 65 years old, preoperative FEV1%<70%,preoperative lung infection and postoperative oxygen partial pressure<100 mmHg were the risk factors for atrial fibrillation after pulmonary lobectomy. The short-term mortality, length of hospital stay, and cost of stay during hospitalization were statistically significantly higher in the atrial fibrillator group than those in the control group (P<0.05).Conclusion There are many risk factors of atrial fibrillation after pulmonary lobectomy, such as age, pulmonary infection,drop of FEV1% and lower partial pressure of oxygen. These factors should be controlled in order to reduce the incidence of atrial fibrillation after pulmonary lobectomy.

    The application of Warden series operation in partial anomalous pulmonary venous connection

    Objective To clarify the efficacy and safety of Warden series operation in partial anomalous pulmonary venous connection (PAPVC). Methods Six patients with PAPVC (pulmonary vein drainage to superior vena cava), who were treated in cardiac surgery of Tianjin Chest Hospital from January 2016 to March 2018, were included in this study. All 6 patients underwent Warden series operation, including 3 with Warden surgery and another 3 with modified Warden surgery. Two patients underwent modified Warden surgery were rebuilt by autologous pericardial patch and 1 cases were connected to the superior vena cava and the right atrium with Gor-Tax vessels. The echocardiography and electrocardiogram were reviewed regularly after operation (1 month, 3 months, 6 months and 1 year after operation,1 time a year for more than 1 year after the operation). The changes of cardiac structure, superior vena cava and pulmonary venous reflux and arrhythmia were observed. Results All patients were discharged successfully. The patients were followed up for 3 months to 2 years. No residual anomalous shunt was found in reexaminating echocardiography, and superior vena cava and pulmonary venous return were unobstructed. Electrocardiogram showed sinus rhythm and no obvious abnormality. Conclusion The application of Warden series operation in partial anomalous pulmonary venous connection is safe and effective.

    The cervus and cucumis polypeptide is widely used in clinical practice, mainly for arthritis, ankylosing spondylitis, various types of fractures, lumbocrural pain and other diseases. Reports of Kounis syndrome (anaphylaxis associated acute myocardial ischemia syndrome) caused by cervus and cucumis polypeptide are rare. In this paper, cervus and cucumis polypeptide was applied for osteoarthritis of the patient with no history of coronary heart disease. Kounis syndrome appeared after intravenous drip for one minute. The patient was cured and discharged after treatment with antianaphylaxis and acute myocardial ischemia. As a common clinical drug, the cervus and cucumis polypeptide should be alert to the possibility of Kounis syndrome if acute myocardial ischemia occurs.

Clinical data of a case with schwannoma in skull plate barrier and left parietal vascular malformation hospitalized in our hospital on September 2016 were retrospectively analyzed. We report the coexistence of intracranial tumors and vascular malformations coexist.

    Meta-analysis of clinical efficacy and safety of bone marrow-derived stem cells in the treatment for patients with acute myocardial infarction

    Objective To evaluate systematically the clinical efficacy and safety of bone marrow-derived stem cell therapies for patients with acute myocardial infarction. Methods Pubmed, EMbase, the Cochrane Library, VIP database,Wan fang database and CNKI database were searched by computer for randomized controlled trials of bone marrow-derived stem cell therapies in patients with acute myocardial infarction. The retrieval time was from the setup of the database to May 2018. Two systematic evaluators screened the literatures according to the inclusion and exclusion standard, and extracted the important information of literature. After assessing the quality of included studies, the risk of bias was analyzed by RevMan 5.2 software, and Meta-analysis was evaluated by Stata 14.0 software. Results There were 39 case-control studies included in systematic screening including total of 3 314 patients of acute myocardial infarction (1 879 cases received transplantation and 1 435 cases were control). Heterogeneity test showed that there was no significant heterogeneity in the included studies, and Begg’s and Egger’s quantitative tests showed that there was no publication bias among the included studies (P>0.1). Compared with the control group, bone marrow-derived stem cells could improve effectively left ventricular end diastolic volume (LVEDV) and left ventricular end systolic volume (LVESV), but there were no significant differences in left ventricular ejection fraction (LEVF) and myocardial infarction area (MIS) after infusion of bone marrow-derived stem cells between the two groups. The sensitivity analysis showed that the 39 included studies had less influence on the combination effect of the outcome indicators, and the stability was good, and the results were more credible. Seven studies reported different degrees of adverse reactions after stem cell transplantation, including death, in-stent restenosis and coronary occlusion. Whether these adverse effects are related to cell infusion remains to be further studied. Conclusion The results from systematic evaluation and Meta-analysis suggest that bone marrow-derived stem cell transplantation can improve left ventricular remodeling of patients with acute myocardial infarction, and its clinical safety requires further evaluation of multicenter clinical trials and long-term follow-up.

    In recent years, it has been found that the lack of vitamin D receptor (VDR) activation is closely associated with the development of cardiorenal syndrome (CRS). Hydroxylation catalyzed by renal 25-hydroxyvitamin D-1α hydroxylase(CYP27B1) is responsible for over 90% circulating concentrations of vitamin D activation, which then exerts biologic actions of vitamin D. Loss of renal CYP27B1 during CRS is associated with gradual decline in circulating 1,25(OH)2D3, resulting in inadequate VDR activation in renal and cardiac tissues, thereby promoting renal and cardiac damages. Therefore, CYP27B1 and VDR may become key targets for the combination of chronic kidney disease and cardiovascular disease. In this paper,the roles of vitamin D / VDR and metabolic regulation of CYP27B1 in CRS are reviewed, which may also provide new therapeutic strategies for CRS.

    Metabolomics is a new discipline in the "post-genomics" period, which constitutes the core of system biology together with genomics, transcriptomics and proteomics. Metabolomics analyzes the dynamic changes of all endogenous small metabolites in physiological or pathological state of biological systems in order to explore the relationship between endogenous small molecules and disease development. At present, metabolomics is widely used in many fields, including screening of tumor markers, disease diagnosis, new drug research and development. In recent years, domestic and foreign scholars have analyzed the characteristics of the metabolite profile of breast cancer by metabolomics techniques, and screened out valuable potential markers, which opened up a new way for the prevention, early diagnosis and individualized treatment of breast cancer. These potential metabolic markers of breast cancer are reviewed in this article.

    Recent advances of three-dimensional printing technology in orthopaedics

    Orthopaedic surgeons mainly use X-ray, CT, and MR imaging data for disease analysis, diagnosis and treatment, which often rely on the experience of doctors and lack of three-dimensional (3D) visual and tactile sense. The 3D printing technology has developed rapidly in recent years, which can quickly convert patient virtual images into threedimensional physical model. It also can build customized personalized structures to fit very specific defects within body. At present, 3D printing technology has been widely used in the diagnosis, treatment and rehabilitation of orthopedic diseases. In addition, the biological printing technology combining materials and cells may be a possible future for achieving regeneration of musculoskeletal tissue. This article aims to review the advanced research of 3D printing technology in orthopaedics, to summarize the existing problems and look forward to the future.

    Autosomal recessive cutis laxa is a common type of hereditary cutis laxa. Its pathogenesis is not completely clear, but involved genes and their corresponding functions and signaling pathways have gradually become the research focus. According to the latest research, the related genes and their responding signaling pathways not only provide a solid foundation for the pathogenesis of cutis laxa, but also indicate a direction for instructing healthy pregnancy and scientific nurture. This paper aims to review the recent advances in pathogenic genes and potential signaling pathways in autosomal recessive cutis laxa.
