Abstract: Objective：To investigate the relation of HLA-B27 in the progression and prognosis of patients with juvenile-onsetspondyloarthropathies（JSpA），we compare the clinical and laboratory examination manifestation between the HLA-B27 antigen positive and negative groups。Methods：The clinical and laboratory manifestations of 94 JSpA patients with positive HLA-B27 were analyzed and compared with those of 80 patients with negative HLA-B27.Results：There was significance difference in the gender，age of disease onset，the involved of articulatio sacroiliaca，the imageology and the IgA，erythrocyte sedimentation rate（ESR），antinuclear antibody，protein electrophoresisβmanifestation between the two groups（p<0.05），in contrast to the peripheral joint involvement，the other immune index，C-reactive protein(CRP) and so on (p>0.05)。Conclusion: HLA-B27 (+) JSpA patients have an later onset of age,severer symptoms and are tend to develop irreversible lesions.
