Abstract: The emergence of de Winter syndrome indicates the occurrence of critical cardiovascular diseases. Serious adverse cardiovascular events can be caused by untimely intervention, but they are easily overlooked because of their rarity and the characteristics of electrocardiogram (ECG). We reported a case of a young man with de Winter syndrome, whose initial complaint was acute chest pain. The result of ECG showed changes of de Winter sign. It was associated with proximal occlusion of the left anterior descending (LAD) coronary artery. Timely reperfusion therapy by means of primary percutaneous coronary intervention (PCI) was the preferred treatment for patient with de Winter syndrome. The subsequent clinical course was uneventful. Therefore, it is necessary to improve the recognition ability of medical workers for such special ECG to improve the prognosis of such patients.
