Abstract: Abstract   Objective   To explore the Expression and Significant of HBV-M in patients of Acute-on-chronic liver failure（ACLF）. Methods   The data in this study were collected from 117 patients with ACLF and 123 patients with chronic hepatitis B(CHB)、21 patients with acute liver failure(ALF) were analyzed retrospectively. Results   The level of HBV-DNA in acute liver failure group was lowest and the proportion of HBeAg –negative patients in Acute-on-chronic liver failure group was highest in three groups. The level of HBV-DNA was lower and the proportion of HBeAg –negative patients was higher in ACLF group than that of CHB group, the difference were statistically significant in both(P<0．05). There are four patients with ACLF who anti–HBs、anti –HBe and anti –HBc are all positive. Three of them died. Conclusion    Patients with Acute-on-chronic liver failure is characterized by lower level of HBV-DNA and the higher proportion of HBeAg –negative patients than ALF group and CHB group. But it is not related to the severity of illness.The high antibody expression was a particular expression or it was closely related with host's invulnerability, which need further research.
