    2026, 54 (10):  1009-1017.  doi: 10.11958/20261146
    Abstract ( 64 )   HTML ( 7 )   PDF (2114KB) ( 58 )

Objective To investigate the mechanism by which Fuyuan Huoxue decoction (FHD) inhibiting chondrocyte ferroptosis in a rat model of knee osteoarthritis (KOA) via the stimulator of interferon genes (STING)/acyl-CoA synthetase long-chain family member 4 (ACSL4) pathway. Methods SD rats were randomly divided into six groups: the sham operation (Sham) group, the model (KOA) group, the low-dose FHD (FHD-L) group, the high-dose FHD (FHD-H) group, the positive control (PC, celecoxib) group and the FHD-H+STING activator (FHD-H+DMXAA) group, with 12 rats in each group. The KOA model was established using the modified Hulth method. After successful modeling, each group received corresponding interventions by intragastric administration or tail vein injection for 8 consecutive weeks. Knee joint function was assessed using the Lequesne MG score method. Cartilage morphology was observed by HE staining. Cartilage degeneration was evaluated by Safranin O-fast green staining combined with the OARSI score. Levels of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), matrix metalloproteinase-13 (MMP-13) and the disintegrin and metalloproteinase with thrombospondin motifs-5 (ADAMTS-5) in cartilage tissue were measured by ELISA. Levels of Fe2+, malondialdehyde (MDA), glutathione (GSH) and superoxide dismutase (SOD) in cartilage tissue were determined using biochemical kits. The level of reactive oxygen species (ROS) in cartilage tissue was detected by DCFH-DA probe method. Iron deposition was observed by Prussian blue staining. Mitochondrial ultrastructure of chondrocytes was observed by transmission electron microscopy. The protein expressions of glutathione peroxidase 4 (GPX4), solute carrier family 7 member 11 (SLC7A11) and cyclooxygenase-2 (COX-2) in cartilage tissue were detected by immunohistochemistry. The protein expressions of STING and ACSL4 were detected by Western blot assay. Results Compared with the Sham group, Lequesne MG score, OARSI score, cartilage inflammatory and matrix degradation factors (IL-1β, TNF-α, MMP-13 and ADAMTS-5), ferroptosis-related indicators (Fe²⁺, MDA, ROS), Prussian blue staining positive area ratio, COX-2 positive expression area and STING and ACSL4 protein expression were significantly increased in the KOA group (P<0.05), while the levels of GSH and SOD, and the positive expression areas of GPX4 and SLC7A11 were significantly decreased (P<0.05). HE staining, Safranin O-fast green staining and transmission electron microscopy revealed severe cartilage structural damage, matrix loss, mitochondrial atrophy and cristae rupture in the KOA group. Compared with the KOA group, all the above indicators were improved to varying degrees in the FHD-L group, the FHD-H group and the PC group (P<0.05), with the FHD-H group showing better efficacy than the FHD-L group (P<0.05). Among them, the protective effects on the above indicators of the FHD-H group was better than that of the FHD-L group (P<0.05). Compared with the FHD-H group, the improvement effects of the above indicators in the FHD-H +DMXAA group were significantly reversed (P<0.05). Conclusion FHD may improve joint function and cartilage damage in KOA rats by inhibiting chondrocyte ferroptosis via suppressing the STING/ACSL4 pathway.

ZHENG Tianhong, WANG Haihui, JIN Zhiyuan
    2026, 54 (10):  1018-1023.  doi: 10.11958/20260411
    Abstract ( 53 )   HTML ( 5 )   PDF (962KB) ( 58 )

Objective To explore the effect of anti-inflammatory decoction on gut microbiota of rats with septic acute lung injury (ALI) by regulating the Toll like receptor 4 (TLR4)/TIR-domain-containing adaptor inducing interferon-β (TRIF) pathway. Methods Rats were stochastically separated into the sham group, the septic ALI group, the anti-inflammatory decoction group (intragastric administration of 14 g/kg anti-inflammatory decoction once a day for 3 days before modeling and within 1 hour after modeling) and the necrosis inhibition agent Necrostatin-1 (Nec-1) group (intraperitoneal injection of 0.6 mg/kg Nec-1 once a day for 3 days before modeling and within 1 hour after modeling), with 10 rats in each group. The septic ALI group and the sham group were given an equal amount of physiological saline by gavage once a day. The lung function of each group of rats [forced vital capacity (FVC), forced expiratory volume at 0.3 seconds (FEV0.3), peak expiratory flow rate (PEF)] were measured. ELISA was used to detect serum levels of TNF-α, IL-1β and IL-6. Fecal gut microbiota was detected. HE staining was used to detect pathological changes in rat lung tissue and small intestine tissue. Western blot assay was used to detect TLR4 and TRIF proteins in lung tissue and small intestine tissue. Results Compared with the sham group, obvious alveolar atrophy, thickening of the alveolar walls, pulmonary interstitial edema, extensive infiltration of inflammatory cells, shortening and detachment of intestinal villi, deformation of crypt structures and abundant infiltration of inflammatory cells were observed in the sepsis ALI group. It was also found that lung function declined (FVC, FEV0.3/FVC and PEF decreased), the serum expression levels of inflammatory factors TNF-α, IL-1β and IL-6 increased, intestinal flora imbalance (decreased counts of bifidobacterium and lactobacillus and increased counts of Escherichia coli) and the protein expressions of TLR4 and TRIF proteins in lung tissue and small intestine tissue increased (P<0.05). The anti-inflammatory agent group and Nec-1 group showed reduced lung and small intestine tissue damage, less inflammatory cell infiltration and effectively reversed the changes of the above indicators in the sepsis ALI group, and the Nec-1 group had a better effect (P<0.05). Conclusion Anti-inflammatory decoction can improve the distribution of gut microbiota in septic ALI rats by inhibiting TLR4/TRIF pathway.

    2026, 54 (10):  1024-1029.  doi: 10.11958/20253703
    Abstract ( 53 )   HTML ( 5 )   PDF (1139KB) ( 54 )

Objective To compare the clinical efficacy of percutaneous spinal endoscopic surgery and traditional posterior laminectomy in the treatment of thoracic ossification of the ligamentum flavum (TOLF), and to analyze prognostic influencing factors. Methods A retrospective analysis was conducted on the clinical data of 167 patients with TOLF who were treated in the spinal orthopedics department of our hospital from January 2017 to December 2023. The patients were divided into the control group (n=54) and the observation group (n=113) based on their own condition and treatment willingness to undergo surgery. The control group received traditional posterior laminectomy surgery, while the observation group underwent percutaneous spinal endoscopic surgery. The surgical indicators, therapeutic effects and complication rates of the two groups were compared. The patients were followed up for 6 months, and the Japanese Orthopedic Association Assessment Scale (JOA) score improvement rate was used to classify them into the favorable prognosis group and the unfavorable prognosis group. Univariate analysis was conducted on the indicators between the two groups to identify the statistically significant ones, and then multiple-factor Logistic regression analysis was further conducted to screen influencing factors for unfavorable prognosis in TOLF patients. Results The bleeding volume was less in the observation group than that in the control group. The hospitalization time, operation time and time to start walking after surgery were all shorter in the observation group than those in the control group. The postoperative Oswestry Disability Index (ODI) and Numerical Rating Scale (NRS) scores were lower in the observation group than those in the control group, while the JOA score was higher in the observation group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). Based on the JOA score improvement rate at 6 months postoperatively, in 167 patients, 116 cases (69.5%) were in the favorable prognosis group and 51 cases (30.5%) were in the unfavorable prognosis group. The multivariate Logistic regression analysis revealed that the intramedullary high signal on the preoperative T2-weighted image (OR=4.667, 95%CI: 2.075-10.497, P<0.001) and postoperative cerebrospinal fluid leakage (OR=6.738, 95%CI: 2.625-17.295, P<0.001) were independent risk factors affecting the prognosis of TOLF patients, while the preoperative residual rate of thoracic spinal canal area>80% (OR=0.421, 95%CI: 0.189-0.936, P=0.034) was an independent protective factor for the prognosis of TOLF patients. Conclusion The therapeutic effect of percutaneous spinal endoscopic treatment for TOLF is superior to that of traditional posterior laminectomy. Preoperative intramedullary high signal on T2-weighted images, preoperative residual rate of the thoracic spinal canal area and cerebrospinal fluid leakage are prognostic factors for patients with TOLF.

GUO Lijuan, KANG Xiaofei, WANG Hui
    2026, 54 (10):  1030-1034.  doi: 10.11958/20260087
    Abstract ( 49 )   HTML ( 5 )   PDF (805KB) ( 49 )

Objective To explore the relationship between serum cathepsin (Cat) B, Cat-L, Cat-S and inflammatory factors in patients with acute exacerbation of chronic obstructive pulmonary disease (COPD). Methods A total of 224 COPD patients admitted to the respiratory department of Hebei Provincial Chest Hospital from January 2022 to December 2024 were collected as the research group, and 62 individuals participated in the health check-up during the same period were selected as the control group. The research group was divided into the acute exacerbation group (71 cases) and the stable group (153 cases) based on changes in the patient's condition. The modified medical research council (mMRC), forced expiratory volume in one second / forced vital capacity (FEV1/FVC) and COPD assessment test (CAT) score were used to assess respiratory distress. Enzyme linked immunosorbent assay was used to detect serum Cat-B, Cat-L and Cat-S levels. Pearson method was used to analyze the correlation between serum Cat-B, Cat-L, Cat-S and serum C-reactive protein (CRP), interleukin-8 (IL-8), tumor necrosis factor-α (TNF-α) and interleukin-10 (IL-10). Logistic multiple regression model was used to analyze the influencing factors of aggravation of COPD. Results Compared with the control group, the serum Cat-B level in the study group decreased, with levels in the acute exacerbation subgroup being lower than those in the stable subgroup (P<0.05). In contrast, Cat-L and Cat-S levels were increased in the study group, in which the acute exacerbation subgroup was higher than that in the stable subgroup (P<0.05). Cat-B was negatively correlated with CRP, IL-8, TNF-α, mMRC and CAT score (P<0.05), and Cat-B was positively correlated with IL-10 and FEV1/FVC (P<0.05). Cat-L and Cat-S showed correlations opposite to those of Cat-B with the above indicators (P<0.05). Elevated levels of Cat-L and Cat-S were identified as independent risk factors for acute exacerbation in COPD patients (P<0.05), whereas elevated FEV1/FVC ratio and Cat-B levels were independent protective factors (P<0.05). Conclusion Cat-B, Cat-L and Cat-S, as key biomarkers, participate in the progress of COPD by mediating inflammatory responses. Their dynamic changes have a significant impact on the acute exacerbation of the disease.

    2026, 54 (10):  1035-1038.  doi: 10.11958/20260009
    Abstract ( 42 )   HTML ( 3 )   PDF (847KB) ( 49 )

Objective To investigate the correlation between serum transient receptor potential vanilloid 1 (TRPV1), DNA methyltransferase 1 (DNMT1) and secreted frizzled related protein 5 (SFRP5) with the severity of allergic rhinitis (AR) in patients. Methods A total of 120 AR patients treated in our hospital from June 2022 to June 2025 were enrolled and categorized into the mild to moderate AR group (75 cases, TNSS total score≤8) and the severe AR group (45 cases, TNSS total score≥9) based on the total nasal symptom score (TNSS). Serum levels of TRPV1, DNMT1, SFRP5 and immunoglobulin E(IgE) were measured using enzyme-linked immunosorbent assay (ELISA) kits. Pearson correlation analysis was used to evaluate the correlation of serum TRPV1, DNMT1 and SFRP5 levels with TNSS score and serum IgE level. Multivariate Logistic regression analysis was performed to assess the impact of serum TRPV1, DNMT1 and SFRP5 levels on AR progression. Receiver operating characteristic (ROC) curve was plotted to evaluate the value of serum TRPV1, DNMT1 and SFRP5 levels alone and in combination for predicting severe AR. Results Compared with the mild to moderate AR group, the proportion of patients with concomitant asthma, TNSS scores and serum levels of IgE, TRPV1 and DNMT1 increased in the severe AR group, and serum level of SFRP5 was decreased (P<0.01). Serum levels of TRPV1 and DNMT1 were positively correlated with TNSS score and IgE (P<0.01), while the serum level SFRP5 was negatively correlated with TNSS score and IgE (P<0.01). Multivariate Logistic regression analysis showed that elevated serum TRPV1 (OR=1.844, 95%CI: 1.283-2.650) and DNMT1 (OR=2.356, 95%CI: 1.552-3.577) levels, and decreased SFRP5 (OR=0.865, 95%CI: 0.798-0.937) level were independent risk factors for progression of severe cases in AR patients (P<0.05). The AUC values of single and combined predictions of serum TRPV1, DNMT1 and SFRP5 levels for severe AR were 0.775 (95%CI: 0.690-0.846), 0.844 (95%CI: 0.766-0.904), 0.808 (95%CI: 0.727-0.875) and 0.921 (95%CI: 0.858-0.963), respectively. Compared with single indicator prediction, the combined prediction value of three indicators was higher (P<0.05). Conclusion In severe AR patients, serum TRPV1 and DNMT1 levels are elevated, while SFRP5 level is reduced. The alterations in these three biomarkers are closely associated with AR disease severity. Combined detection of the three indicators can be used for early prediction of disease progression.

XIE Yilin, XU Dandan, PAN Luxiang, LIU Qin
    2026, 54 (10):  1039-1044.  doi: 10.11958/20253752
    Abstract ( 41 )   HTML ( 3 )   PDF (862KB) ( 50 )

Objective To investigate the relationship between three-dimensional power Doppler ultrasound volumetric blood flow parameters and pregnancy outcome of frozen-thawed embryo transfer (FET) after hysteroscopic adhesion resection (TCRA). Methods A total of 138 patients with TCRA who received FET were retrospectively included in this study, and according to their serum β-human chorionic gonadotropin (β-HCG) levels 12-14 days after transplantation, they were divided into the pregnancy group (76 cases) and the non-pregnancy group (62 cases). The volumetric blood flow parameters [vascular index (VI), flow index (FI), vascular-flow index (VFI)] and general clinical data were compared between the two groups of patients. Logistic regression analysis was used to analyze the influencing factors of pregnancy failure. The predictive value of the above blood flow parameters for pregnancy failure was evaluated by receiver operating characteristic (ROC) curves. Results In the non-pregnancy group, endometrial thickness on the day of transformation, the proportion of transferring two high-quality embryos, VI, FI and VFI were all lower than those in the pregnancy group (P<0.05). Multivariate Logistic regression analysis showed that transferring 0-1 high-quality embryos was an independent risk factor for FET pregnancy failure after TCRA, while high VI, FI, VFI and endometrial thickness on the day of transformation were protective factors (P<0.05). ROC curve analysis showed that the areas under the curve (AUC) for predicting pregnancy failure individually by VI, FI and VFI were 0.776, 0.752 and 0.861, respectively. The combined prediction of the three yielded an AUC of 0.923 (95%CI: 0.865-0.961), and its performance was significantly better than any single parameter alone (Z values were 3.892, 4.117 and 2.848, all P<0.01). Conclusion For patients undergoing FET after TCRA, elevated levels of VI, FI and VFI are independent associated factors for a reduced risk of pregnancy failure, and the combined testing of the three has high value in predicting pregnancy failure.

LIU Shuai, LI Bao, SHANG Heng, SUN Yazhao
    2026, 54 (10):  1045-1049.  doi: 10.11958/20260146
    Abstract ( 50 )   HTML ( 5 )   PDF (911KB) ( 61 )

Objective To investigate the levels of serum adiponectin (APN) and homeostasis model assessment of β-cell function (HOMA-β) in patients with diabetic cardiomyopathy (DCM), and to analyze their relationship with the occurrence of DCM and cardiac function impairment. Methods A retrospective case-control study was conducted. A total of 106 DCM patients admitted to the hospital from March 2021 to March 2023 were enrolled as the observation group. Another 106 patients with type 2 diabetes mellitus (T2DM) without cardiomyopathy, coronary heart disease, or cardiac insufficiency during the same period were selected as the control group. The general data, serum APN, HOMA-β and cardiac function indicators were compared between the two groups. Pearson correlation analysis was used to evaluate the correlation between APN, HOMA-β and cardiac function indicators. Multivariate Logistic regression analysis was used to analyze the influencing factors of DCM. Receiver operating characteristic (ROC) curve analysis was used to assess the evaluation value of APN and HOMA-β for DCM. Results There were no statistically significant differences in baseline data such as age, gender, BMI, smoking history, drinking history, comorbidities, use of hypoglycemic drugs, HDL-C, LDL-C, TC, TG, HbA1c, Scr and UA between the two groups (P>0.05). However, the duration of diabetes was longer in the observation group than that in the control group (P<0.05). Compared with the control group, levels of serum APN, HOMA-β, the ratio of early to late diastolic mitral inflow velocities (E/A) and left ventricular ejection fraction (LVEF) were decreased, while left ventricular end-diastolic volume (LVEDV) and N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels were increased in the observation group (P<0.05). Pearson correlation analysis showed that both APN and HOMA-β were positively correlated with E/A and LVEF, and negatively correlated with LVEDV and NT-proBNP (P<0.05). Multivariate Logistic regression analysis showed that a long duration of diabetes was a risk factor for developing DCM, while high levels of serum APN and HOMA-β were protective factors for DCM (P<0.05). ROC curve analysis showed that the area under the curve (AUC) for APN and HOMA-β alone in evaluating DCM were 0.790 (95%CI: 0.729-0.843) and 0.761 (95%CI: 0.697-0.816), respectively. The AUC for their combined evaluation increased to 0.903 (95%CI: 0.855-0.940). DeLong's test indicated that the diagnostic efficacy of the combined evaluation model was significantly superior to that of APN alone (Z = 2.974,P<0.01) and HOMA-β alone (Z = 5.237, P<0.01). Conclusion Serum levels of APN and HOMA-β in DCM patients are correlated with cardiac function impairment indicators. Both may be potential biomarkers for evaluating DCM and cardiac function impairment, and the combined detection has a higher value.

YANG Xuan, LI Yanhui, ZHANG Liying, SUN Yalin
    2026, 54 (10):  1050-1054.  doi: 10.11958/20260219
    Abstract ( 47 )   HTML ( 3 )   PDF (998KB) ( 49 )

Objective To investigate the clinical efficacy of endovenous radiofrequency ablation (RFA) in elderly patients with lower extremity varicose veins, and to analyze the independent risk factors for venous leg ulcer (VLU) recurrence and establish a predictive nomogram. Methods Clinical data of 211 elderly patients with lower extremity varicose veins complicated by venous leg ulcer (VLU) who underwent RFA from January 2020 to January 2022 were retrospectively analyzed. Operative parameters, perioperative complications and long-term follow-up data were recorded. Univariate analysis and multivariate Logistic regression models were used to screen independent risk factors for ulcer recurrence. A recurrence risk prediction nomogram was constructed based on the identified independent risk factors and internally validated using the Bootstrap resampling method. Results The technical success rate of the procedure was 100% for all patients. The venous clinical severity score (VCSS) showed a significant decrease during the postoperative follow-up (P<0.01). The perioperative complication rate was 8.06% (17/211). Among the 211 patients, 79 experienced recurrence (recurrence group), and the remaining 132 cases were included in the non-recurrence group. Univariate analysis and multivariate Logistic regression analysis revealed that age, body mass index (BMI), history of diabetes, ulcer area, poor compliance with compression stocking therapy postoperatively and deep venous reflux were independent risk factors for venous ulcer recurrence in elderly patients (P<0.05). The nomogram constructed based on these factors had a predictive C-index of 0.832 (95%CI: 0.781-0.887). The Bootstrap-corrected C-index was 0.814. The calibration curve showed good agreement with the ideal curve, and the decision curve analysis indicated high clinical utility of the model. Conclusion RFA treatment for elderly patients with lower limb varicose veins has the characteristics of minimal trauma, fast recovery, definite long-term efficacy and high safety. Age, BMI, history of diabetes, ulcer area, poor postoperative compression stocking compliance and deep venous reflux are key risk factors for ulcer recurrence.

    Abstract ( 47 )   HTML ( 3 )   PDF (925KB) ( 49 )

Objective To analyze the clinical characteristics and risk factors of ureaplasma urealyticum (UU) infection in preterm infants. Methods A total of 462 preterm infants admitted to the Neonatal Intensive Care Unit (NICU) of Nanyang Central Hospital from January 2023 to January 2025 were enrolled. According to the test results of UU testing of lower respiratory tract secretions was performed within 48 hours after birth, and these infants were divided into the UU positive group of 130 cases and the UU negative group of 332 cases. The general data, laboratory examination indicators, and treatment conditions of the two groups of preterm infants, as well as the pregnancy and perinatal related indicators of their mothers were compared. Multivariate Logistic regression was used to analyze the influencing factors of UU infection, and a combined diagnostic model was established and the receiver operating characteristic (ROC) curve was drawn. Results Compared with the UU-negative group, the UU-positive group of preterm infants had lower gestational age, birth weight, 1-minute Apgar score, 5-minute Apgar score, and platelet (PLT), and higher proportions of infants with acute respiratory distress syndrome, bronchopulmonary dysplasia (BPD), sepsis and intracranial hemorrhage, as well as increased levels of white blood cell (WBC), neutrophil (NEUT), and C-reactive protein (CRP), and longer durations of antibiotic use, mechanical ventilation, and oxygen therapy (P<0.05). Compared with the UU-negative group, the mothers of the UU-positive group had a higher proportion of UU infection history, concurrent chorioamnionitis, premature rupture of membranes, antepartum fever, vaginal delivery, and amniotic fluid grade Ⅲ contamination during pregnancy, and longer duration of labor (P<0.05). There were no statistically significant differences in maternal age, parity, gravidity, conception method, concurrent gestational hypertension and gestational diabetes between the two groups. Multivariate Logistic regression analysis showed that concurrent sepsis, concurrent intracranial hemorrhage, maternal UU infection history, concurrent chorioamnionitis during pregnancy and vaginal delivery were independent risk factors for UU positivity in neonates, while higher birth weight, 5-minute Apgar score and longer duration of antibiotic use were independent protective factors. The area under the receiver operating characteristic (ROC) curve of the combined diagnostic model was 0.949 (95%CI: 0.925-0.968). Conclusion Screening for risk factors of UU positivity in the respiratory tract of preterm infants can enable early identification and reduce the risk of UU infection in preterm infants in the NICU.

YANG Cuihong, QI Feifan, GUO Zhiyong, LIU Guangping
    2026, 54 (10):  1061-1064.  doi: 10.11958/20260366
    Abstract ( 42 )   HTML ( 4 )   PDF (899KB) ( 50 )

Objective To analyze the clinical characteristics, diagnosis and treatment of children with second branchial cleft fistulas and cysts, and to improve the understanding of second branchial cleft anomalies (SBCA). Methods A retrospective analysis was performed on the clinical data of 18 children with SBCA who underwent surgical treatment at Tianjin Children's Hospital from July 2024 to June 2025. All children underwent complete resection of the lesion under general anesthesia after infection control. For Bailey type Ⅳ cases, high ligation of the fistula was performed without resection of the ipsilateral tonsil. All patients underwent preoperative MRI examination. The first follow-up cervical ultrasound was performed at 3 months postoperatively, followed by re-examination every 6 months to 1 year, and the postoperative recurrence was recorded. Results Among the 18 children, there were 7 boys and 11 girls, aged from 1 to 16 years, with a median age of 5.5 (3.0, 11.0) years. The disease course ranged from 1 to 196 months, with a median course of 1.5 (1.0, 9.5) months. All cases were unilateral, including 8 on the left side and 10 on the right side. There were 8 cases of cysts and 10 cases of fistulas. According to Bailey classification, there were 6 cases of type Ⅰ, 6 cases of type Ⅱ, 4 cases of type Ⅲ and 2 cases of type Ⅳ. Preoperative MRI showed that most lesions presented isointense or slightly hyperintense on T1WI and slightly or markedly hyperintense on T2WI. All operations were completed successfully, and the intraoperative findings were consistent with the preoperative MRI results. No postoperative complications such as facial palsy, hoarseness, choking, tongue numbness or pharyngeal fistula occurred after the operation. The patients were followed up for 7-18 months, with a median follow-up time of 12 (10, 14) months, and no recurrence was observed. Conclusion MRI has superior imaging quality and safety in the diagnosis of pediatric SBCA. High ligation of the fistula is an effective method to prevent recurrence, and simultaneous resection of the ipsilateral tonsil is unnecessary.

YU Guijun, WEI Suping, LI Linsong, ZHANG Hongwei, ZHAO Ruiling
    2026, 54 (10):  1065-1070.  doi: 10.11958/20253588
    Abstract ( 36 )   HTML ( 3 )   PDF (869KB) ( 49 )

Objective To explore the effects of serum apolipoprotein B/apolipoprotein A-1 (ApoB/ApoA-1) and urinary microalbumin (mAlb) on the severity of diabetic macular edema (DME). Methods From June 2022 to May 2023, 228 DME patients admitted to the Cangzhou Eye Hospital were selected as the research subjects. According to the severity of DME, patients were divided into the mild group, the moderate group and the severe group. The ApoB/ApoA-1 ratio and mAlb level of the three groups of patients were compared. The correlation between the degree of DME and the levels of ApoB/ApoA-1 ratio and mAlb was analyzed using the Spearman method. Univariate analysis and multivariate Logistic regression analysis were used to detect risk factors for developing severe DME. The diagnostic value of the ApoB/ApoA-1 ratio and mAlb for severe DME was analyzed using the receiver operating characteristic (ROC) curve. Results The levels of ApoB/ApoA-1 and mAlb in the mild, moderate and severe groups increased successively (P<0.05). The Spearman correlation analysis showed that the severity of DME was positively correlated with the levels of ApoB/ApoA-1 and mAlb (rs were 0.502 and 0.836 respectively, both P<0.01). Compared with the mild-moderate group, the severe group had a longer duration of diabetes and insulin treatment time, and higher ApoB/ApoA-1 ratio, mAlb and HbA1c levels (P<0.05). The multivariate Logistic regression model showed that higher levels of ApoB/ApoA-1, mAlb, HbA1c and longer duration of diabetes were all risk factors for developing severe DME (P<0.05). The ROC curve analysis results showed that the area under the curve (AUC) for the ApoB/ApoA-1 ratio and the mAlb level alone and in combination in evaluating the risk of developing severe DME was 0.711, 0.782 and 0.927 respectively. The DeLong test indicated that the diagnostic efficacy of the combined model was significantly better than that of the single indicator (Z values were 5.063 and 4.099 respectively, both P<0.05). Conclusion The ApoB/ApoA-1 ratio and mAlb level in DME patients are closely related to the severity of the disease, and they are also risk factors for developing severe DME. The combined detection of these two indicators has important clinical reference value for identifying high-risk individuals with progressive disease and warning of the risk of severe DME occurrence.

WU Yong, MEI Chengqing, LIU Shangxiang, YUAN Ting
    2026, 54 (10):  1071-1074.  doi: 10.11958/20252826
    Abstract ( 42 )   HTML ( 3 )   PDF (871KB) ( 53 )

Objective To investigate the predictive value of the age-adjusted BIG score (aaBIG score) in predicting the risk of in-hospital mortality in adult patients with traumatic brain injury (TBI). Methods A retrospective cohort study was conducted, enrolling a total of 209 adult patients diagnosed with TBI. Patients were divided into the survival group and the mortality group based on in-hospital mortality status, and differences in clinical indicators were compared between the two groups. Multivariate Logistic regression analysis was performed to identify risk factors for in-hospital mortality in TBI patients. Receiver operating characteristic (ROC) curves were used to evaluate the predictive value of the BIG score and aaBIG score for in-hospital mortality in TBI patients. Results Among the 209 patients, 48 (23%) patients died in hospital. The age, respiratory rate (RR), fasting blood glucose (FBG), international normalised ratio (INR) and BIG score were higher in the mortality group than those of the survival group (P<0.05). The blood oxygen saturation (SpO₂), Glasgow Coma Scale (GCS) and Revised Trauma Score (RTS) were lower in the mortality group than those of the survival group (P<0.05). Multivariate Logistic regression analysis revealed that increasing age (OR=1.072, 95%CI: 1.013-1.133, P=0.015), decreased SpO₂ (OR = 0.626, 95%CI: 0.455-0.861, P = 0.004), elevated FBG (OR = 1.593, 95%CI: 1.049-2.419, P = 0.029), increased RR (OR = 1.162, 95%CI: 1.019-1.327, P = 0.025) and a higher BIG score (OR = 1.279, 95%CI: 1.146-1.428, P<0.001) were all significantly associated with the increased risk of in-hospital mortality. Based on this, the aaBIG score was derived as 0.3×age + BIG score. ROC analysis showed that the AUC for the BIG score in predicting in-hospital mortality was 0.775 (95%CI: 0.712-0.830), with an optimal cut-off value of 7.4 points. The AUC for the aaBIG score in predicting in-hospital mortality was 0.810 (95%CI: 0.743-0.876), with an optimal cut-off of 29.6 points. The predictive value of the two scores was similar. Conclusion The aaBIG score has some predictive value for in-hospital mortality in TBI patients, but its performance does not show a significant improvement over the BIG score.

    2026, 54 (10):  1075-1080.  doi: 10.11958/20260422
    Abstract ( 38 )   HTML ( 3 )   PDF (868KB) ( 51 )

Objective To analyze the evaluation value of blood lactic acid to albumin ratio (LAR), heparin-binding protein to albumin ratio (HBP/ALB) and systemic inflammatory response index (SIRI) in the judgement of disease condition and prognosis prediction in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). Methods A total of 215 patients with AECOPD were divided into the severe group (SAPS-II≥40 points, 73 cases) and the mild group (SAPS-II<40 points, 142 cases) according to the simplified acute physiology score-II (SAPS-II) at admission. Based on the prognosis status at 1 year after discharge, patients were classified into the poor prognosis group (acute exacerbation readmission event or death, 37 cases) and the good prognosis group (178 cases). The general data (gender, age, COPD duration) and laboratory indexes (LAR, HBP/ALB and SIRI at admission) were collected. Multivariate Logistic regression was used to analyze the factor influencing severe AECOPD. Receiver operating characteristic (ROC) curve was adopted to evaluate the diagnostic value of the indexes on severe AECOPD and the predictive value on poor prognosis. Results The age and COPD duration were older or longer in the severe group than those in the mild group (P<0.05), and LAR, HBP/ALB and SIRI at admission were higher in the severe group than those in the mild group (P<0.05). Multivariate Logistic regression analysis showed that high levels of LAR, HBP/ALB and SIRI were independent risk factors for severe AECOPD (P<0.05). ROC curve analysis showed that the area under the curve (AUC) of the combination of LAR, HBP/ALB and SIRI for predicting severe AECOPD was 0.872 (95%CI: 0.821-0.922), which was higher than that of each index (P<0.05). LAR, HBP/ALB and SIRI were higher in the poor prognosis group than those in the good prognosis group (P<0.05). ROC curve analysis showed that the AUC of the combination of LAR, HBP/ALB and SIRI was 0.951 (95%CI: 0.925-0.978) in the predicting poor prognosis, which was higher than that of each index (P<0.05). Conclusion The disease condition and prognosis of patients with AECOPD are closely associated with LAR, HBP/ALB and SIRI at admission. Combined detection of the above indicators can effectively aid in evaluating the disease condition and prognosis.

    2026, 54 (10):  1081-1086.  doi: 10.11958/20260425
    Abstract ( 50 )   HTML ( 3 )   PDF (1095KB) ( 49 )

Objective To construct and validate a nomogram prediction model for symptomatic radiation pneumonitis (SRP) in patients with non-small cell lung cancer (NSCLC) after radiotherapy. Methods A total of 360 patients with NSCLC were selected and divided into the training set (270 cases) and the validation set (90 cases) at a 3∶1 ratio using a simple random sampling method. All patients were followed up for 6 months after the end of radiotherapy. Patients in the training set were divided into the SRP group (83 cases) and the non-SRP group (187 cases) according to whether SRP occurred. General clinical data, laboratory indicators and dosimetry parameters were compared between the two groups. Multivariate Logistic regression was used to analyze factors influencing the occurrence of SRP in patients with NSCLC after radiotherapy. A nomogram prediction model for SRP was constructed and validated. Results Compared with the non-SRP group, the SRP group showed higher proportions of patients with history of underlying pulmonary disease, patients with Eastern Cooperative Oncology Group (ECOG) scores of 2 and patients receiving sequential radiotherapy and chemotherapy, and a lower proportion of patients with central-type lesions, higher procalcitonin levels and neutrophil count, lower levels of platelet count and eosinophil count, larger planning target volume (PTV), higher percentage of total lung volume receiving≥5 and ≥20 Gy irradiation (V5, V20, P<0.05). Multivariate Logistic regression analysis showed that having history of underlying pulmonary disease, ECOG score of 2, increased PTV, sequential radiotherapy and chemotherapy and high V20 were independent risk factors for SRP in patients with NSCLC, while central-type lesion was an independent protective factor (P<0.05). The areas under the receiver operating characteristic (ROC) curve of the constructed model for predicting SRP in the training set and the validation set were 0.826 (95%CI: 0.775-0.869) and 0.897 (95%CI: 0.815-0.951), respectively. Hosmer-Lemeshow goodness of fit test showed χ2=3.913, P=0.865 in the training set, and χ2=6.482, P=0.593 in the validation set, indicating good model fit. The calibration curve was relatively close to the ideal curve, and decision curve analysis indicated that the model could obtain net benefits. Conclusion The nomogram prediction model constructed based on risk factors in patients with NSCLC after radiotherapy demonstrates good predictive efficacy.

    2026, 54 (10):  1087-1092.  doi: 10.11958/20260051
    Abstract ( 38 )   HTML ( 3 )   PDF (1251KB) ( 50 )

Objective To analyze the clinical and pathological characteristics, treatment modalities and prognosis of proliferative glomerulonephritis with monoclonal immunoglobulin deposition (PGNMID), and to offer references for the diagnosis and treatment of this disease. Methods A retrospective analysis was conducted on the clinical data, pathological manifestations, treatment regimens and prognostic characteristics of three patients with PGNMID diagnosed by renal biopsy in our hospital. The aim was to explore the pathogenesis and evaluate the efficacy of different treatment methods. Results All 3 PGNMID patients were female, with an average age of (68.00 ± 12.29) years. Throughout the disease course, all patients exhibited glomerular hematuria, massive proteinuria and varying degrees of renal function impairment. Histopathological examination under light microscopy indicated membranoproliferative glomerulonephritis. Immunofluorescence demonstrated the deposition of IgG subtype immunoglobulins and light chains in the capillary loops and mesangial regions, accompanied by complement C3 deposition. Electron microscopy revealed diffuse fusion of podocytes (>95%) and electron-dense material deposition in the subendothelial and mesangial regions. In 2 cases, electron-dense material deposition was also observed in the basement membrane. All three patients received a combination of renin-angiotensin-converting enzyme inhibitors (RASi) and clonal targeted therapy. One month later, their 24-hour urinary protein levels showed varying degrees of reduction. Two patients achieved partial response (PR) after 2-3 months of treatment, achieved complete response (CR) in the 6th to 7th month. One patient maintained PR during the follow-up period up to the month 6. Conclusion Patients with PGNMID usually present with nephrotic syndrome accompanied by renal function decline. Pathologically, it is often characterized by membranoproliferative glomerulonephritis with IgG3 deposition. Early diagnosis and treatment can lead to better clinical therapeutic outcomes.

The relationship between the left ventricular geometric remodeling and clinical worsening in patients with pulmonary arterial hypertension
DENG Yuanlin, TIAN Hexian, ZHANG Zhang, YANG Zhenwen, LI Fengtan, LI Dong, YANG Fan
    2026, 54 (10):  1093-1098.  doi: 10.11958/20261200
    Abstract ( 51 )   HTML ( 4 )   PDF (975KB) ( 53 )

Objective To investigate the association between left ventricular (LV) geometric remodeling derived from short-axis cine cardiac magnetic resonance (CMR) images and clinical worsening (CW) in patients with pulmonary hypertension (PH). Methods A total of 194 patients with PH confirmed by right heart catheterization and who underwent CMR were retrospectively included. Patients were divided into the CW group (n=61) and the non-CW group (n=133) based on follow-up outcome. LV eccentricity, circularity and axis ratio were measured from short-axis cine CMR images at end-diastole (ED) and end-systole (ES). A composite left ventricular geometric index (LVGI) was subsequently constructed. Differences in geometric indices between the two groups were compared. Cox regression was used to evaluate the association between LVGI and clinical worsening. Prognostic discrimination was assessed using the Harrell C-index, 60-month time-dependent receiver operating characteristic (ROC) analysis and Kaplan-Meier analysis. In addition, Spearman correlation analysis was performed to assess the association of LVGI with selected hemodynamic and CMR-derived cardiac functional parameters. Results Compared with the non-CW group, the LV eccentricity and long-to-short axis ratio increased and circularity decreased at both ED and ES in the CW group (all P<0.05). After integration of these geometric parameters, both ED and ES LVGI were significantly higher in the CW group (both P<0.05). In univariable Cox analysis, ED LVGI was significantly associated with clinical worsening (HR=1.684, 95%CI: 1.353-2.107, P<0.001). After multivariable adjustment, ED LVGI remained independently associated with clinical worsening (HR=1.370, 95%CI: 1.028-1.827, P=0.032). The 60-month time-dependent AUC of ED LVGI for predicting clinical worsening was 0.690, and the Harrell C-index was 0.663.Spearman correlation analysis showed that ED LVGI was positively correlated with mean pulmonary arterial pressure, pulmonary vascular resistance and right ventricular end-diastolic volume index (rs=0.436, 0.421, and 0.399, respectively), and negatively correlated with right ventricular ejection fraction and left ventricular cardiac index (rs=-0.247 and -0.227, respectively). After stratification by the median ED LVGI, the clinical-worsening-free survival was higher in the high-LVGI group than that of the low-LVGI group (Log-rank P<0.001). Conclusion LV geometric remodeling is more pronounced in PH patients with clinical worsening. CMR-derived ED LVGI is independently associated with CW and may serve as a complementary imaging marker reflecting ventricular interaction and disease progression.

    2026, 54 (10):  1099-1103.  doi: 10.11958/20260134
    Abstract ( 40 )   HTML ( 3 )   PDF (775KB) ( 50 )

Objective To compare the efficacy, safety and pharmacoeconomics of mecapegfilgrastim and efbemalenograstim alfa in the management of chemotherapy-induced neutropenia (CIN) in cancer patients, and to perform a subgroup analysis stratified by chemotherapy regimen risk, thereby providing evidence-based support for rational clinical drug selection. Methods A retrospective cohort study was conducted, enrolling 163 patients with malignant tumors who developed CIN after chemotherapy. Patients were divided into the mecapegfilgrastim group (n=77) and the efbemalenograstim alfa injection group (n=86). The incidence of adverse reactions after medication, incidence of grade ≥3 CIN,febrile neutropenia (FN) and treatment costs were compared between the two groups. Results In terms of safety, the overall AE rate (particularly fatigue) in the mecapegfilgrastim group was significantly lower than that in the efbemalenograstim alfa group. For efficacy, the efbemalenograstim alfa group exhibited a significantly lower incidence of grade ≥3 CIN in the first chemotherapy cycle, but no significant differences were observed in FN incidence during cycles 2-4 or overall between the two groups. Pharmacoeconomic analysis revealed no significant differences in the number of additional short-acting granulocyte colony-stimulating factor (G-CSF) injection or blood test frequency. However, the hospitalization costs were significantly reduced in the mecapegfilgrastim group. Subgroup analysis by chemotherapy regimen risk showed that fatigue was more prevalent in the efbemalenograstim alfa group in the high-risk regimen subgroup, while hospitalization costs were consistently lower in the mecapegfilgrastim group. For high-risk regimens, efbemalenograstim alfa demonstrated superior efficacy in the first cycle, whereas mecapegfilgrastim performed better in cycles 2-4. For medium-risk regimens, the efficacy of the two agents was comparable. Conclusion Efbemalenograstim alfa has a faster initial onset of action (especially in high-risk regimens), but it is associated with more adverse effects such as fatigue. Mecapegfilgrastim offers better long-term safety and lower hospitalization costs.

A case report of anti-glutamic acid decarboxylase antibody-associated encephalitis presenting with stroke-like symptoms as the initial manifestation
LI Liang, ZHANG Qinli, ZHANG Yaqian, PANG Jin, YU Lining
    2026, 54 (10):  1104-1109.  doi: 10.11958/20260849
    Abstract ( 45 )   HTML ( 4 )   PDF (1186KB) ( 51 )

Anti-glutamic acid decarboxylase antibody-associated encephalitis is a rare and easily misdiagnosed autoimmune encephalitis with diverse clinical manifestations and lack of specificity. Currently, there are few clinical case reports related to this disease. Anti-glutamic acid decarboxylase antibody-associated encephalitis presenting with stroke-like symptoms as the initial manifestation is even rarer. This article presents a case of a 74-year-old female patient with acute stroke-like symptoms (slurred speech, weakness in the right limb) as the initial manifestation. She has a history of atrial fibrillation, hypertension and cerebral infarction. From the perspectives of clinical symptoms, imaging examination, laboratory tests and excluding other causes, analyses of anti-glutamic acid decarboxylase antibody-associated encephalitis suggest that for patients with stroke-like acute focal neurological deficits and atypical imaging (extramarginal lobe system), the possibility of autoimmune encephalitis should be considered, and antibody detection and immunotherapy should be performed as soon as possible.

    2026, 54 (10):  1110-1114.  doi: 10.11958/20260765
    Abstract ( 53 )   HTML ( 5 )   PDF (818KB) ( 55 )

Constipation is a common chronic functional or organic intestinal disease in clinical practice with significant heterogeneity in its etiology, and it has become an important factor contributing to the global disease burden. Traditional etiological studies are easily affected by confounding factors, reverse causality and clinical feasibility, making it difficult to accurately reveal the true causal relationship between risk factors and constipation. Mendelian randomization (MR), which uses genetic variations as instrumental variables, provides a new research idea for exploring the etiology of constipation. This article reviews the potential causal relationship between MR and constipation from aspects such as lifestyle, association with multiple system diseases, intestinal flora and circulating metabolites.

Research progress on Schlemm canal and trabecular meshwork changes in myopic patients
HAO Qian, LI Ju, HE Ziye, FAN Fang
    2026, 54 (10):  1115-1120.  doi: 10.11958/20260419
    Abstract ( 44 )   HTML ( 4 )   PDF (793KB) ( 49 )

Schlemm canal (SC) and trabecular meshwork (TM) are the anatomical and functional basis for achieving the function of aqueous humor outflow. The article focuses on analyzing the morphological remodeling characteristics of "large cavity and thin mesh" presented by both under the state of myopia, especially high myopia. Further, the potential mechanisms of the above structural changes are expounded from three aspects: biomechanical mechanism, mechanical traction and inflammatory microenvironment, and the clinical associations between the changes in the anterior chamber angle microstructure and the increased risk of primary open-angle glaucoma as well as the phenotype of "low intraocular pressure glaucoma" are revealed. This review aims to integrate current imaging, histological and biomechanical research evidence, clarify the pathological characteristics of myopia-related aqueous humor outflow channels, promote the establishment of myopia-specific imaging assessment standards and more rigorous individualized intraocular pressure management strategies, and thereby provide a theoretical basis for the early identification and intervention of glaucoma risk in the population with high myopia.
