    A comparative study on the efficacy and safety of intravenous lanatoside C, dilti- azem and amiodarone in patients with atrial fibrillation of rapid ventricular rate

    Abstract 0bjective: To evaluate the efficacy and safety of intravenous lanatoside C, diltiazem and amiodarone for controlling rapid ventricular rate in patients with atrial fibrillation(AF) in gerontal patient.Methods: 97 AF elderly patients(over 60 year old) with rapid atrial ventricular rate(≥120 beats/min) were randomly divided into 3 groups, the lanatoside C group(30 cases), group diltiazem(32 subjects) and group amiodarone (35 patients).The heart rate, blood pressure, rhythm were recorded before treatment, at 5, 10, 15, 30, 60 and 90 min after treatment.The reacting time,side effects including hypotension, bradyeardia, nausea, vomiting, dizziness, etc, were analyzed. Results: lanatoside C, diltiazem and amiodarone are equally safe and effective on controlling rapid ventricular rate in elderly patients with atrial fibrillation and total effective rate were 75%,90% and 85%. With the mean time response to the bolus was(35.4±15.7)min,(9.7±3.6)min and(18.8±7.6)min, respectively.In lanatoside C group , sinus rhythm was restored in 6 patients and asymptomatic hypotension developed in 1 patient. In diltiazem group, sinus rhythm was restored in 5 patients, asymptomatic hypotension developed in 2 patients and sinus bradycardia developed in 1 patient. In amiodarone group, sinus rhythm was restored in 8 patients and sinus bradycardia developed in 1 patient. All events were resolve without pharmacologic treatment. No patient developed worsening of congestive heart fililure due to lanatoside C, diltiazem and amiodarone. Conclusion: compared with lanatoside C, diltiazem and amiodarone are safe and effective in acutely 1owing of rapid ventricular rate in patients with atrial fibrillation. but diltiazem is more rapid and amiodarone is relatively safe in controlling AF with rapid ventricular response.

    Effect of Silence of the PIM-1 Expression on Proliferation of Prostate Cancer Cell Line

    Abstract Objective: To investigate the effect of RNAi-mediated silencing of PIM-1 on the proliferation of prostate cancer cell. Methods: The recombinant plasmid vectors of RNAi were transfected into PC-3 cell line in vitro and the cell-counting assay were used to determine the proliferation activity of the stably-transfected cells. Flow cytometer(FCM) were used to detect the influence of the silence of PIM-1 on cell cycle and apoptosis in PC-3 cells. Results: Compared with the contrast groups, the ability of proliferation of the PC-3 cell line with silenced PIM-1 gene was suppressed remarkably. The percentage of the cells in G1 stage increased, while which in S stage decreased. At the same time, much more apoptosis cells were observed. Conclusion: Silence of PIM-1 gene can result in the block of the cell cycle, suppression of cell proliferation and increasing apoptosis in PC-3 cells. PIM-1 may serve as a effective target of gene therapy for prostate cancer in clinical practice .

    Effects of Rheumatoid Synovial Fibroblasts on Biological Properties of T cells

    Objective: To investigate the effect of rheumatoid synovial fibroblast on biological properties of T cell. Methods: The sample of synovial tissue was obtained from RA patient during joint replacement surgery. The synovial fibroblasts were extracted by enzyme. The cells of the third passage were co-cultured with Jurkat T cells under the same concentration. MTT method was used to detect the proliferation ability of the co-culture T cells. Flow Cytometer was used to detect the apoptosis of the co-cultured T cells. Real-Time PCR was used to detect the expression of IL-2 mRNA and IL-23p19mRNA. All of the results were compared with the group of T cells alone, respectively. Results: The fast-proliferation phase of the co-cultured T cells had been observed during the previous four days, and reached the proliferation platform at the 7th day, while that of the T cells alone group was observed during the 7th day to the 10th day, and reached the proliferation platform at the 13th day. The proliferation rate was significantly higher in the co-cultured group than that of T cells alone group at the 4th day and the 7th day respectively (P<0.001). The proliferation rate was significantly lower in the co-cultured group than that of T cells alone group at the 10th day and the 13th day respectively (P<0.001). RA synovial fibroblasts co-cultured with T cells inhibited the apoptosis of the T cells (P<0.005). The expression of IL-2mRNA and IL-23p19mRNA in T cells were significantly increased by RA fibroblasts. Conclusion: RA fibroblasts have the potential effect on activating the co-cultured T cells. This may be one of the factors involved in RA pathogenesis and progression.

    Gene Diagnosis of Spinal Muscular Atrophy Using MLPA

    Abstract Objective To perform gene diagnosis for 30 suspected patients of spinal muscular atrophy (SMA) using multiplex ligation dependent probe amplification (MLPA) and PCR-restriction fragment length polymorphism (PCR-RFLP), and compare the results of the two methods. Method Genomic DNA was isolated from peripheral blood of each subject from 30 families using salting-out method.DNA concentration was determined by nucleic acid quantitative instrument. Exon 7 and 8 of SMN gene was amplified by allele specific PCR.The PCR products were digested with DraⅠand DdeⅠ, detected by agarose gel electrophoresis. Simultaneously,the DNA samples were analyzed by SALSA MLPA KIT P021. Results Both PCR-RFLP and MLPA analysis showed the same that 22 patients with exon 7 and 8 homozygous deletion, and 2 patients with only exon 7 homozygous deletion of SMN1. The other 6 cases and parents presented no homozygous deletion by PCR-RFLP, but one child and two morthers of them were detected heterozygous by MLPA. Also MLPA analysis found three “2+0”carriers from 3 families. The data also showed that the SMN2 copy numbers were mainly 4 or 5 in SMA patients, while the carriers and the normal individuals were 2 or 3 and 1 or 2 copies respectively. There were clear statistical significance in the groups of patient -carrier and patient-normal individuals ( p<0.001). The carrier -normal group appeared no statistical significance( p>0.05). Conclution Compared with PCR-RFLP, MLPA is more convenient, precise, high-effective, and it can accuratly quantitative SMN1 and SMN2, so it is a kind of technique of gene diagnosis for common genetic disease .

    The Effect of Arsenic Trioxide(AS203) in Conjunction with Hyper- thermia on Proliferation and Cell Cycle in human esophageal carcinoma EC-1cell line

    [Abstract] Objective: To study the effect of arsenic trioxide(AS203) combined with hyperthermia on the growth of esophageal carcinoma EC-1 cell line and to discuss its possible mechanism. Methods: In every group ,the percentage of survival was calculated with MTT assay, the apoptosis and the cell cycle were observed by flow cytometry and TUNEL. Results: AS203 combined with hyperthermia could obviously inhibit cell proliferation and induce apoptosis as compared with AS203 or hyperthermia.The Flow cytometry assay showed that AS203 combined with hyperthermia could induced G2/M phase arrest and the reduction of the ratio of cells in G0/G1 and S phases expectively. Conclusion: AS203 in combination with hyperthermia can produce synergistic killing effect on EC-1 cell. This may be related with the increase of apoptosis and cell cycle specific killing combined with cell cycle regulator.

    The changes and clinical significance of inflammatory cytokines in saliva and peripheral blood of patients with acute radiation-induced stomatitis

    [Abstract] Purpose: To investigate the changes and clinical significance of IFN-g、TNF-α、IL-2、IL-4、IL-6、IL-10 in the saliva and peripheral blood of the patients with nasopharyngeal carcinoma undergoing radical radiotherapy. Methods: At three doses of radiotherapy, including DT=0 Gy, DT=40 Gy and DT= 70 Gy, six kinds of inflammatory cytokines were examined in the saliva and peripheral blood of 40 patients with nasopharyngeal carcinoma undergoing radical radiotherapy. Results: (1)The levels of the six cytokines in saliva were remarkably higher than those in peripheral blood(P=0.000) at three doses of radiotherapy. (2)The levels of the 6 cytokines in both saliva and peripheral blood at DT=40Gy increased remarkably compared with those at DT=0 Gy (P(IL-10 in blood)=0.003, P=0.000). (3) The level of IL-10 increased in the saliva and peripheral blood at DT=70 Gy, whereas the levels of IFN-g、TNF-α、IL-2、IL-6 decreased compared with those at DT=0 Gy (P=0.000). The level of IL-4 in the saliva at DT=70 Gy was lower than that at DT=0 Gy (P=0.000), .Conclusion: On the progress of acute radiation-induced stomatitis of the patients with nasopharyngeal carcinoma undergoing radical radiotherapy, the levels of IFN-g、TNF-α、IL-2、IL-4、IL-6、IL-10 in the saliva were remarkably higher than those in peripheral blood. Except IL-4, the change of the five cytokines was similar in saliva and blood, which were associated with the pathogenesis of stomatitis.

    Construction and identification of lentiviral vector of RNA interference of ER? gene

    AIM: To construct a lentiviral vector of RNA interference (RNAi) of ERα gene and transfect them into HEK-293T cells to detect the titer of virus. METHODS: The effective sequence of siRNA targeting ERα gene was confirmed in our previous study. The complementary DNA containing both sense and antisense Oligo DNA of the targeting sequence was designed, synthesized and cloned into the pGCSIL-GFP vector, which contained U6 promoter and green fluorescent protein (GFP). The resulting lentiviral vector containing ERα?shRNA was named shERα-LV, and it was confirmed by PCR and sequencing. HEK-293T cells were cotransfected with lentiviral vector shERα-LV,pHelper 1.0 and pHelper 2.0. All virus stocks were produced by LipofectamineTM 2000 transfection. The titer of virus was tested according to the expression level of GFP. CONCLUSION: PCR and DNA sequencing demonstrated that shERα-LV producing ERα-shRNA was constructed successfully. The titer of concentrated virus was 2E+8TU/ml. DISCUSSION: The lentivirus RNAi vector of ERα?was constructed successfully. Our work provides the basis for ERα signal transduction pathway in breast cancer.

    Effects of androgen on PIP mRNA expression in human prostate cancer cell line

    Objective: The purpose of this study was to determine prolactin-inducible protein (PIP) mRNA expression, and effects of androgen in human prostate cancer cell lines. Methods: By different concentrations and different times of DHT or T, PIP and AR mRNA expression in prostate and breast tumor cell lines (LNCaP, PC-3, DU145 and T47D) were determined by RT-PCR and FQ RT-PCR. PIP protein expression in each cell lines was determined by Western blot. Results: By RT-PCR and western blot, PIP mRNA and protein expression was detected in androgen-dependent PCa cell line LNCaP. PIP mRNA expression was increase along with concentration of DHT or T in LNCaP. PIP mRNA level reach maximum peak in 10 nmol/L DHT or T. AR mRNA expression was unchanged treated by different concentration of DHT or T in LNCaP cell lines. Conclusions: The role of PIP may occur in the process of development of prostate cancer, and differentialdiagnosis prostate cancer whether or not androgen dependence.

    Use of Exhaled Nitric Oxide and the association with the Response to Inhaled Corticosteroids for Chronic Cough

    OBJECTIVE: To evaluate our experience with patients who presented with chronic cough and how exhaled nitric oxide predicted response to inhaled corticosteroid (ICS) therapy. METHODS: This retrospective observational study of 114 patients evaluated for chronic cough with measured exhaled nitric oxide and bronchodilatation test was conducted from Jan 1, 2010, through FENO Dec 31, 2010. Clinical records were extracted. Patients with FENO documented follow-up were contacted by telephone and administered a questionnaire. RESULTS: In 62 patients, ICS therapy was started or the current ICS dose increased. There were no difference in age, gender, body mass index (BMI), and FEV1%predicted, et al. Forty patients had elevated exhaled nitric oxide levels (48.13±15.66ppb), which were significantly higher than those of the others. Twenty-two patients with exhaled nitric oxide levels in the reference range were also prescribed ICS, and only 2 had cough improvement. Patients had documented followup that ranged from 4 weeks to 16 months. A cutoff of 34 ppb was found to best differentiate ICS responders and FENO responders. CONCLUSION: Measurement of exhaled nitric oxide accurately predicted response to ICS therapy for chronic cough. Patients with a positive exhaled nitric oxide test result had a strong likelihood of response to ICS, whereas a negative exhaled nitric oxide test result indicated an unlikely response to ICS. This finding may potentially have an impact on how patients with chronic cough are evaluated and treated.

    The Study of Relationship between Plasma Vaspin and Metabolism in Patients of Type 2 Diabetes Mellitus with Nonalcoholic Fatty Liver Disease

    Abstract Objective: To investigate the relationship between the serum vaspin and the metabolism of substance and energy in patients of type2 diabetes mellitus(T2DM) with nonalcoholic fatty liver disease (NAFLD). Methods: The level of the serum vaspin was assayed by enzyme-linked immunosorbent assay (ELISA) in all subjects, patients of T2DM with NAFLD (group A), T2DM without NAFLD(group B)and nomal group(group NC).The indexs of energy metabolism were detected by indirect calorimetry,meanwhile,detect the relating clinical datas . Results: (1)The levels of serum vaspin were significantly increased in group B compared with that of group A and NC(P <0.01), in group A and B, the respiratory quotient (RQ) and oxidation of carbohydrate were significantly lower than in group NC,while the 24 hr urine nitrogen、real resting energy expenditure (REE)、REE/ body wheigt and the oxidation of fat significant higher than the group NC(P < 0.05或P<0.01),but there is no different between group A and B. The level of serum vaspin was positively correlated with body weigt ,body mass index(BMI),REE and sex,meanwhile, it was negatively correlated with respiratory quotient (RQ) in group A; The level of serum vaspin was positively correlated with BMI、waist-to-hip ratio(WHR)、fasting blood glucose (FBG)、fasting insulin(FINS)、homeostasis model assessment of insulin resistance(HOMA-IR)、real REE、real REE/body wheigt and TG, while it was negatively correlated with RQ in group B.The concentration of vaspin is 92.4μg/L could predict the risk of type 2 diabetes mellitus with fatty liver. Conclusion: The level of serum vaspin closely related with the metablism of substance and energy, and vaspin could be use to predict the risk of type 2 diabetes mellitus with fatty liver.

    Study of islet βcell function in obese children with nonalcoholic fatty liver disease.

    Abstract: Objectives To investigate islet βcell function in obese children with nonalcoholic fatty liver disease . Methods Three hundred and sixty-two obese children and one hundred and sixty-four normal children were enrolled in the study form June 2007 to April 2011. 170 of the obese children accompanied with NAFLD , 192 simple obese children without NAFLD. Triglycerides、cholesterol、fasting plasma glucose(FPG)、fasting insulin(FINS)、HbA1c were assayed in these children. Oral glucose tolerance test and insulin releasing test were performed in all obese children. The homeostasis model assessment – insulin resistance(HOMA-IR) ,insulin sensitivity index (QUICKI) and β-cell function index (HOMA-β) were calculated. Results FPG、FINS、HbA1c、HOMA-β and HOMA-IR in NAFL group were significantly higher than the other two groups(P<0.05); QUICKI in NAFLD group was significantly lower than the other two groups (P<0.05). The relevance ratio of abnormal glucose metabolism in NAFLD group was significantly higer than without NAFLD group(P<0.05). Obese children with nonalcoholic fatty liver disease existed impaired function of islet β-cell and glucolipid metabolism disorders. The impaired function of islet β-cell could be one of the pathogenesis for nonalcoholic fatty liver disease.

    Relationship Between Blood lipids levels and weight of newborns in pregnant women with Gestational Diabetes Mellitus

    Objective:To study the influences of blood lipid levels in gestational diabetes mellitus ( GDM ) patients to neonatal birth weight and Provide clinical reference for reasonable management of pregnant women with gestational diabetes mellitus Methods: 106 cases of pregnant women according to the American Diabetes Association (ADA) standards were divided into gestational diabetes (GDM, n = 50) group, gestational impaired glucose tolerance (GIGT, n = 24) group and normal glucose tolerance (NGT, n = 32) 3 groups. Recorded the group of pregnant women lipid levels, weight gain during pregnancy and the group of perinatal data. Results: TC and TG levels on GDM group were higher than the GIGT and the NGT group, the difference was statistically significant (P <0.01). The average weight of pregnant women of three groups before delivery was different, that in GDM group were higher than other two groups. Compared with the other two groups, neonatal birth weight, meconium-stained and the probability into the intensive care room in GDM group were significantly higher than the other two groups, the difference was statistically significant; TG level and growth of body mass during pregnancy were significantly positive correlated to neonatal body weight.( r=0.528,0.632,0.585,P <0.05). Conclusion: Controling lipids level positively on the basis of control blood glucose will reducte fast growth of neonatal body weight、even the happen of macrosomia , thus reduce risk of perinatal complications, ensure safe motherhood.

    The Expression of Thrombospondin-1 and Epidermal Growth Factor Receptor in the Lung of Rats Exposed to Hypoxia

    Abstract Objective To test the role of thrombospondin-1(TSP-1) and epidermal growth factor receptor (EGFR) in the development of hypoxic pulmonary hypertension. Methods Twenty male Wistar rats were divided into two groups and exposed to air and isobaric hypoxia for 3 weeks, respectively. The pulmonary artery pressure was measured by right cardiac catheterization. The expression of TSP-1 and EGFR in the lungs of rats were measured by immunohistochemical staining and in situ hybridization. The histological sections of the lungs were examined using a computerized image analyzer. Results The pulmonary artery pressure in the exposed rats increased. The chronic hypoxia also elicited the thicking of the wall and the narrowing of the lumen of pulmonary arterioles. It led to the increases of the pulmonary artery pressure 、the index of righr ventricular hypertrophy ﹝RV/(LV+S)﹞、 the ratio of vascular wall thickness/vascular external diameter(WA%) and the ratio of vascular wall area/total vascular area(WT%) compared to the controls (PAP2.86±0.39kP vs 1.35±0.28kP;﹝RV/(LV+S): 43.53±3.38 vs 23.68±3.48; WT%:35.24±11.2 vs 23.63±9.74; WA%:55.09±12.38 vs 41.62±12.83 respectively, p<0.05). The positive staining of TSP-1 protein and TSP-1mRNA in the wall of pulmonary arteriole of the exposed rats were significantly stronger than those of controls (P<0.01), so as that of EGFR. Conclusions The TSP-1 and EGFR may play an important role in the pathogenesis process of hypoxic pulmonary vascular remodeling and pulmonary hypertension.

    Abstract Objective: To investigate the relationship between alcohol and Pulmonary fibrosis and possible mechanism by observing the effect of chronic alcohol intake on lung tissues in rats and tissue inhibitor of metalloproteinase-1 glutathione. Methods: Clean healthy male Sprague-Dawley rats 20, weighing 160-200g, were randomly dividedinto control group (C group) 10 and alcohol group (A group) 10, and fed quantitative non-ethanol or ethanol Lieber-DeCarli liquid diet every day in a single cage for 16 weeks. By Masson staining collagen deposition were observed in alveolar interval ; t ere detected.Results: ⑴Masson results show: the alcohol group had more deposition of collagenfibers at intervals of alveolar than the control group.⑵Compared to the group C, the content of GSH in rat lung tissue reduced but HYP increased in group A ,the differences were statistically significant ( P <0.05).⑶TIMP-1 in rat lung tissue of group A were higher than group C, the differences were statistically significant(P <0.05). Conclusion: Chronic alcohol ingestion can cause collagen deposition in alveolar interval and higher expression of TIMP-1 in lung tissue . Alcohol may be one of the reasons for pulmonary fibrosis in rats.

    The effects of tea polyphenols on oxidative stress level, airway inflammation and airway remodeling in asthmatic rats

    Abstract Objective: To observe the effects of tea polyphenols (TP) on oxidative stress level, airway inflammation and airway remodeling in asthmatic rats of early stage and late stage. Methods :SD rats were sensitized and challenged repeatedly by ovalbumin to establish chronic asthma airway remodeling. 48 rats were randomly divided into 6 groups, a normal saline group, an asthma group, an early budesonide group (early BUD group), a late budesonide group (late BUD group), an early TP group and a late TP group . Drugs were administrated 2 weeks before sensitizing in early BUD group and early TP group, while being administrated 5 weeks after sensitizing in late BUD group and late TP group. The lung tissues were harvested from the rats 12 weeks after sensitizing. Smooth muscle area, collagen deposition area were assessed and the expressions of transforming growth factor--?1(TGF-?1)in bronchi and lung tissues were observed by immunohistochemistry. The contents of TGF-?1 and malondialdehyde (MDA) were tested as well as superoxide dismutase (SOD) vitalities were measured. Results: The indicators of airway remodeling significantly improved in the early TP group, late TP group and early BUD group as compared with those in asthma group(P<0.01 or P<0.05), the most obvious in early TP group( all P <0.01). No significant differences were found between late BUD group and asthma group (P>0.05). The asthma group also showed increased MDA content and decreased SOD vitality, both being significantly different from that in normal group (P<0.01). SOD vitality increased and MDA content decreased in each therapeutic group after intervention. SOD vitality had a negative correlation with MDA content. In addition, a positive correlation between collagen deposition area of bronchial wall and MDA content was also found. Conclusion: Tea polyphenols intervention could inhibit or delay the formation and development of airway remodeling maybe by controlling the level of oxidative stress and downregulating airway inflammation in rats. Early drug intervention by Tea polyphenols or BUD could inhibit airway remodeling. Tea polyphenols may delay airway remodeling better than BUD in late stage in asthmatic rats.

    【Abstract】 Objective To investigate the suppressive effect of histone deacetylase and DNA methyltransferase inhibitors on the malignant phenotype of human glioma cells by impacting on the epigenetic mechanism.Methods VPA (Valproic acid) was used as histone deacetylase inhibitor and Aza (5-aza-deoxycytidine) as DNA methyltransferase inhibitor. For in vitro study, U251 glioma cells were divided into four groups: the control group, VPA treatment group, Aza treatment group and the VPA+Aza combined treatment group. The proliferation activity of tumor cells was detected by MTT assay, cell cycle analysis by flow cytometry, apoptosis rate by Annexin V-FITC assay and the invasion ability of glioma cells by 2D Matrigel, 3D Matrigel, and transwell assay. The subcutaneous U251 tumor model was established in nude mice for in vivo study, xenografts were treated with VPA, Aza and VPA+Aza by multi-point local injection into tumor bed every 4 days within the observation period for 28 days and the gross tumor volume was measured. Results Compared with the control group, the tumor cell proliferation activity in VPA , Aza and VPA + Aza combined treatment group, was significantly lowered(P <0.05 for each group), the cells were arrested in G0/G1 phase, apoptotic cell rate was increased and cell invasion ability decreased significantly (P <0.05). The tumor volume in all treatment groups was much smaller than that in control group. The most significant effect on inhibition of malignant biological behaviour of glioma cells was observed in VPA + Aza combined treatment group. Conclusion To treat glioma cells with DNA methyltransferase and histone deacetylase inhibitors can reverse the malignant phenotype by inhibition of their epigenetic alterations, So this study provides the evidence that both of them can be developed as candidates for glioma treatment.

    Heterogeneity of action potential and Ica-L in the left ventricular myocytes of rabbit and the acute electrophysiological effects of amiodarone

    Abstract Objective The heterogeneity of the action potential(AP),transmural dispersion of repolarization (TDR) and Ica-L ion current in left ventricular three layers myocytes were investigated in rabbit by using patch-clamp technique. And the acute electrophysiological effects of amiodarone on both AP and TDR were also studied. Methods Rabbits myocytes were isolated by O2 & CO2 mixed-gas combined with type –II collagenase enzymatic method.And the sub-endocardial (Endo) and sub-epicardium (Epi) tissues were separated from the other regions (midmyocardium,M).Separated ventricular myocytes were perfused with Tyrod’s solution and 10 umol/L amiodarone. And then the whole cell patch clamp technique was used to study the AP and Ica-L. Result 1. The action potential (AP) in the three myocardial layers were significantly different.The action potential duration (APD) in M cells was the longest, while the Endo cells were the shortest. The APD 90 of M cells was 488.13±22.88ms, and that of Endo and Epi cells were 326.36±14.51ms and 391.1±23ms, respectively (P<0.001). TDR was 185ms. The M cells had more pronounced spike-and-dome configuration,with significantly larger phase 1 magnitude and plateau voltage.2) After acute amiodarone perfusion, the APD 20, 50 and 90 in M cells were sharply shortened (P<0.003). On the contrary, Endo cells have a remarkably prolonged period of AP (P <0.0003). So the TDR obviously decreased. Amiodarone has no effect on AP configuration. 3) ICa-L of three myocardial layers was significance different in the current density. The current density of Epi and Endo cells were 6.21±1.35 and 7.20±0.83 pA/pF, respectively. The M cells had a larger current density, the value was 1.59±3.71pA/pF, P<0.05. The three layers myocytes had no significance change in the I-V curved and voltage-dependence. 4) Compared with the normal ventricular myocytes,the current density of ICa-L were all decreased in three layers myocytes perfused by 10μmol/L amiodarone,but the M cells were the significant ones, 11.59±3.71pA/pF vs 7.28±2.01 pA/pF, (P<0.05). And amiodarone had no effect the I-V curved and voltage-dependence of ICa-L. Conclusion 1) The action potential and the L-Ca ion currents in the three myocytes layers were significantly different.Action potential duration (APD) in M cells was longer than that in Epi or Endo cells.M cells had a more pronounced spike—and—dome configuration,with a significantly larger phase 1 magnitude an d plateau voltage.2) ICa-L in M cells were higher than those of Epi and Endo.The ICa-L in Epi and Endo were similar with each other.     Objective: To investigate the role of MitoKATP in rats with SAP and its mechanism of myocardial protection. Methods: 36 healthy SD rats were randomly divided into 4 groups: K group was given sham operation; M group was established SAP model in rats; Respectively consecutive intraperitoneal injection of DZ and DZ+ 5-HD for 3 days,D and H groups were made to SAP model too. Detected the levels of serum CK-MB and LDH at 24 hours after the operation.Applied Spectrophotometry to detect Na+-K+-ATPase activity of myocardial tissue.The alteration of mitochondrial membrane potential was detected by flow cytometry.The technique of TUNEL was used to detect myocardial apoptosis. Heart and pancreas was stored to perform histopathologic examination. Results: Compared with the D group,the levels of serum CK-MB and LDH,the heart pathological scores and the myocardial apoptosis index were higher in M group (P<0.01),while the Na+-K+-ATPase activity and mitochondrial membrane potential decreased in M group(P<0.01)。But there were no significant difference with Mand H group(P>0.05). Conclusion: Opening MitoKATP Channel had myocardial protective effect in Rats with SAP.

    Detection and Clinical Study of Stem Cell Factor in Serum of Acute Asthmatic Patients

    Abstract Objective:To investigate the change and significance of stem cell factor(SCF)level in serum of patients with acute exacerbation of bronchial asthma,To explore the relationship of asthmatic patients with acute exacerbation SCF、T-IgE、FEV1 and FeNO. Methods:65 patients with acute exacerbation of bronchial asthma were divided into 3 groups with severity,37 normal subjects as control.The level of serum SCF 、EOS of 65 patients in each group were measured.The level of serum T-IgE, FEV1%and FeNO were tested quantitively.Results:Serum SCF levels in moderate and severe group were higher than that in control group and slight groups.(P<0.05). Serum SCF levels in asthmatic was higher than that in control group before treatment.(P<0.05). In asthmatic patients the level of SCF level did not correlate with EOS、T-IgE and FEV1% of predicted value,but it corrlated positively with FeNO(r=0.447,P=0.004). Conclusion:The level of serum SCF maybe estimate accurately response airway inflammation changes,it maybe related to the severity of asthma attack.

    The clinical study of thrombus aspiration and different ways of giving tirofiban in Interventional treatment of acute myocardial infarction

    Abstract Objective To investigate the ST-segment elevation myocardial infarction (STEMI) patients in primary PCI, thrombus aspiration and different ways of giving tirofiban acute and long-term effects. Methods Patients were divided into three groups:1) simple thrombus aspiration group. 2) thrombus aspiration + tirofiban given by peripheral venous group, after thrombus aspiration to be the first 10μg.kg tirofiban bolus, followed by maintenance dose 0.15μg.kg-1.min-1 for 24 hours. 3) thrombus aspiration + tirofiban given by intra-infarction related artery group,after thrombus aspiration to inject tirofiban 500μg through the suction catheter , followed by maintenance dose 0.15ug.kg-1.min-1 for 24 hours. To observe the case of recanalization,preoperative and 90minutes after angioplasty ST-segment of ECG,peak of CK-MB, left ventricular ejection fraction 24 hours and 6 months after angioplasty. Follow-up of patients with bleeding and cardiovascular adverse events. Results The basic situation of three groups of patients was not statistically significant, forward flow reached TIMI 3 level after thrombus aspiration.The case of ST-segment after 90 minutes: group 2) and group 3) were significantly different from group 1). And the difference between group 2) and group 3)was not obvious. Peak of CK-MB:group 2) and group 3) were significantly different from group 1). And the difference between group 2) and group 3) was significantly different too.24 hours after angioplasty the left ventricular ejection fraction of group 3) was higher than group 2) and group 1), there was statistically significant between group 2) and group 1) (p <0.05 ). The left ventricular ejection fraction of three groups showed no significant difference after 6 months . Three groups of patients during follow-up were not bleeding and cardiovascular adverse events. Conclusion Primary PCI in acute ST-segment elevation myocardial infarction , thrombus aspiration combined tirofiban is better than a simple thrombus aspirationin in protecting left ventricular function and improving myocardial perfusion in acute phase , Within the intra-infarction related artery is better than the surrounding intravenous administration. Thrombus aspiration and different ways of giving tirofiban had no difference in long-term impact of left ventricular function.

    Superior minor median sternotomy aortic valve replacement in 35 patients

    【Abstract】Objective To summarize the experience of superior minor median sternotomy aortic valve replacement surgery. Methods From 2009.3 to 2011.11, 35 patients underwent superior minor median sternotomy aortic valve replacement operation were collected retrospectively. 13 patients were performed with the extracorporeal circulation through ascending aorta and right atrium, other 22 patients with extracorporeal circulation through femoral artery and femoral vein。Except for 10 patients, all the others got vacuum-assisted venous drainage(VAVD). 7 cases got drainage through pulmonary artery. Results 1 patients died of cerebral infarction on the 30th day after surgery. Delayed wound healing appeared in 2 patients. Cardiopulmonary bypass time was 92.3±15.6 min, aortic cross clamping time was 48.6±19.2 min, duration of postoperative mechanical ventilation was 8.6±1.5 hours, and postoperative hospital stay was 9.5±3.6 days (range, 8 days-16 days). Conclusions Superior minor median sternotomy aortic valve replacement surgery could keep integrity of the sternum. Compared with the traditional midsternalthoractomy, the surgery could offer quick recovery and excellent cosmetic effect to some patients. 【Key words】Superior minor median sternotomy; open heart surgery;Cardiac Surgery

    Objective Evaluation on regular treatment (such as diuretics, angiotensin converting enzyme inhibitor and Digitalis) inefficacy of severe Decompensated congestive heart failure in elderly patients, intravenous injection of Levosimendan (Levosimendan Injection) the effectiveness and safety. Methods General correction of 40 cases of severe congestive heart failure in the elderly patients with heart failure therapy no better randomly divided into Group (20 cases, granting of Levosimendan therapy) and a control group (20 cases, give the metres of milrinone therapy). Observation on the treatment of the two groups, BNP and hemodynamic changes before and after clinical symptoms. Results Comparison of the two groups, after treatment, Levosimendan group BNP loss, difficulty in breathing in patients with improved insight group is more notable than milrinone (p< 0.01), and blood pressure decrease is more pronounced in milrinone Group (p﹤0.01). Between difference in improving the symptoms of heart failure has a significant (p ﹤0.01).. Conclusion: Levosimendan in treatment efficacy and safety are superior to intravenous milrinone in patients with severe heart failure.

    Abstract Objective: To discuss clinical efficacy and safety of low dose fentanyl added to ropivacaine in subarachnoid block for pregnancy hypertension syndrome patients of cesarean section. Methods: Eighty pregnancy hypertension syndrome patients were randomly allocated to two groups of A、B,with 40 cases for each group. The patients of group A received spinal anesthesia with small dose of ropivacaine combined with fentanyl. The patients of group B received epidural anesthesia. Results: The number of hypotension(D)、apgar score after delivery of the neonate 3min and the adverse reaction rate of postoperative nausea, vomiting, back pain, headaches have no significant differences in two groups(P>0.05). The score of anesthesia effect and the time from the beginning of anesthesia to the operation start ( T )of group A were all less than group B ( P < 0.05 ). Conclusion: In this clinical observation, not only the anesthesia effect of small dose of ropivacaine combined with fentanyl for spinal anesthesia in pregnancy hypertension syndrome patient during cesarean section is better than epidural anesthesia block, but also the time of anesthesia begain to the operation start can effectively shorten, and the adverse reactions of hypotension have no obvious difference, so, this method is feasible and safe.
