    The Relationship Between Visfatin Level and Insulin Resistant Degree In Gestational Diabetic Patients

    To measure the serum visfatin level in gestational diabetic patients and study the correlation between visfatin level and insulin resistant degree. Methods: According to the ADA standard, 106 pregnant women were divided into three groups. Gestational diabetes mellitus group (GDM, n=50), the group with gestational impaired glucose tolerance (GIGT, n=24), the control group with normal glucose tolerance (NGT, n=32). Recorded the clinical and perinatal data. Serum visfatin levels were measured by ELISA method and insulin levels were measured bychemiluminescence. Analyze the difference among these groups and study the relationship between visfatin and body mass index, , insulin-resistant degree. Results: The serum visfatin level in GDM group(84.04±22.15 ng/ml)was higher than GIGT group(65.71±13.38 ng/ml) and NGT group (56.11±10.39 ng/ml)(F=26.562, P=0.001)。There were also difference in serum insulin level among three groups, the value in GDM, GIGT and NGT group was 27.91±10.31mIu/L, 26.94±6.69mIu/L and 22.32±3.37mIu/L(F=4.984,P=0.009). Serum visfatin level was positively related with insulin level and HOMA-IR (r=0.636, 0.738, all P= 0.001).There were also relationship between visfatin level and TC, TG, HbA1C value. Conclusion: High-expressed visfatin increases the insulin-resistance in pregnant women. Visfatin participates the metabolic disturbance in glucose and lipid, this maybe a mechanism of GDM occurrence.

    Analysis to food allergy patients sIgG detection results in Tianjin

    Objective: To discuss the relationships between food intolerance and allergenic dermatosis by determination of food allergen specific IgG (sIgG) in the serum of food allergy patients. Methods: Enzyme linked immunosorbent assay (ELISA) was used to detect sIgG for 834 cases of suspect food hypersensitivity disease patients, characteristic of the kinds of food allergens was analyzed. Results: 638 patients were positive in 1~11 kinds of food allergen sIgG, and the total positive rate was 76.5%, among which the cases of intolerance to 1 or 2 kinds of food occupied the most proportion and had a rate of 58.15%.When refer to the 14 kinds of food allergen sIgG in this investigation,the positive rate of egg white/egg yolk was highest,account for 51.9%,and followed by crab (21.6%), shrimp (19.3%), milk (16.5%) ,and the positive rate of pork was 0.5%, which was of lowest rate. Besides,the positive rate of egg, milk, crab, shrimp, wheat and tomato showed age differences (p<0.01). There were 171 patients among the 241 subjects suffered from dermatosis had positive results in food allergen test,and the positive rate was 71.0%. Conclusions: egg white/egg yolk, crustacea (crab, shrimp) and milk are the major food allergens in food allergic diseases. Some kinds of food have great level of intolerance, which is perhaps the main cause of food allergy. The symptoms of food allergy or food intolerance have close correlation with allergenic dermatosis.

    Effects of Ethyl Alcohol Intaking Quantity on Cardiac Structure and Function and on Oxidative Stress

    Objective : Discussing the effects of ethyl alcohol intaking quantity on cardiac structure and function and the effects of oxidative stress on cardiac muscle damage lead by ethyl alcohol. Method: investigating the people who take the physical examination in affiliated hospital of North China Coal Medical University and the Tangshan Steel and Iron Group Limited Liability Company Hospital from March, 2009 to November, 2009. 228 people have answered the questionnaire. The respondents are divided into 4 groups according to the different ethyl alcohol quantity per day: The small quantity of alcohol group, the median quantity of alcohol group, large quantity of alcohol group and zero quantity of alcohol group. Each group takes the echocardiogram test and the determination blood B urine sodium peptide (BNP) in order to examine the cardiac structure and the function. Simultaneously determines in the blood serum the third dial (MDA), the hyperoxide mutase (SOD), the metal sulfur protein (MT) oxidized stress targets and so on. Results: 1、the left room contraction last stage inside diameter (LVSD), the left room diastole last stage inside diameter (LVDD) and the blood serum BNP density of people in large and median quantity of alcohol group improve compared to people in zero quantity of alcohol group.But the left room shoots the menstruation number (EF%), the left room minor axis contraction (FS%), the E/A ratio reduces.LVSD, LVDD and the blood serum BNP density of people in large quantity of alcohol improve compared to people in median quantity of alcohol. But EF, FS, the E/A ratio reduces .LVSD, LVDD,EF, FS, the E/A ratio and the blood serum BNP density of small quantity of alcohol do not change a lot compared to zero quantity of alcohol.2 Regarding blood serum MDA, median and large quantity of alcohol group contain higher than zero and small quantity of alcohol group. 2、Regarding blood serum MDA, median and large quantity of alcohol group contain higher than zero and small quantity of alcohol group. large quantity of alcohol group contain higher than the median quantity of alcohol group, while the small quantity of alcohol group contains lower than zero quantity of alcohol group. Regarding blood serum SOD and MT, median and large quantity of alcohol group contain lower than zero and small quantity of alcohol group. Large quantity of alcohol group contains lower than median quantity of alcohol group, while small quantity of alcohol group contains higher than zero quantity of alcohol group. The difference has statistics significance (P<0.05).Conclusion: Median and large quantity of alcohol make the ventricle expand to a certain extent, the heart function drops, while the small quantity of alcohol doesn’t have any influence to the heart structure and function. The organism oxidation stress condition of those who take large and median quantity of alcohol is active, while taking small quantity of alcohol is possible to cause organism oxidation stress level to drop.

    The relationship between adiponectin gene SNP+45(T/G) polymorphism and 8-isoprostaglandin F2αin T2DM.

    Abstract Objective To investigate the relationship between adiponectin gene SNP +45 (T/G) polymorphism and oxidative stress in type 2 diabetes. Methods The SNP +45 (T/G) polymorphism of the selected 120 cases with T2DM patients was detected by PCR-RFLP. We detected serum 8-isoprostaglandinF2α concentrations by enzyme-linked immunosorbent assay method. Results There was no statistic difference of the serum 8-isoprostaglandinF2α concentration in adiponectin SNP45 site of GG, TG and TT genotype in type 2 diabetes. Conclusion There may be not significant relevance between adiponectin gene SNP +45 (T/G) polymorphism and oxidative stress in type 2 diabetes.

    Study on the injury with endothelial cells of the vascular in the patients with Graves.

    Abstract Objective Study on the change of the level of the endothelial progenitor cells and circulating endothelial cell, and investigate the damage and disorder mechanism of the vascular endothelial system in the patients with Graves. Methods The von Willebrand Factor antigen(vWF:Ag) was measured using the IL ACL-9000 blood coagulation instrument. The flow cytometry was used to measure the circulating endothelial cell(CEC)and endothelial progenitor cells(EPCs). The free-triiodothyronine(FT3)and free thyroxin(FT4)were determined using the ADVIA Centaur chemiluminesent immunoassay analyzer. Results Compared the patients group with the control, the level of the vWF:Ag and CEC were increased obviously and EPCs was decreased remarkably in the Graves patients group(P<0.01); CEC had no change(q=1.35,P>0.05), vWF:Ag(q=21.77) was increased and EPCs(q=8.89) was decreased siginficantly in the patients group(which had not combined with atrial fibrillation)(P<0.01). Compared the Graves patients group(which had not combined with atrial fibrillation) with the patients group (which had combined with atrial fibrillation), vWF:Ag and CEC were increased(q=19.92,q=16.05), EPCs was decreased significantly(q=3.89)(P<0.01) in the patients group (which had combinated atrial fibrillation). There was strong positive relationship between FT3 and vWF:Ag, and negative relationship between FT3 and EPCs(P<0.01) in the patients group(which had not combinated atrial fibrillation); there was no relationship in the patients group (which had combinated atrial fibrillation). Conclusion There was the damage and dysfunction in the Graves patients, the decrease of the level of the EPCs led the ability of the vascular repair to weakness. Lots of the endothelial cell shed from the vascular wall, and the EPCs was lower in the patients which combined with atrial fibrillation, it made the extent of the damaged vascular endothelial more serious and the repair became more difficult., and it brought the worse thrombosis risk.

    Preoperative Evaluation of MRI in the Central Bronchial Carcinoma With 125I Seeds Implantation

    Objective To establish MRI evaluation mode of the tumor (preoperative grouping)in the therapy with interstitial implantation of 125I seeds for central bronchial carcinoma before operation, and to estimate its clinical value. Methods 44 cases central bronchial carcinoma were randomly selected since 2005, which are treated with the particl e 125I therapy. All cases were performed by MRI. On the basis of the clinical requirment of selection of the implantation plan, orientation of the mass target, and the avoidance of the puncture risk, to summarize and analyse the mass signal, mass complications, bronchus abnormal features, metabasis of the hilar and mediastinal lymph nodes, mediastinal invasion, cardiac and vascular invasion, as well as chest wall involved. Results Most of them show oval or irregular shape, equal signal on T1WI, equal or high signal on T2WI. some of them, blood and necrosis in tumor can be found. Adjacent structures can be involved. Conclusion Preoperative grouping of the therapy with interstitial implantation of 125I seeds for central bronchial carcinoma is very valuable for the orientation of the mass target, localization of the puncuture, confirmation of the modus operandi, as well as making sure of the risk in the operation.

    【Abstract】 Objectives To investigate the role of estrogen and the mast cells in the pathogenesis of the interstitial cystitis patients,specifically with respect to activated bladder mast cells. Methods 18 cystoscopy biopsy specimens from patients with interstitial cystitis and 12 control subjects were analyzed blindly for the numbers of mast cells and the presence of estrogen receptors(ER). Immunohistochemical methods were used to detect ER and MC.And double immunohistochemical staining was used to manifest the expression of ER in interstitial cystitis MC. Results Bladder MC alone were shown to express high affinity ER,ER+cells in IC (3.77±3.03/HP) comparing with controls(0.88±0.76/HP)(t=3.870,P<0.01) and there was a higher number of MC present in patients with IC(5.59±3.40/HP)compared with controls(2.48±2.23/HP) (t=2.787,P<0.01).The unique finding that only MC express ER(R=0.884,P<0. 01),in spite of the inflammatory infiltrate present. Conclusions MC high affinity estrogen receptors may help to explain why interstitial cystitis occurs primarily in women,why IC and other associated symptoms tend to be worst at the midcycle. These results have shown that estrogen augment MC histamine secretion, which have a high prevalence in women such as IC.

    Effect of Off-pump Coronary Artery Bypass Surgery on Thyroid Function and Clinical Implications Among Patients Aged Over 80-year-old

    Objective: The aim of this study is to evaluate altered thyroid function after off-pump coronary artery bypass surgery among patients aged over 80-year-old, and to investigate the potential association between thyroid alterations and postoperative parameters. Methods: From 2005-6 to 2009-12, alterations in thyroid function were analyzed in 50 patients. The blood samples were taken on 1 day prior to surgery, upon arrival to ICU, and on postoperative day 1, 2, 5. Meanwhile, according to European System for Cardiac Operative Risk Evaluation, the patients were divided into high risk group or median and low risk group. The altered thyroid function and perioperative parameters were compared between two groups. Results: Among 50 patients studied, except free thyroxine increased transiently, other thyroid hormone decreased significantly. There were statistical differences between the groups in clinical data, including inotropic score, IABP assist, mean duration of ventilatory support and mean length of ICU stay. Additionally, 12 patients of high risk group exhibited significantly lower levels of total and free triiodothyronine as compared with 38 patients of median and low risk group; No significant differences in levels of total and free thyroxine were observed between two groups; Thyroid-stimulating hormone differed significantly between two groups, and the recovery in thyroid-stimulating hormone was observed in median and low risk group, as was recognized the initiation of re-establishment of thyroid hormones homeostasis from suppression. Conclusion: Our findings indicated that off-pump surgery could not prevent the onset of nonthyroid illness syndrome, and the low triiodothyronine in critically cardiosurgical patients was observed to correspond to the period when hemodynamic dysfunction occurred, which had important implications for prognosis and therapy after cardiac surgery.

    The effect of low dose fentanyl on EC50 of propofol for laryngeal mask insertion

    Abstract Objective:To investgat whether supplemental fentanyl 50μg decreased the effective concentration for 50% of the attempts to secure laryngeal mask insertion of propofol using a target controlled infusion technology. Method: eighty-four elective patients were divided into 2 groups by ages and then randomly assigned to four groups (n = 21 for each group) and given saline (control) or fentanyl 50μg. Propofol target concentration was determined and EC50 was calculated by a modification of Dixon's up-and-down method. Complication occurrence of circulation and respiration system and awareness were record during the insertion. Results: The EC50 of normal group without tentanyl was 4.03土0.21μg.ml-1,normal plus fentanyl 3.07土0.23μg.ml-1,and in geriatric group3.76土0.42μg.ml-1,geriatric plus fentanyl2.35土0.22μg.ml-1。The accurrence of circulation depression were comparable in four group,and no awareness happened in all patients, but in group geriatric plus fentanyl the accurence of respiration depression is more than the other groups. Conclusion: A fentanyl dose of 50μg is suffcient to decrease EC50 of propofol TCI to secure laryngeal mask insertion in normal and geriatric patients with minimum cardiovescular depression.     Study on the correlations of adipocyte fatty-acid binding protein and gestational diabetes mellitus

    Objective To study the expression of adipocyte fatty-acid binding protein FABP4 in placental tissues and serum of normal and diabetic pregnant women. The relationship among body weight, substances and the insulin resistance index was discussed,.Methods The expression of FABP4 was detected in serum and placental tissues of diabetic pregnant women and normal ones,which are respectively grouping to normal and overwight according to body weight,with the application of enzyme-lingked immunosorbent assay(ELISA). The fasting blood glucose,insulin levels, triglyeride and total cholesterol were determined separately. The indexes of insulin resistance, insulin sensitivity index, β-cell function, were calculated with homeostasis model (HOMA). All relevant information was statistical analyzed. Restult 1. The body mass index before pregnancy,fasting blood glucose,fasting insulin,triglyeride,total cholesterol,insulin resistance index of GDM normal group were higher than NGT normal group (P<0.05). While the insulin sensitivity index, β cell function of GDM group were lower than NGT normal group. In GDM pregnant women,BMI change value,FBG,FINS,triglycerides,total cholesterol,HOMAIR in the overweight group were higher than normal group (P<0.05),HOMAISI, HOMA-B% are lower than the normal group. 2.The differences among the four groups of the OD dates were significant(P<0.05). 3.Compared with the control normal group,positive cells of GDM normal is higher.The difference was statistically significant(Z=-3.312,P=0.002). The positive cells of GDM overweight is higher the GDM normal.The difference was statistically significant(Z=-2.663,P=0.013). Conclusion The adipocytokine- FABP4 may play an important role in the process of GDM.

    α-Lipoic acid Prevents the Decrease of MafA Gene Expression Induced by Elevated Glucose Concentrations in βTc6 Cell

    【Abstract】 Objective To study the impact of elevated glucose on the expression of insulin gene and insulin transcript facor MafA,and to clarify the mechanism of the protection of α-lipoic acid for β cell under oxidative stress. Methods βTc6 cells were cultured in DMEM, and then cultured for 48 h in media containing 5mmol/L glucose (NG group),16.7 mmol/L glucose (HG group),or 16.7mmol/L glucose and 200μmol/L α-lipoic acid (HG+ALA group) espectively. The insulin concentration in supernatant of cells cultured in different glucose medium was assayed by RIA. The mRNA expression levels of insulin and MafA was determined by RT-PCR. Results In HG group,insulin mRNA levels were decreased by 66.2%(P<0.05),and MafA mRNA levels decreased by 69.5%(P<0.05),as compared with NG group.In HG+ALA group,insulin mRNA levels were increased by 5.45 fold(P<0.05),and MafA mRNA levels by 4.49 fold(P<0.05) respectively,as compared with HG group. There was no significant difference in insulin and MafA mRNA levels between NG and HG + ALA group (P>0.05). Conclusions These data suggested that elevation in glucose concentrations led to a decrease in insulin and insulin transcript factor MafA mRNA levels in βTc6 cells . α-lipoic acid could attenuate this impact of elevated glucose through increasing the expression level of MafA.

    Two-dimensional Gel Electrophoresis Analysis for Effect of Folate on Protein Expression in neural stem cells

    Objective: To analyze the differentially expressed proteins of embryonic rat neural stem cells (NSCs) under folate deficiency or supplementation. Methods: Embryonic rat NSCs were cultured in serum-free suspension culture. Cells were identified by their expression of immunoreactive nestin. Individual cultures were assigned to one of 3 treatment groups that differed according to the concentration of folate in the medium: Control (baseline folate concentration of 4μg/mL), folate deficiency group (Folate-D, folate concentration of 0.6μg/mL) and folate supplementation (4μg/mL above baseline, Folate-L). Protein expressions in 3 groups were separated by two-dimensional electrophoresis (2-DE). Results: The stable and repeatable 2-DE maps of NSCs were successfully established. Compared with Control group, 2 proteins in Folate-D group were lowly expressed, 4 proteins were highly expressed. 2 protein spots were lowly expressed and 1 protein was highly expressed in Folate-L group. Conclusion: Folate deficiency or supplement may impact NSCs protein expression. The isolation of differentially expressed proteins will be useful for exploring the mechanism of folate on proliferation and differentiation of NSCs.

    The Therapeutical Effect of Estrogen on Experimental autoimmune encephalomyelitis mice

    Objective To Observe the effect of estrogen on myelin and axonal injury in the central nervous system(CNS) of experimental autoimmune encephalomyelitis(EAE) mice. Methods 40 mice EAE models were induced with MOG35-55 peptide, all animal ovarectomized. They were divided into treatment group and control group. Treatment group was treated by estrogen. Two groups were compared by clinical symptom scores. The myelin and axonal injury of the brain and spinal cord were studied by Luxol Fast Blue(LFB)-H&E and Bielschowsky staining. The expression of MBP and GAP-43 were examined by quantitative real-time PCR and western-blot. Results Clinical symptom score and incidence of treatment group were lower than that in the control group. Luxol Fast Blue(LFB)-H&E, Bielschowsky staining, quantitative real-time PCR and western-blot showed that myelin and axonal injury was decreased in treatment group. Conclusion Estrogen could decrease myelin damage and promote axonal regeneration to treat EAE mice.

    Abstract: Objective To observe the Effect of the Huangqi Injection on the Mitochondrial Membrane Potential (MMP) and GSH in ADR mice. Methods 60 Kunming mice (weighing 18~22g) are randomly divided into five groups:they are the normal control group ,the adriamycin (ADR) group,the Huangqi injection (large、middle、small) group ,instruct the molder of the doxorubicin induced cardiotoxicity in mice.Test the coefficient of the heart,the contentof the GSH. Flow Cytometry are used to analysis in Mitochondrial Membrane Potential in this study. Results 1. heart coefficient: Compared with the adriamycin(ADR) group, Huangqi injection groups can relieve the heart of tumefy(P<0.01); 2.the content of GSH: Compared with the adriamycin(ADR) group, Huangqi injection groups can increase the GSH(p<0.01) .3. the MMP: Huangqi injection groups can stable the cardio-muscle cell mitochondrial membrane potential, Compared with the adriamycin(ADR) group,there are obvious statistical differences (P<0.01). Conclusion:Huangqi injection can relieve the heart of tumefy,increase the GSH, stable the cardio-muscle cell mitochondrial membrane potential,restrain the apoptosis cardiac muscle cell to protect cardiac muscle cell.

    Construction of high level of stably expression of Prestin protein in vitro YU Li Department of Physiology, Fundamental School, Liao Ning Medical University, Jin Zhou Liao Ning 121001 Abstract Objective To construct stable expressions of prestin in CHO cell line in vitro. Methods CHO cells were transfected by prestin plasmids, which were selected by high dose antibiotic. Multi-colony cells which selected by antibiotic were subcultured and measured for expressing of prestin protein. Result It is successful that stably high expression of prestin protein in CHO cell line was constructed. Conclusion Multi-colony highly stably expression of prestin protein in cell line was constructed in which was transfected and selected by a simple, short-term, easy to operate method. The research is also lay a base for next studies on prestin protein.

    The Experimental Study of Contrast Enhanced Ultrasonography Assessingthe Effect of Treating Hepatic Tumor with Radiofrequency Ablation

    Abstract Objective: To analyze the correlation with contrast enhanced ultrasonography (CEUS), contrast enhanced CT (CECT) and histopathology through observing the findings of hepatic VX2 tumor in rabbits after RFA on CEUS, CECT and histopathology, and explore the reliability of CEUS in assessing the response of hepatic tumors after RFA. Methods: RFA was applied to rabbit hepatic VX2 tumors. Both CEUS and CECT were performed 1 week after RFA in rabbit models with hepatic VX2 tumor and followed with histopathologic examination. The results of CEUS, CECT and histopathologic examinations were compared. Results: Residual viable tumors were found in 22 models and no residual tumors were seen in 8 models after RFA according to histopathology. The average largest diameter and perpendicular diameter and area of ablation areas were 1.37±0.34 cm, 1.02±0.37 cm and 1.52±0.90 cm on CEUS, respectively. Those of ablation areas were 1.32±0.37 cm, 0.95±0.37 cm, 1.38±0.88 cm and 1.27±0.35 cm, 0.94±0.34 cm, 1.31±0.77 cm on CECT and sample, respectively. According to the results of histopathology, the sensitivity, specificity and accuracy for evaluating therapeutic effect were 90.9%, 75.0%, 86.7% for CEUS and 86.4%, 87.5%, 86.7% for CECT, respectively. Conclusion: CEUS is sensitive to capillary vessel. It can accurately discriminate the coagulation necrosis from residual tumors after RFA and offer the reliable foundation for subsequent treatment.

    Objective: To investigate the effect of dantrolene on the activities of enzymes of glycometabolism in rats with myocardial ischemia-reperfusion. Methods: Sixteen healthy male Wistar rats were used in the Langendorff isolated heart perfusion system. The hearts were randomly divided into two groups: control group and dantrolene-treated group, with eight rats in each. The hearts in control group were subjected to 30 min of global ischemia and 60 min of retrograde reperfusion. The hearts in dantrolene-treated group were perfused for 20 min in the working mode, in the presence of 5 μM dantrolene, and then were subjected to 30 min of global ischemia and 60 min of reperfusion. The activities of phosphofructokinase-1(PFK), pyruvate kinase(PK), hexokinase(HK) and isocitrate dehydrogenase (IDH) were determined. Results: The hemodynamic variables had no change except for coronary flow increased (P<0.01). Compared with the control group, the activities of PFK, PK, HK and IDH in dantrolene-treated group were significantly decreased (P<0.05). Conclusion: Dantrolene has a protective effect in myocardial ischemia-reperfusion by reducing the activities of glycolytic enzymes and isocitrate dehydrogenase, slowing the glucose metabolism and thereby enhancing the ability to recover the heart function during ischemia-reperfusion.

    Objective: To investigate HBYYRJ prophylactic effect on mouse liver fibrosis by reduced by ConA. Methods: Eighty rats were randomly divided into 7 groups, normal group(n=8), model group(n=16), HBYYBJ low dosage treatment group(n=14), middle dosage treatment group(n=14), high dosage treatment group(n=14), composed formula treatment group(n=14), and colchicine treatment group(n=14). Rat models of hepatic fibrosis were established by injection of ConcanavalinA .HBYYBJ was used for prevention of hepatic fibrosis. Histologic observation of the liver tissue was done with H & E stain and collagen stain.。Result: The degree of the liver fibrosis staging are significantly higher than normal contrast group (P<0.01). The three groups of HBYYRJ, the degree of hepatic fibrosis are significantly lower than model group (P<0.05).The degree of liver fibrosis The content of HBYYRJ in model group was significantly higher than normal contrast group (P<0.01) HBYYRJ medium and low groups was significantly lower than model contrast (P<0.05). HBYYRJ can decreace the degree of HA and PCⅢ. Conclusion: HBYYRJ has the preventive effect on the liver fibrosis with mice.     [Abstract ] [Objective] Through undergoing the healthy physical examination to understand the staff health condition of a factory in Tianjin. [Methods]Total 4912 workers who were in a factory in Tianjin received physical examination and laboratory test. Detected the Hyperlipidemia by laboratory test, and compared with the normal reference value. Analyzed differentiation between different gender. [Results ] In total 4912 workers , 4848 of them received the examination, occupy 98.7 %,64workers refused to join it, occupy 1.3%. The result showed normal in 3606 workers, abnormal in 1242 workers , occupy 25.6% of the effective data(1242/4848). The detection rate of Hyperlipidemia in male are higher than in female, X2=214.325,P=<0.01, The difference between male and female in detection rate was significant. [Conclusion]We should enhance the health education, adjust diet structure, pay more attention to improving the good change of our life style , to the prevention and control of Hyperlipidemia.

    Abstract: Objective To investigate the protective effect of hydrogen sulfide delayed preconditioning with mitochondrial ATP sensitive potassium channel[mitoK(ATP)] activation on rat myocardial cells. Methods Thirty-two S-D adult male rats were randomly divided into a Sham group, an IR group, a HS group and a HD group(n = 8). In Sham group, we isolated left anterior descending(LAD) coronary artery, but not blocked for 150 minutes; in IR group, we blocked LAD for 30 min and then primed for 2h; in HS group, we injected donor of hydrogen sulfide — sodium hydrosulfide 50μg/kg via vein, after 24h, dealed the same with IR group; in HD group, we injected 5-HD 15min before blocking LAD via vein, others dealed the same with HS group. the myocardial infarct size and area at risk were determined; the myocardial ultrastructure was observed by electron microscope. Results Compared with IR group, the infarct size was reduced and the damage of myocardial ultrastructure was depressed in HS group(P<0.05). Compared with IR group, there was no significant difference in HA group(P>0.05). Conclusion Hydrogen sulfide delayed preconditioning can protect the cardiomyocytes by activating the KATP channel.

    The correlation between PASP and brain natriuretic peptide,endothelial function, inflammatory mediators in pulmonary artery hypertension

    Abstract Objective: To investigate the correlation between PASP and brain natriuretic peptide (BNP), endothelial function, inflammatory mediators and evaluate mechanism of pulmonary artery hypertension (PAH). Methods: Pulmonary artery systolic pressure (PASP), plasma BNP, endothelin-1 (ET-1), serum nitric oxide (NO), serum hyper sensitive C-reactive protein (Hs-CRP), tumor necrosis factor-α (TNF-α) were measured in 92 patients with PAH and 50 healthy persons. Results: Compared with healthy persons, BNP, ET-1, hs-CRP and TNF-αwere significantly increased (P<0.05), while serum NO were reduced in patients with PAH (P<0.05). PASP in patients with PAH were positively correlated with plasma BNP(r=0.574), hs-CRP(r=0.423), ET-1(r=0.402) and TNF-α(r=0.366); while negatively correlated with serum NO(r=-0.378). Conclusion: The occurance of PAH is decided by interrelation of natriuretic peptide system, endothelial system and inflammation.

    Clinical observation of subcutaneous hemorrhage caused by combined usage of Low Molecular Heparin and Intermittent Pneumatic Compression in patients of total knee arthroplasty

    [Abstract] Objective:To analyze the clinical characteristics of the Subcutaneous hemorrhage caused by combined Low Molecular Heparin(LMH) and Intermittent Pneumatic Compression therapy used in patients of TKA. Clinical data: 80 cases of total knee arthroplasty (TKA) patients were distributed into 2 groups: the experimental group (EG), and the control group (CG). In CG, we inject 5000u LMH subcutaneously every night; while in EG, besides the same LMH injection, IPC was used as a combination. And perimeters of extremities were matured, as well as temperature and color of skin were observed; the Doppler ultrasounds to vessels in lower extremities were examined, tests of coagulation and the PLT were tested before and after the operation. Finally, these data was managed statistically. Results: When examined by the Doppler ultrasound after the operation, 6 cases (15.0%) in the control group were found to have developed DVT; while the incidence in the experimental group was 1case (P<0.05), this means it is statistically effective. And 3 cases in the control group were found to have light subcutaneous hemorrhage; while in the experimental group the subcutaneous hemorrhage incidence was 37 cases (92.5%), in which there were 30 cases with generalized subcutaneous hemorrhage. When 2 groups compared together, P<0.05, it is statistically effective. Conclusions: It displays that combined Low Molecular Heparin and Intermittent Pneumatic Compression treatment can effectively prevent the morbidity of DVT in lower extremity, but it’s at the price of causing subcutaneous hemorrhage.

    Objective: To evaluate the clinical value of Two-dimensional strain echocardiography in the quantitative evaluation of myocardial infarction left ventricular regional wall motion abnormalities. Methods : 40 patiengts with myocardial infarction(According to history, signs, ECG and myocardial enzymes diagnosis, part of the coronary angiography) and 40 healthy subjects. Access to apical left ventricular long axis, apical four-chamber and apical two-chamber left ventricular cardiac cycles of 3 consecutive dynamic two-dimensional gray scale image. Quantitative analysis of two-dimensional strain software, quantitative analysis of left ventricular segments, Automatically appear in each region of interest along the myocardial wall segment fiber direction of the longitudinal peak systolic strain and strain curves, And automatically derived bovine strain of paragraph 18 chart. Results: Control group, left ventricular systolic longitudinal strain of each segment is negative. Absolute value of the strain from the basal part of the middle section to the apical segments gradually increase, but the difference was not statistically significant (P> 0.05). Wall myocardial infarction, no corresponding changes in the corresponding set of rules.However, the absolute value of each segment strain than the normal control group were significantly lower (P <0.05). Conclusion: Two-dimensional strain echocardiography for myocardial infarction left ventricular regional wall motion abnormalities after an objective quantitative evaluation.

    The Diagnosis and Treatment of Idiopathic Normal Pressure Hydrocephalus (with report of 35 cases)

    Abstract Objective: To explore the clinical diagnosis and treatment in patients with idiopathic normal pressure hydrocephalus (iNPH). Methods: Clinical data of 35 patients with iNPH in our department from September 2008 to June 2010 were analyzed, the symptoms before and after operation have been compared. Results: Among 35 iNPH patients have been operated by ventriculoperitoneal shunt, all of them have improved in clinical symptoms in varying degrees. gait disturbance(85.7%), dementia(91.4%), urinary incontinence(82.4%). 13 patients have improved in all symptoms, the remains have one symptom improved at least. Conclusion: iNPH is one of the common diseases in neurology which can be misdiagnosis and missed diagnosis. Ventriculoperitoneal shunt is effective method for it. Lumbar puncture and tap test can be used to select patients for operation.

    The Management of the Traumatic Brain Injury with limbs fractures

    Abstract Objective To investigate the principle of diagnosis, therapy and the impact factors of prognosis in traumatic brain injuries (TBI) associated with limbs fractures. Methods 59 TBI cases associated with limbs fractures during 2005 and 2008 in our hospital were retrospectively analyzed. The characteristics of these patients were summarized including their injury mechanisms, brain injury types, clinical characteristics, diagnostic methods, treatment measurements and prognosis. Fish exact test was used to assay the discrepancy in the prognosis of the TBI cases in different Glasgow coma scale (GCS) scores. Results According to the GCS scores, the TBI cases were divided into 3 different groups with 39 mild, 7 moderate and 13 sever ones respectively. The Glasgow outcome scale (GOS) was evaluated to reveal 3 died, 4 sustained in vegetative state, 8 suffered from severe disability , 12 suffered from moderate disability, 32 survived well. There was a straight line correlationship between the admitting GCS scores and the discharging GOS ones. Based on the GCS scores, the prognosis of the different types of TBI cases is different. Conclusion The TBI is a pivotal impacting factor on the prognosis of the patients of TBI associated with limbs fractures. However, it should be emphasized to pay special attention to the diagnosis and treatment of limbs fractures, especially the big bone fractures, when the brain injury is treated actively, which is also very important in reducing the mobility and mortality of this kind of injuries
