    2011, 39 (12):  1091-1094 .
    Abstract ( 1388 )   PDF (2981KB) ( 2237 )
Abstract Objective:To study the capacity of different mesenchymal stem cells (MSCs) to be differentiated into nerve-like cells in vitro. Methods: The fetal derm-derived MSCs (group A), fetal bone marrow-derived MSCs (group B) and adult bone marrow-derived MSCs (group C) were separated and cultured in expanded medium. The morphology and doubling time of MSCs were detected by inverted microscope. The phenotype and cell cycle of MSCs were detected by flow cytometry. All-trans-retinoic-acid (ATRA) was used to induce three different MSCs to differentiate into nerve-like cells. The differentiated cells were observed by phase-contrast microscopy and detected by immunocytochemistry for neuron-specific marker-neurofilament (NF) and astrocyte specific marker-glial fibrillary acidic protein (GFAP). Results:Our results showed that MSCs derived from three groups were similar in phenotype, morphology and cell cycle. However, the abilities of expansion and passage of MSCs derived from C group were lower than that of MSCs derived from A and B group. After treatment with ATRA, the generations of the three different MSCs group showed typical morphological characteristic of nerve cells. Results of immunocytochemistry staining and Western blot showed that the differentiated nerve-like cells expressed NF and GFAP. Conclusion:Three different sources of MSCs can be induced into nerve-like cells in vitro. But, MSCs in different development stage have different ability of being differentiated into nerve-like cells in vitro.

    Abstract ( 1457 )   PDF (2988KB) ( 2171 )
Abstract Objective: To investigate the association between C667T point mutation of N5, 10-methylenetetrahydrofolate reductase (MTHFR) and Hui nationality patients with essential hypertension in Ningxia. Methods: The study was performed in 146 patients with essential hypertensive (EH) and 112 normotensive (NT) control subjects. Blood samples of two groups were collected and then DNA was extracted. MTHFR gene polymorphism was detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Results: The frequencies of three genotypes (CC, CT and TT) showed statistics differences between EH group and NT group (P<0.05). The frequency of genotype 677TT and T allele were significantly higher in patients of EH group than those of NT group (P<0.05). There was no significant difference in diastolic blood pressure between CC genotype group and CT/TT genotype group (P>0.05), but there were significantly higher systolic blood pressure, pulse pressure and mean arterial pressure in CT/TT genotype group than those of CC genotype group (P<0.05). Binary stepwise logistic regression analysis showed that CT+TT genotype, female, body mass index (BMI), triacylglycerol (TG) and urea nitrogen (BUN) were independent risk factors for essential hypertension. Conclusion: There is a correlation between MTHFR C677T gene polymorphism and essential hypertension in patients of Ningxia Hui nationality. The T allele may be the susceptible gene of essential hypertension in Ningxia Hui nationality.

Research of the level and relationship of the gastric cancer with VEGF-D and MLVD
    2011, 39 (12):  1108-1111 .
    Abstract ( 1399 )   PDF (2891KB) ( 2181 )
Objective: To detect the amount of VEGF-D (vascular endothelial growth factor-D ) and MLVD(micro-lymphatic vessel density) in the gastic cancer, analyze the relationship of VEGF-D and MLVD with the following factors: age, gender, clinical stage, lymph node metastasis,histo-differentiation and explore the use of antilymphangiogenic treatment. Methods: 52 samples of the gastric cancer tissue and bordeline tissue of cancer were gathered . 52 samples of normal tissue at least 5cm away from the tumor were also collected. We adopted an immunohistochemistry method to determine the levels of VEGF-D and MLVD in the samples. This was accomplished by bingding the clinical data and stastistical analysis to find how much VEGF-D and MLVD were present in the gastric cancer and normal tissue and the relationship with age, gender, clinical stage ,lymph node metastasis ,histo-differentiation . Results: The expressive rate of VEGF-D in gastric cancer and the normal tissue adjacent cancer had significantly difference (P<0.01 ). The positive rate of VEGF-D in gastric cancer in the samples which had lymph node metastasis was significantly higher than which had no lymph node metastasis(P<0.01 ). The positive rate of VEGF-D in gastric cancer of stage Ⅲ、Ⅳ was significantly higher than that in stage Ⅰ、Ⅱ(P<0.01). The expression of VEGF-D had no significantly different from poor and well differentiation(P>0.05). The expression of VEGF-D had significant correlation with lymph node metastasis ( P<0.01). The positive rate of VEGF-D was also correlated with the clinical stage(P<0.01). The MLVD in the normal tissue was significantly higher than that in the gastric cancer ( P<0.01 ). The MLVD in tumor borderline was higher than that in the normal tissue and gastric cancer ( P<0.01). The MLVD average value in tumor borderline was significantly increased in the VEGF-D positive group than the negative group ( P<0.01). No significantly difference in the MLVD of VEGF-D positive group with the MLVD average in negative group(P>0.05). Conclusion:The expression of VEGF-D in the gastric cancer is high, and it has correlation with clinical stage, lymph node metastasis. It suggests that the expression of VEGF-D is important in promoting tumor growth and metastasis. MLVD in the tumor borderline was significantly higher than in gastric cancer and normal tissue . The increase of MLVD may be related to the invasion and metastasis of gastric cancer in the tumor borderline. The detection of VEGF-D and MLVD in the tissue of gastric cancer can be accepted as important affiliated markers to evaluate the lymph node metastasis. It may prove to be markers to predict the malignant biology of gastric cancer and prognosis.

    2011, 39 (12):  1112-1115 .
    Abstract ( 1514 )   PDF (2879KB) ( 2486 )
Objective The aim of the study was to evaluate the prognostic value of the postprocedural tumor necrosis factor-α (TNF-α) and high sensitivity C-Reactive Protein(hs-CRP) in patients with first acute myocardial infarction(AMI) treated with successful percutaneous coronary intervention(PCI). Methods A total of 152 consecutive AMI patients underwent successful PCI were enrolled. TNF-α and hs-CRP were measured within 12 hours after PCI. All patients were followed up for 1 year. Logistic regerssion analysis was used to evaluate predictive values of postprocedural TNF-α and hs-CRP for MACE at 30 days and 1 year post PCI. Time-to-event analyses were performed using the Kaplan-Meier survival curves in patients with various ranges of postprocedural TNF-α and hs-CRP. Results The percentage of anterior myocardial infarction, the peak of Creatine Kinase MB, left ventricular ejection fraction, TNF-α and hs-CRP levels were all significantly different between MACE group and non-MACE group after PCI. Multivariable logistic analysis showed the OR of TNF-α were 1.43 (P=0.006) at 30 days and 1.62 (P=0.002) at 1 year. The OR of hs-CRP were 1.14 (P=0.004) at 30 days and 1.22 (P=0.005) at 1 year. The patients with higher postprocedural TNF-α level had significant lower survival rates at 30-day and at 1-year, compared with the patients with lower postprocedural TNF-α level. The patients with higher postprocedural hs-CRP level had significant lower survival rates at 1-year, compared with the patients with lower hs-CRP level. In higher TNF-α group recurrent myocardial infarction rates at 1-year, revascularization rates at 30-days and at 1-year were higher than those in lower TNF-α group. Compared with those in lower hs-CRP group, recurrent myocardial infarction rates and revascularization rates at 30-days and at 1-year all apparently increased in higher hs-CRP group. Conclusion Postprocedural TNF-α and hs-CRP levels are closely related with short and long-term MACE in first AMI patients treated with successful primary PCI.

Objective: To investigate the clinical efficacy of puncture and drainage for the treatment of spontaneous basal ganglia hematoma through a catheter at frontal region at different time window and its impact on brain edema. Methods: A total of 113 patients with spontaneous basal ganglia hematoma were divided into an ultra-early group (n=31, surgery started within 6 hours of the ictus),an early group (n=42, surgery started from 6 hours to 24 hours of the ictus) and a delayed group (n=40, surgery started from >24 hours to 72 hours of the ictus) according to the time window of surgery. The differences in incidence of common complications, the evacuation rate of blood clots, brain edema volume change, and neurological deficit scores (NIHSS) change and long-term follow-up results were compared among the three groups. Results: There were no significant differences in the incidence of common complications, residual hematoma volume, evacuation rate of blood clots and initial brain edema volume among the three groups. 7 days after the ictus perihematomal edema volume was (6.6±4.8) mL and (8.2±5.9) mL in the ultra-early and early groups, respectively. Both volumes were significantly lower than (15.8±15.4) mL in the delayed group (P<0.01). The neurological deficit score (NIHSS) change was (11.3±6.0) and (12.5±6.3) in the ultra-early and early groups, respectively. Both changes were significantly higher than (6.3±5.4) in the delayed group (P<0.01). Six months later, the ADL score in ultra-early and early groups was significantly more favorable than the delayed group (P<0.01). But there were no significant differences between the ultra-early and early groups in these variables. Conclusion: The puncture and drainage through a catheter at frontal region for the treatment of spontaneous basal ganglia hematoma could remove blood clot effectively. Early surgery could alleviate the degree of perihematomal edema, improve neurological outcome and reduce the possibility of rebleeding.

Results:(1)The incidence of abnormal glucose metabolism was 80.70%.(2)There is a higher incidence of renal impairment, showing abnormal UAER and obvious renal insufficiency (53.91% vs21.39% ,respectively) (3)Compared with normal UAER group, there were lower absolute value of eGFR in UAER abnormal group patient regardless of how their blood glucose levels, and the proportion of the obvious decline of renal function was significantly higher. In UAER abnormal group, the levels of eGFR have a statistically difference among patients with different blood glucose levels, eGFR in T2DM group was the lowest in three group. (4) With eGFR> 60 ml / min .1.73 m2 group comparison: age and blood pressure were higher, more common women , with blood glucose levels elevated ,percentage of the obvious decline of renal function increasing in eGFR ≤ 60ml / min .1.73 m2 group, this percentage was the highest in T2DM group . Conclusion:(1) Abnormal glucose metabolism were very common in elderly patients with hypertension hospitalized.(2) The incidence of kidney damage were higher, performance for UAER abnormal or eGFR reduce or both cocurrence. (3) Renal impairment occurrence involves glucose metabolic disorder, elevated blood pressure and the growth of the age.

    Abstract ( 1586 )   PDF (10197KB) ( 1944 )
[abstract] objective To evaluate the analytical performance of ADVIA Centaur TnI-Ultra testing method, in order to help the clinical diagnosis of AMI . Methods To collect the serum of emergency department patients with chest pain immediately and detect TnI-Ultra. consult 2000 ESC/ACC for AMI diagnostic criteria to diagnos AMI. Calculate the positive rate of immediated detection of patients. according to ROC curves of TnI-Ultra test results in the diagnosis of characteristics with AMI for evaluation. Evaluation of this method function sensitivity (CV = 10%), within- and between run imprecision. Results confirmed immediated positive rate of AMI patients was 34.2%, the area under the ROC curves was 0.956, definite diagnosis threshold value was 0.436 ng/ml, at this threshold value sensitivity was 90.2%, specificity was 87.3%. TnI-Ultra function sensitivity when CV=10% was 0.042 ng/ml, when CV=20% was 0.0195 ng/ml. Low concentration serum (nearly 0.04 ng/ml) within-run CV was 7.9%, between run CV was 8.1%; High concentration serum (0.11 ng/ml) within-run CV was 4.6 %, between run CV was 5.4%. Conclusion ADVIA Centaur TnI-Ultra detection method analysis performance reach "guideline acceptable" (CV=10% or less), but when AMI happened, judgment threshold the provided materials by manufacturers have difference, the application of routine clinical diagnosis still needs further observation and study.
