    Effects of berberine on inflammatory factors, adipokines and fatty acid metabolism in 3T3-L1 adipocytes

    Objective To observe the effects of berberine on inflammatory factors, adipokines and fatty acid metabolism in 3T3-L1 adipocytes, and to investigate the molecular mechanism underlying berberine improving insulin resistance. Methods Inflammatory molecules , adipokines, key enzymes and protein in fatty acid metabolism mRNA in 3T3-L1 cells were determined with quantitative real time polymerase chain reaction(QRT-PCR) after treated with different concentrations of berberine (0μmol/L,5μmol/L,10μmol/L,20μmol/L and 40μmol/L) for 24 hours while treated with 10μmol/L berberine at different durations (0 h,4 h,8 h,24 h,48 h). These inflammatory factors and key enzymes in fatty acid metabolism mainly included interleukin-6 (IL-6) , tumor necrosis factor-α (TNF-α), leptin, adiponectin, visfatin, fatty acid synthase (FAS), acetyl-CoA carboxylase (ACC), adipose triglyceride lipase (ATGL), adipocyte fatty acid binding protein (AFABP). Results In 3T3-L1 adipocytes, expression of IL-6, TNF-α, leptin, FAS, ATGL, AFABP mRNA reduced with dosage at 10~40μmol/L berberine(P<0.05)while visfatin mRNA level was increased(P<0.05). No significant difference was found in the expression of adiponectin(P>0.05). IL-6, TNF-α, leptin, AFABP,ATGL,FAS mRNA decreased with time after 10μmol/L berberine intervention (8-48h) when compared with the control group(P<0.05) while visfatin mRNA level was increased(P<0.05). Adiponectin mRNA was found to decreased only after treated for 48 h(P<0.05). No significant difference was found in the expression of ACC mRNA between groups treated with berberine(P>0.05). Conclusion Berberine has dose and time dependent effects on mRNA expression of inflammatory factors, adipokines, key enzymes and protein in fatty acid metabolism in 3T3-L1 adipocytes, suggesting the mechanisms underlying berberine improving insulin resistance.

    【Abstract】Objective Mutation screening was performed for genes including ASXL1,TET2, IDH1, IDH2, SETBP1, MPL515, JAK2exon12 and JAK2V617, in 135 polycythemia vera (PV) patients. The correlation between the genomics characteristics and disease progression of PV were evaluated. Methods DNA sequencing of ASXL1(Exon12),TET2 (Exons 3-11),IDH1(Exon4),IDH2(Exon4),SEPBP1(Exon4), JAK2exon 12 and MPL515(Exon 10) genes were carried out. The burden of mutant JAK2V617F allele (V617F%) was evaluated by real-time PCR in the meantime. The significative gene mutations and clinical outcomes or PPMF was analyzed. To study the risk factors of PPMF, Kaplan–Meier estimation, Cox and Logistic regression were applied. Results ASXL1、TET2、IDH1、IDH2 were mutated in 7.69%(8/104)、5.26% (1/19) 、0.08% (1/120)、and 0.08% (1/121) JAK2 was mutated in 82.22%(111/135) , among them, the mutation rate of JAK2exon12 was 2.96%(4/135) and there were no mutation of MPL515 and SETBP1 in these PV patients. The mutation of ASXL1 was 31.82%(7/22)in PPMF patients. The result of Spearman’s analysis showed that ASXL1 is correlated with V617F% (r=0.298,P=0.002). The hemoglobin was lower in patients with ASXL1 mutation. The result of Kaplan-Meier survival analysis showed ASXL1 mutation (Log Rank =9.189, P=0.002), V617F% higher than 50% (Log Rank=9.339,P=0.002) and leukocyte count >25×109/L(Log Rank=4.191,P=0.041) were the risk factors for survival of no PPMF. ASXL1 mutation(β=3.47,Wald =3.932,OR=32.151,P=0.047, 95%CI=1.041-992.968), V617F% higher than 50% (β=-1.582,Wald =5.157,OR=0.206,P=0.023, 95%CI=0.052-0.805) were the risk-factors of PPMF by multiple-factor analysis of Logistic was used. Conclusion The ASXL1 mutation was higher in Chinese. The ASXL1 mutation was correlation with V617F%. The ASXL1 mutation may is the risk-factor of PPMF. ASXL1 mutation may promote the V617F% by some mechanism. The mutation in the TET2, IDH1, IDH2, SEPTBP1 and MPL were rare.

    Cellular Localization Analysis of Enhanced Green Fluorescent Protein Tagged hnRNP A1 Under Stress Condition

    Objective:To construct eukaryotic enhanced green fluorescent protein(EGFP) expressing recombinant plasmid,pEGFP-C1-hnRNP A1,which contains coding sequence of human hnRNP A1 (heterogeneous nuclear ribonucleoprotein A1),and perform the cellular localization analysis of EGFP tagged hnRNP A1 under stress condition. Methods:The total RNA was isolated from HeLa cell and used for the first-strand cDNA synthesis with reverse primers specific for the 3'-untranslated region of hnRNP A1.The gene of hnRNP A1 fragment was then amplified by touch-down PCR from the cDNA and inserted into pEGFP-C1 fluorescent expressing vector through EcoRI/BamHI double enzyme digestion and T4 DNA Ligase connection. The recombinant pEGFP-C1-hnRNP A1 plasmid was transfected into HeLa cells and the expression of green fluorescent fusion proteins was examined by Western blotting assay and confocal fluorescence microscopy. RNA Fluorescence in situ Hybridization and Immunofluorescence assays were also performed to detect the co-localization of EGFP-hnRNP A1 with poly(A)+ mRNA (the marker of the stress granules),or DCP1a (the marker of processing bodies). Results: The pEGFP-C1-hnRNP A1 was sequenced and digested correctly by restriction single/double enzyme. The green fluorescent fusion protein was also detected in transfected HeLa cell by Western blotting assay and confocal fluorescence microscopy. EGFP-hnRNP A1 localizes with poly(A)+ mRNA, but not DCP1a. Conclusion:The recombinant eukaryotic plasmid of pEGFP-C1-hnRNP A1 was constructed successfully and expressed effectively.EGFP tagged hnRNP A1 takes part in the formation of stress granules.

    Analysis of CD4+CD25+CD127low regulatory T cells in the peripheral blood of hepatocellular carcinoma patients with HBV infection

    Objective To investigate the level of CD4+CD25+CD127low regulatory T cells in the peripheral blood of hepatocellular carcinoma (HCC) patients with HBV infection and to explore the relationship between these cells and HBV infection. Methods Peripheral blood mononuclear cells were isolated from 41 HCC patients infected with HBV, 34 HCC patients without HBV infection and 29 healthy persons. These mononuclear cells were labeled with the monoclonal antibodies of CD4, CD25 and CD127, and then they were determined by flow cytometry (FCM) to analyze the level of Tregs. Moreover, the load of HBV DNA in the blood serum of HCC patients with HBV infection was detected by fluorescent quantitative polymerase chain reaction (FQ-PCR). Results The percentage of CD4+CD25+CD127lowTregs in CD4+T cells in the HCC patients with HBV infection was higher than that in the HCC patients without HBV infection [(8.24±2.20)% vs (7.06±2.46)%, P<0.05] and that in the healthy persons [(8.24±2.20)% vs (6.51±1.99)%, P<0.01]. Furthermore, the percentage of CD4+CD25+CD127lowTregs in CD4+T cells in the HCC patients without HBV infection was higher than that in the healthy persons [(7.06±2.46)% vs (6.51±1.99)%, P<0.05]. In addition, the level of CD4+CD25+CD127lowTregs in the HCC patients with HBV infection was gradually increased following the increase of HBV DNA load and there was a positive correlation between the level of CD4+CD25+CD127lowTregs and the load of HBV DNA (r=0.350,P<0.05). Conclusion The level of CD4+CD25+CD127low Tregs in HCC patients is increased remarkably and associated with the copy of HBV DNA, which may be an important mechanism of immune evasion in HCC.

    【Abstract】: Objective: Doxycycline on gastric cancer cell line BGC-823 proliferation and Notch1,MMP-9 expression, in order to provide a new new anticancer drugs. Method: Respectively 0, 5, 10, 20, 40 μg/mL final concentration of doxycycline effects on human gastric cancer cell line BGC-823.Cell proliferation was detected was detected by MTT degree; flow cytometry cell cycle distribution; Western Blot detection Notch1, MMP-9 protein expression. Results: Doxycycline can inhibit the proliferation of human gastric cancer cells line BGC-823, and the effect of drug dose and time-dependent(P<0.01); doxycycline alter gastric cancer cell line BGC-823 cycle distribution, with increasing concentration, S the proportion of lower phase(P<0.01); as the concentration increase, Notch1 expression enhancement, MMP-9 expression decreased (P<0.01). Conclusion: Doxycyclinecan inhibit gastric cancer cell line BGC-823 proliferation, Notch1 up as one of the possible mechanisms.

    Expression and Correlated Analysis of autophagy-related proteins in Human squamous cell carcinoma and adenocarcinoma of lung

    Objective:To explore the expression of Beclin1、autophagy-related protein 1 (Atg1)、target of rapamycin (mTOR) in different squamous cell carcinoma(SCC) and adenocarcinoma of lung and relationship among the three factors. Methods: The expression of Beclin1、Atg1、mTOR in different human SCC and adenocarcinoma of lung were detected by immunohistochemical techniques .There were 17 cases of normal tissue,82 cases of lung cancer. Results: The expression of Beclin1 and Atg1 showed a tendency to increase from high differentiated SCC and adenocarcinoma of lung to low differentiated ones. But mTOR showed a tendency to decrease. Compared with the expression of Beclin1,the expression of Atg1 in SCC and adenocarcinoma of lung presented a positive correlation. And Compared with the one of mTOR,Beclin1 and Atg1 show a negative correlation. Conclusion:Autophagy is related with development and SCC and adenocarcinoma of lung.

    Liu YiBing1,Ma Yun Fu2,Chen Jianshe3,Wang YinHui4 Abstract Objective: To evaluate the diagnostic value of TSSC1 in glioma patients and to the role influence on cell biology of glioma U87 cells. Methods: RT-PCR and Western blot were used to examine the expression of TSSC1 in samps, including 80 normal tissues and 80 primary tumors. MTT and transwell were used to analyze the ability of proliferation, migration, and invasion in TSSC1 siRNA-transfected U87 cells. Results: TSSC1 is down-regulated in glioma and related to higher grade. Furthermore, knockdown of TSSC1 expression results in increased proliferation and invasion in U87 cells in vitro. Conclusion: Our results may provide a strategy for blocking glioma and progression.

Nanchang,330004,China) [Abstract ]Objective To observe the effects of moxibustion pretreatment on the caspase-3 expression in cortex of rats with cerebral ischemia-reperfusion injury, and the mechanism thereof. Methods Twenty-one male SD rats were randomly divided into three groups, sham group(n=6), model group(n=8) and moxibustion group(n=7). Focal cerebral ischemia-reperfusion injury rat model was induced for 2h by middle cerebral artery occlusion (MCAO) followed by reperfusion. Brain infarct was examined by TTC staining, brain morphology was with optical microscope, the ultrastructure of cell was observed with electron microscope, and the expression of caspase-3 protein in cortex was examined by immunofluorescence histochemistry. Results Percentages of infarct volume were significantly lower[(24.9±4.7)%] in rats of moxibustion pretreatment group than those of model group[(45.8±4.6)%], nerve cells experience less morphological and structural changes in moxibustion pretreatment group than those of model group, and caspase-3 positive cells(39.17±7.28) are lower in moxibustion pretreatment group than those of model group(58.17±16.53). Conclusion Moxibustion pretreatment could reduce the damage of cerebral ischemia-reperfusion, which might be through decreasing caspase-3 expression.

    Effects of exercise training on expressions of brain-derived neurotrophic factor and tyrosine kinase receptor B in spinal cord of rats with spinal cord injury

    Objective To investigate the effect of exercise training on the expression of BDNF (brain-derived neurotrophic factor) and tyrosine kinase receptor B(TrkB) in spinal cord of spinal cord injury(SCI) rats. Methods Spinal cord injury models were produced by universal spinal cord impact system. Twenty-four female adult Sprague-Dawley rats were randomly divided into 3 groups, exercise group (SCI-induction and exercises group, n=8), control group (SCI-induction without exercises group, n=8) and sham-operation group (no operation, without SCI and exercises, n=8). Exercise training began from 7th day after injury and lasted for 4 weeks. The locomotor function was assessed by Basso-Beattic-Bresnahan(BBB) scale before injury and at the 1st, 2nd, 3rd, 4th and 5th week post injury. The expression of BDNF and TrkB protein in spinal cords were detected by method of immunohistochemistry and Western blot. Results BBB scores in exercise and control groups were higher than that at 1 week and 2 week post injury, and BBB scores in exercise group increased more significantly than that in control group (P<0.05). In immunohistochemistry test positive immunologic reaction to BDNF protein was mainly located in anterior horn of spinal cord, the expression level in exercise group was significantly higher than that in sham-operation group and control group (P<0.05). In immunohistochemistry test positive immunologic reaction to TrkB protein was located in anterior horn and dorsal horn and central canal of spinal cord, the expression level in exercise group was significantly higher than that in sham-operation group and control group (P<0.05). Expression of BDNF and TrkB in exercise group rats after exercise for 4 weeks were significantly increased, compared with control group (P <0.05). Conclusion Exercise training may effectively induce the expression of BDNF and TrkB in spinal cords and promote the recovery of locomotor function of SCI rats.

    The Improved Technology of Inducing Coronary Microembolization Animal Model of Pigs by Coronary Intervention

    Abstract Objective: To develop an animal model of miniature pigs with coronary microembolization (CME) by easy and economical technology. Methods: A total of 11 miniature pigs were divided into control group (n=5) and CME group (n=6). Femoral artery puncture was made with a 21 gauge needle for transradial procedures. Microspheres were injected into the left anterior descending artery of the CME group by 5F coronary diagnostic catheter and 1.8F coronary micro-guide catheter. Serum concentrations of brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) were evaluated just before CME and 6h after CME. Apical myocardial pathological lesions 6h after CME were evaluated by optical microscope. Results: All the miniature pigs of the control group survived, but one died in the CME group. 5F coronary diagnostic catheter and 1.8F coronary micro-guide catheter successfully arrived at the designated location. Before CME, the serum concentrations of BNP (ng/L:143.00±13.51 vs 134.00±15.57) and cTnI (μg/L:0.39±0.09 vs 0.38±0.10) showed no significant differences between these two groups (t values are 0.976 and 0.294 respectively,both P>0.05). After CME, the serum concentrations of BNP (561.00±80.65) and cTnI (2.75±0.58) much higher than those (BNP 139.00±13.87; cTnI 0.54±0.14 ) of the control group (t values are 11.530 and 8.337 respectively,both P<0.001). In CME group, microspheres, micro-infarction and inflammatory cell infiltration were found under an optical microscope. Conclusion: The use of new surgical consumables can successfully develop an animal model of miniature pigs with CME, and the technology is simple, economical, practical and worth promoting.

    【Abstract】Objective: To investigate the enhanced osteogenesis on bone fracture healing at a segmental bone defect of injecting leptin into intracerebroventricular (ICV) through Foramen magnum. Method: Segmental critical-sized defects were created at the right tibial bone of skeletally mature New Zealand white rabbits. Recombinant rabbit leptin (leptin group) and normal saline (control group) were injected into cerebellomedullary cistern through foramen magnum. Bone fracture healing was evaluated through callus formation and bone mineral density (BMD) by using qCT at 1st, 3rd, 7th days and the 2nd ,3rd week. At the 3rd week, all rabbits were euthanized to collect the right tibia for histomorphology. Result: The result showed that ICV leptin exhibited significantly high osteoinduction activity compared with that of NS group: qCT showed leptin group healed faster than control group, formulation of callus appeared early than that of control group, and the BMD analysis results showed that the BMD of tibias in LEP group were significantly different with NS group. Conclusion: ICV leptin could accelerate bone fracture healing at the bone defection model

    Abstract Objective:To research for the mechanism of carbonated drinks affecting health of the children primary and permanent teeth replacement. Methods:The dogs tooth enamel samples were soaked in coca-cola, sprite and pure soda, and the calcium, phosphorus concentrations were detected. Dental papilla stem cells were separated and cultured in the conditioned medium by adding three drinks. PCR and western blot method were used to detect the mRNA and protein expression levels of activator of nuclear factor-k B receptor ligand (RANKL), osteoprotegerin (OPG) and vitamin D receptor (VDR) , then the possible role of each gene and interactions relationship were analyzed. Results:Compared with saline, coca-cola and sprite showed their significantly decalcification and dephosphorization role, while plain soda water showed calcium and phosphorus protective effect. The three drinks had no effect on the mRNA and protein levels of RANKL gene (P> 0.05). Coca-cola and sprite can reduce OPG mRNA and protein levels, and at the same time increase the expression of the VDR gene. Plain soda water has no effect on the OPG gene expression, but can significantly reduce the level of the VDR gene. Conclusion:Carbonated drinks may affect the dental health of the children's primary and permanent teeth replacement by regulating bone related gene expression and vitamin D receptor family.

    Effect of Pioglitazone on Expression of Interleukin-18 and Toll-Like Receptor 4 in OLETF Rats with Insulin Resistance

    [Abstract] Objective To investigate change of TLR4 in OLETF rats with insulin resistance(IR), and study the effect of peroxisome proliferator-activated receptor γ(PPAR-γ)pioglitazone (PIO) on the expression of TLR4, and the possible mechanisms of the PIO reducing the risk of cardiovascular diseases. Methods: 24 OLETF rats were fed with high-fat diet for 20 weeks to establish the IR model. 24 OLETF rats with IR were randomly assigned into two groups: the model group (group M), in which the rats were fed with high-fat diet; the PIO group (group P), in which the rats were fed with high-fat diet and PIO. 12 LETO rats fed with normal diet were served as normal control group (group NC). After 20 weeks of drug intervention, the plasma levels of FINS, FBG, blood lipid, IL-18 and TLR4 were detected in every group. Results Comparing with group NC, the FBG, Blood lipid, IL-18 and TLR4 were significantly increased in group M(P<0.05 or P<0.01); comparing with group M, the FBG, Blood lipid were improved in group P, and The serum IL-18,TLR4 were significantly lower in the group P than that of group M(P<0.05 or P<0.01). Conclusion TLR4 may be involved in IR by promoting inflammatory response, and PIO can significantly improve IR and inflammatory stage, and reduce the risk of cardiovascular diseases by inhibiting the expression of TLR4.

    Application of psycho-cardiology medical mode in patients with clinical heart failture complicated depressive disorder

    To explore the application of psycho-cardiology medical mode in patients with clinical heart failture complicated depressive disorder. Methods:Hospitalized patients with clinical heart failture were assessed with PHQ-9 health questionnaire. A total 100 cases with depressive disorder were selected and randomly divided into routine trentment group(n=50,received routine medication) and intervention group (n=50,receive intervention of psycho-cardiology medical mode based on routine medication). Tests were done after three months and changes of scors of the two groups before and after therepy were statistically analyzed. Results :Compared with routine treatment group, total scores of PHQ-9 significantly decreased in intervention group (p<0.01)。Conclusion: Psycho-cardiology medical mode can treat chronic heart faiture and solve psychological problems of patients and conducive to comprehensive rehabilitation of patients.

    The influence of current perception threshold(CPT) by different frequency and time of transcutaneous electrical nerve stimulation (TENS)

    Objective Analysis the influence of current perception threshold(CPT) by different frequency and time of transcutaneous electrical nerve stimulation (TENS).It measured the CPT of the foramen area and the arm area to evaluate the effect of TENS. It gave different frequency and time of TENS to 30 Healthy volunteers Hegu and measured the CPT of the foramen area and the arm area on the same side. Results The CPT of the foramen area increased. The low frequency of TENS inhibited the slow pain significantly, the high frequency of TENS inhibited both of the slow pain and the fast pain. The inhibition to the pain is more obviously influenced by the frequency of TENS than the time of TENS. Conclusion It is confirmed the channel and point TENS can influence the CPT of the specific reaction area,the choice of the appropriate frequency of the treatment is more important than extending the treatment time alone.

    The expression and clinical significance of sTn in endometrial carcinoma

    Object To investigate the expression and clinical significance of tumor-associated carbohydrate antigen sTn in endometrium from patients with atypical hyperplasia of endometrium , endometrial carcinoma and benign diseases of the uterine. Also analyze the association between sTn and the pathologic stage , pathological grading of endometrial carcinoma. Methods 160 cases were detected by immunohistochemistry, including 60 subjuects with endometrial cancer, 50 subjuects with atypical hyperplasia of endometrium, and 50 subjuects with benign uterine lesions. Results The positive rate of sTn in atypical hyperplasia of endometrium and endometrial carcinoma and benign diseases of the uterine were71.7%、32%、14%, it’s significantly different from each other ( p<0.05 ) . Patietns with endometrioid carcinoma got sTn positvie in 78% among them, significantly higher than that in patients with benign uterine lesions, atypical hyperplasia of endometrium, endometrial serous cystadenocarcinoma (all p<0.05 ).The positive rate of sTn in patietns with poorly differentiated endometrial carcinoma was significantly higher than that in patients with highly differentiated endometrial carcinoma ( P<0.05 ). Conclusion: sTn may play a role in the tumorigenesis and development of endometrial carcinoma, it should be taken as an potential biomarker for endometrial carcinoma.

    The Study on Expression of Snail and RKIP in non-small cell lung cancer invasion and metastasis.

    Abstract: Objective To detect the expression of Snail and RKIP in NSCLC and its relationship with the clinicopathologic characteristics .Methods Immunohistochemical staining was used to detect Snail/RKIP expression in 124 cases of NSCLC and 67 cases of adjacent normal lung tissue .Results Immunohistochemistry revealed that of the 124 cases, there was a high expression of Snail and a low RKIP protein, 79.03% (98/124) and 36.29% (45/124) respectively. There was correlation of Snail and RKIP protein expression with the degree of differentiation of tumor 、TNM stage、 lymph node metastasis and the survival time(P<0.05, respectively). Snail protein levels were negatively correlated with RKIP in lung carcinoma (P <0.05). Conclusion The overexpression of Snail and the low expression of RKIP might be important biological markers for invasion and metastasis of NSCLC.

    Objective To evaluate the clinical significant and expression of the GDNF/ GFRα1/RET in the distal of rectum with congenital Imperforate anus (CIA). Methods Specimens were collected from resected colon in the 3 cm away to the anus and the distal of rectum with 12 CIA patients in Tianjin Children’s Hospital. Haematoxylin and eosin(HE) staining, immunohistochemical(IHC) staining and real-time quantitative(RT-PCR) methods were done in the different site of samples with CIA for RET, GDNF and GFRα1 expression. Results It was decreased for expression of ganglia and negative for expression of RET, GDNF and GFRα1 in the distal of rectum with 12 CIA patients. There were ganglionic cells in the 3cm away to the anus with 12 CIA patients. It was positive for expression of RET, GDNF and GFRα1 in the 3cm away to the anus with 12 CIA patients. middle and low imperforate anus were more expression than high imperforate anus in the expression of neurotrophic factors that had RET, GDNF and GFRα1. The former were more complications than the latter in terms of postoperative function recovery. Conclusions The unsatisfactory anorectal function is possible related to the decrease or lost of neurotrophic factors(GDNF/ GFRα1/RET) and ganglia in the myenteric plexuses.

    Safety and Efficiency of Anticoagulation Methods during CVVH in the Patients with MODS with High-Risk Bleeding

    [Abstract] Objective To investigate safety and efficiency of anti- coagulation methods in the patients with high-risk bleeding multiple organ dysfunction syndrome (MODS) during continuous veno-venous hemofiltration(CVVH). Methods Forty patients with high-risk bleeding MODS during CVVH in our hospital were divided into heparin-free group(A group) and low-dose heparin group(B group). The changes of blood coagulation function, the counts of platelets, blood urea nitrogen, serum creatinine, PaO2/FIO2 and Apache Ⅱscores in two groups before treatment, at 24h,48h after treatment were measured. Filter lifespan, median ventilation time, ICU admission time and bleeding complications were observed. Results 1. There was significant difference between the levels of blood urea nitrogen, serum creatinine, PaO2/FIO2 and Apache Ⅱscores at 24h,48h after treatment in low-dose heparin group and that in heparin-free group (P<0.05).2. The levels of APTT, TT were prolonged,the counts of PLT were significantly lower at 24h after treatment than that before treatment in low-dose heparin group, the levels of APTT, TT, the counts of PLT had no changes comparing prolonged time of CVVH therapy. 3. Average ventilation time, ICU admission time were obviously shorter in low-dose heparin group than that in heparin-free group, Filter lifespan was longer in low-dose heparin group than that in heparin-free group, there was significant difference (P<0.05). 4. Bleeding in skin and mucosa was observed in 1 case in low-dose heparin group, there was no other severe bleeding complications. Conclusion The results of monocentic study show that low dose of heparin ensure smooth operation of CVVH in the Patients with MODS with high-risk bleeding, clinical application is safe, efficient.

    Protective Effect of Compound coenzymes with Vitamin E on Liver and Kidney Injury Induced by Neonatal Hyperbilirubinemia

    Objective To study the effect of compound coenzymes with vitamin E on liver and kidney Injury Induced by neonatal hyperbilirubinemia. Methods 150 term neonatals with hyperbilirubinemia were chosen as observation groups(According bilirubin divided into mild, moderate and severe groups),45 healthy term or non- hyperbilirubinemia newborns were selected as control group. Serum total bilirubin ,γ-GGT , MDA and Cys-c were measured within 24 hours after charge in.The observation groups were randomly divided into regular and combined treatment groups to study the effect of medicines.Compound coenzymes with vitamin E as well as regular methods were given to combined group while regular group only recived regular methods.All the above biochemical indexes were tested in 7th day.Results MDA in blood were higher in all observation groups (mild, moderate and severe groups)than in control group(P<0.05);But the levels of γ-GGT and Cys-c increased compared with control group in moderate and severe groups (P<0.05),There were positive corelations(P<0.05) among the levels of serum total bilirubin and MDA、γ-GGT and Cys-c. Positive linear correlation were found between MDA and γ-GGT、Cys-c(P<0.01). After early treatment bilirubin decreased and γ-GGT 、Cys-c and MDA declined,with a faster recovery in combined treatment group than regular group(P<0.05).Conclusions In hyperbilirubinemia newborns, Lipid peroxidation activated by bilirubin may cause damages of liver and kidney. Compound coenzymes with vitamin E have protective effect to these damages based in conventional treatment in hyperbilirubinemia.

    The diagnostic value of myocardial enzyme on cardiac insufficiency in patients with severe acute pancreatitis

    [Abstract] Objective To explore value of myocardial enzyme, such as cardiac troponin I(cTn-I), on diagnosis of cardiac insufficiency in patients with severe acute pancreatitis(SAP). Methods Fifty patients with severe acute pancreatitis from emergency, gastroenterology and Intensive Care Unit of our hospital were collected from 2008 to 2013. Serum biochemical markers, such as cTnI, myoglobin (MYO), Creatine kinase MB (CK-MB), lactate dehydrogenase (LDH) and aspartate aminotransferase (AST) were examined, APACHE Ⅱ score was also calculated. All patients were divided into two groups: cardiac insufficiency group and non-cardiac insufficiency group. The values of myocardial enzyme were compared using t test for normally distributed variables and rank-sum test for variables not normally distributed. The sensitivity and specificity of myocardial enzyme for cardiac insufficiency diagnosis in patients with SAP were evaluated using ROC cure. The relationship between myocardial enzyme and APACHE Ⅱ score was assessed by Pearson correlation coefficient. Results The level of cTn-I、LDH and AST in case of cardiac insufficiency group was significantly increased than that in case of non-cardiac insufficiency group(P <0.05). The area under ROC curve for cTn-I was most large (0.940) than that for LDH (0.762) and AST (0.770). With a cut-off value of 0.07ng/ml, the sensitivity and specificity of cTn-I for diagnosing cardiac insufficiency in patients with SAP were 0.857 and 1.000, respectively. cTn-I was positively correlated with APACHE Ⅱ(P<0.05). Conclusion Serum cTn-I show the better ability in diagnosis of cardiac insufficiency, and can preferably reflect the severity of SAP.

    Blood oxygen level dependent magnetic resonance imaging of normal kidney and correlation with physiological indexes: A preliminary study

    Objective To explore the characteristics of blood oxygen level dependent (BOLD) magnetic resonance imaging (MRI) in healthy native kidneys. To investigate the relationship between BOLD-MRI and common physiological indexes. Methods Ninety patients who were ruled out chronic kidney diseases underwent BOLD-MRI with T2*-spoiled gradient recalled echo (T2*SPGR) sequence. BOLD images were analyzed on R2*map software to yield R2* pseudo-color map. Cortical and medullary R2* values were analyzed in bilateral kidneys and in different gender. Different regional R2* values in cortex and medulla were also analyzed. Physiological indices including age, body height and weight, body mass index (BMI), body surface area (BSA), estimated glomerular filtration rate (eGFR) was recorded. Correlation between R2* value and physiological indices were analyzed. Results Bilateral renal cortical R2* values were less than values in medulla. Female and male cortical R2* values were also less than corresponding values in medulla. In bilateral renal medullary region, R2* values in low pole was less than values in middle and upper pole (F =15.184 , P <0.001). Age positively correlated with R2* values in medulla(r =0.284, P =0.002). However, eGFR negatively correlated with medullary R2* value (r =-0.232, P =0.007). Conclusions BOLD MRI can directly visualize renal oxygenation. Oxygenation level discrepancy exists in different regions of renal parenchyma. In healthy kidneys, renal medullary oxygenation level attenuated when patient age increase and eGFR decrease.

    Nephroscope combined with S-shaped urethral dilator for the treatment of male urethral stricture

    Objective To evaluate the clinical value of the combination of S-shaped urethral dilator and nephroscope for the treatment of male urethral strictures. Methods Under topical anesthesia or topical anesthesia combined with intravenous anesthesia, the urethral dilation with S-shaped urethral dilator were carried out in 18 cases. All dilators were inserted over guidewires in nephroscope after the examination of urethral stricture. All catheters were located across the strictures and remained for 4-6 weeks. Regular follow-up were done on all cases to assess the clinical effect of urine flow. Results All surgeries were successful without serious complications. Mean operative time was 40 min. The voiding symptoms and urinary flow rate situation were significant improved after catheter removed in all case who received urethral dilation at least 6 week during a mean follow-up time of 34 weeks. No case has incontinence, diminished sexual function and other complications. Conclusion Nephroscope combined with S-shaped urethral dilator for the treatment of male urethral stricture is intuitive, accurate, feasible, minimally invasive, safe and has a high success rate .This method is worthy of recommendation.
