    The Relationship between Glycosylated Hemoglobin, Serum Visfatin Level and Perinatal Outcome in Pregnant Women with Normal Glucose Tolerance

    [Abstract]   Objective   To investigatethe relationship between the levels of serum glycosylated hemoglobin (HbA1c),
    serum visfatin, glucose and lipid metabolism and the perinatal outcome in pregnant women with normal glucose tolerance
    (NGT).   Methods   A total of135NGT pregnant women diagnosed by oral glucose tolerance test (OGTT) entered this study. Patients were divided into two groups, high HbA1c group (HbA1c≥5.5%,n=66) and normal HbA1c group (HbA1c<5.5%,n=69). The clinical data were collected in two groups. HbA1c levels were measured by HPLC, and the serum visfatin levels were measured by ELISA method. The values of body mass index (BMI), pregnant weight gain, visfatin, glucose and lipid metabolic levels were analyzed between two groups. The relationship between levels of HbA1c, visfatin, glucose and lipid metabolism and perinatal outcome were compared between two groups.  Results  The values of mean pregnancy weight gain, visfatin, total cholesterol (TC), triacylglycerol (TG), low density lipoprotein (LDL) and high density lipoprotein (HDL) were higher in high HbA1c group than those of normal HbA1c group. In NGT women with high HbA1c level,there were higher average birth weight, lower blood glucose level in newborn babies and higher rates of cesarean section. The incidences of polyhydramnios, meconium- stained fluid, macrosomia and neonatal hypoglycemia were also higher in NGT women with high HbA1c than those of normal HbA1c group. There was a positive relationship between HbA1c level, pregnant weight gain and visfatin andbirth weight of newborn babies in135NGT pregnant women.  Conclusion   The excessive weight gain during pregnancy can induce the up- regulation of the visfatin level, resulting in the increased blood glucose level. The optimal timing and mode of delivery could prevent the occurrence of adverse pregnancy outcomes.

    [Abstract]   Objective  To investigate the aberrant methylation and expression of growth arrest and DNA-damage-in- ducible45gamma (GADD45G) gene in gastric cardia adenocarcinoma (GCA).  Methods  Bisulfite conversion-methylation specific polymerase chain reaction method (BS-MSP) and immunohistochemistry method were used respectively  to detect the methylation status and protein expression of GADD45G in138GCA tumor tissues and corresponding normal tissues.  Results  The methylation status of GADD45G distal promoter (region1) was not detected in GCA tumor tissues and corresponding normal tissues. For GADD45G region2and region3, the BS-MSP results of region 3were identical to that of region2. The methylation frequency of proximal promoter and exon 1in GADD45G island2(region2and region3) in GCA tumor tissues (49.3%,68/138) was significantly increased compared to that in corresponding normal tissues (0, P<0.01). The methylation status of this two sites in tumor tissues was associated with TNM stage of tumors (P<0.05). The protein expression of GADD45G in tumor tissues was significantly decreased than that in corresponding normal tissues (P<0.05),and threre was a significant negative correlation with methylation status of GADD45G proximal promoter and exon1(rs=-0.398).  Conclusion   Thedecreased expression of GADD45G by hypermethylation of proximal promoter exon1of the gene may play an important role in gastric cardia adenocarcinoma.

    [Abstract]   Objective   To study mechanisms of lipopolysaccharide (LPS) release from Acinetobacter baumannii (A.
    baumannii) induced by biapenem (BIPM), meropenem (MEM) and levofloxacin (LVFX).  Methods   A. baumannii was exposed to different concentrations of BIPM, MEM, LVFX, and ceftazidime (CAZ) at different time points. The LPS release was measured by tachypleus amebocyte lysate (TAL) test. Quantities of bacteria exposing to0.5MIC antibiotics were measured.The morphology of bacteria treated with different antibiotics for7-hour was evaluated by light microscope.  Results   The LPS releases of A. baumannii treated with four kinds of antibiotics were significantly less than those of control group. In the later period, LPS releases of A. baumannii exposed to the high concentration of antibiotics tended to smooth. The cell quantities of bacteria were significantly lower in treatment group than that of control group. The cells of A. baumannii treated with CAZ and LVFX showed long filamentous shape. The bacteria exposed to MEM and BIPM showed obviously globular. At the same time, some bacterial cells were induced to lysis rapidly by BIPM.  Conclusion   The level of LPS release is influenced by kinds of antibiotics, concentration and incubation period. All kinds of antibiotics could induce morphological changes of A. baumannii, which may be related to the different mechanisms that bacteria induce the different levels of LPS release.

    Effects of Celecoxib on the Cell Migration, MMP-9and VEGF Expression in Human Gastric Cancer SGC-7901

    [Abstract]   Objective   To observe selective cyclooxygenase-2(COX-2) inhibitor celecoxib on the cell migration,
    MMP-9and VEGF expression of human gastric cancer SGC-7901.   Methods   In vitro culture of SGC-7901cells, the cells were divided into blank control group and celecoxib treatment group (different concentrations of celecoxib25, 50and 100μmol/L). SGC-7901cells were treated with celecoxib for24h. The effect of celecoxib on cell migration of SGC-7901 was detected by the damage repair experiment. RT-PCR assay was used to detect the effect of celecoxib on MMP-9mRNA and VEGF mRNA expressions in SGC-7901cells.  Results   The migration distance of SGC-7901cells was decreased with the increased concentration of celecoxib. There were significant differences between medium-dose and high-dose celecoxib groups and the control group. RT-PCR assay showed that MMP-9mRNA and VEGF mRNA expressions in gastric cancer SGC-7901were significantly decreased with the increased concentration of celecoxib, and there was a significant difference compared with those of control group  (P<0.05).  Conclusion   Celecoxib can inhibit migration of human gastric cancer SGC-7901cells, which may be related to the inhibition of COX-2downstream of MMP-9and VEGF expressions.

    Relationship between Renin-Angiotensin System Blockers and Plasma Level of Adiponectin in Patients with Metabolic Syndrome

    [Abstract]???Objective???To explore the relationship between renin-angiotensin system (RAS) blockers and plasma level of adiponectin (APN) in patients with metabolic syndrome (Mets).??Methods?? Fifty-three patients with Mets were randomized into control group (n=23), angiotensin-convertingenzyme inhibitor (ACEI) group (benidipine, 5mg /d,n=15) and angiotensinll recepter blockers(ARB)group (candesartan,4mg/d,n=15),and were given4-week treatment. Twenty-three patients, who were avoiding application related drugs affecting the RAS for four weeks, were used as control. The values of plasma APN, fasting blood glucose (FBG), fasting insulin (FINS), triglyceride (TG), high-density lipoprotein cholesterol (HDLC), total cholesterol (TC), low-density lipoprotein cholesterol (LDL-C), creatinine(CR), aspartate aminotransferase (ALT), bilirubin (BIL) and insulin resistance index (HOMA-IR) were detected and compared before and after treatment.???Results??? The levels of FBG and LDL-C were significantly decreased after treatment than those before treatment in ACEI group. The level of CR was significantly lower after treatment than that before treatment in ARB group (P<0.05). The plasma APN level was significantly higher after treatment compared to that before treatment in ACEI and ARB groups (P<0.01).There were no significant differences in the values of FINS,HOMA-IR,TC,ALT and BIL between'before and after treatment' in three groups (P>0.05). The levels of CR, TC, ALT and BIL before treatment were influencing factors for the APN level. The levels of FBG and TC after treatment were influencing factors for the APN level.??Conclusion?? APN may serve as potential therapeutic targets for treating Mets and its related complications by ACEI and ARB.

    Modified V Shaped Osteotomy on the First Distal Metatarsal for Mild to Moderate Hallux Valgus

    [Abstract] Objective To introduce the surgical strategy of modified V shaped osteotomy on the first distal metatarsal, and to discuss its benefit for the treatment of mild to moderate hallux valgus. Methods Totally 18 hallux valgus patients with refer metatarsalgia (24 feet) were operated by modified V shaped osteotomy on the first distal metatarsal from January in 2008 to January in 2012, all females, with an average age of 42.6 years (range, 30 to 55 years). Pre- and postoperation, HVA and IMA1~2 were measured by X-Ray. Plantar pressure was measured by Plantar pressure measurement system. AOFAS scoring system was employed to evaluate clinical curative effect. Results HVA was corrected from 30.2°±3.3° preoperatively to 11.8°±1.8° at the last visit(P<0.05), IMA1~2 was corrected from 13.8°±1.9° preoperatively to 6.6°±1.0° at the last visit(P<0.05), The plantar pressure return to normal postoperatively, AOFAS score was raised from (58.2±8.6) to (92.8±3.4)(P<0.05) postoperatively. Conclusion The surgical strategy of modified V shaped osteotomy on the first distal metatarsal is proved to be effective in treating mild to moderate hallux valgus with refer metatarsalgia, in terms of strong capability of correction, lower incidence of nonunion, allowance of early weight bearing.

    188Re-fo?late-CDDP/HAS MNP) on human SKOV3ovarian cancer cells in vivo.  Methods   The human SKOV3 ovarian cancer model was established in mice. Sixty-four tumor-bearing mice were randomly divided into eight groups: (A) negative control group,(B) chemotherapy group, (C) radiotherapy alone group, (D) hyperthermia alone group, (E) chemotherapy combined with radio?therapy group, (F) chemotherapy combined with hyperthermia therapy group, (G) radiotherapy combined with hyperthermia therapy group and (H) hyperthermia, chemotherapy and radiotherapy combined treatment group. After treatment, the cell proliferation and tumor growth were observed. The inhibitory rate of tumor mass was measured. The histopathological changes of tumor were observed in all groups.   Results   The quality of tumor was significantly lower in treatment groups than that of control group (P<0.05). There was the lowest quality of tumor in hyperthermia, chemotherapy and radiotherapy combined treatment group than that of other treatment groups (P<0.05).   Conclusion   The combination of magnetic induction hyper?thermia, chemotherapy, targeted radionuclide of radiation exposure can effectively inhibit the growth of ovarian cancer,which has the potential application for ovarian cancer treatment

    [Abstract]   Objective   To investigate the effect of1, 25- dihydroxy-vitamin D3(1, 25(OH)2D3 ) on adipocyte differentiation and the underlying mechanism.   Methods   The mesenchymal stem cell line C3H10T1/2was randomly divided into6 groups including control group, differentiation group and4different doses of 1, 25(OH)2D3 groups. The control group wastreated with vehicle. The differentiation group was supplemented with adipocyte differentiation reagent. And the 1, 25(OH)2D3 groups were treated with adipocyte differentiation reagents and10-9, 10-8, 10-7and10-6 mol/L of1, 25(OH)2D After culturing for5days, the cells were stained with oil red O, and the expression levels of adipocyte-specific transcription factors and Wnt/ β-catenin signaling pathway related genes were examined by RT-PCR or Western blot methods. Results   1, 25(OH)2D3 significantly reduced the number of differentiated adipocytes and blocked the mRNA levels of adipocyte specific transcription factor PPARγ(peroxisome proliferator-activated receptor gamma), C/EBP α(CCAAT enhancer binding proteinα) and adipocyte characterization factor aP2(fatty acid binding protein 4). These were paralleled by the decreased mRNA expression of Wnt/β-catenin signaling pathway inhibitor sFRP1(Secreted frizzled-related protein 1) and the increased level of β-catenin protein.  Conclusion   1, 25(OH)2D3 inhibits adipocyte differentiation, which may be related to the activation of Wnt/β-catenin signaling.

    Transfection of Lentivirus Recombined with Marker Gene into Human Umbilical Cord Wharton’s Jelly-Derived Mesenchymal Stem Cells

    [Abstract]   Objective   To investigate the optimal condition of lentivirus,which was recombined with marker gene of
    enhanced green fluorescent protein (Lentivirus-EGFP) transfect human umbilical cord wharton’s jelly-derived mesenchymal stem cells (HUWMSCs) and the effect of transfection on the proliferation in HUWMSCs.  Methods   HUWMSCs were transfected with EGFP by lentivirus vector in vitro via different multiplicity of transfection (MOI) in four different transfection methods (A, B, C and D). The fluorescence expression and the transfection efficiency in different methods were analyzed by both fluorescent microscope and flow cytometry. The proliferation rates of infected HUWMSCs was evaluated by MTT method.  Results   The transfection efficiency was10.6%-87.3% after4days in all experimental groups, which showed the dose-effect relationship with MOI. Polybrene (5mg/L) could significantly increase the transfection efficiency (P<0.05). Results of MTT assay showed that there were significant differences in the proliferation rates of infected HUWMSCs between different transfection methods (P<0.001). There was better cell proliferation in method A (MOI=10) group and method B (MOI=10) group than that of other groups.   Conclusion   Method B (MOI=10) is the optimal transfection method in this experiment. HUWMSCs could be transfected by lentivirus-EGFP with high efficiency and could stably express transfected gene within2weeks, which is a safe and effective gene transfer vector.

    [Abstract]   Objective  To investigate the effect of hypoxia preconditioning (HPC) on endoplasmic reticulum stress
    induced by cerebral ischemia-reperfusion (I/R) in rats.  Methods   Thirty-six male SD rats were randomly divided into three groups equally with twelve rats each: sham operation group (group S), group I/R and group HPC. The rat model of focal cere?bral I/R was produced by occluding right middle cerebral artery for1h followed by24-h reperfusion. In group HPC,8% oxy?gen was inhaled for3h at12h before ischemia. No nylon suture was inserted in group S. Neurological deficits were assessed at the end of24-h reperfusion. And then rats were decapitated. The expressions of growth arrest and DNA-damage-inducible gene 153(GADD153) protein in ischemic penumbra cerebral cortex were determined by immunohistochemical staining.The apoptotic neurons were detected by TUNEL assay, and the apoptotic index was calculated.  Results   The neurological deficit score and apoptotic index were significantly higher, the expression of GADD153protein was significantly up-regulated, in group I/R and group HPC than those in group S. The neurological deficit score and apoptotic index were significantly lower, the GADD153protein expression was significantly down- regulated, in group HPC than those in group I/R (allP<0.05).   Conclusion   Hypoxia pretreatment can reduce the focal cerebral I/R-induced neuronal apoptosis mediated by endo?plasmic reticulum stress, which may be related with the down-regulation of GADD153protein expression in cerebral cortex.

    [Abstract]   Objective   To investigate the effects of celecoxib on the expressions of nuclear factor-κB (NF-κB) p65
    and inducible nitric oxide synthase (iNOS) in rat model of acute lung injury induced by lipopolysaccharide (LPS).  Methods   Sixty rats were randomly divided into control group, LPS group, treatment group and celecoxib group (15rats for each group).To copy the rat model of acute lung injury, LPS(5mg/kg)was injected into the tail vein in LPS group. Celecoxib (20mg/kg)was administered by gavage after30-minute modeling in treatment group. Celecoxib (20mg/kg)was also administered by gavage in celecoxib group. The same volume of normal saline was treated in control group. Rats were sacrificed after3h in four groups. The expressions of cyclooxygenase-2(COX-2), NF-κB p65and iNOS protein were detected by Western blot analysis. The expressions of NF-κB p65and iNOS mRNA were evaluated by reverse transcription polymerase chain reaction (RT-PCR) assay.  Results  Compared with control group, expression levels of COX-2, NF-κB p65, iNOS protein,NF-κB p65 and iNOS mRNA were significantly higher in LPS group (P<0.01). The NF-κB p65, iNOS mRNA and protein expressions were significantly decreased in treatment group than those of LPS group (P<0.01).   Conclusion   The selective COX-2inhibitor celecoxib has a protective effect on acute lung injury in rats.

       Study on correlation between contrast-enhanced sonography parameters and microvessel density of pleural-based pulmonary carcinoma

       Abstract: objective to investigate the relationship between contrast-enhanced sonographic characteristics and tumor microvessel density (MVD) in pleural-based lung cancer. Methods 40 patients with biopsy-proven pleural-based lung cancer (22 patients with adenocarcinoma and 18 patients with squamous cell carcinoma) performed contrast-enhanced sonography prior to biopsy procedure. MVD of the biopsy specimens were assessed by histochemical staining using a mouse antihuman CD34 monoclonal antibody. Peak enhancement and enhancement index derived from time intensity curve were correlated with MVD. Results Lung adenocarcimoma showed statistically higher value than squamous cell carcinoma in respect to PE , EID, and MVD(P<0.01). A linear positive correlation was reveal between enhancement parameters (PE, EID) and MVD for both adenocarcimoma and squamous cell carcinoma(P<0.01). Conclusion Peak enhancement and enhancement index of pleural-based lung cancer on contrast-enhanced sonography were correlated with tumor’s MVD. This indicates that contrast-enhanced sonography can be used to evaluate angiogenesis of pleural-based lung cancer.

    Clinical distribution and drug resistance of Pseudomonas aeruginosa from ophthalmic disease

    【Abstract】Objective To master the clinical distribution and drug resistance of Pseudomonas aeruginosa in eye hospital, to direct the doctor reasonably use the antibiotics. Methods A retrospective analysis was performed for the distribution and drug resistance of 40 Pseudomonas aeruginosa strains isolated from Eye hospital during Jan 2009-Dec 2012. Results The detection rate of Pseudomonas aeruginosa was 5.3% in eye hospital; ranking the first in the Gram-negative bacteria, most Pseudomonas aeruginosa isolates were isolated from corneal ulcers(67.5%). This bacterium was high drug resistance rate to ampicillia/sulbactam, CoSMZ and cefotaxime(100%); and well sensitive to amikacin, polymyxin E, meropenem, piperacllin/tazobactam and tobramycin. Conclusions The drug resistance of Pseudomonas aeruginosa in eye hospital is lower than them in general hospital, but it should strengthen the monitoring of drug resistance of Pseudomonas aeruginosa in order to reduce the emergence of multidrug-resistant strains.

    Expression of Bcl-2、Ki-67 protein in prostatic carcinoma and their clinical significance

    Abstract Objective:To study the expression and clinical significance of Bcl-2 and Ki-67 in prostatic carcinoma.Methods:Immunohistochemical ABC was used to determine the expression of Bcl-2 and Ki-67 in 20 cases of BPH tissue and 60 cases prostatic carcinoma.Results: The positive incidences of Bcl-2 and Ki-67 expression in BPH tissue were5% 、15%,and prostatic carcinoma were 63%±48.2%、70%±46.9%.The expressionof Bcl-2 and Ki-67 correlated to preoperative serum prostate-specific antigen level,extraprostatic extension,the degree of histobogical grade and clinical stage(P<0.05).The expression of Bcl-2 had a positive relation with expression of Ki-67(r=0.167 P<0.05). Conclusion;The combined detection of Bcl-2 and Ki-67 proteins can be used to evaluate the incidence of prostate carcinoma development, prognosis, and provide an important basis for drug treatment .

    The Clinical Diagnosis and Surgery Treatment of Hepatic Angiomyolipoma

    Objective To evaluate the clinical diagnosis and surgery treatment of hepatic angiomyoilpoma(HAML).Methods 5 HAML cases were reviewed retrospectively.The average age of the 5 patients(1 male and 4 female) was 50.8 years old.Serum tumor markers levels were all within normal limits. Imaging studies include abdominal US, CT scan in all patients, contrast enchanced US(CEUS) in 3 patients,and MRI in 2 patients. All these 5 patients underwent liver resection,The diagnoses were confirmed by pathologic examination and immunochemical assays with a broad panel of antibodies.Results The postoperative pathology showed HAML in all the patients; and the positive rate of HMB-45 staining by immunohistochemistry was 100%(5/5).Accurate preoperative diagnosis of HAML were made in 1 patients by CEUS,2 patients by CT,and 1 patients by MRI. Folow up of 6 months to 36 months in all 5 patients with no evidence of recurrence.Conclusions There are no specific signs and symptoms in HAML patients;The preoperative imaging diagnosis is difficult. surgical resection should be considered as a treatment choice for HAML.

    Endometriosis is a common gynaecological disorder, and although extensively studied its etiopathogenic mechanisms remain unclear. Stem cells are undifferentiated cells that have the ability to self-renew as well as to produce more differentiated daughter cells in particular conditions. Recently, the identification of putative endometrial stem/progenitor cells (SPCs) has emerged as a possible origin of endometriotic lesions and a new mechanism for endometriosis etiology. This new hypothesis could account for the etilogy of all kinds of endometriosis, it has been raising considerable scientific and clinical interest not only for the breakthrough of the pathogenesis, but also for the new target on therapy.

    [Abstract]   Objective   To investigate the expression and clinical significance of four histamine receptors: H1R, H2R,
    H3R and H4R in urinary bladder of patients with ketamine-induced cystitis. Methods   Immunohistochemical S-P method was used to detect the expression levels of histamine receptors: H1R, H2R, H3R and H4R in bladder tissues of10patients with ketamine-induced cystitis (experimental group) and distal tissue away from bladder tumors of10patients with cystectomy (control group). The average optical density (OD) values of four kinds of different histamine receptors were separately calculated by Imagepro-plus 6.0in two groups. At the same time, mast cells were marked by toluidine blue special dyeing and were counted.  Results   Comparing with control group, the expression levels of H1R, H2R, and H4R were significantly increased in experimental group (P <0.05). Mast cells diffused interstitial bladder infiltrates (P<0.05). There was no significant difference in the expression of H3R in two  groups. Conclusion  Mast cells, H1R, H2R, and H4R are closely related to the ketamine-induced cystitis, which may be new diagnostic indicators and new treatment targets of ketamine-induced cystitis.

     [Abstract]   Objective   To explore the clinical value of the different components of glycosylated hemoglobin in pa-
    tients with coronary artery disease (CAD).   Methods   A total of217patients were divided into3groups: CAD group (groupⅠ,n=60), CAD patients without acute coronary syndrome (ACS) and with diabetes mellitus group (groupⅡ, n=60) and ACSpatients with diabetes mellitus group (group Ⅲ,n=97). Fifty-eight healthy volunteers in the same time period were selected as control group. The values of fructose glycosylated hemoglobin (HbA1a), lactose glycosylated hemoglobin (HbA1b), glucose glycosylated hemoglobin (HbA1c), hemoglobin P3component (HbP3), hemoglobin A0component (HbA0), unstable glycosylated hemoglobin (LA1c/CHb1) and alkali-resistant hemoglobin (HbF) were measured. These parameters were compared between4groups. Logistic regression was used to analyze factors that influencing CAD and CAD with diabetes mellitus. The receiver operating characteristic (ROC) curve was used to analyze the diagnostic efficiency of these factors.  Results  There were significant differences in titers of HbA1b, HbA1c, HbP3and HbA0between groupⅠ, groupⅡ, group Ⅲ and control group (P<0.01, or P<0.05). There were no significant differences in levels of glycosylated hemoglobin parameters between group Ⅱ and group Ⅲ (P>0.05). Logistic regression and ROC curve analysis showed that HbA1c and HbP3were independent effects of CAD, and there were some diagnostic efficiency of CAD. The diagnostic efficiency of ROC curve was consistent in HbA1c and HbP3between group Ⅱ and group Ⅲ.  Conclusion   Levels of HbA1b, HbA1c, HbP3and HbA0are closely related to CAD and CAD with diabetes mellitus. HbA1c and HbP3are independent effects of CAD and, there are some diagnostic efficiency in CAD.

    gen induced arthritis (CIA) in rats.  Methods   The expression vectors of siRNA against TNF-αgene were constructed successfully and were injected by tail veil into CIA rats. Twenty-four CIA rats were randomly divided into4groups including model group, empty vector group, TNF-α-siRNA1group and TNF-α-siRNA2group. CIA rats were injected with the same dose of phosphate buffered sodium (PBS) and pGFP-V-RS vector respectively in model group and empty vector group, while TNF-α-siRNA1group and TNF-α-siRNA2group were injected with TNF-α-siRNA1eukaryotic expression vector andTNF-α-siRNA2eukaryotic expression vector respectively. Another6rats, which were not established CIA model, were in-jected with PBS (blank control group). The serum expression levels of IL-1were detected by ELISA on day1, 5, 9and13after injection. The expression level of TNF-αmRNA was detected by reverse transcriptase polymerase chain reaction (RTPCR) on day13.   Results   The expression level of IL-1was significantly higher on day1, 5, 9and13in model group than that of blank group (P<0.05). The expression levels of IL-1were significantly lower on day1, 5and9in TNF-α-siRNA1group and TNF-α- siRNA2group than that of model group and blank group (P<0.05). The expression level of TNF-αmRNA was significantly higher on day13in model group than that of blank group (P<0.05). The expression levels of TNF-αmRNA were significantly lower in TNF-α-siRNA1group and TNF-α-siRNA2group than those of model group and empty vector group (P<0.05).   Conclusion   TNF-αspecific siRNA can suppress the levels of TNF-αmRNA and IL-1, which provides experimental basis for gene therapy of rheumatoid arthritis.

    [Abstract]   Objective   To investigate the protective effects and mechanisms of rosiglitazone on lung injury in severe
    acute pancreatitis (SAP).  Methods   Seventy- two SD rats were randomized into three groups: sham operation (SO) group,SAP group and rosiglitazone-pretreated group. The model of SAP was induced by retrograde injection of5% sodium taurocholate into the bili-pancreatic duct in SD rats. Rats of rosiglitazone-pretreated group were given10mg/kg rosiglitazone in?traperitoneally 30min before inducing SAP. The amylase plasma levels, levels of TNF-αandp(O2), the myeloperoxidase (MPO) and the wet/dry ratio of lung tissue were measured. The expression of NF-κB in pulmonary tissues was assayed by immunohistochemistry. The expressions of TNF-αmRNA and intercellular adhesion molecule1(ICAM-1) mRNA in pulmonary tissues were detected by reverse transcript PCR (RT-PCR). The histopathological changes of pulmonary tissues were evaluated.  Results   Compared with SO group, the plasma levels of amylase and TNF-α, the intrapulmonary MPO were significantly increased in SAP group (P<0.05). The expression levels of NF-κB, TNF-αmRNA and ICAM-1mRNA in lung tissue were also significantly increased in SAP group (P<0.01). There were significantly lower levels of the above indicator in rosiglitazone-pretreated group than those of SAP group (P<0.05). Compared with SO group, the lung wet/dryratio was significantly higher at6h and12h in SAP group (P<0.05). The values of lung wet/dry ratio were significantly lower at 6h and 12h in rosiglitazone-pretreated group than those of SAP group (P<0.05). The lung tissue damage was significantly reduced. The level ofp(O2)was significantly decreased in SAP group than that of SO group (P<0.05). The level of p(O2)was significantly increased at6h and12h in rosiglitazone pretreated group than that of SAP group (P<0.05).   Conclusion   Rosigli?tazone might have protective effects on SAP-related lung injury by inhibiting NF-κB and decreasing the production of TNF-αand ICAM-1in the lung tissues

    Changes of c-fos mRNA in Endothelial Cells Exposed to Various Intermittent Hypoxia

    [Abstract]   Objective   To measure the mRNA expression of immediate-early gene c-fos in endothelial cells by the
    use of different intermittent hypoxia (IH) protocols.  Methods   A gas control delivery system was homemade, which produced IH/ re-oxygenation(ROX) environmental exposure. The endothelial cells were exposed to IH/ROX cycles, and were divided into three groups (A, B and C). There were five sub-groups in each group. Group A included intermittent normoxia group, standard incubator control group, continuous hypoxia group (CH),1.5%O2 IH group and10%O2 IH group. There were different frequency IH of1.5%O2 and10%O2sub-groups in group B and group C. Hypoxia time was15s. The intermittent hypoxia cycle was60and the ROX time was different. The total cycle time was different, including1.5h group,3h group,5h group,6.5h group and9.5h group. The mRNA expression of immediate-early gene c-fos was measured by real-time PCR.  Results   There was no significant difference in the level of c-fos mRNA between intermittent normoxia group and CH group. The expression of c-fos mRNA was significantly higher in1.5%O2 IH group and10%O2 IH group than that of intermittent normoxia group, and there was a higher expression level of c-fos mRNA in 1.5%O2 IH group than that of 10%O2 IHgroup (P<0.01). It was found that the mRNA expression of c-fos increased gradually at first, and then gradually decreased. The expression of c-fos mRNA was significantly higher in5h group than that of other groups (P<0.01).   Conclusion   The mRNA expression of immediate-early gene c-fos is increased after exposing to IH/ROX in endothelial cells, which isclosely related with IH extents and frequencies.

    Clinical Study of Coupled Plasma Filtration Adsorption Combined with High Volume Hemofiltration Treatment in Patients with Severe Acute Pancreatitis

     [Abstract]   Objective   To observe the clinical efficacy of coupled plasma filtration adsorption (CPFA) combined
    with high volume hemofiltration (HVHF) for severe acute pancreatitis (SAP) and explore the practicality, the safety and the therapeutic mechanism.  Methods  This was a prospective, randomized clinical trail. A total of29patients with SAP were divided into two groups: treatment group (n=15, CPFA and HVHF) and control group (n=14, HVHF). The APACHEⅡscore, serum triglyceride, liver and kidney function, white blood cell (WBC), heart rate (HR), respiration, mean arterial pressure (MAP) and oxygenation index were compared before and after treatment between two groups. The organ function was assessed, and the adverse reactions and prognosis were observed in two groups. Results   After treatment, there were significant decreases in APACHEⅡscore and WBC, and significant increase in PaO2/FiO2 in two groups, especially significant in treatment group. There was a significant decrease in the level of triglycerides in 7-d treatment group than that of3-d treatment group (P<0.05). There were no thrombocytopenia, bleeding, allergies, blood clotting and other adverse reactions in two groups of patients. There were significant decreases in the blood purification time and the length of hospital stay in treatment group than those of control group (P<0.05). There was no significant difference in mortality between two groups.   Conclusion   CPFA combined with HVHF can effectively control inflammation, reduce the serum level of triglyceride and improve the prognosis in patients with SAP.
