    Expressions of Bcl-2and Caspase-3in Ulcerative Colitis

    [Abstract]  Objective   To study the correlation between the expressions of Bcl-2and Caspase-3in intestinal epithelial and inflammatory cells of lamina propria and the staging and pathogenesis of ulcerative colitis (UC). Methods The  pathomorphological changes of 25 samples of UC in the active phase and remission phase and25samples of control were observed under microscope. The expressions of Bcl-2and Caspase-3were detected by immunohistochemistry SP method. The correlations between expressions of Bcl-2, Caspase-3and UC staging were analyzed. Results   There were no significant differences in expressions of Bcl-2and Caspase-3in intestinal epithelial cells between three groups (P>0.05). The expression of Bcl-2in inflammatory cells of lamina propria was significantly higher in active UC group than that of normal control group(P<0.05), no significant difference between UC remission group and normal control group (P>0.05). There was a higher positive expression of Caspase-3in inflammatory cells of lamina propria in UC remission group. There was correlation between Bcl-2, Caspase-3and UC staging (r=0.371and0.4453respectively). Conclusion   The expression levels of Bcl-2 and Caspase-3were related with pathogenesis of UC. The detection of expression levels of Bcl-2and Caspase-3has a certain application value in UC staging.

    [Abstract] Objective   To investigate the expressions of PTEN and p21genes in Hazak patients with esophageal can
    cer. Methods  The expressions of PTEN and p21genes were detected by RT-PCR in48samples (cancer tissues and normal tissues) of patients with esophageal cancer. The relationship between the expressions of PTEN and p21genes, tumor differentiation, TNM stage, clinical phase and lymph node metastasis were analyzed.  Results  The positive rates of PTEN genewere75% and45.8% in cancer and distant normal tissues. The expression of PTEN was significantly higher in cancer tissues than that of distant normal tissues (χ2=8.537,P<0.05). The positive rates of p21gene were95.8% and97.9% in cancerand distant normal tissues, and no significant difference between them (χ2=0.344,P>0.05).There was no correlation between expressions of PTEN and p21and the tumor differentiation, the depth of invasion and lymph node metastasis in esophageal cancer.Conclusion   PTEN and p21genes are not the primary genes for the carcinogenesis of esophageal cancer inHazak.

    [Abstract]  Objective   To investigate the effect of tripterine on wear particle-induced inflammatory reaction by cell culture in vitro. Methods  Wear particles from artificial joints were prepared by vacuum ball milling, and made into particle suspension by using serum-free medium. RAW264.7 cells were cultured, subcultured and divided randomly into four groups according to different treatment factors: blank control group (group A), wear particle group (group B), wear particle + tripterine group (group C) and tripterine group (group D). After 24 hours, toxicity of tripterine was detected by CCK-8 assay; ELISA was used to detect the expressions of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) ; RT-PCR was used to detect the expressions of tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β) and nuclear factor-κB (NF-κ B) at the level of gene;Western blot was used to detect the expression of nuclear factor-κB (NF-κ B) at the level of protein. Results  The results showed 1μg/ml tripterine was little cytotoxic;the expressions of pro-inflammatory cytokines and the activation of NF-κB in wear particle group were significantly higher than those in blank control group (P<0.05),and the indexes after the treatment with tripterine (group C)decreased statistically (P<0.05). Conclusion  Tripterine can inhibit the wear particle-induced expression and release of pro-inflammatory cytokines at the level of gene and protein and the activation of NF-κB in RAW264.7 cells.

    [Abstract]   Objective   To investigate the expression and significance of trefoil factor3(TFF3) and stromal cell de-
    rived factor-1(SDF-1) in papillary thyroid carcinoma (PTC) and adjacent tissues, and to provide the basis for clinical diagnosis and prognosis evaluation.  Methods   The expressions of TFF3and SDF-1proteins in PTC and adjacent tissues were detected by immunohistochemical SP method. The relationship between expressions of TFF3, SDF-1and clinicopathological features was analyzed.   Results   (1) The positive rate and the expression level of TFF3protein were significantly higher in PTC than those in adjacent tissues, which were higher in patients with lymph node metastasis than those in patients without lymph nodes metastasis, and were higher in PTC patients of stage Ⅲ/Ⅳ than those in patients of stageⅠ/Ⅱ(P<0.05). The strong positive rate of TFF3protein in stage Ⅲ/Ⅳof PTC patients was higher than that in stageⅠ/Ⅱ(P<0.01). (2) The positive rate and the expression level of SDF-1protein were significantly higher in PTC than those in adjacent tissues, which were higher in patients with lymph node metastasis than those in patients without lymph nodes metastasis, and were higher in PTC patients of stage Ⅲ/Ⅳ than those in patients of stageⅠ/Ⅱ, and were significantly higher in patients over45years old than those of patients ≤45years old (P<0.05). (3) There was a positive correlation between expression levels of TFF3and SDF-1proteins in PTC (r=0.266,P<0.05).   Conclusion   The higher expressions of TFF3and SDF-1in PTC were closely correlated with carcinogenesis and progression, which may play a significant value in judging the malignant degree and progression of PTC.

    Femur Biomechanical Changes Caused by Femoral Stem Extension Implant in Total Knee Arthroplasty

    [Abstract]   Objective   To evaluate the femur biomechanical changes of finite analysis with different femoral stem extension implants in total knee arthroplasty (TKA). Methods   The full- length femoral CT scan was performed in selectedhealthy volunteers. The femoral digital model was established with Mimics10.0and Geomagic Studio10.0software. The models of femoral prosthesis and stem extension implant were created by laser scanner. According to the common three hiploading modes (1586N,1690N and2132N), the maximum principal stress (MPS), the maximum principal elastic deformation (MPES) and MPES distribution changes were recorded and analyzed by three-dimensional finite element analysis with ANSYS Workbench13.0software. Results   The MPS values before TKA were98.54, 147.36and143.51MPa respectively when values of femoral head loading were1586N,1690N and2132N. With the increase in the length of the femoral prosthesis extension rod, the MPS and MPES showed a decreasing trend. There was a positive correlation between free femur ratio (FFR) and MPES (P<0.05). The MPES distribution was gradually concentrated at the top point of femur stem extension implant with the increase in the length of the femoral prosthesis extension rod. Conclusion   The values of MPS and MPES were gradually decreased with the increase in the length of the femoral stem extension implant. There was a positive linear correlation between FFR and MPES. The stress concentration was focused obviously at the top end of femoral stem extension implant with its length more than120mm.

    [Abstract]   Objective  To observe the effects of nonsteroidal anti-inflammatory drugs (NSAIDs) on the proliferation
    of chondrocytes and metabolism of human osteoarthritis (OA) in short- term (48hours).   Methods  The human knee OAchondrocytes were cultured in the presence or absence of NSAIDs (celecoxib, diclofenac and ibuprofen). BrdU assays wereused to evaluate the effects of NSAIDs on the proliferation of OA chondrocytes. ELISA was used to detect the contents of proteoglycan and type-Ⅱ collagen in chondrocytes and culture supernatant. The differences of those indexes were compared between groups.   Results  The positive rate of BrdU labelling index of chondrocytes increased significantly in ibuprofen groupthan that of other groups (P<0.05). The content of type-Ⅱcollagen in the culture supernatant increased significantly in diclofenac group than that of other groups (P<0.05orP<0.01). The content of proteoglycan in the culture supernatant increased significantly in ibuprofen group than that of other groups (P<0.05orP<0.01). There were no significant differencesin the content of type-Ⅱcollagen and proteoglycan between chondrocytes of NSAIDs groups (P>0.05).   Conclusion   Ibuprofen may stimulate the proliferation and the secretion of proteoglycan of human OA chondrocytes. Diclofenac may stimulate the secretion of type-Ⅱcollagen of human OA chondrocytes. There were no effects of celecoxib on the proliferation andmetabolism of human OA chondrocytes.

    The Laboratory Monitoring of Aspirin Therapy in Perioperative Patients with General Surgery

      [Abstract] Objective   To investigate the reasonable time limit for stopping the aspirin treatment in preoperative patients with general surgery and the effects on platelet function in postoperative patients with recovering the therapy of aspirin.Methods   A total of121patients undergoing elective general surgery were divided into stopping aspirin treatment5d group(n=59) and stopping aspirin treatment 7d group (n=62). Fifty healthy volunteers were used as the control group. The arachidonic acid (AA)-induced platelet aggregation test was used to detect the platelet agglutination rate in all groups. Aspirin was reused24~48h after surgery. The level of urinary11-dehydro- thromboxane B2 (11-DH-TXB2) was assayed by ELISA 7 and10d after retreatment. Results   The levels of the PAgT (5min,8min and10min) were decreased significantly in patients with stopping aspirin treatment 5d group compared with those of patients with stopping aspirin treatment7d group and control group (P<0.05). There was no significant difference in the level of PAgT between patients with stopping aspirin treatment7d group and control group. The platelet aggregation showed two different characteristic curves after stopping aspirin treatment for5d. And the normal curve of platelet aggregation was found after stopping aspirin treatment for7d. The levels of PAgT and urinary11-DH-TXB2were significantly lower in patient recovered the aspirin treatment for7d and10d than that of control, and which was significantly higher in 7d group than that of10d group (P>0.05). Conclusion   The platelet aggregative function returned to normal level in patients with7-d preoperative stopping aspirin. The laboratory monitoring of aspirin therapy should be more than7d after postoperative reusing aspirin

    The Effects of Preemptive Antiviral Therapy on HBV DNA Negative HBV-Related Hepatocellular Carcinoma Patients Receiving Transcatheter Arterial Chemoembolization

    [Abstract]   Objective   To investigate the effects of prophylactic antiviral therapy for HBV DNA negative HBV-relat-
    ed hepatocellular carcinoma (HCC) patients undergoing hepatic arterial chemoembolization (TACE).Methods   Fifty-four consecutive patients with HBV-related HCC and received TACE were enrolled in this study. Thirty patients received preemptive antiviral drugs before TACE were defined as the treatment group. Twenty-four patients, who did not use antiviral drugs until HBV reactivation after TACE, were included in control group. The incidence of HBV reactivation, duration from HBV DNA positive point to the last time of TACE, the occurrence of abnormal alanine aminotransferase (ALT) caused by HBV reactivation, the peak of aspartate aminotransferase (AST) and the number of liver failure caused by HBV reactivation were observed after TACE in two groups.Results   The incidence of HBV reactivation, the occurrence of abnormal ALT, the occurrence of abnormal ALT caused by HBV reactivation, the peak ALT and peak AST were significantly lower in treatment group than those of control group (P<0.05). No liver failure caused by HBV reactivation was found in treatment group. There were four patients with liver failure caused by HBV reactivation in control group. There was no significant difference in cumulative survival rate between two groups (P=0.071). Conclusion   It is suggested that preemptive antiviral therapy can prevent the reactivation of hepatitis B virus, prevent the deterioration of liver function,and decrease the occurrence of liver failure caused by HBV reactivation in patients receiving TACE

    substance1(IRS-1) in type 2diabetes (T2DM) rats.Methods  T2DM rat model was established by feeding with high-fatdiet and intraperitoneal injecting streptozotocin (STZ). T2DM model rats were randomized into four groups: T2DM control group, high dose Peng-Hui (150mg/kg) group, medium dose Peng-Hui (100mg/kg) group and low dose Peng-Hui (50mg/kg) group. The T2DM control group was fed with only high-fat-diet. The normal control group was fed with common diet. The expression levels of IRS-1in pancreas and skeletal muscle tissues were detected by immunohistochemistry method after 8-week treatment, and fasting blood glucose (FBG) and insulin levels were also detected in five groups.Results   FBG levels were significantly lower in medium dose Peng- Hui group than those of T2DM control group and low dose Peng- Hui group (P<0.05). There were no significant differences in insulin levels between high, medium and low dose Peng-Hui groups and T2DM control group (P>0.05). The expression levels of IRS-1of pancreas and skeletal muscle tissues were significantly lower in high dose Peng-Hui group than that of T2DM control group (P <0.05). Conclusion  The expression of IRS-1 was decreased in high dose Peng-Hui group, but no effects on FBG and plasma insulin.

    [Abstract]  Objective   To verify the validity of the sucrose density gradient ultracentrifugation method for lipid rafts
    from cerebral cortex.Methods  Extract lipid rafts from cerebral cortex in mouse were extracted by the sucrose density gradient ultracentrifugation method. The properties of lipid rafts were detected by Western blotting method, double enzyme and light scattering methods. HPLC MS/MS proteomics and bioinformatics were used to locate proteins of lipid rafts in cells. Results Lipid rafts from cerebral cortex were provided with the model properties of lipid rafts such as high light scattering and cholesterol and high expression of Flotillin-1. HPLC MS/MS proteomics identified total 647proteins. Most of these proteins were from plasma membrane, endoplasmic reticulum, cytoskeleton and cytosol, however, there were 21% proteins among total647proteins were from nucleus, mitochondria and ribosomes.Conclusion  The sucrose density gradient ultracentrifugation method is a effective method to extract lipid rafts from cerebral cortex, however, the properties of mixture should be considered.

    [Abstract]   Objective   To explore the effects of homocysteine (Hcy) on neural stem cell (NSC) proliferation and the
    mRNA expression level of Notch1and Hes1related Notch signaling pathway from neonatal rats in vitro.  Methods NSCs from neonatal rats were cultured by serum-free culture method in vitro. Cells were divided into four groups: control group (Hcy-C), low dose Hcy (Hcy-L) group, middle dose Hcy (Hcy-M) group and high dose Hcy (Hcy-H) group. NSCs were identified by immunofluorescent staining using the antibodies against Nestin, β-tubulin Ⅲ and GFAP. The proliferation ability of NSCs was detected by MTT. The mRNA expressions of Notch1and Hes1were detected by Real-time PCR.   Results   In the serum free suspension medium, neurospheres that consisted of a great number of nestin-positive cells were found. β-tubulin Ⅲ positive neurons and GFAP positive astrocytes were detected by immunofluorescence staining on the6th day of cell induction. MTT assay showed that the cell viability was significantly lower in three Hcy treatment groups than those of control group (P<0.05). And the effect of concentration-dependent was observed. The results of RT-PCR showed that mRNA expression of Hes1was significantly lower in three Hcy treatment groups than that in control group (P<0.05). The mRNA expression of Notch1was significantly lower in Hcy-H group than that of other three groups (P<0.05). The mRNA expression of Notch1was significantly lower in Hcy-M group than that of Hcy-L group and control group (P<0.05).   Conclusion   Hcy could inhibit the proliferation of NSCs by down- regulating mRNA expression levels of Notch1and Hes1genes related to Notch signal pathway

    [Abstract]  Objective   To observe the effect of suramin combinated with PG-Rg3on xenograft growth of lung adeno- carcinoma in mice, and the related mechanism thereof.  Methods   Forty C57BL/6J mice bearing Lewis cells were random?ized into five groups: control group, cisplatin (DDP) group, suramin group, PG-Rg3group and combination group. Appropriate interventions were given in five groups of mice. Mice were sacrificed at day24after tumor inoculation. The subcutaneous tumors were stripped for histological examination. The tumor inhibitory rate was measured. The expressions of erythropoietinproducing hepatoma amplified sequences (Eph) B4protein, Bcl-2and tumors microvessel density (MVD) were determined by immunohistochemistry method with image analyze system. The apoptosis of tumor cells was measured by biotinyated dUTP nick and labeling (TUNEL) method.  Results   There were significantly lower values in subcutaneous tumor volume and weight in drug-treated groups than those in control group (P<0.05). The inhibitory rates were39.20%,49.11%,54.86% and62.49% in cisplatin group, suramin group, PG-Rg3group and combined group (P<0.05). The values of EphB4, MVD and Bcl-2grey values were significantly decreased, the apoptotic index was significantly increased, in suramin group, PGRg3group and combined group than those of control group and DDP group (P<0.05). The values of EphB4, MVD and Bcl-2 grey values were significantly increased, the apoptotic index was significantly decreased, in suramin group and PG- Rg3 group than those of combined group (P<0.05).   Conclusion   Suramin combinated with PG-Rg3can produce a synergetic inhibitory activity against tumor growth of lung adenocarcinoma, which may be associated with the effect of suppressing the expression of EphB4and angiogenesis, and the promotion of tumor cell apoptosis

    [Abstract]   Objective   To observe the expressions of Akt and ERK phosphorylation in abdominal aortic aneurysm of
    rat model, and explore the pathogenesis of abdominal aortic aneurysm. Methods   The rat model of abdominal aortic aneurysm was established. The diameter of abdominal aorta was measured and the extended rate of the aorta was calculated. HE staining was used to observe the change of pathology. Immunohistochemistry and Western blot methods were used to detect the expressions of Akt and ERK phosphorylation in the level of protein. Results   The dilation of aorta was significantly higher in abdominal aortic aneurysm group than that of saline group and normal group (P<0.05). HE staining showed structural disorder and inflammatory cell infiltration in abdominal aortic aneurysm group. The results of immunohistochemistry and Western blot results showed that phosphorylation of Akt expression was significantly higher in abdominal aortic aneurysm group than that of saline group and normal group (P<0.05). There was no significant difference in phosphorylation of ERK expression between three groups (P>0.05). Conclusion   PI3K/Akt signaling pathway may be involved in the development of abdominal aortic aneurysm.

    Effect of RNA interference targeting HIF-1a gene on the proliferation of human tongue cancer cells

    [Abstract]   Objective  To explore the effect of RNA interferencetargeting hypoxia-inducible factor-1α(HIF-1α)
    gene on the proliferation of human tongue cancer cells (Tca8113and Tscca).  Methods  HIF-1α- specific short hairpin
    RNA (shRNA) expression vector and empty shRNA vector were respectively transferred into two kinds of cultured human tongue cancer cells. The expression of HIF-1αgene was detected by RT-PCR and Western blot method. The clone formation assay was used to observe the cell proliferation. The cell counting was used to monitor the number of living cells. Flow cytometry was applied to detect cell cycle distribution. Additionally, the cells were injected into nude mice, and tumor forming condition was observed.   Results   After the shRNA HIF-1αplasmid was transfected into tongue cancer cells, the expressions of both HIF-1αmRNA and protein were significantly decreased in tongue cancer cells under hypoxia condition. The number of colony formation was significantly lower in HIF-1αsmall interfering RNA (siRNA) group than that in empty and negative control group. The cell counting revealed that the number of living cells was remarkably lower in HIF-1αsiRNA groupthan that in negative control group. Also, the cell cycle in HIF-1αsiRNA group was arrested at G0/G1phase after transfection and 48hours hypoxia culture. Besides, the tumor volume wasremarkably smaller in HIF-1αsiRNA groupthan that in negative control group after thetongue cancer cells transplanted into nude mice for several days. The statistical analy sis indicated that the difference was significant (P<0.05). Conclusion   RNA interference targeting HIF-1αgene could suppress the proliferation of human tongue cancer cells effectively. HIF-1αgene could be researched as a new therapeutic target of tongue cancer in the future.

    Observation on Epigallocatechin Gallate Treating the Bladder Tumor Induced by N-methyl-N-nitrosourea in Rats

    [Abstract]   Objective   Toinvestigate the therapeutic effect of epigallocatechin gallate (EGCG) on the bladder neo-
    plasm induced by N-methyl-N-nitrosourea (MNU) in Wistar rats.Methods   Seventy-two female rats were randomly divided into two groups, treatment group and control group. The rat model of bladder tumor was established by intravesical instillation of MNU (2mL/time, once every two weeks) inside bladder. Rats in treatment group were given EGCG (2mL/time, once/day), and control group were given the same volume of distilled water. Six rats were executed respectively at the3rd, 5th,7th,9th, 11th and13th  week, and the bladder histopathological changes were studied in two groups.Results  The untypical hyper-plasia was observed after5weeks. The bladder neoplasm was observed at the11th week in control group. The bladder neo-plasm was not found in treatment group. There was a significantly lower   pathological score in treatment group than that of control group except for the first two weeks (P<0.05).   Conclusion  EGCG is effective for treatment of the bladder neo-plasm induced by MNU.

    [Abstract] Objective??? To explore the damage of intestinal mucosal barrier caused by vinegar retention enema, and
    compare the degree of injury in intestinal mucosal barrier between isotonic vinegar and traditional hypertonic vinegar treatment.? Methods?? ?Sixteen adult rabbits were randomly divided into two groups. The solution of vinegar mixed with normal sodium was given to hypertonic group. Isotonic group was administered with isosmotic solution made of vinegar and double distilled water, with the same volume fraction and pH as hypertonic group. Then two different osmosis solutions were given as enema solution to rabbits under the same condition. Finally, damages of intestinal mucosa were observed by light microscope and transmission electron microscope (TEM) in two groups.?? Results?? It was observed that injuries of rectum and distal part of colon were more severe in hypertonic group than those of isotonic group through light microscope. Besides, cell structures were morphologically normal, with in-line microvilli and intact tight junction in isotonic group. In hypertonic group, microvilli of intestinal cells decreased or disappeared, the cell junction is widened, and cell apoptosis and necrosis were found through TEM.? ?Conclusion??? Results showed that isotonic vinegar had less damage to mucosa. Consequently, isotonic vinegar can be considered as enema solution for hepatic encephalopathy to avoid damage by osmosis in clini

    [Abstract]   Objective    To discuss the impacts of different doses of alcohol on the level of microparticles (EMPs) of
    rat endothelial cells.   Methods   Sixty male SD rats were randomly allocated into four groups: high dose group (group A), medium dose group (group B), low dose group (group C) and the blank contrast group (group D). There were15rats in each group. Rats were fed alcohol for8weeks. Flow cytometer was used to measure the level of circulating CD31+ /CD42- EMPsin four groups, and which was compared with the alcohol dosage.    Results   Compared with group D, the level of circulating EMPs was significantly increased in group A and B (P<0.05). There was no significant change in the level of circulating EMPs in group C (P>0.05).   Conclusion   The moderate and high doses of alcohol are harmful to the function of vascular endothelial cells in rats, which show a significant dose-effect relationship. The low dose of alcohol shows no effect on the function of vascular endothelial cells in rats. The protective effect of alcohol needs further investigation

    [Abstract]   Objective   To study the influence of qiqishen granules on the cellular and humoral immune functions in
    model mice.  Methods   Six-week-old mice were divided into control group and qiqishen granule (high, medium and lowdose) groups. The suspension of chicken red blood cells was injected into the mouse abdominal cavity. The influence of qiqishen granules on the phagocytic function of the macrophages in mouse abdominal cavity was observed. The sheep red blood cells (SRBC) were prepared. The blood corpuscle coagulation was observed, and the serum hemolysin was detected. The activity of the mouse natural killer (NK) cells were detected by the interaction between the target cell (YAC-1) and spleen cell (the response cell). The influence of qiqishen granules on the cellular immunity was detected by the lymphocyte transforming assay. The influence of qiqishen granules on organ/body weight ratio was measured by calculating the thymus/body and spleen/body ratio.    Results    Compared with the control,qiqishen granules significantly improved phagocytic function of the macrophages in mouse abdominal cavity. The humoral immune function was also improved in mice. The activity of NK cell was enhanced by qiqishen granules, which also enhanced the lymphocyte transforming induced by the concanavalin (Con) A. There was no significant influence in the organ/body weight ratio.   Conclusion   The qiqishen granules could increase the immunomodulating effect, indicating it would have a good using prospect.

    Treatment of deep venous thrombosis of lower extremities with fibrinolysin and rivaroxaban

    Objective To observe the effect of the fibrinolysin and rivaroxaban to treat deep venous thrombosis. Methods 63 patients from Janurary, 2010 to August, 2011 were divided into two groups: observation group (n=32) was treated with fibrinolysin 200U/L and rivaroxaban 10mg/d; controll group (n=31) was treated with fibrinolysin 200U/L and Low Molecular Dextran. Fibrinogen, coagulation time, and prothrombin time were tested. Doppler ultrasound was performed to evaluate the effect of thrombolysis. Results There were significant difference bewteen two groups in effect of thrombolysis and fibrinogen. Fibrinolysin and rivaroxaban could have better effect than controll group. There were not significant difference in coagulation time, prothrombin time, and severe bleeding events. Conclusion Fibrinolysin and rivaroxaban could be an effective option for deep venous thrombosis.

The score of mRS in the group of intravenous thrombolysis after 90 days between 0~1 has 6 cases and 2~6 has 16 cases. The score of mRS in the group of Intra-arterial thrombolysis and embolectomy with stent after 90 days between 0~1 has 14 cases and 2~6 has 9 cases.The results of Intra-arterial thrombolysis and embolectomy with stent are better than intravenous thrombolysis to cure Acute posterior circulation infarction(P<0.05). Conculation: To treat Acute posterior circulation infarction, there are better results in Intra-arterial thrombolysis and embolectomy with stent than intravenous thrombolysis.

    Effect of compound lidocaine cream inhibit the cardiovascular responses of extubation after the Cerebral aneurysm clipping surgery

    Objective To observe clinical effect of the compound lidocaine cream to inhibit the cardiovascular responses of tracheal extubation after the cerebral aneurysm clipping surgery. Methods 32 patients (ASA I - II level) of cerebral aneurysm clipping surgery were randomly divided into lidocaine cream group (L) and the control group (C group), 16 cases in each group. L group was coated with 5% lidocaine cream in the front of the tracheal tube before intubation, group C coating the same amount of vaseline. Recorded two groups such as age, height, weight in general. Recorded operation time, anesthesia time, dosage of remifentanil, dosage of fluid replacement. The SBP, DBP, and HR of before anesthesia (T1), 2min before extubation (T2), extubation (T3), 2min after extubation (T4) in patients were recorded. Calculation of the two groups at T3and T2 to SBP and DBP, HR difference (ΔSBP, ΔDBP, ΔHR), and calculate the ratio of the difference between T2 to corresponding data. Results The SBP and DBP, HR in T3, T4point of group L significantly lower than group C (P <0 05). The ratio of ΔSBP, ΔDBP, ΔHR and the corresponding data of T2 of group L was significantly lower than group C (P <0 05). Conclusion Tracheal tube coated compound lidocaine cream before intubation can effectively inhibit the cardiovascular responses of extubation after cerebral aneurysm clipping surgery.

    Use of Dezocine combined with different dosage of Dexmedetomidine for postoperative intravenous analgesia(PIA)

    [Abstract] Objective Observe the clinical effect of dezocine combined with different dosage of dexmedetomidine for postoperative intravenous analgesia. Method Formulas of 120 cases of laparotomy which were under general anesthesia in the same induction and maintenance process and randomized into four groups were as follows: Group D1: dezocine 0.6mg/kg + Dexmedetomidine 0.045μg/(kg?h) + droperidol 5mg; Group D2: dezocine 0.6mg/kg + Dexmedetomidine 0.1μg/(kg?h) + droperidol 5mg; Group D3: dezocine 0.6mg/kg+ Dexmedetomidine 0.15μg/(kg?h) + droperidol 5mg; Group F: dezocine 0.6mg/kg+ fentanyl 10μg/kg + droperidol 5mg, all diluted into 100ml with normal saline. The background infusion rate was 2ml/h, PCIA dose 0.5ml, and lock time 15min. Outcome measures Pain score of patients at 1, 3, 6, 12, 24, 36 and 48h postoperativly, postoerative sedation scale and postoperative comfort score (BCS) at 24h and adverse reactions, e.g. nausea and vomiting, dizziness, itchy skin, respiratory depression, bradycardia, hypotension muscle rigidity and stupor. Results VAS scores in all group and at all time point were less than 4. The VAS scores of Group D1 at 3, 6, 12 and 24h postoperativly were greater than those of the other 3 groups (P<0.01). BCS scores of all groups were greater than 2. The BCS score of group D1 was less than the other 3 groups (P<0.001). The 24h postoperative sedation score of group D3 was notably higher than those of the other three groups (P<0.001). The incidence rate of nausea and vomiting and EPS (extrapyramidal symptoms) of group F was notably higher than those of the other groups (P<0.05).The incidence rate of bradyarrhythmia of group D3 was higher than those of the other three groups (P<0.05). Conclusion It is effective and has less adverse effects to use Dezocine mixed with a basal infution of 0.1μg/(kg?h) of dexmedetomidine for postoperative analgesia.

    Comparative Analysis of transperineal saturated prostate biopsy with traditional prostate biopsy modes

    Objective: To evaluate the clinical value of different prostate biopsy modes for the diagnosis of prostate cancer. Methods: Analyzed the clinical data of 355 cases that underwent prostate biopsy. 110 cases (group A) received transrectal prostate biopsy. 144 cases (group B) received 12-core transperineal prostate biopsy and 101 cases (group C) received 24-core transperineal prostate biopsy. Compare the positive rates and complication rates of three groups in different PSA levels. Results: The positive rate of biopsy with PSA<10.0ng/ml in group C was 32.3%, which was higher than that of group A (10.8%) and group B (13.0%). The positive rates of biopsy with PSA10.0-60.0ng/ml or PSA>60.0ng/ml of the three groups had no statistics significance. The complication rates of gross hematuria, fever, bloody stool in group A were significantly higher than that in group B and group C. Group A had advantages in operating time, hospitalization time and cost (P<0.05). Conclusions: The ideal method for the diagnosis of prostate cancer is the TRUS guided 24-core transperineal prostate biopsy, which has high diagnostic accuracy, safety and fewer complications.

    The Analysis of the Risk Factors and Prevention Measures for Postpartum Hemorrhage

    【Abstract】 Objective To analyze the risk factors and prevention measures for postpartum hemorrhage. Methods 3786 cases who gave birth in our hospital from May 2011 to December 2011 were analyzed retrospectively. The Logistic regression was used to analysis the risk factors of the postpartum hemorrhage. Results Based on the amount of blood loss, measuring by the weight and volume estimation, there were 135 cases out of 3786 defined as postpartum hemorrhage and the incidence was 3.57%. Due to the single factor analysis, there were 11 factors associated with postpartum hemorrhage, including age, delivery history, the manner of delivery, placenta previa, placental abruption, adherence implantation or residual of placenta, uterine atony, injury of birth canal, anemia, coagulation disorders, pregnancy-induced hypertension history. According to the multiple factor analysis, 4 factors entered the logistic regression model, including uterine atony (OR=15.132,95%CI 13.342~16.701), placenta previa (OR=10.897,95%CI 9.256~11.347 ), adherence implantation or residual of placenta (OR=30.310,95%CI 23.312~35.514)and cesarean section (OR=4.018,95%CI 3.331~4.522 ). Conclusion Uterine atony, placenta previa, adherence implantation or residual of placenta and cesarean section were the risk factors of the postpartum hemorrhage. Pay attention to prevent the uterine atony. Strengthen the knowledge of perinatal care as much as possible and reduce the rate of cesarean section can help reducing the incidence of postpartum hemorrhage and obstetric mortality.

    Objective To explore the clinical features,diagnosis and treatment for allergic perpura accompanied with acute intussusceptions(APAI) in children.Methods The clinical data of 26 cases was analysed retrospectively.Results APAI had no typical clinical manifestation.B type ultrasounography(BUS) is very useful for diagnosis . It was observed a high rate of air enema reduction.Surgical procedure should be made if air enema failed. Intestinal necrosis usually occurred in cases with enteric intussusception .Conclusion APAI had no typical clinical manifestation. BUS is very useful for diagnosis.     Expression of VEGF in the Endometrial-Myometrial Interface of Adenomyosis

    Objective:To observe the expression of VEGF in the endometrial-myometrial interface of adenomyosis.Methods:Immunohistochemistry SP method was adopted to determine the expression of VEGF in EMI of twenty cases of adenomyosis and of twenty cases of control subjects.Results:In adenomyosis group the expression level of VEGF was higher in EMI versus outer layer in the whole menstrual cycle (p<0.05).Between the groups, the expression level of VEGF was higher in EMI of adenomyosis group than that of control group in the whole menstrual cycle(p<0.05).Conclusion:The higher expression of VEGF in EMI is closely related to the pathogenesis of adenomyosis possibly.

    Ureteroscopic Lithotripsy for Urgent Decompression of Urosepsis Caused by Acute Obstructive Ureteral Calculi

    Objective: To investigate the feasibility and safety of ureteroscopic lithotripsy for urgent decompression of urosepsis caused by acute obstructive ureteral calculiu . Methods: 35 consecutive patients diagnosed with ureteral stone-related sepsis received ureteroscopic lithotripsy as urgent decompression at our institution(29 patients of general sepsis,3 patients of sever sepsis,3 patients of sepsis shock) .Patients underwented ureteroscopic lithotripsy and placement of an indwelling ureteral double J stent under general or regional anesthesia. Results: All patients successfully underwent emergency drainage for decompression, and all cases improved after urgent decompression. There were no anesthesia related morbidities, postoperative exacerbations of the clinical condition, or postoperative deaths .Clinical outcomes(mean time to normal temperature and time to normal white blood count) :patients of general sepsis 1.6(0-7) days and 2.1(1-8) days; patients of severe sepsis 8.0(6-11) days and 9.3(7-13) days ; patients of septic shock 17.7(13-26) days and 19.7 (14-29) days .Conclusion: Ureteroscopic lithotripsy for urgent decompression of urosepsis caused by acute obstructive ureteral calculican can be safely and successfully performed by skilled endourologists in select clinical situations,such as patients of general sepsis, patients of severe sepsis and septic shock who fail to underwent primary retrograde indwelling double- J stent with cystoscope drainage and would have technically diffcuilty with mild hydronephrosis or with bleeding diathesis to perform percutaneous nephrostomy drainage.

    Objective To explore clinical manifestation and related risk factors of adrenal insufficiency after coronary artery bypass grafting;Methods To summary 13 cases with AI after coronary artery bypass grafting,and clinical manifestations, diagnosis , treatment and related risk factors were analyzed;Results The incidence rate of this group was 3.9% , no significant difference in the incidence between men and women,the main clinical manifestations were hyponatremia,vomiting,diarrhea,conscious indifference,fatigue, low blood sugar –based and rare pigmentation.Under hormone replacement therapy,11 patients were cured, 2 cases of patients rely on long-term hormone replacement therapy.By logistic regression analysis, operative time , intraoperative round-robin fashion , postooerative infection correlated with postoperative adrenal insufficiency(p<0.05); Conclusion Adrenal insufficiency may occurred after coronary artery bypass surgery and operation time,cardiopulmonary bypass, postoperative infection were risk factors for the occurrence of AI.And after active treatment,most patients can get better treatment effection.

    Application of Stem Cells in Orthopedics

    [Abstract] Stem cell research plays an important role in orthopedic regenerative medicine today. Current literature provides us with promising results from animal research in the fields of bone, tendon, and cartilage repair. While early clinical results are already published for bone and cartilage repair, the data about tendon repair is limited to animal studies. The success of these techniques remains inconsistent in all three mentioned areas. This may be due to different application techniques varying from simple mesenchymal stem cell injection up to complex tissue engineering. However, the ideal carrier for the stem cells still remains controversial.This paper aims to provide a better understanding of current basic research and clinical data concerning stem cell research in bone, tendon, and cartilage repair. Furthermore, a focus is set on different stem cell application techniques in tendon reconstruction, cartilage repair, and filling of bone defects.

    Sublober resection in early-stage lung cancer

    Currently,lobectomy is the preferred treatment for early-stage non-small cell lung cancer for its relatively lower recurrence rate compared to sublobar resection .However, recent retrospective studies reveal a similarity in recurrence and survival rate between sublobar resection and lobectomy in the early treatment of peripheral lung cancer. When deciding on the operational means of local excision, scholars generally support sublobar resection rather than lobectomy .This study reviews the current status of sublober resection for early-stage lung cancer, with emphasis on prognostic factors.
