    Lyophilization-Preparation of recombinant IFN-α-2b-BCG and Study of its bioactivity

    Objective: To explore the preparation process of lyophilization of recombinant IFN-α-2b-BCG(rBCG), and study the biological characteristics of lyophilized rBCG for clinical immunological activity to provide biological stability, safe and effective freeze-dried formulations.. Method: Vacuum-freeze-drying of recombinant IFN-α-2b-BCG, drawing the curve of lyophilization process, screening the best processing parameters. Comparing the number of viable cells with the plate count, before and after lyophilization, to calculate the survival rate. The acid-fast staining characteristics and morphology of the lyophilized rBCG was analyzed. Continuous measurement of the OD value of freeze-dried rBCG, drawing the growth curve. The plasmid stability of lyophilized rBCG was analysed by PCR and kanamycin-resistance. The level of IFN-α-2b expressed by rBCG was determined with ELISA. Results: The number of viable rBCG reached 6.5×106cfu/ml after lyophilization, the value was 1×107cfu/ml before freeze-drying, so survival rate was about 65%. The morphology, growth rate and acid-fast staining characteristics of lyophilized rBCG was not significantly different, from that of un-lyophilized. The stability rate of gene insertion(hIFN-α-2b)was 91.7%, loss rate 8.3%. The level of secretion of rIFN-α-2b by rBCG had no significant difference after lyophilization (p>0.05). Conclusion:Freeze-dried powder of hIFN-α-2b BCG was successfully prepared. After lyophilization, viable rBCG achieved 1×107cfu/ml, which is the standard of bio-immune-therapy needed. There was no significant difference in morphology, growth and expression of exogenous IFN-α-2b,comparing before with after lyophilization. Adverse effects of freezing, low temperature and vacuum drying of lyophilization process has on significant effects on hIFN-α-2b expression, plasmid insert, genetic stability of rBCG.

    The Correlation of NGAL Expression in Gastric Carcinoma with Clinicopathologic Progress , Metastasis and Prognosis

    Abstract Objective: To investigate the expression of neutrophil gelatinase-associated lipocalin (NGAL) in gastric carcinoma and evaluate the correlation of NGAL expression with clinicopathologic progression , metastasis and prognosis. Methods:The expression of NGAL was observed in 102 gastric carcinoma tissues and 32 normal gastic tissues by immunohistochemistry. The correlation of NGAL expression with clinicopathological parameters and metastasis was analyzed. Results: The positive rate of NGAL expression in gastric carcinoma was 56.9%, which was significantly higher than in normal gastric tissue 21.9% (χ2=11.938, P <0.01). The expression of NGAL was significantly different in Patients with different histological types (χ2=10.180, P <0.05) .The expression of NGAL was observed to correlate positively with serosal invasion, TNM stage, lymph node status and T stage(rs =0.307, 0.211, 0.220, 0.201, P <0.01 or P <0.05). NGAL expression of positive and nagative groups of five-years cumulative survival rates were 24.3% and 43.7%. Kaplan-Meier curves showed a significant difference (P =0.018).Conclusion: NGAL expression of positive in gastric carcinoma, was correlated significantly with tumor progression and lymphatic metastasis, NGAL might be a poor prognostic factor for human gastric carcinoma, and also might be a novel molecular marker to predicate the prognosis of gastric carcinoma.

    Initial Study of Real-time Three-dimensional echocardiography Assessment of Left atrial Synchronicity in Patients with Dilated Cardiomyopathy

    Abstract Obiective: To evaluate left atrial asynchrony in patients with dilated cardiomyopathy (DCM) by real-time three-dimensional echocardiography (RT-3DE). Methods:Thirty-one patients with DCM and thirty healthy subjects underwent RT-3DE. Left atrial 16-, 12-, 6- segment models asynchrony parameters including the standard deviation(Tmsv 16-SD、Tmsv 12-SD、Tmsv 6-SD) and maximal difference of QRS to Tmsv(Tmsv 16-Dif、Tmsv 12- Dif、Tmsv 6- Dif)were measured, and maximun left atrial volume index(LAVmax index), minimum atrial volume (LAVmin index), volume before atrial active contraction (LAVpre index) were measured. Left atrial emptying passive atrial emptying fraction(LAEF), passive atrial emptying fraction (LAEFpassive), active atrial emptying fraction (LAEFactive) were calculated. Results: Left atrial 16-, 12-, 6- segment models asynchrony parameters were significantly higher in the DCM group than those in the control(P<0.05). LAVmax index, LAVpre index and LAVmin index were higher, whereas LAEF, LAEFpassive、LAEFactive were lower in the DCM groups than those in the control group(P<0.05). Moreover, there were correlation between left atrial 16-, 12-, 6- segment models asynchrony parameters and left atrial volume and function. There were positive correlations between left atrial Tmsv16-SD,Tmsv 16Dif and LAVmax index(r=0.71、0.74, P<0.01, respectively), and negative correlations between left atrial Tmsv16-SD,Tmsv 16 Dif and EF(r=-0.69、-0.72, P<0.01,respectively). Conclusion: RT-3DE is a useful ultrasonic technique that can allow accurate evaluation of left atrial asynchrony in patients with dilated cardiomyopathy, left atrial asynchrony have intenly relation with left atrial dilate and decrease atrial function.

    Objective: Investigating the polymorphisms of variable number tandem repeat (VNTR) in Mycobacterium tuberculosis, developing high discriminatory power of VNTR typing, especially for recognizing Beijing family. Method: 22 known VNTR loci (12 MIRU, 3ETR,6 QUB and Mtub21) were assessed among 102 Mycobacterium tuberculosis strains isolated from Tianjin.     Function of MMP-3 5A/6A polymorphism on acute myocardial syndrome

    【Abstract】 Objective In order to find the relationship between MMP-3 5A/6A polymorphism and acute myocardial syndrome. Methods 389 patients were divided into ACS group (n=232) and control group (n=157) according to clinical syndrome and coronary angiography result after admission. Collected two groups’ clinical data and blood sample to test and compare. Results Compare two groups polymorphism we found the rate of patients who carried 5A/6A+5A/6A gene type in ACS group significant higher than control group(36.21% vs 24.84% P=0.001), and 5A allele frequency also higher in ACS group than in control group(37.93% vs 13.06 P<0.001). on the other sides compared the MMP-3 serum content between two groups we also found the same result(12.37±2.96 vs 9.59±2.77 P=0.002).Conclusions The patient who carried MMP-3 5A allele trend to occur ACS than other patients.

    The significance of APACHEⅡ and IL-6 in patients with traumatic hemorrhagic shock by limited fluids resuscitation

    ABSTRACT Objective This study was performed to investigate the significance of APACHEⅡ and IL-6 in patients with traumatic hemorrhagic shock by limited fluids resuscitation, Method 106 eligible patients with traumatic hemorrhagic shock were randomly divided into two groups and the healthy control group (n=20). The group of conventional liquid (n=53), Anohter group of limited fluids resuscitation (n=53). The blood samples were taken for lactate, IL-6, and IL-10 measurements. The Acute physiology and chronic health evaluation(APACHEⅡ) were calculated; complications and final clinical outcomes were monitored. Result IL-6, IL-10 in two groups were significantly higher than healthy control groups(p<0.01).IL-6 in the group of limited fluids resuscitation were significantly lower than the group of conventional liquid (p<0.05). IL-6 in the group of the non-survival group were higher than those in the group of survival group(p<0.01).There were no obvious significances in IL-10 between two group(p>0.05).There were obvious significances in complicationsand mortality(p<0.05). Conclusion Limited fluids resuscitation can reduce the levels of IL-6, improve the crue rate, lower the daeth rate. Early test of IL-6 and calculated APACHEⅡscores in paitients with traumatic hemorrhagic shock can be used to evaluate the state of illness and prognosis

    Discussion of the relationship between albuminuria and carotid artery intima-media thickness in type 2 diabetes mellitus

    to discuss the relationship between albuminuria and carotid artery intima-media thickness (IMT) in type 2 diabetes mellitus.Methods: 146 type 2 diabetes mellitus (T2DM) patients with 24-hour urine microalbumin detection,who based on the results were divided into normal albuminuria group (86 cases) and abnormal albuminuria group (60 cases) (including micro - albuminuria(MAU) group and macroalbuminuria group) .Two groups of patients were carried out on various biochemical indices and carotid artery intima-media thickness (IMT) detection, and two sets of data were analyzed by the single factor, multi-factor logistic regression analysis.Further compare the difference of carotid IMT among the three groups of normal urine, microalbuminuria and macroalbuminuria by variance analysis. Results: ①Single-factor logistic regression analysis showed that duration, the incidence of retinopathy, triglyceride (TG), carotid artery intima-media thickness (IMT) and serum creatinine (Cr) were the risk factors of albuminuria; ②multi-factors logistic regression showed longer duration, higher triglycerides, carotid artery intima-media thickness thickening, increased serum creatinine in patients with type 2 diabetes were the independent risk factors of albuminuria.(the OR values were 1.105,1.528,12.781,1.029respectively).③variance analysis showed: Among the three groups including normal albuminuria group, microalbuminuria group, macroalbuminuria, the difference of carotid artery IMT was statistically significant (P = 0.000), while there were no statistically significant difference between the groups of microalbuminuria and macroalbuminuria (P = 0.805). Conclusion: In patients with type 2 diabetes, carotid artery IMT is an independent risk factor of albuminuria(OR = 12.871), while the severity of diabetic kidney disease and carotid IMT was no significant correlation (P = 0.805).

    ABSTRACT Object:To investigate the effects of age and the number of embryo transferred on pregnancy outcomes in infertile patients undergoing FET.Methods:A retropective analysis was performed in our center on the clinical data of 1052 thawing cases from Jan. 2004 to December 2008 .Results:The implantation rate was significantly lower in A group compared with those of B group(P<0.05), but higher than that C group (P <0.05). The abortion rate was significantly lower in A group compared with those of B group(P<0.05).There were significant differences in multiple pregnancy rate and fetal reduction rate between E group and F group. Ectopic pregnancy rate among groups were no difference (P> 0.05). Conclusion: Patients ≥40yrs implantation rate decreased significantly.The number of transferred embryos has nothing to do with ectopic pregnancy and abortion rate occurs. Transplantation 2 embryos is the reasonable choice and elective single embryo transfer(eSET) is a new choice .

    【Abstract】:Object: To understand the status of Novorirus infection in infants and children in Tianjin area , and study the genotype and the genetic identity of Norovirus . Methods : The fecal specimens from infants and young children with viral gastroenteritis were collected from Tianjin Children hospital from August 2008 to July 2009 .The viral RNA was amplified by a reverse transcription polymerase chain reaction (RT-PCR). The amplied genes were the sequenced and compared with the Norovirus sequences deposited in GeneBank. By DNAstar and Bioedit software to evaluate sequence homology and generated the phylogenetic tree .Results: NV was detectable in 174 cases out of the 540 specimens. The positive rate was 32.22% .Nucleotide sequence analysis showed that the NrV strains all belonged to GⅡ-4 . The sequences of specimens were closer to the viral sequence obtained from Beijing . Conclusion: NV is one of the important diarrhea pathogens of viral gastroenteritis in infants and young children in Tianjin area and the GⅡ-4 is the major prevalent strain.

    Clinical and effectiveness analysis of dendritic cells and cytokine-induced killer cells therapy for acute leukemia.

    ABSTRACT:Objective To investigate the methods, safety and influence on the immune function about the treatment of dendritic cells(DC)and cytokine-induced killer cells(CIK)cultured in vitro on acute leukemia(AL). Methods Mononuclear cells (MNCs) were isolated from peripheral blood from 34 AL patients and cultured in vitro in the presence of combined cytokines to generate DC and CIK. All patients were treated with the DC and CIK by subcutaneous injection and venous transfusion. Adverse effect and safety were observed after treatment. The change of T cells subsets, the secretion of cytokines, gamma-interferon, and tumor necrosis factor, minimal residual disease were determined before and after the treatment. Results All the patients have no significant adverse effect after the treatment and continue to keep up bone marrow complete remission. The level of CD3+CD56+ T cells, cytokines, gamma-interferon, and tumor necrosis factor enhances after the treatment. Conclusion The review has demonstrated that this treatment is feasible and safety in AL patients and holds promise as a possible new treatment for AL patients with minimal residual disease. 【Key words】 Dendritic cells;Cytokine-induced killer;Acute leukemia;Minimal residual disease

    Comparison between effect of highly permeable dialysis and low permeable dialysis on uremic pruritus

    ABSTRACT: Objective To investigate the mechanism and the effect of highly permeable dialysis and low permeable dialysis on uremic pruritus. Methods Thirty-eight patients of uremic pruritus studied were randomly divided into observed group and control group. Nineteen cases received high permeable dialysis (observed group) and the others received low permeable dialysis (control group) with three times-weekly session of 4 hours following up one year. All patients were dialyzed with Fresenius 4008S. Blood urea nitrogen(BUN) , creatinine ,phosphate ,β2- microglobulin and parathyroid hormone(PTH) were tested and solute clearance and Kt/ V value was figured out. The degree of skin itch was evaluated by the visual analogue scales (VAS). Results There was no significant difference between observed group and control group in the decrease of BUN ,creatinine(P>0.05). But the decreases of phosphate andβ2-microglobulin of observed group were superior to that of control group, (30.4±14.95) % and (35.12±17.82) % vs (22.06±8.32) % and (1.64±4.93) %(P<0.05) . And there was no difference in Kt/ V value between the two groups(P>0.05) . After one year ,the decrease of PTH of observed group was superior to that of control group (27.54±19.43)pmol/ L vs (43.31±19.59) pmol/ L(P<0.05) and compared to pretreatment ,skin itch was relieved in both groups(P<0.05) ,but observed group was superior to control group , (2.66±1.16) scores vs (5.53±1.56) scores (P<0.05) . Conclusion Compared to low permeable dialysis, highly permeable dialysis,which not only cleared the micromolecule toxins,but increased the clearance of macro-middle molecule toxins,can significantly relieve uremic pruritus.

    A clinical control trail of polonosetron plus dexamethasone versus tropisetron plus dexamethasone to prevent nausea and vomiting induced by cisplatin-based chemotherapy

    Objective: To observe and assess the efficacy and safety of palonosetron made in China plus Dexamethasone for preventing nausea and vomiting induced by cisplatin-based chemotherapy. Methods: This study was performed as a randomized,clinical control trial. The patients were received a single dose of palonosetron 0.25mg i.v. bolus (treatment group), or tropisetron 2mg i.v. drop (control group), each administered 30 min before initiation of highly emetogenic cisplatin-based (≥50mg/m2) chemotherapy,both with Dexamethasone (16 mg i.v. drop on day 1, 8 mg i.v. drop on day 2, 3) for prophylactic antiemesis. To observe the proportion of patients with a complete response(CR) and emesis-free during the acute phase (0-24h postchemotherapy )、delayed phase (24-120h postchemotherapy ) and both phases (0-120h postchemotherapy ),and administer rescue medication(tropisetron or Methoxychlorproeainamide ) if the patient needed. Results: 72 patients were included in the clinical trail: 35 patients in the palonosetron group and 37 patients in the tropisetron group. CR rates(CRR) were slightly higher (P >0.05) for palonosetron 0.25 mg than tropisetron during the acute (0-24 h) (82.9% versus 75.7%, respectively), delayed (24–120 h)( 65.7% versus 43.2%) phases , but CRR were significantly higher (P <0.05) for treatment group than control group during the both phases (0-120 h)( 65.7% vs40.5%).The percent of patients emisis-free for treatment group were numerically higher but not statistically different from control group during the acute phase(91.4% vs 83.8%, P>0.05), but were significantly higher (P <0.05) than control group during the delayed and both phases(77.1% versus 48.6% 、74.3% versus 43.2%, P<0.05).The adverse reaction of two groups patients included mainly headache、constipation、dizzy and Abdominal discomfort, which were very slight and well tolerated for the patients. Conclusions: Conclusions: palonosetron made in China plus Dexamethasone is effective and safe for preventing both acute and delayed nausea and vomiting with highly emetogenic chemotherapy, and it is superior to tropisetron in preventing chemotherapy-induced delayed nausea and vomiting.

    Abstract: Objective:To study the effects of low frequency repetitive transcranial magnetic stimulation (rTMS) on the hippocampus ultrastructure of temporal lobe epilepsy(TLE), by observing the pathological lesions of neurons, nerve fibers, synapses, glial cells and capillaries of TLE rats hippocampus with electron microscopy. Methods:Lithium-pilocarpine induced TLE rat models were randomly divided into two groups, TLE group and rTMS group. The ultrastructural changes in hippocampus of both groups were viewed with electron microscope. Results:TLE has significant pathological damage in the hippocampus ultrastructure, including neuronal apoptosis and necrosis, nuclear condensation, chromatin aggregation, mitochondrial cristae fracture, processes of nerve cells and glial cells edema, local medullary sheath of nerve fibers degradation, microtubule and microfilaments of axoplasm disarrangement, vascular endothelial cells swelling, glial cells demage. rTMS treatment can significantly improve the pathological damage, including relieving of tissue edema, reducing of apoptosis and damage cells, decreasing of synaptic and neuropil lesions, and alleviating of angioedema. Conclusion:TMS can relieve the pathological damage of TLE in the hippocampus ultrastructure and have neural protective effects in TLE.

    Abnormal expression of G protein in the brain of the senescence-accelerated prone mouse(SAMP8)

    Abstract Objective: To explore abnormal expression of G protein in the cortex and hippocampus of 8-old-month and 18-old-month senescence-accelerated prone mouse(SAMP8). Methods: senescence-accelerated mice resistant 1(SAMR1) as control mice, Detection of Gαq/11, Gαi and GαS protein abnormal expression in 8-month-old and 18-month-old SAMP8 by using Western-blot method. Results: Compared with the month-old SAMR1, expression of GαS, Gαi and Gαq protein in 8-month-old SAMP8 cortex and hippocampus were not different (p> 0.05). Gαq expression in 18-months-old SAMP8 cortex was significantly lower than that of SAMR1 (p <0.05). Gαq and Gαi expression in 18-months-old SAMP8 hippocampus was significantly lower than SAMR1(p <0.05). Conclusion: Abnormal expression of Gαq and Gαi in 18-month-old SAMP8 brain involved in Alzheimer Disease (Alzheimer's, AD) pathological progress and is expected to be a target for AD treatment.

    Effect study of Yiqihuoxue treatment on cardiac morphology in rabbits with pulmonary heart disease

    Objective: To observe the changes in heart morphology of Yiqihuoxue treatment in Rabbits with pulmonary heart disease . Methods: We had randomly divided 32 healthy Japanese white rabbits into 4 groups, Normal Group(NG), Model Group(MG),Chinese Medicine Group(CMG) and Captopril Group(CG). Three groups of above mentioned, MG,CMG and CG ,had been given a single dose of Fecl3 induced the model of pulmonary heart disease . Then after that, we treated the 2 groups of them, CMG and CG, respectively 20 days. We had observed and recorded the data of the general conditions, and detected all above groups by echocardiography before & after modeling, and after treating. Results: After modeling, there emerged some value of statistical difference on the field of echocardiographic parameters, such as right ventricular diameter(RVD), pulmonary artery diameter(PAD), the thickness of right ventricular anterior wall(RVAW), tricuspid valve (TV)and pulmonary artery(PV) blood flow velocity(P <0.05 or P <0.01). After treating,We found the weight and breathe of CMG had been improved better than the CG.. We also observed the right ventricular diameter (RVD) was significantly less in the CMG than the other groups(P <0.05). Conclusion: Yiqihuoxue treatment can be effectively alleviate the symptoms,reduce the right ventricular diameter (RVD) and lower pulmonary artery pressure.

    Effects of ShenMai Injection on Lipid Peroxidation in the Lung Induced by Intestinal Ischemia/Reperfusion(I/R)

    Abstract Objective: To explore the effects of shenmai injection(SM) on lipid peroxidiation in the lung induced by intestinal ischemia/reperfusion(I/R). Methods: 24 SD rats were randomly divided into three groups:sham group,I/R group,treatment group. In sham group ,superior mesenteric artery was separated only;Models of intestinal I/R injury in I/R group and treatment group were estabished. In treatment group, SM(0.9mL/100g) was injected through tail vein before ischemia, before reperfusion and after reperfusion 30min respectively. The state of leukocyte adhesiveness/aggregation(LAA) in peripheral blood, lung wet/dry weight ratio,lung coefficient, Superoxide dismutase(SOD), malondialdehyde(MDA) , myeloperoxidase (MPO) in serum and lung tissue were measured, as well as the effects of SM were observed. The changes of lung histology were examined by light microscope. Results: (1)In I/R group ,LAA in peripheral blood, lung wet/dry weight ratio,lung coefficient were significantly increased than that in sham group(P<0.01); the damage to lung tissue was increased in severity with light microscope. The above-mentioned indexes of treatment group were obviously lower in comparison with I/R group(P<0.01).(2)The activity of SOD in serum and lung tissue were significantly lower in I/R group than that in sham group (P<0.01); The content of MDA , the activity of MPO in serum and lung tissue were significantly increased in I/R group than that in sham group (P<0.01).The above-mentioned indexes of treatment group were obviously ameliorated in comparison with I/R group(P<0.01). Conclusion: Shenmai injection can protect the lung injury induced by intestinal ischemia/reperfusion(I/R) ,which may be mediated by inhibiting lipid peroxidation and aggregation of neutrophils.

    Objective To investigate the changes of cytokines during ALI and anti-oxidant effects of cytokines on the effects of ALI. Methods Healthy, mature, Wister rats were randomly divided into two groups 60, the model (control) group and treatment (experimental) group 30 rats in each group are based on intraperitoneal injection of E. coli in order to oxygenation index, lung wet / dry weight ratio, pathological changes in lung tissue as a standard, the establishment of animal models of ALI. Treatment (experimental) group at 30min after the injection of E. coli by rat tail vein injection of anti-oxidants - reduced glutathione. E. coli, respectively, after injection of 3h, 6h, 12h Determination of blood IL-8, IL-10 changes. Results Model (control) group 3h after injection of E. coli ALI symptoms occur, 6h more obvious, 12h vomit pink secretions, anatomical lung after the naked eye can see significant edema and bleeding. Oxygenation index and W / D reached the diagnostic criteria of ALI. Treatment (experimental) group 3h no significant change, 6h part of the rats were breathing a little promotion, 12h only a mild edema in lung tissue. Treatment (experimental) group the lung wet / dry than in the 3h, 6h, 12h test values are lower than the model (control) group, 6h, 12h, when differences were statistically significant (p <0.05). Treatment (experimental) group rats after the use of glutathione, 3h, 6h, 12h measured IL-8 detection value (units of PG / ml) lower than the model (control) group, there were statistical differences between each period significance (p <0.05). Treatment (experimental) group rats after the use of glutathione, 3h, 6h, 12h measured IL-10 measured values (in PG / ml) than the model (control) group, 6h, 12h, when differences exist in statistical significance (p <0.05). Conclusions Of inflammatory mediators in acute lung injury there is loss of control and balance, the application of anti-oxidants - reduced glutathione after the effective regulation of inflammatory mediators, contribute to reducing the release of super-oxide and lysosomes, reducing the inflammatory response , thereby reducing the risk of the occurrence of ALI.

    Expression of pro-apoptotic protein P53、Bax and caspase3 after traumatic optic nerve injury in rats

    Objective: To explore the retinal ganglion cells (RGCs) pathological changes and expression of Bax, P53 and caspase3 protein in RGCs after the damage of the traumatic optic nerve injury in rats at different time points, to investigate the mechanism of traumatic optic nerve injury and the role of pro-apoptotic genes. Methods: the animal model of optic nerve injury in rats was built by fluid percussion brain injury device (FPI). Rats were killed on 1,3,5,7,9,14,28 days after injury. The changes of RGCs were observed by histopathology, the expression of Bax, P53 and caspase3 in RGCs were studied by immunohistochemistry method. Results: On the first day after the optic nerve injury the number of RGCs reduced. On the seventh day after the injury the number of RGCs decreased rapidly. On the fourteenth day after the injury the number of RGCs decreased slowly. On the twenty eighth day after the injury the number of RGCs stabilized. Expression of P53, Bax and Caspase3 mRNA increased in RGCs after the injury. The expression of those increased more significantly in the injury group than in control group (P<0.05). Conclusion: The decrease in the number of RGCs after the optic nerve injury is an important pathological basis in the degression of visual function. Apoptosis is one of the mechanisms of the death of RGCs. The pro-apoptotic protein P53, Bax and Caspase3 play an important role in RGCs apoptosis .

    The Study on Complementary Role of Dexmedetomidine in Abdominal Surgery Patients undergoing General Anesthesia

    Abstract Objective: To evaluate the effect of haemodynamic changes during induction of general anesthesia and the dexmedetomidine-sparing effect of anesthetics in abdominal surgery patients undergoing general anaesthesia. Methods: This was a prospective, randomized, double-blind, placebo-controlled study. 48 patients (ASA I~II) aged 18 to 65 years old undergoing abdominal operation under general anesthesia were randomly divided into group D(intravenous 1μg ?kg-1 Dex for 10 min before induction of general anesthesia) and group C (0.9% normal saline was given as control). The haemodynamic changes including blood pressure and heart rate during induction of general anesthesia was observed, and the consumption of anesthetics was recorded. Results: The heart rate was significantly decreased in group D than that in group C after DEX administration(P<0.01). The minimum of blood pressure was significantly increased in group D than that in group C before tracheal intubation(P<0.01), while the minimum of heart rate was contrary. The maximum of heart rate was significantly decreased in group D than that in group C after tracheal intubation(P<0.01).The consumption of anesthetics was significantly less in group D than that in group C(P<0.05). Conclusion: Dexmedetomidine can attenuate hemodynamic responses and reduce requirements of propofol, fentanyl and sevoflurane. Dexmedetomidine play a very good complementary role in general anesthesia.

    Abstract:【objective】Investigate the Presentation and Clinical Significance of CyclinD1 in Endometriosis of Ovary. 【Methods】 Apply immunohistochemistry (s-p) method to detect the presentation of CyclinD1 in 43 tissues of endometriosis of ovary and 27 tissues of normal endometrium. 【Results】CyclinD1 in endometriosis of ovary is clearly higher than that in normal endimetrium with statistical significance (P<0.05). The high expression of CyclineD1 in endometriosis of ovary is not clearly related to dysmenorrheal, abnormal menstruation and barren, but is clearly related to the sizes of cysts,the depths of ectopia intima imbibition and the imbibition condition of pelvic cavity. Their relation has statistical significance.(P<0.05). 【Conclusion】 CyclinD1 is closely related to the development of endometriosis of ovary.

    Abstract Objective:To study the impact of glucose monitoring and controlling in women with abnormal glucose metabolism on maternal and neonatal outcomes.Methods:A retrospective study of maternal and neonatal outcomes was conducted in 172 pregnant women who were diagnosed and treated for abnormal glucose metabolism and delivered in the Second Hospital of Tianjin Medical University from January 2007 to January 2009. After diet control or diet control plus insulin therapy,172 patients were devided into two groups according wether blood glucose controlled satisfiied. While blood sugar was satisfied named A group(91 cases),the other patients named B group(81 cases). Compared the general situation and outcomes. Results:The incidence of HDCP,fetal distress, C-section and postpartum hemorrhage in group A was significantly higher than that in group B. The incidence of fetal macrosomia,hypoglycemia,neonatal asphyxia,transferring to NICU in group A was significantly lower than that in group B.No difference was shown between the two groups in the rate of ketonemia,intrauterine infection,polyhydroamnios deformity,NRDS.Conclusions: The glucose metabolism disorders is harmful for maternal and neonatal .The screening and diagnosis of glucose metabolism disorders during pregnancy should be strengthened. Controlling blood sugar in satisfied level is the key to decreasing maternal and neonatal complications.

    The value of 99mTc-MIBI thyroid imaging in the differential diagnosis of amiodarone-induced thyrotoxicosis

    Object To investigate the value of 99mTc-MIBI (MIBI) thyroid imaging in the differential diagnosis of amiodarone-induced thyrotoxicosis (AIT). Material and methods 15 consecutive AIT patients were performed with thyroid color-flow Doppler sonography (CFDS) , thyroid 99mTcO4- imaging and thyroid radioiodine uptake (RAIU)) within 1 week, as the standard criteria for initially classified. The final diagnosis of AIT was based on the response to different treatments for the thyrotoxicosis. All patients were submitted to thyroid MIBI imaging before any treatment was initiated. Results According to standard criteria, 15 patients were initially classified as having type 1 AIT (n=5) or type 2 AIT (n=10). In the final analysis, 12 of 15 patients could be subdivided into four with type 1 AIT, eight with type 2 AIT. For the other three patients, required the combination of methimazole and glucocorticoids to restore euthyroidism, who had the resistant response to initially therapy, met a posteriori criteria for the diagnosis of a mixed form of AIT. Assessment of MIBI scan results showed that MIBI diffuse retention was present in all four patients with a final diagnosis of type 1 AIT, while no significant uptake, was found in the 8 patients with a final diagnosis of type 2 AIT. Of the other three patients whose final diagnosis was mixed AIT, had a faint persistent MIBI uptake. CFDS was suggestive of type 1 AIT in three patients with type 2 AIT and in one patient with mixed AIT. RAIU was measurable in all patients with type 1 AIT, but also in three out of the eight with type 2 AIT as well as two of three mixed AIT patients. Even in the combination of RAIU and CFDS, 3 of 15(20%) patients were unable to make a clear-cut diagnosis of type 1 or type 2 AIT and propended for a diagnosis of mixed form of AIT. Conclusion Thyroid MIBI imaging may represent a useful tool in the differential diagnosis of different forms of AIT, particularly for the identification of mixed forms, the most resistant to medical treatment.

    Genetic diagnosis and clinical phenotype analysis of sporadic Huntington disease

    Objective: To detect gene and analyze clinical manifestations of sporadic Huntington disease (HD). Methods: Non-denaturing polyacrylamide gel electrophoresis method was used to detect CAG repeat. Clinical manifestations were scored by UHDRS and MMSE. Results: 5 out of 11 sporadic HD patients had CAG expansions. 5 genetic diagnosis patients with late onset, average 47 years, had 40-48 CAG repeat, often with milder chorea and no marked intellectual decline. Conclusions: The diagnosis of sporadic HD patients was confirmed by genetic detect. The clinical manifestations of sporadic HD patients were different from those of patients with family history.

    Clinical Characteristics of Splenic Artery Steal Syndrome After Orthotopic Liver Transplantation

    Objective: to analyze the clinical characteristics, diagnosis and treatment methods and therapeutic effect of splenic artery steal syndrome. Methods: Retrospective clinical observational cases. All patients with splenic artery steal syndrome underwent examinations with blood routine, liver function, color Doppler image, and celiac angiogram. Results: Eight patients with splenic artery steal syndrome were examined. There were 6 case males and 2 case females. The age of these patients ranged from 22 to 55 years, average 43.9 years. The time of final diagnosis ranged from 4 to 30 days. Clinical signs included hypersplenism and elevated liver enzymes. Color Doppler image showed low hepatic artery velocity and high resistance index. Celiac angiogram showed the hepatic artery was patent but presented with sluggish flow, delayed filling of intrahepatic arterial branches, poor peripheral parenchymal perfusion, associated with early and abundant filling and of the splenic artery. All patients were treated by splenic arterial embolization with coils. The 8 patients improved quickly with significant change in the biochemical and clinical parameters. There were no complications during follow-up period. Conclusion: Splenic artery steal syndrome has low incidence rate. Clinical signs include hypersplenism and elevated liver enzymes. Color Doppler image can be used to screen the patients with splenic artery steal syndrome. Celiac angiogram can be used to diagnose the patients with splenic artery steal syndrome. The patients with splenic artery steal syndrome can be effectively treated by splenic artery embolization with coils.

    Abstract: Objective To explore the relationship between high-sensitivity C-reactive protein(hsCRP) and Microalbuminuria(mALB) in patients within 24 hours with essential hypertention. Methods According to the WTO criteria, 103 patients were diagnosed to hypertention. Hypertension stage Ⅰ(n=37)、Hypertension stageⅡ(n=34)、Hypertension stageⅢ(n=32) and 35 healthy subjects as control. hsCRP and mALB within 24 hrs were determined. Results Compared to the control, Cr within 24hrs of Hypertension stageⅡand Hypertension stageⅢ increased (P<0.01),hsCRP and mALB within 24 hrs of Hypertension stageⅠ,Hypertension stageⅡand Hypertension stageⅢ increased (P<0.01). which is correlated to the high blood pressure level. (P<0.01). Conclusions hsCRP and mALB within 24 hrs all increased in different hypertensive stage. The kidney damage of hypertension is related to the hsCRP and mALB within 24 hrs

    [ Abstract] Objective: To explore the application of the flap supported by SMAS (superficial muscle aponeurosis system) pedicle for the immediate repair of the facial soft tissue defect after accident. Methods: The flap supported by SMAS pedicle were designed at the edge of a facial subregion and were used for the immediate repair of the facial soft tissue defect after accident. The biggest area of the transplanted flaps was 7.5cm ×4.5 cm. Results: All transplanted flaps survived well without any obvious complications,and were all matched with the surrounding tissue in texture and colouration.The donor-site incision lines were obscured, facial contour and appearance were good , and cosmetic results were satisfactoring. Conclusions: Based on the principles of facial aesthetic subregions ,use of the flap supported by SMAS pedicle with sufficient blood supply is easily completed and is an ideal choice in immediate repair of a middling or small areas of facial soft tissue defect after accident

Methods With a case report and review of the related literatures , the pathogenesis , clinical manifestations ,diagnosis , management and prognosis of pulmonary benign metastasizing leiomyoma were described. Results The pathogenesis of PBML remains controversial . The pathologic feature is that the tumor appears as interlacing bundles of smooth muscle cells and connective tissue , containing glandular appearing structures .Leiomyomas may undergo degenerative changes. There is few symptoms in most patients with PBML . Chest radiograph or computed tomography usually reveals multiple bilateral pulmonary nodules . PBML can be definitely diagnosed by histopathology and treated with pulmonary surgery and hormonal manipulation . The clinical course of PBML in most patients is benign. Conclusions  Being rare and often misdiagnosed , PBML should be considered in patients showing multiple bilateral pulmonary nodules.
