    The characteristic study on the mycolic acids fingerprintings of 50 standard strains of mycobacteria

    Tianjin Hospital,Tianjin 300211,China Abstract Objective: The fingerprintings library of 50 standard strains of mycobacteria was constructed by mycolic acid analysis using high performance liquid chromatography(HPLC) .The characteristic study on the fingerprintings was carried out. Methods: Mycolic acids were extracted ,derivatized and analyzed by the HPLC method after mycobacterium species were cultured ,saponified and acided. The mycolic acids fingerprintings library was set up. On the basis the HPLC patterns of relative retention time and relative peak height, the characteristic of the fingerprintings was analyzed and compared. Results: The mycolic acids fingerprintings library of 50 standard stains of mycobacteria was constructed by mycolic acid analysis using HPLC .These patterns may be divided into three groups,such as single-,double- and triple-clusters peak patterns,each groups comprising 10,23,and 17 species, respectively.50 strains were successfully typing by its characteristics. Of 92 isolates tested,89 were given species names in concordance with the known names. The concordance rate was 96.74%.Conclusions: All of patterns have characteristics. The established method can be used to identify above mycobacterium species rapidly and reliably.

    Expression of Egr-1 protein and heparanase in bladder transitional cell carcinoma and its clinical significance

    Objective: To investigate the relationship between the expression of Egr-1 protein andheparanase.Methods: The expression of Egr-1and heparanase was examined by SABC(StreptAvidin-Biotin Complex)immunohistochemistry in 58 Paraffin-embedded tissue specimens and 10 control groups of normal bladder tissues.The expression and relationship of Egr-1 and heparanase with clinicopathologic factors and the relationship of Egr-1 and heparanase were analysed . Results: The positive expression rate of Egr-1 in normal bladder tissue was 0%(0/10),while the positive expression rate of Egr-1 in BTCC was statistically significant(P<0.01).No heparanase expression was detectable in 10 normal bladder tissues,while heparanase expression was detected in 34 of 58 transitional cell carcinoma of bladder(58.62%)。The positive expression of Egr-1 and heparanase in transitional cell carcinoma of bladder had no distinct difference in age,sex,tumorsize and tumor quantity(P>0.05),but it had a close correlation with the degrees of histological grades and clinical stages(P<0.01). The expression of Egr-1 was closely correlated with that of heparanase (liner index of Pearson=0.3875,P<0.01). Conclusion: the positive experssion of Egr-1 and heparanase might be the referencing indxe of the development and invasion of bladder carcinoma and support a potential involvement of Egr-1 in the progression from non-muscle invasive bladder cancers to muscle invasive bladder cancer. It is shown that Egr-1 may provide theoretical fundation of chemopreventive stategy for bladder cancer in the future.

    【Abstract】Objective To construct a shRNA eukaryotic expression vector of dual promoters. Methods The pcDNA3.1(+) plasmid,which was frequently used by the laboratory, was reconstructed and inserted with dual promoters (human U6 promoter). Results The recombinant vector was obtained with the correct sequence in the insert. Conclusion Successful construction of the eukaryotic expression vector may provide us with practical tools for shRNA expression study.

    Effects of comprehensive intervention on elderly diabetes patients

    Abstract Objective: The aim of this project was to investigate the effects of a community comprehensive intervention program on middle aged and the elderly diabetes patients. It includes improvement of exercise, blood glucose, blood pressure and diabetes -related risk factors. Methods: A randomized controlled community trial was conducted. 124 diabetes patients were selected and randomly assigned to intervention group and control group. The intervention group was given comprehensive intervention for 6 months. The control group was not given any intervention. The changes of two groups’ physical activity, blood glucose, blood pressure and diabetes-related risk factors before and after intervention were analyzed using SSPS 13.0 software package carry out covariance and multivariate analysis of variance. Results: The results showed that intervention group ’ s total physical activity MET on average increased by 446.01 ± 801.74MET / week, fasting serum glucose level reduced by1.40±2.16 mmol/L, two-hour post prandial blood glucose fell by an average 1.39 ± 2.99 mmol / L, waist to hip ratio reduced and body mass index reduced over an 6-month integrated intervention. Both of them had significant statistically difference between two groups (P<0.05). Conclusion: Results of the study indicates that integrated intervention model can be effective in control of diabetes and its related risk factors in elderly diabetes patients.

    The Relativity of Microalbuminuria of Youth Abdominal Obesity and Hemorheology

    【Abstract】 Objective : To observe the relativity of microalbuminuria (MAU)of youth abdominal obesity , biochemical index and Hemorheology. Method: Participants were categorized into three groups which were abdominal obese group, general obese and health volunteer, MAU, biochemical index and hemorheology were examined in the same standard. Result : The average value of MAU、TG、CHOL、UA、ALT、BUN、CRE、GLU、hemorheology index are obviously heightened, as far as abnormal rate three group(abdominal obese group , general obese group and health volunteer group) were compared ,there is obvious difference (P<0.01). Conclusion : There is abnormal index in different degree in MAU, biochemical index and hemorheology, there is obvious difference in abnormal rate (P<0.01)and MAU are positively related with abnormal index rate of hemorheology.

    The Clinical Effect Observation Of Feishu Capsule On The Steady Stage Period Of COPD

    Abstract:To observe the clinical effect of Feishu capsule which has been renamed Shengeyifei capsule(drug approval number Z20080756) on the steady stage period of COPD. Methods: With randomized, single blinded trial 87 patients with steady stage period of COPD were randomized divided in two groups. One is the cure group(44 cases) and the other is the control group(43 cases). Indexes evaluated were following: indexes (syndrome in TCM , pulmonary function, ratio of FEV1/FVC, BODE ,BMI, air current obstruct degree (O,FEV1%pred),dyspnea (D,MMRC)), movement resistance (E,6MWT)。Result: the effective rate of syndrome indexes in TCM is 88.64%,the control group is 67.44% (P<0.05);pulmonary function (FEV1%、FEV1/FVC)were increased significantly after treatment compared with that before(P<0.05) in cure group, there was no difference in control group(P>0.05). the cure group was elevated compared with the control group after the treatment (P<0.05) ;BODE indexes were decreased significantly after treatment compared with that before(P<0.05) in cure group, there was no difference in control group(P>0.05). the cure group was decreased compared with the control group after the treatment (P<0.05)。Conclusion: The elemental research indicates Feishu capsule on the steady stage period of COPD has probably advantages in improvement of syndrome indexes in TCM, pulmonary function, BODE indexes and personal living ability.

    Relationship between Diabetic Nephropathy and Subclinial Inflammatory Factors

    Abstract:0bjective To investigate the changes of the levels of high sensitivity reactive protein C (hsCRP), white blood cell (WBC) and fibrinogen (FIB) in diabetic nephropathy(DN) pa- tients. Methods:The clinic documents of 506 patients with type 2 diabetes were divided into two groups based on with/without DN.The changes of the levels of hsCRP,WBC and FIB in the two groups and the relationship between their levels and DN were analyzed.Results:Compared to non-DN group,the levels of hsCRP,WBC,FIB in DN group were increased obviously(3.653±4.634vs1.974±2.878; 7.04±1.94vs6.49±1.65; 3.46±0.89vs3.16±0.72,P<0.05).Using univariate logistic regression analysis it was found that the nephropathy had a positive correlation with WBC,hsCRP,FIB(OR=1.127, 95%CI:1.072-1.186;OR=1.188, 95%CI:1.072-1.317;OR=1.630, 95%CI:1.284-2.070).In a multiple logistic regression analysis,after adjusting the age,gender,the ascend duration of diabetes,smoking , blood pressure, lipid and body mass index(BMI),the levels of WBC,hsCRP,FIB still had a predominant relation with DN(OR=1.163,95% CI:1.034-1.308;OR =1.772,95% CI:1.343-2.338;OR=1.144,95% CI:1.082-1.211). Conclusion :The levels of WBC,hsCRP,FIB are closely related with DN.These subclinical inflammatory index can be used for prognosis and evalutation of DN.

    Relationship between Nonalcoholic Fatty Liver Disease and Subclinial Inflammatory Factors in Patients with Type 2 Diabetes

    Abstract Objective:To investigate the changes of the levels of high sensitivity reactive protein C (hs-CRP),white blood cell(WBC) and fibrinogen(FIB) in Nonalcoholic Fatty Liver Disease (NAFLD) with type 2 Diabetes. Methods: Four hundred and thirty patients with type 2 diabetes were divided into NAFLD group and non-NAFLD group according to with or without NAFLD. The changes of the levels of hs-CRP,WBC and FIB in the two groups and the relationship between their levels and NAFLD were analyzed. Results:Compared to non-NAFLD group,the levels of hs-CRP,WBC,FIB in NAFLD group were increased obviously(3.86±4.09 vs2.41±3.48; 6.84±1.66vs6.42±1.70; 3.65±0.76vs3.42±0.81,P<0.05).Using univariate logistic regression analysis it was found that NAFLD had a positive correlation with hs-CRP,WBC,FIB(OR=1.098,95%CI:1.038~1.163;OR=1.351, 95%CI:1.006~1.317; OR=1.395, 95%CI:1.063~1.831),In a multiple logistic regression analysis,after adjusting the age,gender,the ascend duration of diabetes,smoking , blood pressure, lipid and body mass index(BMI),the levels of hs-CRP still had a predominant relation with NAFLD(OR=1.197,95%CI:1.098~1.305). Conclusion: Subclinial inflammatory factors, especially the levels of hs-CRP, are closely related with NAFLD.Changing the inflammatory condition of the patients with type 2 diabetes can be used for prevention and cure for NAFLD.

    The Effects of Estradiol on Apolipoprotein AⅠ mRNA Transcription in HepG2 Cells

    Objective To observe the effect of estrogen on modulating apolipoprotein AⅠ gene transcription in HepG2 cells. Methods Exposure HepG2 cells to different concentration of estradiol (0.01,0.1,1.0,10.0,20.0 μmol/L) for 24 hours, respectively; HepG2 cells were incubated with estradiol at 10μmol/L concentration for 1,6,24,48 hour. RNA was extracted from HepG2 cells collected at above time points and the expression levels of apoAⅠmRNA was analysized by RT-PCR. Results The level of apoAⅠmRNA expressed in HepG2 cells stimulated by different concentrate of estradiol was in a dose-dependent increased. Compared with 0.01, 0.1, 1μmol/L of estradiol, apoAⅠ mRNA was significantly higher in 10μmol/L of estradiol; A time-course experiment at the 10μmol/L of estradiol indicated that the expression of apoAⅠmRNA was significantly increased at 6 hour(P<0.05)and the highest at 24 hour(P<0.05). Conclusions Estrogen may play an important role in the transcription of apoAⅠgene, the effect for mRNA expression show a dose-dependent and time-dependent partly.

    The Clinical Significance of Oxidized Low-Density Lipoprotein,Endothelin-1 and Nitric Oxide in Pregnant Woman With Pre-eclampsia

    Objective: To investigate the clinical significance of oxidized low-density lipoprotein, endothelin-1 and nitric oxide in pregnant woman with pre-eclampsia.Methods: From June 2008 to January 2009 ,59 woman with hypertension who delivered in the department of Obstetrics, The Second Hospital of Tianjin Medical University, were recruited,including 20 women with mild pre-eclampsia.(MPE group) and 19 women with severe pre-eclampsia.(SPE group).and 20 healthy pregnant women were taken as control group. Enzyme –linked immunosorbent assay(ELISA)was used to measure the oxLDL and ET-1 level in these women. Nitrate reduction was used to measure the NO level in the plasma and investigate the correlation of them. Results: the oxLDL level in MPE group (10.06±3.61)mg/L, SPE group (13.25±3.15)mg/L,were significantly higher compared with control group (3.77±0.71)mg/L (both P﹤0.01).(2) the plasma concentrations of oxLDL 、ET-1 and NO in healthy pregnant women group were (3.77±0.71)mg/L、(1.15±0.35)ng/L、(58.4±9.80)μmol/L. the oxLDL level has no obvious correlation with ET-1、NO、SBP and DBP(all P>0.05).(3) the plasma concentrations of oxLDL 、ET-1 and NO in mild pre-eclampsia. (MPE group) were (10.06±3.61)mg/L、(2.47±0.73)ng/L、(34.8±5.70)μmol/L。the oxLDL level has obvious correlation with ET-1 and NO (r=0.572、0.619, both P﹤0.01). the oxLDL level has obvious correlation with SBP and DBP (r=0.516、0.691,both P<0.01). (4) the plasma concentrations of oxLDL、ET-1 and NO in severe pre-eclampsia(SPE group) were (13.25±3.15)mg/L、(3.19±0.67)ng/L、(25.1±3.80)μmol/L. the oxLDL level has obvious correlation with ET-1、NO、SBP and DBP(r=-0.529、-0.598、0.474、0.537,all P<0.01).Conclusion : oxLDL in plasma may be involved in the pathophysiological processes of pre-eclampsia by affecting the endothelial systolic and diastolic factors'expression such as ET-1 and NO. And closely related to the incidence of pre-eclampsia.

    Empirical study on reducing hepatic and renal toxicity during hemopoietic stem cell transplantation by supplying antioxidants

    Objective To study the effect on supplying antioxidants to reduce hepatic and renal toxicity during pretreatment in hemopoietic stem cell transplantation. Methods 100 patients were randomly divided into 2 groups by single blind design. The test group was given Vit C (300 mg/d) and Vit E (600 mg/d) 15 days Before the pretreatment and the control group was given nothing. Other interventions were the same. The ALT,BUN,Cr and TAC,MDA were detected before and after HSCT. Results Significant differerces of the TAC,MDA,ALT,BUN and Cr between the two groups after HSCT. Conclusion Supplying antioxidant vitamins before pretreatment improved the antioxidant capacity and reduced lipid peroxidation of HSCT patients, and helped to control the hepatic and renal toxicity from pretreatment efficiently.

    The relationship between the serum homocysteine and oxidative stress in patients with Diabetic nephropathy

    Objective to investigate the relationship between the content of serum maleic dialdehyde (MDA), superoxide disdmutase (SOD)and reduced glutathione (GSH) with complication of type 2 diabetic mellitus. the influence and mechanism of oxidaive stress on diabetic nephropathy,so as to ofter theoretic evidences for clinical therapy. Methods:50 cases of the control group of healthy patients;150 cases with type 2 diabetes mellitus is divided into three groups of patients with normal albuminuria group, microalbuminuria and clinical proteinuria group, each group contens 50 cases. ELISA assay with serum homocysteine, spectrophotometer serum MDA, SOD, GSH content. Results:All patients with diabetes serum homocysteine, MDA levels were significantly higher than the normal control group (P <0.01); micro-albuminuria albuminuria group and clinical group were higher than normal albuminuria group and normal control group (P <0.01); clinical albuminuria group was higher than the microalbuminuria group (P <0.01); Serum SOD, GSH content of all the diabetic patients were lower than normal control group (P <0.01); micro-albuminuria albuminuria group and clinical group lower than those without nephropathy (P <0.01); serum homocysteine level and the level of serum MDA was positively correlated with serum homocysteine levels and SOD, GSH content showed a negative correlation( P <0.01). Conclusion:Serum homocysteine damage through oxidative stress involved in the occurrence of diabetic nephropathy development process.

    The related mechanism of artificial joint loosening caused by monocyte-macrophage cells challenged with metal ions

    Abstract: Objective Research the cytotoxicity of chromium (Cr3+) and cobalt(Co2+) ions and the mechanism of osteolysis after artificial joint replacement by RANK express in rats monocyte/macrophages(RAW264.7) stimulated with Co2+ and Cr3+. Methods Culture monocyte-macrophage cells in vitro. Monocyte-macrophage cells exposed to Co2+ and Cr3+ ions, The cell viability was assured by MTT test. The cells were divided into 5 groups, Group A: monocyte /macrophages; Group B: monocyte /macrophages + 500ppm Cr3+ ;Group C: monocyte /macrophages +500ppm Cr3++ 20μM SP600125; Group D: monocyte /macrophages + 10ppm Co2+; Group E: monocyte /macrophages + 10ppm Co2+ + 20μM SP600125 ; Then,Semi-quantitative reverse transcription PCR method was used to detect the level of RANK mRNA after 24h and 48h. Results Compared to the control, MTT test demonstrated that Co2+and Cr3+ ions can both obviously decrease the cell viability of monocyte /macrophages. Cells exposed to Co2+ and Cr3+ ions,compared to the control(Group A),mRNA expression of RANK was both incresaed at 24h and 48h(P<0.05),and SP600125 (JNK Inhibitor) could inhibite cells to express of RANK mRNA in cells stimulated with Co2+ and Cr3+(P<0.05). Conclusion Co2+、Cr3+ ions have a cytotoxic effect on monocyte/macrophages and can stimulate the expression of RANK in monocyte /macrophages.In addition,metal ions could stimulate monocyte /macrophages to express RANK through JNK pathway.

    Role of leptin in the mouse myocardial ischemia / reperfusion injury*

    【Abstract】 Objective To study the effect and role of leptin on myocardial ischemia / reperfusion injury (MIRI) in mouse。Methods:Twenty four male rat were randomized into 4 group:①the sham-operation group;②the myocardial ischemia reperfusion group (MIRI group),established by blocking left coronary artery anterior descending branch for 30 min followed by reperfusion ③the leptin pretreatment group(leptin+ MIRI group),established by injecting intraperitoneally leptin at 30 min before ischemia; ④the leptin post—treatment group(MIRI +leptin group),established by injecting intraperitoneally leptin at 30 min after reperfusion。Blood and heart samples were harvested from each mouse at 120 min after reperfusion。We then analyzed the samples for leption level associated with MIRI。Results: Leptin treatment increased myocardial infarct size and myocardial leptin levels and serum leptin、creatine kinase (CK)、lactate dehydrogenase (LDH)、aspartate aminotransferase (AST)、Troponin T(TnT) levels. Leptin pretreatment reduces myocardial infarct size and myocardial leptin levels and serum leptin、creatine kinase (CK)、lactate dehydrogenase (LDH)、aspartate aminotransferase (AST) 、Troponin(TnT)levels. Conclusion: Treatment and high level of leptin increased the myocardial ischemia / reperfusion injury, leptin pretreatment reduced myocardial ischemia / reperfusion injury。

    The study for 5α-R1 and adrenal cortex function in juvenile rat with metabolic syndrome

    ABSTRACT Objective: To investigate the Gene expression of hepatic 5α-reductase type 1 (5α-R1)and its effects for adrenal cortex in juvenile rat with metabolic syndrome. Methods: 50 grams of male, female SD rats (3 weeks, just weaned) were randomly divided into 3 groups, 12-14 animals in each group, were given routine diet (NC) and high fat diet(FC) and high fat-salt diet (FSC). Then the body weight,, body length, abdominal circumference, blood pressure, visceral fat weight, plasma lipids, plasma cortisol and corticotropin-releasing hormone were measured 4 weeks later, at the same time oral glucose tolerance test and insulin release test were performed. Hepatic 5α-R1 mRNA were assessed . Results: In the FSC group, body weight, abdominal circumference, blood pressure, visceral fat, plasma glucose and insulin lever significantly increased than the NC group, plasma lipid disorders increased and significant insulin resistance occurred, hepatic 5α-R1 mRNA were significantly increased , serum cortisol,corticotropin-releasing hormone level and adrenal weight had the significant relationship with the5α-R1 mRNA expression . Conclusions: We successfully induced metabolic syndrome in juvenile rat by giving high fat-salt diet .High liver 5α-R1 expression may increase GC metabolic disorder and be related to the occurrence and development of children metabolic syndrome. Targeted regulation prompted 5α-R1 expression and activity may be a early intervention for children metabolic syndrome.

    Abstract Objectives :To evaluate the factor of signal pathway-protein kinase-C(pkc) in protective effect of one MAC sevoflurane post-conditioning on rat’s myocardial ischemia/reperfusion injury in Vivo. Methods:Rats model of Myocardial ischemia-reperfusion Injury were randomly divided into 4 groups:groupⅠsham operation (S); groupⅡischemia/reperfusion injury (I/R); groupⅢone MAC sevoflurane post-conditioning (Sevo); groupⅣSevo +Chelerythrine(Sevo+C). The activity of CK-MB、SOD、MPO and concentration of Calcium ion were measured at the end of reperfusion. Myocardial infarction size was determined by dyeing with hematoxylin eosin (HE) and triphenyltetrazolium staining at the end of reperfusion. Results :As far as the activity of CK-MB、SOD、MPO and concentration of Calcium ion, there are great significant differences among four groups at the end of reperfusion(P<0.05). The concentration of Calcium ion in groupⅡ,groupⅢand groupⅣ are higher than groupⅠ(P<0.05). Compared with groupⅡ, infarction size in groupⅢ、 groupⅣreduce significantly(P<0.05). There is also great significant difference of infarction size between groupⅢand groupⅣ(P<0.01).Conclusion: One MAC sevoflurane post-conditioning can attenuate ischemia/reperfusion injury through the activation of PKC signal pathway.

    The activity of E2F-1 transcriptional factor in gastrointestinal cancer cell lines and its effect on cell cycle

    Abstract:Objective To investigate the activity of E2F-1 transcriptional factor in gastrointestinal cancer cells and its effect on cell cycle. Methods The expression of E2F-1 factor in 8 gastrointestinal cancer cell lines was detected immunohistochemically, and the cell cycle of cancer cells was analyzed by flow cytometry. Results The gastrointestinal cancer cells were divided into two groups, the E2F-1 high-expression group including HGC-27, BGC-823, SGC-7901, HT29, the E2F-1 low-expression group including MKN45, MKN28, SH288, SW-620. The ratios of G1/G0 phase, S phase and apoptosis in high-expression group were 33.48±4.61%, 63.91±5.02%, 1.50±0.56%, and in low-expression group were 58.58±3.41%, 35.38±4.03%, 5.61±1.44%. There were differences between these two groups. Conclusion The status of E2F-1 expression in cancer cells has a great effect on cell cycle. The E2F-1-positive cancer cells have a potency to grow with high rate.

    Objective: To understand the characteristics of phase of destruction of tight junctions in intestinal epithelial cells after the occurrence of acute intestinal obstruction. Methods: Healthy rabbits were randomly divided into normal control group (Norm), sham-operated group (Sham), obstruction 1h group (H1), obstruction 6h group (H6), obstruction 12h group (H12), obstruction 24h group (H24), obstruction 48h group (H48), obstruction 72h group (H72), n = 6. the relative quantitative analysis of the animals of the last paragraph of the intestinal epithelial tight junction-associated protein Claudin1’s mRNA’s expression were made through the real-time fluorescence quantitative RT-PCR technology. Results: The experiments showed thatClaudin1 mRNA expression has been found to have significantly reduced levels to normal about 43% (p <0.05), 12h to 18% (p <0.01), 24h to 11% after 6h obstruction , 48h-72h phase expression was very low. Conclusions: mechanical barriers have been destroyed at Early stage after the occurrence of intestinal obstruction (6h), the manifestation was lowly expressed genes for the tight junction protein upstream gene expression, with the obstruction of progressing, the low expression was more prominent at 48h-72h phase of obstruction. Indicated that this stage of intestinal epithelial integrity is completely lost, seriously destruction of intestinal mechanical barrier .     Objective To investigate the remission and efficacy for higher-dose daunorubicin applied in induction-therapy for elder acute myeloid leukemia (AML) patients. Methods 123 elder patients with acute myeloid leukemia were chosen to be randomly assigned to receive first induction therapy including cytarabine, at a dose of 100 mg per day by continuous infusion for 7 days, plus daunorubicin for 3 days, at the conventional dose of 30 mg per square meter (41patients), at the conventional dose of 45 mg per square meter (41patients) and at an escalated dose of 60 mg per square meter (41 patients). The primary end point was two year event-free survival. Adverse reactions and complete remission rate were analyzed. Results In the higher- dose daunorubicin group, the complete remission time, rates and event-free survival were better than the conventional dose group (P < 0.05); There was no significant difference between the groups in the incidence of hematologic toxic effects. Conclusions Higher dose DNR can be optimal choice to induce elder AML to get remission.

    Objective To investigate the changed characteristics of oxygen dynamic and treatment Strategies of high altitude pulmonary edema(HAPE) patients with secondary acute respiratory distress Syndrome(ARDS).Methods 8 HAPE with secondary ARDS patients data of oxygen dynamic were Monitiored with Swan-Ganz catheter;treatment methods were put forward. Results At the beginning Of monitor, the patients data of RAP、mPAP、PVRI、Qs/Qt increase significantly,cardiac output index(CI)、oxygen delivery(DO2)、oxygen consumption(V O2)、oxygen extraction rate(O2ext)、 PaO2/FiO2 decrease significantly;At the end of monitor, the patients data of RAP、mPAP、PVRI、Qs/Qt decrease significantly(P<0.05),CI、DO2、VO2、O2ext、PaO2/FiO2increase significantly(P<0.05),8 patients all survivaled.Conclutions HAPE with secondary ARDS patients data of hemodynamic、oxygen dynamic has disordered greatly.Therefore,early support of ventilation to improve hypoxia,enhance oxygen delivery is the most important treatment mesure..

    Diagnosis and treatment of pulmonary sclerosing hemangioma

    Objective  To investigate the diagnosis and treatment of pulmonary sclerosing hemangioma (PSH) and improve our understanding of this disease. Methods  The clinical features , radiographic manifestations and treatment of PSH pathologically verified in 61 patients were analyzed retrospectively. Results  None of the 61 patients was diagnosed as PSH preoperatively. There were 44 females and 17 males, and the average age was 40.36 years in this group. 36 patients were symptom free. The plain chest roentgenograms or CT showed a well defined, homogeneous, round or oval nodulous shadow in most cases. All patients received operation. There was no postoperative morbidity and mortality. Postoperative follow-up showed a good prognosis. Conclusion  Preoperative diagnosis of PSH is quite difficult . A good prognosis can be achieved through surgical treatment.

    An initial observation on the variation of the enzyme in serum of patients with type A(H1N1) influenza

    [ Abstract ] Objective : To find out the difference of the enzyme in serum between the severe case and mild case with H1N1 influenza and between the groups at different time points, to understand its clinical significance, and infer possible reasons. Methods :We selected H1N1 influenza in patients with severe and mild respectively 10 cases who admitted to hospital the first day, the third day and the seventh day of alanine aminotransferase (ALT), aspartate aminotransferase (AST), lactate dehydrogenase (LDH), serum creatine kinase (CK) and creatine kinase isoenzyme (CK-MB) of all values between the two groups, and severe group and mild group comparisons between different time points. Results : There were significant difference (p <0.05) between severe group and mild group in ALT, AST, LDH, CK-MB at all time points. There was significant difference between severe group and mild group in CK at the admission day ,but no significant difference between the two groups in CK at the the third day and the seventh day. There was significant difference between the admission day and the third, the seventh day respectively in CK in severe group. There was no statistical difference among different time points in severe group in ALT, AST, LDH, CK-MB. There was no statistical difference among different time points in mild group. Conclusion : The enzyme in severe group patients with H1N1 influenza was significantly higher than those in mild patients, suggesting the liver and myocardial damaged in severe patients . The mechanism of liver and myocardial damage in severe patients would need further study .The enzyme between two groups at different time points changed little in one week after hospitalization.     C-reactive protein in elderly community-acquired pneumonia in the clinical value

    Abstract : Objective To study C-reactive protein (CRP) measurement in elderly patients with community-acquired pneumonia diagnosis and antibiotic use in the clinical value. Methods Detected C-reactive protein in 50 elderly patients with community-acquired pneumonia in patients before treatment and 3 days 7 days after treatment , and C-reactive protein with the same period 50 cases of non-infectious diseases in hospitalized elderly patients .Results community-acquired pneumonia in patients before treatment with non-infected patients were significantly different (P <0.05); treatment 3 days before and after treatment were significantly different (P <0.05); treatment after 3 days and 7 days after treatment compared with significantly different (P <0.05); treatment 7 days before and after treatment were significantly different (P <0.05); treatment after 7 days compared with the control group no significant difference (P> 0.05). Conclusion C-reactive protein can be used as a diagnostic elderly community-acquired pneumonia and a sensitive indicator of therapeutic effect, but also could serve as a guide clinical timely, correct or adjust the use of an indicator of efficacy of antibiotics.

    【Abstract】 Objective:To study the chest image appearances of Influenza A(H1N1). Methods:The chest X-ray imaging features of Influenza A(H1N1). Results:Of 17 cases,pulmonary parenchymal exudation and consolidation were in 5 cases;interstitial infiltration was in 2 cases;and parenchymal and interstitial lesions were in 10 cases. Conclusion: Patches,large areas of lung consolidation,ground-glass shadow,diffuse lung diseases,and bilateral lung involvement were the chief image appearances in Influenza A(H1N1).

    Impact of Live Donor Age (>55) on Living Related Liver Transplantation

    Objective: The question of whether donor age impacts donor and recipient outcomes in adult-to-adult living related liver transplantation (LRLT) was rarely discussed. The aim of this study was to evaluate the impact of older donor age (>55) on donor and recipient outcomes in adult-to-adult LRLT. Methods:LRLT Clinical data of both recipients and donors of the transplantation department of Tianjin first central hospital were retrospectively evaluated from our 4 LRLT cases of older donor age (>55). Results: Postoperative recovery of liver function for donors and recipient/graft survival were not influenced by donor age except for one donor. Conclusion:Selected right lobe donors from individuals who were 55 years of age or older can be used in adult-to-adult LRLT.

    The complication of laparoscopic Soave pull-through for Hirschsprung’s disease in newborn and small infants [abstract] objective To explore the complication of laparoscopic Soave pull-through for Hirschsprung’s disease in newborn and small infants and find it’s prevention method. Methods Twenty-eight cases with Hirschsprung’s disease were operated by a modification Soave’s procedure under laparoscopy form May 2006 to June 2009. Results The complications were found in seven cases after operations, one case with abdominal bleeding, two cases with postoperative enterocolitis, one case with hematuria and one case with colon partly recovery. Conclusions Although laparoscopic Soave pull-through for Hirschsprung’s disease had some avoidable complications by disposing appropriately, it would be a satisfactory method for Hirschsprung’s disease in newborn and small infants.

    【Abstract】Objective: To observe the clinical results of a pedicle screw- lamina hook system applied to treat the lumbar spondylolysis without spondylolisthesis.Method: Since august, 2005, 10 cases of lumbar spondylolysis without spondylolisthesis have been treated with a pedicle screw- lamina hook system.Eight cases is male,tow cases is female.The mean age of the group was 22, from 18to 26.Tow cases of spondylolysis located at L2,one case at L3,three cases at L4,four cases at L5.All the patients suffered from severe back pain for more than six months.The average VAS before the operation was 7.2.During the operation, the presence of the pars interarticularis defect was confirmed.The defects were cleared of any fibrous and cartilaginous tissues.The sclerotic bone margins were curetted until bleeding was noted.Autograft havested from posterior ilium was done.The pedicle screw- lamina hook system was placed onto the index vertebra.Result: All the patients were followed- up for more than 18 months ( 12~36months) .The significant back pain relief was found in all the cases.The postoperative VAS was 2.2. The bony healing was comfirmed in 10 cases.Conclusion: The simple spondylolysis in youth can be treated by the pedicle screw- lamina hook system without spinal fusion.The pedicle screw- laminahook system is proved to be safe and effective.

    Surgical Treatment of Tricuspid Regurgitaion After Mitral Valve Replacement

    Abstract Background: Functional tricuspid regurgitation (TR) can often be observed in patients with rheumatic mitral valve disease, including those who undergo the operation for the first or second time. The aim of this study was to analyze TR in a group of patients who underwent successful mitral valve surgery. Methods and Results: 45 patients with TR after mitral valve replacement were involved in this study. 43 patients (95.6%) had right heart failure symptoms (edema of double lower extremity, ascites, hepatic congestion, etc.). 40 patients (88.9%) had atrial fibrillation. Twenty-six patients (57.8%) were in New York Heart Association (NYHA) functional class III, and 19 (42.9%) in class IV. Prior operations included: 41 for mechanical mitral valve replacement (91.1%), 4 for bioprosthetic mitral valve replacement (8.9%), and 7 for tricuspid annuloplasty (15.6%). All the patients received medicine to improve the heart function. The tricuspid valves were repaired with Kay's (7 cases, 15.6%) or De Vega technique (4 cases, 8.9%). Tricuspid valve replacements were performed in 34 cases (75.6%). 1 patient died (2.2%). Postoperative low cardiac output syndromes (LCOS) were observed in 5 patients and treated effectively. Postoperative echocardiography showed obvious reduction of right atrium and ventricle. Comparing with the preoperative data (33.7±6.2mm), the anteroposterior diameter of right ventricle decreased to 23-41mm (25.5 ±7.1mm) (P<0. 05). Conclusions: If TR occurs or progresses late after valve surgery, annuloplasty or replacement to tricuspid valve may be required and could get a satisfied outcome even if it could carries a high operative risk.

    Abstract Objective:To discuss the pathological feather and correlated risk factor of malignant of endometriosis.Methods:4234 endometriosis cases from 2000 to 2005 were analyzed retrospectively, Surgical treatment and pathological examination confirmed endometriosis.Results:Patients were divided into two groups based on pathological diagnosis.Dysmenorrhea and alter of menses were more frequent in group A(81.8%), abdominal distension and mass of pelvic cavity were more frequent in group B(χ2=56.88,P<0.01). There were 2041 diameter of tumor in group A in 4178 cases exceed 8cm(49.5%),diameter of tumor of 41 cases of group B exceed 8cm (73.2%), the distinction has statistical significance (P< 0.01) . Beyond surgry 357 cases’s value of CA125 is higher than 100U/ml in group A(8.7%),15 cases’s value of CA125 is higher than 100U/ml in group B(26.8%)。Age of 2953 cases in group A less than 40(71.6%), age of 19 cases in group B less than 40(33.9%), the distinction has statistical significance (P< 0.01) . Makes a follow-up visit for 5 years, five year survival rate of pure ovarian cancer of group A is 92.4%, group B is 46.9%. Condiusion:Patients with risk factors of diameter of tumor exceed 8cm or CDFI accompanied by substantial mass or CA125 is higher than 100U/ml ,surgical treatment is used for patients with risk factor,so to find the patients who was conbined with ovarian cancer.So we shoule pay higher attention to it in the linical work.

    Abstract Depression affects 340 million people around the world. It has become one of the major cause of death and disability. Depression occurs in women about twice as in men ,which suggest that estrogen may play a role .Many researches have shown that E2 in plasma is significantly lower in women with depression than those with no depression and increased after treatment .But there is few research on men about that. Efficacy of estrogen for the treatment of depression is positive in some research. But the mechanism is to be further probed. The side effect of estrogen has restricted its clinical use. Further research might gain more meaningful result.

    Effects of Chinese herbs on Th1/Th2 cytokines in peripheral blood mononuclear cells of the patients with anterior uveitis

    To investigate the effects of on the levels of Thl/Th2 cytokines in the cultured supernatants of the peripheral blood mononuclear cells(PBMC) in the patients with anterior uveitis. Methods:The levels of Th1 cytokines(IFN-γ, TNF-α) and Th2 cytokines(IL-4, IL-10) in the supernatants of PBMC of the 41 cases with anterior uveitis were detected by enzyme linked immunosorbent assay before and after the treatment by using Chinese herbs. 30 cases of healthy persons were collected as controls. Results: The levels of IFN-γ, TNF-α, IL-4 and IL-10 in supernatants of PBMC of the acute patients were different significantly than those of the patients after the treatment and the healthy controls(F=8.96~41.01, P=0.00), the IFN-γ an d TNF-α levels before treatment were higher than thecontrols(t=4.93/7.75,P=0.00),the levels of IL-4 and IL-10 were not different than the controls(t=1.55/1.46, P>0.05). After the treatment with Chinese herbs, the levels of IFN-γ and TNF-α in the recovery patients were decreased than the acute patients(t=16.94/21.21, P=0.00), IL-4 and IL-10 levels were increased(t=6.77/13.04, P=0.00), the levels of IFN-γ, IL-4 and IL-10 were higher than the controls(t=2.42~4.32, P=0.02~0.00), the levels of TNF-α was not different significantly with controls(t=0.87, P=0.39). The IFN-γ and TNF-α levels before and after treatment had positive correlation(r=0.84/0.75, p=0.00), and the IL-4 and IL-10 levels had positive correlation(r=0.88/0.86, P=0.00). Conclusion:The results indicated that the Th1 cytokines were increased in the patients of the acute anterior uveitis. After the treatment with Chinese herbs, the Th1 cytokines were decreased and the Th2 cytokines were increased. Chinese herbs could adjust the balance of Th1/Th2 cytokine of the anterior uveitic patients.
