    The effects of XPD and P53 on growth of HepG2.2.15 and the expression of Hepatitis B virus X protein

    Abstract Objective:To investigate the effects of xeroderma pigmentosum D(XPD) and P53 on growth of HepG2.2.15, human hepatoma cell, and the expressions of Hepatitis B virus X protein(HBx),Bcl-2 and Bax. Methods: Recombinant plasmid pEGFP-N2/XPD and vacant vector plasmid pEGFP-N2 were transfected into HepG2.2.15 by liposome.On the next day, these cells were incubated with Pifithrin-α,P53 inhibitor, at a 20μM concentration for 24 h. The experiments were divided into five groups: Blank control group;pEGFP-N2 group;pEGFP-N2/XPD group;pEGFP-N2/XPD + Pifithrin-α group;Pifithrin-α group. Through RT-PCR and Western blotting, the expressions of XPD,HBx , Bcl-2and Bax were detected. The cell growth was detected by MTT. The cell cycles were examined with flow cytometry. Results: RT-PCR and Western blotting results showed that the transfection of pEGFP-N2/XPD increased the expression of XPD(P <0.001,respectively). The increase of XPD expression down-regulated the expressions of HBx and Bcl-2 and up-regulated the expression of Bax, while Pifithrin-α abolished the above-mentioned effects of XPD(P <0.01,respectively). MTT results showed that the increase of XPD expression inhibited the cell growth and the inhibition of cell growth by XPD was reduced by Pifithrin-α(P <0.001,respectively). Flow cytometry results showed that the up-regulation of XPD expression increased the cell number of G1 phase and decreased that of S phase, while the effect of XPD on cell cycle was abolished by Pifithrin-α(P <0.001,respectively). Conclusions: XPD can suppress growth of hepatoma carcinoma cells, down-regulate the expressions of HBx and Bcl-2 and up-regulate the expression of Bax through P53 pathway. There may be mutual influences among XPD, P53 and HBx that co-regulate hepatocarcinogenesis.

    Effects of Recombinant Human Growth Hormone Replacement Therapy on Linear Growth and Fasting Blood Glucose in Children with Growth Hormone Deficiency

    Abstract Objective:To explore the therapeutic effects of replacement therapy with domestic recombinant human growth hormone(rhGH) on the liner growth and the fasting blood-glucose in children with growth hormone deficiency(GHD) . Methods: 25 GHD children were treated with rhGH and the dose is 0.1-0.15U/(kg.d).They use it 6-7times every week by subcutaneous injection .Height (Ht) and weight (Wt) were measured by designate personnel using the same measuring tool before and after treatment for 3 and 6months .Body mass index (BMI) and Height standard deviation score (HtSDS) were computed. Fasting blood-glucose test were performed to test fasting blood-glucose (FBG) during 8 am. And children had no food or drink at least 8 hours before the test. Results:After 3 and 6 months of rhGH therapy, the HtSDS increased from -2.30±0.44 to-1.77±1.23(P<0.05), -1.74±0.58 (P<0.001) and the HV increased observably (P<0.001). FBG increased after the 3 and 6 months treatment (P<0.05), but still in a normal range and the FBG was age-related (r=0.418、0.456,P<0.05). Conclusion:The replacement of the rhGH can improve the height of children with GHD and affect the fasting glucose in children with GHD. We should dynamically monitor the fasting glucose during the treatment especially for the children with bigger initial treatment age.

    Application of computer-assisted 3-dimensional quantitative assessment and virtual surgical system in precise hepatectomy

    Objective: To assess the effect of computer-assisted 3-dimensional quantitative assessment and virtual surgical system for precise hepatectomy. Methods: The clinical data of 17 patients who had undergone precise hepatectomy at Tianjin First Center Hospital from June 2010 to June 2011 were retrospectively analyzed. EDDA’s IQQA?-Liver was used for constructed the 3-dimensional imaging of liver, and the anatomic relationship between hepatic vessel and tumor was analyzed. Total liver volume, predicted resection volume, the remnant liver volume and remnant-standard liver volume ratio (RSLVR) were calculated, and the operative program was planned according it. Results: Of the 17 patients, total liver volume was calculated as 1744.5±881.6ml, predicted resection volume as 919.2±884.4ml, the remnant liver volume as825.3±228.6ml, and RSLVR as 67.6%±22.3%. All patients received anatomical hepatectomy, and postoperative complications included bile leak in 1case, pleural effusion in 6 cases, moderate or massive ascites in 4 cases and pulmonary infection in l case. Conclusion: computer-assisted 3-dimensional quantitative assessment and virtual surgical system helped to determine the tumor localization and to plan optimal liver resection operations. It should be wide used for precise hepatectomy.

    Clinical Significance of BNP for risk stratification,treatment and prognosis in submassive PTE

    Objective To explore the clinical significance of BNP for risk stratification,treatment and prognosis in submassiv PTE.Methods A total of 84 patients with confirmed submassiv PTE were divided into two groups according to serum BNP levels,including Group A(n=50),BNP>90pg/ml and Group B(n=34),BNP≤90pg/ml.Meanwhile,the patients in Group A and Group B were respectively divided into two groups according the treatment:thrombolysis combined with anticoagulant therapy or simple anticoagulant therapy.The clinical feature,ECG,ABG,echocardiography and prognosis were analyzed and compared. Results There were significant differences in dyspnea,chest pain,heart rate,respiratory frenquency,cyanosis,pericardial or pleural effusion,accentuated S2,abnormal myocardial enzyme,arterial blood PaO2,A-aDO2,oxygenation index,severity of ECG,RAE or RVE,PASP,mid to deep degree of tricuspid regurgitation between Group A and B (P<0.05).There were significant differences in clinical key events:PTE recurrence,pulmonary artery hypertension,pneumocardial disease between Group A and B(P<0.05).There were significant differences in △PASP,therapeutic effect,pulmonary artery hypertension,pneumocardial disease between two sub-groups according to the treatment:thrombolysis combined with anticoagulant therapy or simple anticoagulant therapy of Group A.There were no significant differences between two sub-groups of Group B. Conclusion Serum BNP level is significant for submassive PTE patients to stratify the risk,select the fittest treatment and test the prognosis.

    Correlation factors of brachial ankle pulse wave velocity in healthy subjects

    Abstract Objectives: To investigate the features and correlation factors of brachial ankle pulse wave velocity ( baPWV) in healthy subjects. Methods: 1052 healthy persons were enrolled into this research. The baPWV of each person were tested by the full-automatic arteriosclerosis diagnostic instrument, and analyzed the correlation of baPWV and other clinical indexes including age, height, BMI, abdomen circumference, blood pressure, heart rate, drinking, smoking, FBG, TG and TC. Results: The values of baPWV were higher in male, smoking, drinking, or person with abnormal FBG, TG or TC (P=0.000). BaPWV also increased along with the ages (P=0.000). Conclusions: BaPWV in healthy subjects are mainly related with age, BP and FBG. BaPWV should be tested routinely in healthy persons for indication of early change in stiffness and elasticity of blood vessels.

    Comparative Study of Less Invasive Stabilization System (LISS) Fixation and Anatomical Plating for Treatment of Complicated Proximal Tibial Fractures

    Abstract Objective: To study comparatively the clinic outcomes of the treatment of the complicated proximal tibial fractures by the less invasive stabilization system (LISS) or the anatomical plate. Methods: From July 2002 to December 2008, 58 consecutive patients which beared the high energy and complicated proximal tibial fractures were treated with the LISS or anatomical plate. The anatomical plate group has 32 patients. There were 21 males and 11 females, with the average age of 41.2 years (range, 23 to 68 years). The type of AO/OTA concluded the A2 of 5 cases, A3 of 9 cases,C2 of 16 cases and C3 of 2 cases. The LISS plate group has 26 patients. There were 16 males and 10 females, with the average age of 41.5 years (range, 21 to 61 years). The type of AO/OTA concluded the A2 of 4 cases, A3 of 7 cases,C2 of 13 cases and C3 of 2 cases. All patients were followed for 12 to 18 months. Outcomes were assessed by recording the surgical experience, post-operative complications and improvements in knee function as measured by the Hospital for Special Surgery (HSS) scores. Results: No differences in the fracture type, mean age, gender distribution were observed between the two groups. The surgical time, wound size, blood loss and HSS scores of 12 months were significantly difference in the LISS group than in the anatomical plate group. The bone graft was used more frequently in the anatomical plate group. Overall post-operative complications or radiographic healing times were all similar in both groups. A trend toward significance of a higher incidence of symptomatic hardware irritation were observed in the LISS group compared to the anatomical plate group. Conclusion: LISS and the anatomical plate both obtained the satisfactory outcomes in our study. However, LISS was characterized by the less invasion and more excellent recovery, and is new trend for the complicated proximal tibial fractures.

    The study of the relationship between Osteoporosis of type 2 diabetes and A-SAA

    Abstract Objective: To investigate the relationship between osteoporosis of type 2 diabetes and its association with risk factors preliminarily. Methods: Selected a total of 100 male patients with type 2 diabetes who came to Endocrine department of the third affiliated Hospital of Liaoning Medicine College from April 2010 to April 2011 as the object of study.All the patients were divided into the normal bone quantity group (A group), the low bone quantity group (B group) and the osteoporosis group (C group) according to the lumbar vertebra bone density(The T value). To contrast 3 groups of patient's ages, BMI, disease duration, FPG, glycosylated hemoglobin (HbA1c), the blood fats, A-SAA、UAER, TNF-α, IL-6 and so on, and then carried on the relevant analysis .Results: (1)There was significant difference among threnn groups in the facets of ages, BMI, the course, FPG, glycosylated hemoglobin (HbA1c), the blood fats, A-SAA, UAER, TNF-α and IL-6.(p﹤0.05).(2)Multivariate correlation analysis of bone mineral density in patients with diabetes showed that BMD had negative correlation with age, disease duration, HbA1c, A-SAA、UAER, TNF-α and IL-6 (p <0.05), but had positive corelation with body mass index (BMI)(p﹤0.05).Conclusion: High blood sugar and the increased level of A-SAA, TNF-α and IL-6 are the risk factor for osteoporosis of diabetes.

    Effects of Thyroid Hormone on the Expression of Synaptophysin in the Offspring Hippocampus of Hypothyroidism Rat Model

    Objective:To explore the effects of thyroid hormone, given to the hypothyroidism pregnant rats, on the expression of synaptophysin in the offspring hippocampus. Methods:Eighty Wistar rats (female)were randomly divided into eight groups: normal group, hypothyroidism control group, hypothyroidism with pregnancy (1-17 d) + thyroid hormone (high, medium and low dosage) groups, and hypothyroidism with pregnancy (18-20 d) + thyroid hormone (high,medium and low dosage) groups. Rats were fed with low-iodine diet. Rats of normal group were given KIO3 solution, and rats of other groups were given deionized water. Rats were mated with normal male rats (n=80) after three months of feeding. According to 100 g of rat body weight, the concentrations of thyroid hormone for high, medium and low dosage were 3.5, 2.0, 0.5 μg/d.The hippocampus samples were taken from new-born, 7-day-old, 14-day-old,21-day-old and 28-day-old offspring rats in eight groups. Immunohistochemistry method and image analysis technique were used to detect the expression of synaptophysin in the hippocampus . Results:In different period rats , the expression level of synaptophysin was higher in normal group than that of hypothyroidism control group(P<0.01). In the newborn rats ,there was no significant difference in the expression level of synaptophysin between hypothyroidism with pregnancy (18-20 d) group supplied with medium dosage of thyroid hormone and hypothyroidism control group (P > 0.05), the expression level of synaptophysin was lower in hypothyroidism with pregnancy (1-17 d,18-20d) groups supplied with low dosage of thyroid hormone than that of hypothyroidism control group (P<0.05), the expression level of synaptophysin was higer in hypothyroidism with pregnancy (1-17 d) group supplied with thyroid hormone (me?dium ,high dosage)and hypothyroidism with pregnancy (18-20 d) group supplied with high dosage of thyroid hormone than that of hypothyroidism control group(P<0.01). In the 7-day-old,14-day-old,21-day-old and 28-day-old offspring rats, there was no significant difference in the expression level of synaptophysin between hypothyroidism with pregnancy (1-17 d,18-20d) groups supplied with low dosage of thyroid hormone and low-iodine control group (P > 0.05), the expression level of synaptophysin was higher in other groups than that of hypothyroidism control group(P<0.05).Conclusion:Hypothyroidism of pregnant rats results in the low expression of synaptophysin in off-spring hippocampus,the expression of synaptophysin in off-spring hippocampus is highly related to the supply dosage and supply time of thyroid hormone in hypothyroidism pregnant rats.

Methods:Twenty-four adult male Sprague-Dawley rats were randomly divided into three groups ( eight per group ) , including sham-operated control group(group S),vehicle control group (group C), CPB plus NAC group (group N).Rats in group N were treated with NAC 100 mg/kg into CPB prime followed by infusion at 20 mg/(kg?h) until the cessation of CPB,which in group C were treated with the same volume of natural saline. The CPB model of rat was established. The animals of group C and group N underwent one hour CPB and were observed two hours later. The samples of plasma and brain tissue were collected at two hour after CPB. The plasma levels of neuron specific enolase (NSE),S-100β protein,tumor necrosis factor -α (TNF-α),interleukin-6 (IL-6) were determined.The contents of brain tissue of malondialdehyde (MDA) and glutathione peroxidase (GSH-px) were determined.The ultrastructure of hippocampal neuron was also examined under an electron microscope. Results :The plasma levels of NSE,S-100β protein ,TNF-α,IL-6 of group N were significantly better than those of group C. The contents of brain tissue of MDA and GSH-px of group N were significantly better than those of group C. Compared with group C ,the damage degree of ultrastructure of hippocampal neuron of group N significantly reduced. Conclusion:NAC can reduce brain injury induced by CPB in a rat model by antioxidant and anti-inflammatory mechanism.     The Study on Kim-1 Expression in Rat Model of Acute Kidney Injury Induced by Chemicals

    Objective:To study the expression of kidney injury molecule-1(Kim-1)in rat models of acute kidney injury (AKI) induced by cis-diamminedichloroplatinum (DDP), Gentamycin(GM) or cyclosporine A. Methods:3 kinds of AKI rat models were made and renal tissue was obtaind. Using those of normal rats as control, the kidney tissue was observed for histopathology.The expression of Kim-1 mRNA was detected by RT-PCR, and that of Kim-1 protein was detected by Western Blot. Results:Compared with those in control group, histopathological observation of AKI rats indicated pathological changes at various degrees, RT-PCR proved that the level of Kim-1 mRNA in renal tissue of AKI rats was up regulated, and Western blot indicated that positive expression of Kim-1 protein in renal tissue of AKI rats was progressively increseaed. Conclusion:The increase of Kim-1 in kidney tissue directly depended on the acute kidney injury induced by DDP,GM or cyclosporine A, which supported that Kim-1 can be used as a sensitive and specific biomarker for early diagnosis of acute renal tubular injury induced by chemicals.

    The Effect of Interleukin-1β on MMP-9 Expression of Human Pulp Cells in Vitro

    Objective: The purpose of this study was to investigate the effect of interleukin-1β (IL-1β), the key factor of dental pulp inflammation, on the synthesis and secretion of MMP-9 of human pulp cells in vitro. Methods: Human pulp cells were separated and cultured in vitro. Cells of 4th-8th generation were incubated in 6 wells plates and divided into two groups. The cells in experimental group were treated with IL-1β, while the cells in control group were not. The streptavidin peroxdase conjugated method was applied to investigate immunohistochemical positive stained rate of cells and the distribution of positive stained particles. immunohistochemical The gelatin zymography method was also applied to investigate the expression of MMP-9 in human dental pulp cells with IL-1β or not, and gelatin zymography to examine the production of MMPs by dental pulp cell in course of cultures by the number of integrated optical density. Results: The results of immunohistochemical method and gelatin zymography revealed that MMP-9 were expressed in normal human dental pulp cells, and IL-1β could increase this level significantly(P<0.01). The immunohistochemical positive stained particles were found mainly in the cytoplasm, while a few of them were found in the extra-cellular matrix. Upon treated with IL-1β, the cells demonstrate significantly elevated levels of synthesis and secretion of MMP-9, showing the higher number of integrated optical density(P<0.01). Conclusion: MMP-9 is probably one of the matrix-degrading MMPs in dental pulp. IL-1β could have the potential to stimulate the production of MMP-9 in human pulp cells and thus result in the inflammation of dental pulp.

    Platelet-derived Microparticles Affects Vascular Endothelial Growth Factor Expression in Endothelial Cells

    Abstract Objective: To investigate the possible mechanism of platelet-derived microparticles (PMPs) effecting on vascular endothelial growth factor(VEGF)releasing and its clinical significance in endothelial cells.Methods: Human umbilical vein endothelial cells (HUVECs) CRL-1730 were commonly cultured. Different concentrations of PMPs intervene HUVECs and incubate for different time. We used semi-quantitative reverse transcription polymerase chain reaction techniques to measure VEGF、phosphatidylinositol-3 kinase(PI3K)、extracellular signal-regulated kinase(ERK)and VEGF type II receptor (KDR) mRNA levels. Results: (1)VEGF mRNA level in 0 mg/L group was significantly higher than the 10、30、50、100 PMPs mg/L groups(P<0.05,respectively).In contrast, Flt/KDR mRNA mRNA level in 0 mg/L group was significantly lower than the 10、30、50、100 mg/L PMPs groups(P<0.05,respectively).ERK mRNA level(1.141±0.093) in 50 mg/L PMPs group was significantly higher than that of the 0 mg/L group(0.749±0.205),P=0.004.PI3K mRNA level(1.344±0.133) in 100mg/L PMPs group was significantly higher than that of the 0 mg/L group(0.999±0.246),P=0.004.(2)Just as the effect of different concentration PMPs on mRNA expression, VEGF mRNA level(0.746±0.179) in 0 hours group was significantly higher than that of the 24 hours intervention group(0.318 ±0.091),P<0.001.KDR mRNA level in 0 hours group was significantly lower than the 4hours and 24hours groups (P<0.05, respectively).PI3K mRNA level(0.999±0.246) in 0 hours group was significantly lower than that of the 24hours group (2.622±1.213),P=0.004)..Conclusions: PMPs may induce the biological processes of blood vessel and angiogenesis via VEGF receptor and its downstream signal transduction pathways.

    Changes of coagulative and fibrinolytic function in patients with both hypertension and diabetes

    Objective:To investigate the changes of coagulative and fibrinolytic function in elderly patients with both hypertension and type 2 diabetes. Methods:Plasma levels of von Willebrand’s factor(vWF), fibrinogen(Fbg), prothombin fragment 1+2(F1+2), tissue-type plasminogen activator (t-PA) and plasminogen activator inhibitor (PAI-1)were detected in 50 patients with essential hypertension, 50 patients with both hypertension and type 2 diabetes and in 30 healthy controls. Results:The plasma levels of vWF, Fbg, F1+2, t-PA and PAI-1 had significant differences among groups (P <0.05).The hypertension and type 2 diabetes group, the hypertension group presented a significant difference to the control group for the plasma levels of vWF, Fbg, F1+2, t-PA, PAI-1. The levels of F1+2, t-PA and PAI-1 had significant differences in the hypertension and type 2 diabetes group compared with those in the hypertension group (P <0.05). Conclusions:There was coagulative and fibrinolytic disturbance in elderly patients with both hypertension and type 2 diabetes. Molecular markers should be measured in those patients for prevention and treatment of thrombotic diseases.

    Clinical study on influence of different blood pressure to perfusion in extracorporeal circulation.

    [Abstract] OBJECTIVE To explore the influence of different blood pressure in mixed venous oxygen saturation(SvO2) and lactic acid(Lac). METHODS Forty patients undergoing coronary artery bypass graft(CABG) surgery with extracorporeal circulation(ECC) were selected and divided randomly into two groups: the patients in group I were applied with drugs to keep blood pressure between 60 and 90mmHg duringECC. Those in group II were to keep blood pressure between 40 and 60mmHg. The arterial and venous blood gas were examined at time points of before extracorporeal circulation(T1), five minutes after clamping the ascending aorta (T2), before rewarming (T3), before the end of ECC (T4) and before delivering (T5). Hematocrit, arterial oxygen saturation and glucose, and electrolytes were maintained within the normal range during ECC. The values of SvO, Lac and temperature at all time points were recorded. RESULTS 1. The normal data, ECC clinical data and the result of blood gas had no significant statistical. differences between the two groups(P>0.05). 2. There were no significant statistical differences in SvO2 of the two groups. 3. There were significant statistical difference in Lac of the two groups with point T4 and T5: the results of Lac in group II were higher than in group I (P<0.05). CONCLUSION Maintenance of a higher blood pressure with vasoactive drugs during extracorporeal circulation is favorable to decrease the blood lac after rebeating.

    abstract Objective: To evaluate the effect of probucol and atorvastatin combined medication on blood levels of liquids and Lp-PLA2 in patients with acute coronary syndrome(ACS).Method: A total of 94 patients with acute coronary syndrome who had been diognosised by CAG were randomly divided into two groups: single medication group(n=48),the patients were taken atorvastatin 20 mg/d;and combined medication group(n=46),the patients were taken atorvastatin 20 mg/d with probucol 500 mg/d.The plasma levels of Lp-PLA2 ,total cholesterol(TC),triglyceride(TG),low density lipoprotein-cholesterol (LDL-C) were measured in both groups before and 6-8 weeks after the medication.Then the results were compared and analysed .Resultes: There was no significant diffierence in the levels of TC、TG、LDL-C、HDL-C and Lp-PLA2 before treatment between the two roups(P>0.05).The values of TC、TG、LDL-C and Lp-PLA2 were significantly decreased after the treatment in both two groups(P<0.01).Compared with single medication group,the value of TC, LDL-C, Lp-PLA2 were more significantly decreased in combined medication group (P0.05).The values of HDL-C was increased after the treatment in the single medication group(P<0.O1),while decreased in the combined medication group(P<0.O1). Conclusion: The values of TC、LDL-C and Lp-PLA2 can both decrease by single atorvastatin medication or probucol and atorvastatin combined medication but more significantly by combined medication . Combined treatment played an important role in stabilizing the plaques and antiatherosclorosis in ACS patients.

    The efficacy of colon dialysis with integrated traditional Chinese and Western medicine and artificial liver support system in treating liver failure with hepatic encephalopathy

    Objective To investigate the efficacy of colon dialysis therapy with integrated traditional Chinese and Western medicine and artificial liver support system with hepatic encephalopathy. Methods 74 cases of hepatic encephalopathy were divided into two groups: treated group(38 cases) and control group(36 cases) and treated with comprehensive management and artificial liver treatment. The patients in treatment group were treated with colon dialysis therapy on the base of routine treatment. The changes of symptom, prognosis, liver function, PTA, blood ammonia were observed before and after the treatment. ? Results The survival rate in treated group(84.21%) was higher than that in control group(58.33%)(χ2=6.091,P<0.05). The recovery period of unconsciousness of patients in treated group (32.88±8.34)h was much shorter than those in control group (45.12±9.68)h (P<0.01). The totle effective rate for hepatic encephalopathy treatment in treated group (78.95%) was much higher than those in control group (52.78%)(χ2=5.659,P<0.05). Significant difference of Bil, PTA, and blood ammonia were shown in the treated group(P<0.01) before and after treatment, and between groups after treatment (P<0.01). Conclusion The colon dialysis is effective in hepatic encephalopathy treatment, which can shorten the unconsciousness time, increase the survival rate, decrease the levels of bilirubin and blood, ammonia and shorten prothrombin time.

    the Role of Sniff Test in the Diagnosis and Treatment of Diaphragmatic Eventration

    Abstract Objective:In this study we aimed to evaluate the outcome of diaphragmatic plication for unilateral diaphragmatic Eventration and to explore the role of sniff test in the diagnosis and treatment of diaphragmatic Eventration. Methods:Sixteen patients who underwent unilateral diaphragmatic plication between 2003 and 2009 were evaluated. Accroding to sniff test, ten patients(positive group) were shown to have paradoxical movement of the diaphragm and six patients(negative group) weren’t preopreatively. Before and after plication, pulmonary function, dyspnea index and arterial oxygen tension were assessed. Results:All variables showed signi?cant improvement after plication in the positive group(P<0.01), however, in the negative group only arterial oxygen tension showed significant improvement after operation. The positive and negative groups showed significant difference of operative effective. Conclusion:Diaphragm plication provides excellent results for patients with positive result of sniff test.Sniff test has important clinical value in predicting the outcomes of diaphragmatic plication for unilateral diaphragmatic Eventration.

    Abstracts Objective: To explore expression and signification of FHIT mRNA in bladder transitional cell carcinoma(BTCC). Methods:We detected the expression of FHITmRNA in BTCC of 62 cases and normal bladder tissues of 35 cases by reverse transcription polymerase chain reaction (RT-PCR). Results:FHITmRNA is higher in the normal tissues than in bladder transitional cell carcinoma(BTCC),and FHIT Gene was relatively with the pathological grade. Conclusion:It is of great important role in the initiation and development of BTCC. FHIT gene may be a reference indicator of bladder transitional cell carcinoma.

    Abstracts Objective:To investigate the clinical value of percussive mobilizer sputum elimination apparatus in pulmonary infection combined with respiratory failure. Methods:From July 2008 to July 2010, 58 patients with pulmonary infection combined with respiratory failure were randomly divided into two groups: 29 patients in group A (treatment goup) and 29 patients in group B (control group). A group were treated with G5 Percussive Mobilizer made in America, B group were treated with knock expectoration.The SpO2, PaO2, PaCO2, moist rales of lung, expectoration, the daily sputum volume and ICU stay were observed and compared. Results:Compared group B, the efficiency of sputum elimination and improvement of moist rales were better in group A.There were significant differences between group A and group B in SpO2 ((94.48±3.18) % versus (92.21±2.88) %), PaO2 ((70.38±6.90) mmHg versus (66.31±7.49) mmHg), PaCO2 ((40.34±3.63) mmHg versus (43.41±3.27) mmHg) ICU stay ((9.31±2.59)d versus (11.66±3.53)d) (P<0.05). Conclusion:Percussive mobilizer sputum elimination apparatus had signifeant effect in assisting expectoration in patients with pulmonary infection and respiratory failure, which could help to control the infection, improve the pulmonary function and reduce ICU days. It is worthy to be popularized.

Methods: Taking the questionnaire on 2355 health technical personnel who have received 16 kinds of technologies training and were from rural three-tier institutions in Jixian County and Ninghe County. Gender, educational background, professional title, the situation of understanding, master and application were investigated. Results: The trained can understand and master the technology. 2 121 personnel used the technologies, accounting for 90.06% of the trained. 10 technologies were well applied. Conclusions: Rural health appropriate technology were understood, mastered, and applied with better.

    Clinical Observation of Coelom Hyperthermic Perfusion Chemotherapy in the Treatment of Malignant Pleural Effusion and Ascites

    Abstract Objective:To observe the efficacy and adverse reaction of coelom hyperthermic perfusion chemotherapy in the treatment of malignant pleural effusion and ascites. Methods: 58 cases in the treatment group were from oncology inpatient of our hospital from February 2010 to August 2010, while 58 cases in the control group were from June 2009 to February 2010. All patients were placed Central Venous Catheter by ultrasound –guided, and the effusion was drained properly. Treatment group : two-thirds of the chemotherapeutic drugs with Lidocaine Hydrochloride 0.1g and Dexamethasone 5mg were put into 2000~3000ml normal saline,and then the medicinal liquid was perfused into body cavity by indwelling drainage tube circularly,maintainning at 43℃~45℃with coelom hyperthermic perfusion therapy machine and keeping 60 minutes,finally the remaining one-third of the chemotherapeutic drugs with normal saline 100ml was perfused into body cavity. Control group : the chemotherapeutic drugs were put into 50ml (thoracic cavity) or 1000-2000ml (abdominal) normal saline heated to 45℃ in advance,and then the medicinal liquid was perfused into body cavity by indwelling drainage tube quickly. The therapeutic schedule was applied once every three weeks in each group. The efficacy and adverse reaction were evaluated after two cycles. Results : The total effective rate in the treatment group was 89.66%, while that in the control group was 67.24%(P<0.01). The adverse reactions were similar with no significant differences in the two groups.Conclusion:Coelom hyperthermic perfusion chemotherapy can effectively control the growth of malignant pleural effusion and ascites with mild adverse reaction. It’s worthy of applying to the clinical.

    Endovascular treatment of poor grade ruptured aneurysms:a review of 36 cases

    Objective To investigate the therapeutic effects of endovascular management for aSAH caused by ruptured aneurysm of poor grade(Hunt-Hess grade,4-5). Methods we retrospectively 36 patients with poor grade aneurismal SAH admitted to our hospital ,of which 28 were Hunt-Hess Grade Ⅳ and the rest grade Ⅴon admission. Early endovascular treatment was carried out in these patients ,the data of these patients including patient demographics, morphologic features of the aneurysms ,treatment results and follow-up data were collected and analyzed .Results The early interventional procedures were completed in all patients ,Angiography immediately after the treatment showed that the aneurysm of the total 32 patients were obliterated ,After a follow up period of 3- 18 months ,good prognosis was 55.5%(20/36),the poor prognosis was 13.8%(5/36),and the mortality rate was 8.39%(3/36). Conclusion Patients with poor grade aneurismal SAH should be treated aggressively; Interventional embolization by using detachable coils is a safe ,mini-invasive and effective therapy with few complications, and also can receive satisfying prognosis.

    The Application of Preoperative Acute Normovolemic Hemodilution Combined with Intraoperative Autologous Blood Recovery in the Surgery of Cranial

    【Abstract】 Objective: To investigate the effects to the patients’ blood coagulation function and the function of anemia cure as preoperative acute normovolemic hemodilution combined with intraoperative autologous blood recovery is used in elective surgery of brain. Methods: Comparing 16 selective neurosurgery patients’ APTT, PT, PLT, HB, HCT respectively at the time: before surgery, after normovolemic hemodilution, when the bleeding basically ended, intraoperative transfusion of autologous blood, and the preoperative collection of autologous blood. Results: after normovolemic hemodilution, as compared with preoperation, there were no significant changes in the patients’ PLT, APTT, PT, but the patients’ HB, HCT decreased significantly; when the bleeding basically ended, as compared with preoperation, the patients’ PLT, HB, HCT decreased significantly, and the patients’APTT, PT pronged; after intraoperative transfusion of autologous blood, as compared with those at the time when the bleeding basically ended, the patients’ APTT, PT pronged, the patients’ PLT further declined, but the patients’ HB, HCT increased significantly; after transfusion of the preoperatively collected autologous blood, as compared with those after the transfusion of intraoperative recovery autologous blood, the patients’ APTT, PT shorted significantly, the patients’ PLT ,HB, HCT increased significantly. Conclusions: In surgery with bleeding in larger amount, peoperative acute normovolemic hemodilution combined with intraoperative autologous blood recovery may help to correct the patients’ anemia. This method may also help to improve the patients’ coagulation function and surgical safety.

    Development of Glucagon-like Peptide-1 Analogs and GLP-1 Receptor Agonists

    Glucagon-like peptide-1(GLP-1) is a hormone secreted from enteroendocrine L cells of the intestine. Upon binding to its receptor, it results in many effects such as increasing insulin biosynthesis and secretion from β cells; suppressing glucagon secretion from α cells; enhancing islet neogenesis and proliferation of β cells; decreasing apoptosis of β cells; enhancing body sensibility to insulin and so on. However, GLP-1 is rapidly degraded by DPP IV enzyme and clearance in renal; the in vivo half-life is less than 2 min which limited the clinical utility of GLP-1 in type 2 diabetes. Many efforts were focused on finding the novel GLP-1 receptor agonist or GLP-1 analogs retaining the biological effects of GLP-1. This review summarized the physiological properties of GLP-1 and the ongoing development of GLP-1-based therapies for treatment of diabetes.

    Canonical Wnt signaling(Wnt/β-catenin signaling) exits in the osteoblasts,which is composed of LRP5,β-catenin and GSK-3β et al.The changes of these conmponents in the cells can affect the proliferation and differentiation of osteoblasts,then control the formation and remolding of bone.The inactivation of LRP5 and β-catenin will suppress the Canonical Wnt signaling,then the differentiation of osteoblasts,which will induce the formation of bone. The inhibition of GSK-3β will increase the espression of the specific gene in the osteoblasts,contributing to the formation of bone.Osteoblast plays an important role in the formation and remolding of bone.The review is about the Canonical Wnt signaling how to regulate the proliferation and differentiation of osteoblasts.
