    The Surface Preparation on Pure Ti Implant Using Femtosecond Laser and Short-term Implants Research

    Abstract Objective: To elucidate new implant surface treatment methods, in order to enhance bone implant interface bonding strength and promote initial osseointegration. Methods: Pure titanium specimen submerged in HA supersaturated liquid surface processed by femtosecond laser as the test group, untreated group as control group. Surface topography of the samples was examined by SEM and the element concentration of the samples was analyzed through EDX. The samples were implanted in New Zealand rabbits femoral. After 4 weeks, making histological sections to observe experimental samples of initial bone regeneration and osteoconduction. Results: It is found that three-stage hierarchical surface structures are produced on the surface of implants which was composed of grooves, islands, nanoholes under SEM. Remarkably, Ca/P deposition were revealed on the micro-structured surfaces firmly by EDX. The surface of the control group was smooth. Histological results showed the formation of osteoid and the deposition of calcium phosphate in the text group were more active than the control group. Conclusion: The femtosecond lasers can induced topo patterns surface on pure Ti surface in HA suspension and promoted Ca/P element to deposited on the micro-patterns. Contrast with the control group, the samples processed by femtosecond laser were benefit to bone regeneration and initial stabilization.

    E-tracking technology in evaluating carotid artery elasticity in drinker

    Objective To explore the value of E-tracking technology ( ET) in evaluating carotid artery elasticity in drinker . Methods One hundred and forty one subjects were enrolled in the study and grouped to 35 ones of non-drink group,35 ones of modicum-drink group,35 ones of moderate-drink group and 36 ones of heavy-drink group,that all of the subjects exclude high bloodpressure,hyperlipidemia,diabetes and serious heart、brain、kidney、hypothyroid disease,carotid arteries have no plaque. Application of E-tracking technology torack real-time polygraph subjects bilateral common carotidarter wall movement, Calculatedanalysis of stiffness parameter(β), epressure-strain elastic coefficient (Eρ),arterial compliance(AC),one-point pulse wave velocity calculate from β (PWVβ), and carry on statistics analysis to various parameters to assess carotid artery elasticity change of drinker. Results Comparison parameter among all groups:with the increase in alcohol consumption,stiffness parameter(β), elasticity modulus(Eρ),one point pulsewave velocity calculated from β value(PWVβ) values gradually increased,arterial compliance(AC)decreased; heavy-drink group、moderate-drink group and non-drink group values in each parameter between groups showed with significant difference(P<0.05);heavy-drink group、moderate-drink group compare with modicum-drink group,the statistical test with significant difference(P<0.05); the heavy-drink group compare with moderate-drink group,the statistical test with significant difference(P<0.05); modicum-drink group compare with non-drink group with no significant difference(P>0.05),there is no statistical significance. Conclusion Heavy-drink、modicum-drink can bring the decrease of carotid arterial elasticity ,ET technique can early detection the change of carotid artery elasticity .

    Analysis of the Application of Pregnancy-associated Plasma Protein A to Diagnose the Acute Coronary Syndrome

    Abtract Objective: To detect the concentration of PAPP-A in the serum of the normal individuals, SAP and ACS, and analyze its clinical value. Methods: Picked 117 ACS samples, and followed all of them after therapy three weeks later. Also, picked 68 samples of SAP and 84 of nomal controls. Then, all of the above were detected with the ELISA method. Results: the concentration of PAPP-A in the samples of the normal individuals, ACS and SAP, the concentration in the ACS was highest, which had significant statistical meanings, (q1=23.73, q2=31.32, P<0.01). However, the findings between SAP and normal ones had non-statistical meaning, (q3=0.54, P>0.05). Also, we found that the PAPP-A congcentration had significant statistical meanings, (t=12.831, P<0.01). Conclusion: PAPP-A was of great value in detecting the ACS samples, complexed with the symptoms and other markers, it can have a much better application in predicting and preventing the ACS, if the sensitivity becomes higher, the clinical application of PAPP-A can be better.

    Abstract:【Objective】To observe the changes of the probability of inhibition and the mitogen-activated protein kinase phosphatase-1(MKP-1)expression in human breast adenocarcinomal cell after dexamethasone pretreatment with difficult concentration and treatment of paclitaxel. 【Methods】MTT assay (mononuclear cell direct cytoxicity assay) : to estimate the survival rate of paclitaxel on breast adenocarcinomal cell MCF-7 , screening the concentration of IC50 in this medicine. To place different concentration of dexamethasone pretreatmently and paclitaxelsuccedently on MCF-7 cell,MTT assay to estimate the survival rate of paclitaxel on MCF-7 cell and reverse transcriptase-polymerase chain reaction(RT-PCR)to detect the expression level of the mitogen—activated protein kinase phosphatase-1 (MKP-1)mRNA in MCF-7 cell( P < 0.05). 【Results】After being pretreatmented with dexamethasone with difficult concentration,MCF-7 cell was induced inhibition of chemotherapy to paclitaxel,the inhibition rate increased gradually with the increasing concentration of dexamethasone and the expression level of MKP-1 mRNA increased gradually with the increasing concentration of dexamethasone also. 【Conclusions】After being pretreatmented with dexamethasone with difficult concentration,MCF-7 cell was induced inhibition of chemotherapy to paclitaxel,which make clear that MKP-1 may be involved in the path-ways of chemotherapy inhibition to breast carcinomas through resistance cellular apoptosis.

    Effects of Platelet Factors on Haemodialysis Patients with Recurrent Native Arteriovenous Fistula Dysfunction

    Objective:To investigate the relationship between platelet Factors and recurrent native AVF dysfunction in MHD(maintenance hemodialysis) patients.Methods:The studied subjects were divided into three groups(group A included 18 MHD patients with recurrent native AVF dysfunction, group B included 30 MHD patients with longer native AVF survival and group C included 12 healthy volunteers as controls). The expression of platelet activation markers(CD62P,CD63) were measured by flow cytometry(FCM), and all studied subjects were detected platelet aggregation indued by arachidonic acid(AA) and adenosine diphosphate(ADP). All those results were compared among groups.Results:The level of activated platelet and platelet aggregation in group A is much higher than in group B and group C(P <0.05 or P <0.01). The logistic analysis showed that CD62P and PAgTADP were the risk factors of recurrent native AVF failure. Conclusion: The higher level of platelets activating and platelet aggregation is associated with recurrent native AVF dysfunction in MHD patients

    Clinical observation of PA-MSHA and talcum powder administration by thoracic cavity injection in the treatment of malignant pleural effusion

    Abstract Objective: To prospectively study the clinical efficacy of combined administration of pseudomonas aeruginosa-mannose sensitive hemagglutinin (PA-MSHA) and talcum powder in the treatment of malignant pleural effusion. Methods: 72 patients suffered from malignant pleural effusion were randomly divided into one observation group (28 cases) and two control groups (24 cases and 20 cases respectively). Patients in the observation group were given combined use of PA-MSHA and talcum powder, while patients in two control groups received PA-MSHA and talcum powder respectively. All the patients received thoracic close drainage by central vein catheterization set. Then the medicine was injected into thoracic cavity through the catheter. The weekly administration lasted 1~3 weeks depending on clinical efficacy. Results: The total effective rate of treatment group was higher than control groups (P<0.05). Compared with control groups, the treatment group showed no significant difference in serious side effects. Conclusion: Combined administration of PA-MSHA and talcum powder through thoracic drainage catheter proved to be a safe and effective method.

    Abstract Objective:To evaluate the effect of therapy that Paroxetine to post-stroke anxiety(PSA).Methods:Ninety-four patients with PSA were received the therapy of Paroxetine or Lorazepam at random.The clinical conditions were reviewed by NIHSS and HAMA scale until 8 weeks after beginning of the therapy .Results:By the end of observe period,the scores of NIHSS and HAMA in two group were significantly decreased(P<0.01),but the scores of NIHSS and HAMA in Paroxetine group were more decreased than those in Lorazepam group(P<0.01). Conclusion:To treat the PSA,the therapy of Paroxetine has more obvious effects than Lorazepam.

    Expression and significance of NF-κBp65 and MMP-9 in non-small cell lung cancer

    To investigate the protein expression of nuclear factor kappa B (NF-κB) and MMP-9 in NSCLC tissues and the relationship between their expression and metastasis. Methods: Expression of NF-κBp65 and MMP-9 protein in non-small cell lung cancer tissues (n=95,)and normal tissues(n=27) was detected by immunohistochemical staining. Results:(1)The expression rates of NF-κBp65 protein in NSCLC are 44.2%;In the normal tissues are 22.2%.Significant difference were found for NF-κBp65 expression.(2)The expression of NF-κBp65 protein has significant correlation to tissue typing, clinical classification and clinical stage(P<0.05 or P<0.01), but it has no correlation to age、sex and tumor size (P > 0.05).(3)The expression of MMP-9 protein has significant correlation to tissue typing, clinical classification and clinical stage(P<0.05 or P<0.01), but it has no correlation to age、sex and tumor size (P > 0.05).(4) There was obviously positive correlation between NF-κBp65 and MMP-9( r=0.217,P<0.05).Conclusion:The results indicate that NF-κBp65 and MMP-9 protein play some role in suppression of the metastasis of non-small cell lung cancer.

    Construction of a shRNA Lentiviral Vector Aiming at The Target of HPV18E6 Oncogene

    Abstract Objective:To construct a lentiviral vector which contains short hairpin RNA(shRNA) interfering sequence aiming at the target of human Papillomavirus 18E6 (HPV18E6) oncogene.Method:Based on the information of HPV18E6 oncogene,design and synthesize shRNA interfering sequence,recombinate into pGCSIL-GFP plasmid to construct a recombination plasmid pGC-shRNA which contains shRNA interfering sequence and apprase it;Recombination plasmid and packaging plasmid pHelper 1.0 and pHelper 2.0 co-transduce the 293T cell to package the lentiviral vector,collects the lentiviral vector solution,concentrates and purifies it;The lentiviral vector solution after concentrating and purifying transduce 293T cell to determine the concentration of the lentiviral vector solution.Results:Indexing shRNA of interfering sequence aiming at the target of HPV18E6 oncogene with human genes in BLASTER,there is no homogeneous,the second constructure of the transcripts of interfering sequence can conform short hairpin;The recombination plasmid pGC-shRNA was obtained with the correct sequence in the insert after the appraisal;After packaging and collecting and concentrating the lentiviral vector successfully, the concentration of lentiviral vector solution is 3x108TU/ml.Conclusion:The lentiviral vectors of HPV18E6-RNAi-LV was constructed successfully,which provides practical tool for later study.

    Effect on the Expression of Related Protein of AQP2 and Alteration of Morphology and Function in Kidney of Endotoxemic Rats

    Objective: To investigate the effects on the expression of related protein of AQP2 and alteration of morphology and function in kidney of endotoxemic rats. Methods: 24 rats were randomly divided into 3 groups (n=8 each): C1 group (sham control group), C2 group (saline and fat milk group), and L group (endotoxin and fat milk group). The morphology and function of kidney and the levels of AQP2, ICAM-1, and TNF-α with RT-PCR and Western blot were detected. Results: Endotoxemia led to morphological damage and functional impairment, and downregulation of AQP2 and upregulation of ICAM-1 and TNF-α in rat kidney. Conclusions: Endotoxemia led to renal damage, and decreased AQP2 level, and increased ICAM-1 and TNF-α level.

    The Effects of Intravenous Infusion with Paclitaxel on the Plasma Level of Catecholamine in Breast Cancer Rats during Perioperative Period

    Abstract Objective To investigate the effects of intravenous infusion with Paclitaxel on the plasma level of catecholamine. Methods From July 2008 to May 2009, 50 rats with tumor formed by planting Walker-256 cell were distributed into 4 experimental groups and 1 control group. Each group included 10 rats. The rats in experimental groups received intravenous infusion of Paclitaxel, while the rats in control group received intravenous infusion of normal sodium. The tumor resection operation for the control group was taken 1 week after infusion of normal sodium, while the same operation for the experimental groups were given at the end of the first, the second, the third and the fourth week after receiving intravenous infusion Paclitaxel. The blood of all the rats were taken before and during the operation of tumor resection, and then the plasma concentration of catecholamine were measured respectively. Results the intravenous infusion of Paclitaxel before operation could greatly improve the plasma level of norepinephrine, and it reached peak at the end of the second week. Conclusions Intravenous infusion of Paclitaxel can induce significant stress response which is characterized by the increase of plasma level of norepinephrine. It indicates that the stress system become to the better state 2 weeks after intravenous infusion of Paclitaxel.

    The Portective Efect of Levosimendan in STH-2 Cardioplegic SoIution on Ischemic/reperfusion Injury in IsoIated Rat Heart

    Objective:To observe the portective effect of levosimendan(Levo) in cardioplegic soIution on isoIated rat heart. Methods:Thirty-two male Wistar rats were randomly divided into four groups(n=8 in each group): no Levo(L0), 0.03μmol/L Levo(L0.03), 03μmol/L Levo(L0.3), 03μmol/L Levo and 10μmol/L glibenclamide(LG) in St.ThomasⅡ(STH-2)cardioplegia solution.The hearts was connected to Langendorff perfusion apparatus immediately.After perfused with K-H solution for 30 minutes,perfuseed by different cardioplegic solution at 25℃ for 2 hours,and then were reperfused with K-H solution for 30 minutes. Observed the changes of hemodynamics,myocardial enzyme leakage in the coronary effluent.Results: There were no significant diferences in hemodynsmics, creatine kinase(CK)and lactate dehydrogenase(LDH)before hearts stopped beating in all groups(P>0.05).After reperfusion ,the recovery of myocardial hemodynamic parameters in group L0.3 better than other groups(P<0.01 or P<0.05).LDH and CK significant dcreease in group L0.03 compared with other groups (P<0.05)..Conclusion: Levo added into STH-2 cardioplegic soIution has protective effects on myocardial isehemia-reperfusion injury,the mechanism of the protection may be related to the opening of the adenosine triphosphate-regulated potassium channel (KATP).

    Effects of Nonylphenol on the Structure and Function of Thyroid gland in Rats

    Abstract Objective:To study the effects varying concentrations of nonylphenol on the structure and function of the thyroid gland in rats and mechanism.To provide the available information on endocrine disrupting chemical-NP to rats has on the toxic effect. Methods:40 healthy 4-weeks-old SD white rats were divided into 4 groups at random, group comprised of 5 male and 5 female rats. The treated rats were administered varying doses of NP(40mg/kg , 80mg/kg and 200mg/kg) by pouring stomach. The changes in serum hormone , histopathology and follicular cells ultramicroscopic structure were examined after 15 days. Results:1.Serum FT4 and FT3 levels decreased gradually as the dosage increased after 15 days and there is a negative correlation on doses of NP(FT4 r=-0.932; FT3 r=-0.926).The difference was statistical between random group and the treated groups(p<0.05).There was a significant difference between 40mg/kg and 80mg/kg ,200mg/kg on the doses of NP(p<0.05). 2.TSH levels increased. There is a positive correlation on doses of NP(r=0.967).The difference was statistical between random group and the treated groups(p<0.05).There was a significant difference between 40mg/kg and 80mg/kg ,200mg/kg on the doses of NP(p<0.05). 3.SDH activity of the subject rats increased obviously , The difference was statistical between random group and the treated groups(p<0.05). 4.The hyperplastic thyroid structure can be observed on each group. The hyperplastic effect was in a dose dependent manner.

    Abstract Objective: To observe the effect of creatine phosphate on cerebral infarction volume in focal cerebral ischemia/reperfusion rat model.Methods: Rats model of focal cerebral ischemia/reperfusion injury were randomly divided into 5 groups:groupⅠ,shame operation, groupⅡ,ischemia/reperfusion injury (I/R); groupⅢ,low dosage of creatine phosphate 20mg/kg(L); groupⅣ, medium dosage of creatine phosphate 40mg/kg (M); groupⅤ,high dosage 80mg/kg(H); The activity of SOD、MPO 、MDA and Na+-K+-ATP enzyme were measured at the end of reperfusion. Infarction conditions and volume of cerebral was determined by dyeing with hematoxylin eosin (HE) and triphenyltetrazolium staining at the end of reperfusion. Results :As far as the activity of MDA、SOD、MPO and Na+-K+-ATP enzyme, there are great significant differences among five groups at the end of reperfusion(P<0.05). There is also great significant difference of infarction volume between group Ⅲ and groupⅣ(P<0.05).Conclusion: pre-conditioning of creatine phosphate can attenuate ischemia/reperfusion injury of focal cerebral.

    The study on therapy for inferior genicular arterial occlusive disease

    objective To evaluate the therapy for inferior genicular arterial occlusive disease. Methods 71(101 limbs)patients with infrapopliteal artery ischemia were divided into two groups according to their therapy. Group I were treated with PTA;group II were only drug treatment. Their clinical features, ABI, clinical results, cumulative patency were collected and analyzed statistically. Results In this sudy, the average increased ABI is 0.52±0.04 in group I, 0.04±0.01 in group II at 14-day after-therapy. The degrees of increase had significant difference between two groups (P<0.05). In group I, The arterial cumulative patency rate was 88.5% at 1-year, 65.2% at 2-year postoperatively, respectively. The secondary arterial patency rate was 96.2% at 1-year, 83.3% at 2-years postoperatively, respectively. The limb salvage rate was 100%. Conclusion PTA is a feasible, safe, and effective procedure, and it should be recommended as the first choice for patients with critical infrapopliteal limb ischemia. But the long term patency needs to be observed in the future.

    Correlation of serum HBV DNA levels with HDL-C in patients with chronic hepatitis B infection

    Abstract Objectives The aim of the present study was to determine the correlation between high density lipoprotein cholesterol (HDL-C) levels and serum hepatitis B virus (HBV) DNA levels in patients with chronic hepatitis B infection. Methods 156 patients with chronic hepatitis B infection were enrolled in this study. Serum lipid profiles and HBV DNA levels were measured. All the patients were divided into three groups according to the HBV DNA levels: High-DNA levels group (>107copy/ml, n=76), Medium-DNA levels group (106~105copy/ml, n=42) and Low-DNA levels group (<105copy/ml, n=38). Results A significant negative correlation was found between increments in serum HBV DNA levels and HDL-C levels (P<0.01). HDL-C levels were inversely associated with HBV DNA levels (r=?0.336, P<0.01). Conclusions HDL-C levels in patients with chronic hepatitis B infection were inversely associated with disease severity of OA. These results demonstrated that HDL might affect the bioactivity and the ongoing replication of HBV, and HDL-C levels could be used as a biochemical marker for assessing the replication of HBV.

    A clinical study of spontaneous reversal of depressed ventilatory during sevoflurane-remifentanil anesthesia

    【Abstract】Objective To investigate the effect of sevoflurane-remifentanil anesthesia on spontaneous ventilation recovery. Methods Four hundreds ASAⅠorⅡpatients aged 18-63 yr,weighing 52~80 kg scheduled for elective surgery were randomly divided into 2 groups(n=200 each): sevoflurane group(Ⅰ) and sevoflurane-remifentanil group(Ⅱ).Anesthesia was induced with fentanyl,propofol and vecuronium.The patients were mechanically ventilated after tracheal intubation. Anesthesia were maintained with 1MAC sevoflurane and intermittent iv boluses of fentanyl in groupⅠand 1MAC sevoflurane and iv infusion remifentanil 0.1~0.2μg.kg-1.min-1 in group Ⅱ.All patients recieved in bolus of vecuronium when spontaneous respiratory was reversed.The times of spontaneous ventilation recovery per patient,the amount of vecuronium,the cases of TOF≥0.9 at immediately point of opertion ending and the duration of tracheal extubation were recorded.Results The 189 cases in groupⅠand 187 cases in group Ⅱwere taken into statistical analysis .There were a higher incidence of spontaneous ventilation recovery and more amount of vecuronium in groupⅠthan that in group Ⅱ(p<0.05); the cases of TOF≥0.9 at immediately point of opertion ending in groupⅠ(48 cases) were significantly less than that in group Ⅱ(163cases)(p<0.05); meanwhile,the duration of tracheal extubation was shorter in group Ⅱthan that in groupⅠ(p<0.05).Conclusion Sevoflurane-remifentanil anesthesia caused so strongly respiratory depression that less amount of vecuronium was required during perioperative.

    Objective To detect the anesthetic efficiency of etomidate emulsion,which was used in total intravenous anesthesia (TIVA) combined with remifentanil. Methods 50 patients in ASA Ⅰ~Ⅱ underwent elective general anesthesia for middle and lower abdominal surgery were selected, the operations time were all within 3 hours, these patients were randomly divided into two groups: group E(etomidate) and group P(propofol), they were induced with the same method, we transfused etomidate and remifentanil in group E and propofol and remifentanil in group P continuously to maintain anesthesia, maintain the bispectral index(BIS)between 40 and 60,which guided the transfused speed of etomidate and propofol.; The hemodynamic changes of the patients in the two groups were recorded; The transfused speed and the dosage inside unit time of etomidate and propofol were recorded and calculated; The consciousness recover time of patients were recorded. The happenings of postoperative nausea and vomiting, dysphoria were recorded. Results The hemodynamic changes were stable in the two groups; Maintained the BIS between 40 and 60,the intravenous speed of etomidate was( 7.74±1.11) ug.kg-1.min-1,the dosage was(1.61±0.39)bottle per hour; The intravenous speed of propofol was(5.62±0.83) ug.kg-1.min-1,the dosage was(1.92±0.47)bottle per hour; The consciousness recover time of patients in the two groups were close, there was no happening of postoperative nausea and vomiting、dysphoria during anesthesia resuscitation; Conclusion Combined with remifentanil in total intravenous anesthesia to maintain anesthesia within 3 hours, etomidate emulsion had higher anesthetic efficiency and cost performance compared with propofol.

    Abstract:Objective To evaluate the clinical features and Pathogenic Candida species in midtrimester-late pregnancy with vulvovaginal candidiasis. Methods A total of 294 pregnant out-patients in Tianjin Obstetrics & Gynecology Hospital were admissed between Jul 15th 2007 to Jan 30th 2008. Incidence surveys and routine laboratory examination were used. The Pathogenic Candida species were identified and the antifungal sensitivities in the Candida species were analyzed. Resulsts A total of 196 of 294 pregnancy women were involved in VVC with fungal culture, the morbidity is 66.67% (196/294). The mortidity with hanging drop method is 67.35%(132/196),the mortidity with smear method is 72.69%(143/196).No significant difference was found(p>0.05). Candida albicans was account for 95.41% (187/196),Non-albicans 4.59% (9/196).Candida albicans was observed in 131 cases by hanging drop method and in 137 cases by smear method, No significant difference was found(p>0.05). Non-candida albicans was observed in 1 cases by hanging drop method and in 6 cases by smear method.No significant difference was found(p=0.05). Drug-resistance was found in Candida albicans cases. 160 cases were resistant to tebinaifen. Non-albicans resistance against clotrimazole was 66.67% (6 / 9), and Candida albicans resistance to clotrimazole was 2.67% (5 / 187), the difference was significant ( p <0.01). Conclusion Candida albicans was still the most commonly identified species of pathogenic parasilosis yeasts. a higher detection rate of smear.different degrees resistance to commonly used antifungal drug were found in Candida albicans. resistance to clotrimazole in non-albicans was significantly higher than in Candida albicans.

    Role of Ultrasonography in the Intervention Therapy of Cesarean Scar Pregnancies

    Abstract Objective To evaluate the role of ultrasonography in the diagnosis and intervention therapy of cesarean scar pregnancy. Methods Collected records of 32 patients of our hospital with cesarean scar pregnancy diagnosed and managed with intervention therapy.The sample patients were enrolled from Jan.2006 to Dec. 2009.Analyzed Color Doppler sonographic features, ultrasonography after intervention therapy and follow up with pathologic finding. Results Among the 32 patients with cesarean section scar pregnancy , 6 patients were treated with intervention therapy solely, 16 patients with intervention therapy and curettage, 4 patients with intervention therapy and MTX ,4 patients with intervention therapy and MTX and curettage ,2 patients with intervention therapy and MTX and hysterectomy. In the course of the intervention therapy, ultrasound showed the focus size and the blood flow changes, combined with clinical blood HCG examination, provide the technical support for clinical treatment. Conclusions Color Doppler ultrasonography can provide relatively accurate localization and qualitative diagnosis for cesarean section scar pregnancy, Color Doppler Ultrasonography may assist with the assessment of the clinical treatment program course and outcome of patients based on imaging features.

    Objective:To observe the curative effects of injectable calcium sulfate bone graft combined with percutaneous reduction by leverage for treatment of intraarticular calcaneal fracture.Methods: 16 cases of fresh close caleaneal fractures were treated from April 2007 to April 2009.Of them,there were 11 males and 5 females,aged 23-56 years, averaged 34.5 years.According to Sanders classification,there were 8 cases of type II fractures,6 cases of type III fractures and 2 cases of type IV fractures,the course wag from 3 to 16 days,averaged 7.3 days.All the cases were treated by percutaneous reduction by leverage for the collapsing bone graft under the C-arm fluoroscopy,the bone grafts were fixed on the cuboid by round pin through calcaneal tuberosity. The injectable calcium sulfate was implanted into bone defects under subtalar joint assistancing with plaster slab fixation.The postoperative radiologic evaluation criteria is displaced intra-articular<2mm,and satisfactory shape on calcaneus. The function was evaluated according to the the ankle- hindfoot score of American Orthopedic Foot and Ankle Society Score(AOFAS).Results:The injectable calcium sulfate implanted into bone defects were absorbed in all patient,no one case happened rejection and pinhole infection,the calcaneal fracture in all patient was healed. X-ray films of patients showed well reduction on the caleaneal posterior subtalar articular facet and satisfactory shape on calcaneus.According to the score of AOFAS, the effect was excellent in 9 cases, good in 6 cases,and fair in 1 case,with 93.8% rate of good and excellent.     Coronary Artery Bypass Surgery in Octogenarians

    Abstract Objective: To investigate the peri-operative treatments of coronary artery bypass grafting in octogenarians. Methods: From January 2008 to March 2011, 37 patients(30 male,7 female) 80 years of age or older(80-91years,mean 83.11±2.90 years) underwent coronary artery bypass grafting. 31 patients underwent off pump coronary artery bypass grafting (83.8%) and 1 patients underwent conventional on pump coronary artery bypass grafting (2.7%) and 3 patients underwent simultaneous mitral valve replacement and coronary artery bypass grafting (8.1%) and 2 patients underwent simultaneous aortic valve replacement and coronary artery bypass grafting (5.4%). Results: One patients died post-operatively. The mortality was 2.7%. Major postoperative complications included atrail fibrillation in 18 cases, respiratory dysfunction in 8 cases( 1 patients underwent trachyotomy), low cardiac output in 6 cases,acute kidney injury in 2 cases(2 patients underwent continuous renal replacement treatment) and stroke in one case. Hemorrhage or tamponade requiring reoperation in 1 cases. Intra-aortic balloon pump was used in 7 patients (18.9%). The average ICU stay was 3.35±1.99 days. Mean postoperative in-hospital stay was 13.03±4.39 days. Thirty-seven patients were followed up from 3-39 months. One patient died during the follow-up period. Conclusions: Coronary artery bypass grafting in octogenarians is safe. Intensive perioperative treatment should be stressed in octogenarians to reduce the morbidity and mortality.

    【Abstract】 Objective : To analyze the features of hepatic biliary cystadenoma and cystadenocarcinoma on MRI, and to discuss the diagnostic value of MRI in the diseases. Methods:The MRI features of 2 cases of cystadenoma and 6 cases of cystadenocarcinoma proved by pathology were analyzed retrospectively and correlated with the pathologic findings. Results:All of 8 cases were revealed as solitary cystic lesions. Variant contents in the intracystic fluid showed different signal intensity on MRI. Hemorrhagic fluid was seen in one cystadenoma and one cystadenocarcinoma respectively. The 2 cases of cystadenoma were well-defined, with smooth cystic wall and internal septa, and showed no enhancement or slightly enhancement after contrast. Irregular thickening in cystic wall and internal septa were found in these cystadenocarcinomas, and mural nodules were found in the 4 cases. After contrast,the solid parts were obviously enhanced, particularly in port vein phase and delay phase, but for the 2 cases which were enhanced particularly in arterial phase. Ductal dilatation near the lesions was seen in 2 cases, furthermore communication between the lesion and the bile duct was seen in one of them. The margins of the cystadenocarcinoms were mostly smooth, except for that of one recured after resection. Conclusions: MRI can reveal the characteristic morphological features of hepatic biliary cystadenoma and cystadenocarcinoma, and play an important role in the diagnosis before the operation. Solid parts enhanced can be seen much more frequently in cystadenocarcinoma than in cystadenoma. Hemorrhagic internal fluid and involved bile ducts are also more common in cystadenocarcinoma. The cystadenocarcinomas with ill-defined margins may have unfavourable prognosis.

    Objective:To explore the clinical application of asymmetric z-plasty in repair of congenital unilateral incomplete cleft lip. Methods:By individual design,65 patients with incomplete unilateral cleft lip were renewed lip height, repaired orbicular muscle of mouth and repaired nasal deformity. Results:The method was used in 65 patients, all cases were treated with primary healing. Followed up 6 months to 18 months,the ideal operative effects were obtained in all cases. Conclusion: the individual reconstruction of incomplete unilateral cleft lip is rational and simple,and is an ideal method for unilateral cleft lip repair.

    Acute pericardial tamponade during percutaneous coronary intervention was usually caused by coronary artery perforation which requires cardiologist to identify and treat as soon as possible. Recently, many cases of pericardial tamponade caused by cardiac rupture were reported, which often occurred unexpectedly. A case of cardiac rupture after successful PCI with myocardial reinfarction is herein reported in order to remind physician this possibility and rescure the patient promptly.

    【Abstract 】 Objective To reported a case of Ductal Adenocarcinoma of the Prostate g and the relevant literature was reviewed to investigate the diagnosis and treatment of this disease.evaluate the clinical and pathological characteristics treatment ,prognosis of ductal adenocarcinoma of the Prostate.Methods A 82-year-old male suffered from "frequency, urgency, 7 years, increased for 3 months" misdiagnosis of patients. His serum prostatic specific antigen(PSA)level was 2.38 ng?ml-1.transurethral prostatic resection,(TURP) was performed on him.Results The postoperative pathology showed a primary Ductal Adenocarcinoma.Gleason score was 8.With immunohistochemical staining,the tumor cells were positive for PSA ,PAP (prostatic acid phosphatase) ,CKAE and CKHW,but negative for androgen receptor.Postoperatively,the patient recovered fairishly but long-term effects need further follow-up.Conclusion Ductal Adenocarcinoma of the Prostate is a clinically rare subtype of prostate cancer and difficult to be found in early stage.The diagnosis depends on histopathologic and immunohistochemical studies.Radical prostatectomy is recommended for this disease.

    Intralabyrenth Acoustic Neuroma

    Objective The present study reports a patient with a rare acoustic neuroma that aroses from the cochlea. Methods In order to diagnose and treat, a detailed analysis is performed according to the case history, physical examination and radiographic findings {CT (computerized tomography) and MRI (magnetic resonance imaging) scanning}. And literature review. Results Some specialists discuss this case. Consequently, intralabyrinth acoustic neuroma is identified so that other possibilities are excluded. Surgical treatment is regarded as the optimal choice. Because the patient doesn’t agree with it, gamma knife (γ-knife) is used at last. Conclusions Intralabyrinth acoustic neuroma has no typical clinical symptoms, and gadolinium enhanced MRI examination is its best diagnostic tool. Early tumors confine to the labyrinth and no severe dizziness symptoms can be observed. If the patient has severe vertigo, or tumors spread out of the labyrinth, surgical resection of the tumors or the use of r-knife treatment should be applied.
