Abstract Objective: To evaluate the clinic application of anterograde ileum tubal fistulation for obstructed left–sided colorectal cancer by a one-stage resection and an anastomosis. Methods: Studied Clinical data of 20 patients with obstructed left-sided colorectal cancer who underwent ileum tubal fistulation after A one-stage resection and an anastomosis. After the decompressive procedure, A catheter was inserted to terminal segmen of the ileum through the stump, on-table lavage, resection, and primary anastomosis be performed, then the catheter was fixed on abdominal wall. Results: There were no complication such as anastomotic leakage, postoperational stool leakage and peritonitis. The stool diversion was efficient. No deaths occurred. Conclusion: The Application of anterograde Tubal Typical ileum fistulation for obstructed left-sided colorectal cancer by a one- stage resection and an anastomosis was safety and efficient.

Catheter Interventional Treatment of systemic-to-pulmonary arterial shunt obstruction
    2010, 38 (9):  0-793 .
    Abstract ( 1185 )   PDF (387KB) ( 1913 )
【Abstract】 Objective: Creation of a systemic-to-pulmonary shunt is still the firstline treatment in neonates with ductdependent pulmonary circulation, or in patients with hypoplastic left heart syndrome as a part of the first stage of the Norwood sequence. Acute complications after such surgery, such as stenosis, thrombosis, or kinking, are potentially lifethreatening. These complications require immediate revision or exchange of the shunt. In this report, we discuss interventional treatment as an alternative to surgery in 3 patients with acute stenosis of a shunt. Methods:The age of the patients ranged from 4 to 6 months, and their weights ranged between 4.2 and 6.2 kg. In 1 patients, the shunts were modified Blalock-Taussig type, while the 2 patients had shunts from the right ventricle to the pulmonary arteries. Results:We implanted 5 coronary arterial stent systems in 4 interventions. The diameter of the balloon used for implantation had a ratio to the shunt of 1. to 1. All shunts were successfully reopened by stenting. During follow-up, 3 patients underwent further procedures on an elective basis. Conclusion: In our limited experience, implantation of stents is an effective treatment for complications of shunts.

    Abstract ( 1091 )   PDF (1001KB) ( 1896 )
Abstract 0bjective: To investigate the value of serum AFP-L3 variants assay for differential diagnosis of liver disease. Methods : serum AFP-L3 variants were isolated by micro centrifugal Column, and the contents of AFP and AFP-L3 were detected by Elcteochemo luminescence immuno assay(ECLIA). Result:When AFP-L3/AFP>10% was used as a positive criterion ,in the cases of AFP>400ng/ml and AFP<400ng/ml, the positive rate of AFP-L3 were 79.0% And 75.9%, respectively in hepatocellular carcinoma(HCC), which in liver cirrhosis(LC)、chronic hepatitis(CH) and autoimmune hepatitis(AIH) were 8.7%、7.9% and 7.3% ,respectively. Conclusion :AFP-L3 is very useful in the differential diagnosis of HCC and low AFP level liver disease.

The Initial Study of Relationship between ATGL and Type 2 diabetes mellitus With Non-alcoholic Fatty Liver
    2010, 38 (9):  0-793 .
    Abstract ( 1604 )   PDF (1103KB) ( 1949 )
Objective: To explore the relationship between plasma ATGL and related metabolic indexes of Type 2 diabetes mellitus with fatty liver. Methods: Fasting plasma ATGL level of one hundred and thirty-one patients with incipient T2DM,T2DM group (seventy four patients without fatty liver, A group; fifty seven patients with fatty liver, B group) and forty four healthy subjects (NC group)were assayed by enzyme-linked immunosorbent asssy(ELISA),A comparison about basal materials and ATGL was made between A group 、B group and NC group, correlation of ATGL and basal materials were measured as well, then multiple stepwise regression analysis were made. Results: Fasting plasma ATGL level in T2DM without fatty liver was significantly lower than controls(P<0.01),39.13?23.98ng/ml vs 69.21?27.05 ng/ml. Fasting plasma ATGL level in T2DM with fatty liver was significantly lower than controls(P<0.01),18.25?14.85ng/ml vs 69.21?27.05 ng/ml. Fasting plasma ATGL level in T2DM with fatty liver was significantly lower than without fatty liver(P<0.01),18.25?14.85 ng/ml vs 39.13?23.98 ng/ml. Correlation analysis showed that the fasting ATGL level was negatively in correlation with TC、TG、、Fins 、BMI、HOMA-IR(r=-0. 222 、-0.424、-0.317、-0.200、-0.178,P<0.05). Multiple stepwise regression analysis showed that Fins、BMI、TC were independent related factors that influenced the level of plasma ATGL. Conclusion: ATGL may contribute to the pathogenesis of T2DM with non-alcoholic fatty liver.
