    The Effects of Hedan tablet on Oxidant Stress in Atherosclerosis Rats

    Objective To evaluate the effect of Hedan tablet on oxidant stress in atherosclerosis rats. Methods Forty-eight healthy male rats were randomly divided into the normal control group、the model group、the three Hedan tablet treated groups(3g.kg-1.d-1 、1.5g.kg-1.d-1、0.5g.kg-1.d-1)and the simvastatin treated group.Hyperlipidie feed was adopted to build the athersclerosis animal model.Rats were sacrificed 12 weeks later,their histopathological changes in abdominal aorta were observed by electronic microscope;serum SOD and NO were determined by by colorimetry, and serum MDA level was determined by Thiobarbituric acid method. Results In comparing with the model group, Hedan tablet reduced the serum level of MDA as well as increased the serum level of SOD、NO(P<0.05)。Hedan tablet treatment(3g.kg-1.d-1) showed the best effect of inhibiting the atherosclerosis formation in the three Hedan tablet treated groups. Hedan tablet showed actions in lessening the atherosclerosis lesion. Conclusion Hedan tablet treatment(3g.kg-1.d-1) can inhibit the atherosclerosis formation in experimental rats .Its mechanism might be related with its anti-oxidation effect.

    Prognostic Effect of Different Methods of Radiotherapy to early stage Nasal NK/T Cell Lymphoma

    Objective: To investigate the prognostic effect of different scopes and doses of radiotherapy and preventive neck eradiation to nasal NK/T cell lymphoma at early stage. Methods: Analyze the clinical feature of 68 patients with stage I and Ⅱ nasal NK/T cell lymphoma who received radiotherapy in our department from January 1985 to January 2005 in retrospect. According to the different scopes of radiotherapy, the patients can be divided into two groups, one group is involved field with 27 patients, another group is extended field with 41 patients. According to the different radiotherapy doses , the patients can be divided into 4 groups as less 45Gy group with 11 patients,45~50Gy group with 32 patients , 50~60Gy group with 17 patients and more than 60Gy group with 8 patients. 38 patients have received preventive neck eradiation and other 30 haven’t. Kaplan-Meier method was used to calculate the local control rate and 5-year survival rate, Log-rank test was used to inspect the significant difference. Results: The total local control rates and 5-year survival rates are 79.4% and 52.9%. The local control rates of group of involved field radiotherapy and group of extended field radiotherapy are 66.7% and 87.8%, the 5-year survival rates of these two groups are 29.6% and 68.3% (P<0.05). The local control rates of radiotherapy dose less 45Gy group, 45~50Gy group, 50~60Gy group and more than 60Gy group are 45.5%、84.4%、87.5%、88.9%(P<0.05), the 5-year survival rates of these groups are 36.4%,56.3%,56.3%,55.6%(P<0.05). Analysis on the local control rate and 5-year survival rate of the radiotherapy dose less 45Gy group have significance in statistics and the comparison among other three groups has no significance in statistics(P>0.05). The neck lymph node metastasis rate of the patients group who have received preventive neck eradiation and the other patients group who haven’t are 7.9% and 36.7%(P<0.05), 5-year survival rates of these two groups are 55.3% and 50.0% (P>0.05). Conclusion: Extended field radiotherapy at early stage has good effect for the local control rate and long term survival rate of the patients with nasal NK/T cell lymphoma. The dose of radiotherapy should be more than 40Gy. Preventive neck eradiation achieved lower neck lymph node metastasis rate but has no effect on the survival.

    Influence of Anti-CD44 mAb A3D8 on Proliferation and Apoptosis of three Sphere-forming Cells from Ovarian Cancer

    Abstract Objective: To explore the effects and mechanisms of A3D8(mAb) against CD44 on proliferation and apoptosis in three kinds of sphere-forming cells of ovarian cancer. Methods: The MTS assay was used to evaluate the cell proliferation. The PI staining and flow cytometry was used to analyze the cell cycle distribution. Annexin V-FITC/PI apoptosis detection kit was used to analyze cell apoptosis. Rhodamine123 apoptosis detection kit was used to detect the change of mitochondrial transmembrane potential (ΔψM). Western blotting assay was used to detect the CDK2,cyclinA,Bcl-2 and caspase-3 protein levels. Results: A3D8 inhibited the proliferation in three sphere-forming cells of ovarian cancer, the inhibition was in concentration-and time-dependant manner. A3D8 could cause S arrest with G0/G1 decrease via p21/CDK2/cyclinA pathways. A3D8 can increase the apoptosis rates of three kinds of cells, compared with DDP treatment alone, the apoptosis rates caused by A3D8 combined with DDP was increased significantly.Meanwhile A3D8 can lead ΔψM to decrease, CDK2,cyclinA,Bcl-2 protein level decrease and caspase-3 increase in above three kinds of sphere-forming cells. Conclusion: A3D8 inhibit the proliferation in three sphere-forming cells of ovarian cancer probably by the cell cycle arrest via p21/CDK2/cyclinA pathways and increase the apoptosis rates via mitochondrial pathways.

    The Expressions and Clinical Significance of LKB1 and VEGFR2 in Non-Small Cell Lung Cancer

    Objective: To investigate the protein expressions of LKB1 and VEGFR2 in Non-small Cell Lung Cancer (NSCLC). Methods: The samples of tumor and normal lung tissues were from 78 patients with NSCLC. Immunohistochemical stainings for LKB1 and VEGFR2 were performed and the expression levels of both proteins were analyzed. Results: LKB1 expression as well as VEGFR2 expression was found both in tumor and normal lung tissues. Statistics data showed that significant differences of the positive expressions of LKB1 protein in different organizations, different degree of differentiation and composition of lymph node metastasis(P <0.05). There was also statistically significant in VEGFR2 in different tissue origin, differentiation, and lymph node metastasis(P <0.05). Conclusion: An opposite tendency existed in the expression of LKB1 and VEGFR2 in NSCLC. Moreover, low expression of LKB1 and high expression of VEGFR2 protein were related to the cells differentiation and lymph node metastasis in NSCLC, which may affect the prognosis of lung cancer.

    Clinical significance of ANXA5 expression in human uterine cervical carcinoma of different pathological differentiation

    [ABSTRACT] Objective: To study the possibility of ANXA5 as a differentiation type marker of uterine cervical carcinoma by examining expression of ANXA5 in the three kinds of pathology differentiation. Methods: All the specimens are classified as well-differentiated,intermediate-differentiated and poor-differentiated groups according to HE stainning. RT-PCR method was used to determine the ANXA5 mRNA expression; Western Blot method and immunohistochemistry were employed to determine the ANXA5 protein expression. The results between any two groups were analysed by t test. Results: ANXA5 expression in poor-differentiated and intermediate-differentiated groups increased drastically while the expression in well-differentiated group only lightly increased, which has significant difference with the other two groups(P<0.05). Conclusion: ANXA5 maybe a potential marker for the differentiation types of uterine cervical carcinoma.

    Study On The UVB-Induced MMP-9And TIMP-1 mRNA expression on HaCaT cells

    [Abstract] Objective:To investigate the effect of UVB irradiation induced the matrix metalloproteinase (MMP-9) and Tissue inhibitor of Matrix Metalloproteinase (TIMP-1) mRNA expression on immortalized human keratinocyte cells (HaCaT cells).Methods:Culture the HaCaT cells ,Draw cell growth curve, and the cells was treated with different doses (0、30、60、90 mJ/cm2)of UVB irradiation ,MTT method for determining cell proliferation.Semiquantitative-PCR (RT-PCR) was used to measure the expression levels of MMP-9 mRNA and TIMP-1 mRNA in HaCaT cells.Results:With the UVB irradiation accumulated, There was a decrease in the proliferation of HaCaT cells and an increase in the inhabation ratio, and there were significant differences in the values of OD (P < 0.05).The expression levels of MMP-9 mRNA increased,Comparative differences between groups were statistically significant(P<0.05).The expression levels of TIMP-1 decreased,There were significant differences in compare with0 mJ/cm2(P<0.05). Conclusion:UVB irradiation can induce the injure and apoptosis of HaCaT cells , MMP-9 and TIMP-1 maybe have some role in the pathogenesis of skin photoaging.

    A Study on Relationship of Metabolic Syndrome with Diabetic Peripheral Neuropathy in Type 2 Diabetes Patients

    Objective: To research the relationship between peripheral neuropathy and Metabolic Syndrome (MS) in 2 type diabetes. Methods: 302 type 2 diabetes patients Were examined by Michigan diabetic neuropathy score and nerve electrophysiological test. Patients were divided into 2 groups according to their blood pressure, glucose levels, blood lipids and waistlines. Results: In the metabolic syndrome group, the prevalence of peripheral neuropathy (57.5%)and Michigan diabetic neuropathy symptom score(4.12±1.89) and sign score(2.15±1.60) were higher than those in non-MS group(44.3%,1.69±1.54,1.56±1.01)(all P<0.05). Peroneal sensory nerve conduction velocity (43.7±4.6m/s in left, 40.0±4.9m/s in right) in MS group were lower than those in non-MS group(46.1±4.0m/s in left, 45.9±4.5m/s in right).The left median sensory nerve conduction velocity(51.7±4.2m/s) in MS group was lower than that of non-MS group(55.5±4.3m/s) (all P<0.05).Multiple regression analysis show that MS was independently related to peripheral neuropathy after adjustment for sex, age, disease duration, blood pressure and blood glucose. OR value was 1.195, P<0.05. Conclusion: Metablic syndrome may be related to the progress of diabetic peripheral neuropathy in type 2 diabetes.

    The expression and clinical significance of PPARγ、p27kip1 and Skp2 in hepatocellular carcinoma

    Objective The aim of this expriment is to investigate the expression and significance of PPARγ, p27kip1 and Skp2 in hepatocellular carcinoma. Method Immunohistochemistry was performed for PPAR-γ, p27Kip1, and Skp2 in 57 cases HCC patients who had undergone operations at the Fourth Hospital of Hebei Medical University, between 2004 and 2007. Results A significant inverse correlation was found between p27Kip1 andSkp2 expression in HCC(r=-0.307,P=0.020).There was no significantcorrelation between PPAR-γ and P27kip1、Skp2 expression in HCC. Conclusion The expression of Skp2 was negatively correlated with P27kip1 in HCC,High Skp2 expression was significantly associated with poor prognosis. Skp2 might serve as the key prognostic factors in HCC.

    Causes Analysis of Abortion during IVF-ET Treatment

    Abstract Objective: to investigate the causes of abortion during IVF-ET in order to increasing the pregnancy rate and preventing the occurance of abortion. Methods: 203 patients including 150 patients who had early stage abortion (early abortion group) and 53 patients who had late stage abortion (late abortion group ) were enrolled. The age, BMI, COH and clinical pregnancy results between the two groups were analysed in order to find the causes of spontaneous abortion during IVF-ET treatment. Results: The clinical pregnancy rate in our center is 38.3% while the 16.9% including early abortion rate 12.6 % and late abortion rate 4.4%,.The β-hCG level of 14 days after ET is quite low in the early abortion group while the BMI, Multiple pregnancy and fetal reduction method in late abortion group is high. Conclusion: early abortion is correlated with embryo quality and more than 60% is due to the chromosome abnormalities. Low level of hCG after 14 days of ET will suggest the high risk of early abortion. Additionally increasing BMI and large dose of Gn during COH can infect the receptivity of the endometrium and the embryo implantation. Multiple pregnancy and fetal reduction method can cause prematue membrane rupture and is one of the reason causing late stage abortion. More over fetal malformation is another reason for pregnancy induction.

    Study on quality of life for perimenopausal women after different operations of uterus and ovary

    Abstract:Objective:To investigate the influence on quality of life for perimenopausal women whether reservation of ovary on the hysterectomy.Methods:Perimenopausal women with uterine or ovarian surgery for benign disease are divided into 3 groups.group A(40 cases): hysterectomy plus bilateral adnexectomy;group B(20 cases): hysterectomy plus ani-lateral adnexectomy;group C(40 cases): simple hysterectomy..The levels of FSH、LH、E2 were measured before operation and 2,6months after operation,and the symptoms of peri-menopausal period was observed in 2,6 months after the operation.Results:Blood hormones of group A were significantly differential compared with the group B(P<0.05),and more symptoms of prei-menopausal period.Blood hormones of group A were more significantly differential compared with the group C(P<0.05), and most symptoms of prei-menopausal period.Two months after operation decline in hormone levels of group B were significantly differential compared with the group C,but no significantly differential in 6 months after operation .No differences of symptoms of peri-menopausal period were observed between groups(P>0.05). Conclusion:Ovary removal in hysterectomy will appear more menopausal syndrome, so ovaries should be preserved and slow decline in hormone levels to delay the emergerce of menopausal syndrome and improve the quality of life of women.

    Abstract Objective:To explore the achievement rates of porcelain laminate restoration based on different preparation designs. Methods:29 patients and 97 porcelain laminate veneers were prepared by facies labialis overlap (typeI), incisive edge overlap (type L) and finishing lines extended proximally (type U). And veneers' integrity, fracture, the marginal situation and color match were evaluated at 1week, 6months, 12 months and 18months.Results: There was 1 failure at 12 months preparaded by typeI, and 1failure at 12months and 1 failure at 18 months prepared by typeL, and 1failure at 6months and 1 failure at 18 months prepared by typeU. There ware no significent difference about achievement rates between 3 type preparations. Conclusion: Excellent effects porcelain laminate restorations were obtained by choiced reasoned tooth preparation.

    The role of the immune imbalance in patients with severe acute secondary systemic infection

    Objective: To study the role of immune imbalance in patients with severe acute pancreatitis secondary to systemic infection. Methods: 55 cases of patients with severe acute pancreatitis was divided into secondary to systemic infection group (A group) and suspected systemic infection with or without systemic infection group (B group).Assayed the level of human leukocyte antigen DR (HLA-DR), T helper lymphocyte 1 / 2 (Th1 / Th2) ratio, the regulation of T cell (Treg) level in peripheral blood on the 1st, 3rd, 7th, 10th day. The mortality rate at 28 days and the length of stay in surgical intensive care unit(SICU)were recorded. Results: In 55 patients, length of SICU stay were(9.82± 4.32) days, length of hospital stay were (24.72±10.35)days, the mortality rate at 28 days was 9.09% (5 / 55). The mortality of Group A was higher than the B group .At each time point, the value of HLA-DR i n A group were significantly lower than B group. Two groups of the Th1 / Th2 value increased firstly, after reducing, at each time point in A group were significantly lower than B group. Two groups of Treg decreased, increased after, the A group were significantly higher than that of B group. All patients at each time point Th1 / Th2 value was closely related to Treg. Two groups of SICU stay stay, mortality significantly associated with HLA-DR%. The number of involved organ was closely related to HLA-DR and the mortality rate ,wich was significantly increased in group A, decreased in group B. Conclusion: The group of patients with systemic infection occurs early antigen presenting function hindrance, immune imbalance, increased body internally, exogenous susceptibility to infection, the body can not effectively remove pathogens ,appearing systemic infection, prolonged SICU stay days, increased the mortality rate.

    Relationship of lipids and prognosis in elder patients of chronic heart failure with coronary artery disease

    Abstract Objective:To study the association between the lipid levels and prognosis in elder patients with chronic heart failure(CHF) who had coronary artery disease(CAD). Methods:970 patients enrolled from the cardiology department of 2nd Hospital Tianjin Medical University who had clinical diagnoses of CHF with CAD from January 2005 to December 2007. Total cholesterol (TC), Triglycerides(TG), Low-density lipoprotein cholesterol (LDL-C) and High-density lipoprotein cholesterol (HDL-C) were divided into quartile. Patients were grouped with death (206 cases) and living (764 cases). According to statin-using,compared the mortality in various lipid levels, statin groupe (531 cases) and non statin groupe (439 cases). Cox regression analysis was used to confirm the associations of lipid levels and prognosis of the patients. Results:The different of mortality in each level of HDL-C was significant (P<0.05), but there was no significant association between the levels of the TC, TG, LDL-C and prognosis (P>0.05). Compared with living groupe, death patients were older, had more proportion of old myocardial infarction, anemia, atrial fibrillation and renal failure, while had lower HDL-C level and ejection factor, less using of angiotensin converting enzyme inhibitor(ACEI)/Angiotension receptor blocker(ARB) and statin (P<0.05 or P<0.01). Multivariate Cox regress analysis showed age, NYHAⅢ~Ⅳ, the lowest and the lower level HDL-C had significantly greater survival. ACEI/ARB was protected factor for prognosis. But none of the quartiles of TC or TG or LDL-C could predict for the mortality. Conclusion:Our study shows HDL-C represented a novel prognostic factor for patients with advanced heart failure. TC, TG and LDL-C were not related to the prognosis of heart failure.

    High risk factors of coronary artery lesions caused by Kawasaki disease

    0bjective: To explore the high risk factors of coronary artery lesions(CALs)caused by Kawasaki disease. Methods :Clinical data of 1070 patients with KD admitted in our department from January 1997 to September 2011 were collected and retrospectively reviewed.Risk factors associated with coronary artery lesions including gender,age,duration of fever,use of intravenous gamma immune globulin (IVIG),white blood cell count(WBC),platelet count(PLT),hematocrit(Hct), C-reactive protein (CRP),erythrocyte sedimentation rate(ESR),and serum albumin(Alb) were analysed.Chi-square test and Logistic regression were performed. Results: Chi-square test showed that gender,age,duration of fever,timing of IVIG treatment,WBC,CRP,and PLT were significantly related to CALs(P <0.05).Multiple Logistic regression analysis identified CRP ,PLT,age ,sex as risk factors of CALs (P < 0.05). Conclusions :Less than 1 year old age ,male,and high level of CRP and PLT are the high risk factors of CALs caused by Kawasaki disease.

    Nitric oxide in persistent pulmomnar hypertension of newborn

    【Abstract】 Objective TO study the therapeutic effects of inhaled intric oxide(iNO) and high-frequencey oscillatory ventilation in neonatal perisistent pulmonary hypertension (PPHN).Methods 30 neonates with pulmonary angiospasm and 6 patients with anatomic vascular obstruction were thrapied.During high-frequencey oscillatory ventilation,NO was added into ventilator circle. Oxygenation indes(OI),blood gas analysis,blood pressure,pulmonary arterial systolic pressure(SPAP), heart rate,mean airway pressure(MAP),saturation of blood oxygen per cutem(TcSaO2),methaemoglobin (MetHb),and nitrogen dioxide (NO2)30 minutes and 24 hours after NO therapy were monitored. Results In group Ⅰtwenty-six (86.67%)neonates survived,the oxygenation were improved.There were no significant adverse reaction.In group Ⅱ,there were no remarkable changes. Conclusion Inhaling minimum effective dosage NO in PPHN with pulmonary angiospam was beneficial as soon as possible. Premature infants(≥34W)who was treated with high-frequencey oscillatory ventilation and NO were effective.

    Articular Cartilage Injury in Rabbits with Knee Osteoarthritis and the Effects of Calcitonin

    Objective: To study the articular cartilage injury in rabbits with knee osteoarthritis and the effects of calcitonin. Methods: 36 rabbits were randomly divided into three groups: normal group (group A), model group (group B) and CT treatment group (group C).There were 12 rabbits in each group. The rabbits in group B and C were made into knee osteoarthritis by Hulth’s method. The rabbits in group A weren’t dealed by special process, except for incision of skin and articular cavity. The rabbits in group C were interfered by hypodermic injection CT everyday (5∪/kg). The other rabbits were interfered by normal saline. The pathomorphism and changes of TNF-α, NO,collagen Ⅱ and MMP-1 were observed. Results: The scores of JP and Mankin in group B were significantly higher than those in group A, articular cartilage injury was more serious, meanwhile, the content of TNF-α、NO in synovial fluid increased (P<0.05 ), the expression of MMP-1 increased, but the expression of collagen Ⅱ decreased (P<0.05 ). Compared with group B, the scores of JP and Mankin in group C were significantly decreased, the articular cartilage injury relieved evidently, expression of MMP-1 and the content of TNF-α, NO in synovial fluid decreased significantly (P< 0.05 ), but the expression of collagen Ⅱ increased significantly (P<0.05 ). Conclusion The protection of articular cartilage injury by CT on KOA may relevant to the inhibition of TNF-α and NO.

    The effect of H2O2 pretreatment on mouse myocardial hypoxia/reoxygenation injury

    Abstract Objective To investigate the H2O2 pretreatment on mouse myocardial hypoxia/reoxygenation caused apoptosis of H9C2 cells. Methods Mice cardiac cells (H9C2) cultured in vitro amplification, using the logarithmic growth phase cells to do the experimental treatment. Use respectively 0, 5, 10, 20, 50, 100 μ mol/L concentration H2O2 treatment H9C2 cells, flow cytometry ( FCM ), MTT and immunoblotting method ( Western blot ) detection of H2O2 with different concentration on H9C2 cell apoptosis rate, Proliferation activity and the STAT3 phosphorylation level effects, in order to determining the optimal concentration of H2O2 pretreatment. Watch the effects of low concentrations of H2O2 pretreatment on hypoxia/reoxygenation caused cell injury. Cells were divided into four groups namely control group, hypoxia/reoxygenation group, H2O2 pretreatment group, AG490 group.① Control group: conventional cultivation; ② hypoxia/reoxygenation group: pre in 5% CO2, 95%N2 incubator 120min, reoxygenation 30min; ③H2O2 pretreatment groups: pre-treatment given to determine the concentration of H2O2 90min for liquid 24h after hypoxia/ reoxygenation; ④AG490 group: give 10min before the H2O2 pretreatment 10μmol/L AG490 treatment, after treatment with H2O2 pretreatment group.AnnexinV-FITC / PI double staining method and flow cytometry (FCM) detection of apoptosis, MTT assay H9C2 cell proliferation activity, immunoblot (Western blot method) to detect STAT3 phosphorylation level. Results 20μmol/L group of STAT3 phosphorylation significantly higher than all other level of concentration group (P < 0.01), myocardial cell apoptosis rates were also significantly lower than 50 μ mol/L and 100 μ mol/L concentration group (P < 0.01), and the myocardial cell proliferation activity and cell apoptosis rate and 5 μ mol/L and 10 μ mol/L concentration group there were no obvious difference ; H2O2 pretreatment can reduce the small rat myocardial cells apoptosis (P <0.05), increased myocardial cell viability (P<0.05) increased P-STAT3 expression (P<0.05). Oxygen injury to AG490 after treatment can make the H2O2 pretreatment protection disappear.Conclusions 20μmol/L H2O2 pretreatment on myocardial hypoxia/reoxygenation injury adaptive protection. The possible mechanism is activated JAK2-STAT3 pathways play to the role of inhibiting cell apoptosis.

    Abstract: Objective To study the inhibitory effect on allergic asthma by adoptive transfer of    CD8α-CD11c+ dendritic cells (DC) from Schistosoma japonicum-infected mice. Methods CD8α-CD11c+DC from S.japonicum-infected mice and naive were isolated and purified using microbeads respectively,. Twenty BALB/c mice were divided into four groups randomly.5×105 CD8α-CD11c+DCs from S.japonicum-infected mice were injected into the tail vein of each mouse in Group A,while 5×105 CD8α-CD11c+ DCs of na?ve were injected into the tail vein of each mouse in Group B. One hour after injection,mice in Group A、B and C were sensitized and challenged with ovalbumin to induce asthma.Four weeks later,all the mice were killed,and the left lungs were cut for histopathologica1 analysis.Results Compared with mice in Group B and C,pulmonary inflammation of mice in Group A diminished markedly.According to Underwood’s standard,average total scores of Group A,B,C and D were 7.67±2.34,l0.86±1.35,11.75±2.22and 4.5±1.73 respectively ( P< 0.05). Conclusion CD8α-CD11c+ DC from S.japonicum infected mice can significantly inhibit allergic asthma.

    The Study on Nimodipine on the Effect of Blood-brain Barrier of Mouse with Diffuse Axonal Injury

    [Abstract] Objective:To observe the therapeutic effect of nimodipine in treatment of Diffuse axonal injury (DAI) . Methods:100 mouse with DAI were divided randomly into two groups. In treatment group nimodipine was injected intravenously for 7 days. In control group mouse receive the same treatment measures but no nimodipine was given. The brain water content and the EB content at every time point ( before DAI and 6h, 1d, 3d, 7d after DAI) were closely observed. Results:The climax of the brain water content came to at 1d after DAI, and one of the EB content came to at 3d after DAI. The proportion of the brain water content and the EB content at 6h, 1d, 3d, 7d after DAI in treatment group were significantly lower than the control group(P<0.05). Conclusion:Nimodipine decreases the permeability of BBB and has definitive effect for brain edema after DAI.

    Abstract Objective: To study the Correlations with CRP, IL-6 and the severity of stenosis of coronary artery. Methods: 120 patients undergoing quantitative coronary angiography were involved in this study, and we measured the titer of CRP and IL-6. The severity scale of coronary artery stenosis was quantitatively assessed according to coronary angiography by Gensini score. The patients were categorized into control group and CAD group The CAD patients were classified to1, 2 and 3-vessel-lesion group according to the results of Angiography. Results: The CAD group had higher titers of CRP and IL-6 than the control group. In muiltvariate analysis, CRP and IL-6 were demonstrated to be independent risk factors. Neither CRP nor IL-6 were along with the number of lesion-vessels. In the correlation analyses, Gensini were significantly associated with IL-6, but neither CRP nor IL-6 were independent risk factors of Gensini score. Conclusion: CRP and IL-6 have correlation with CAD and are demonstrated to be significant predictors of CAD. But, nither CRP nor IL-6 are correlated with severity of CAD assessed by coronary angiography.

    Trans-intercostal mini-thoracotomy cardiac surgery

    Objective To summarize the experience of intercostal mini-thoracotomy cardiac surgery. Methods From 2010.5 to 2011.8, 28 patients underwent right trans-intercostal mini-thoracotomy cardiac operation were collected retrospectively. The surgery included 13 cases of mitral valve replacement, 2 cases of mitral valve repairment, 2 cases of mitral valve replacement with tricuspid valve repairment, 1 case of mitral valve replacement and CABG, 3 cases of ASD repairment, 3 cases of ASD repairment with tricuspid valve repairment, 4 cases of left atrial myxoma resection, 2 cases of aortic valve replacement. Results All the patients were survived. Subcutaneous emphysema occurred in 2 patients. One patient got re-thoractomy. Cardiopulmonary bypass time was 55.6±14.3 min, aortic cross clamping time was 31.4±12.5 min, duration of postoperative mechanical ventilation was 7.2±1.8 hours, and postoperative hospital stay was 8.2±2.4 days (range, 5 days-12 days). Conclusions Trans-intercostal mini-thoracotomy cardiac surgery could keep integrity of the sternum. Compared with the traditional midsternalthoractomy, the mini-thoracotomy cardiac surgery could offer quick recovery and excellent cosmetic effect to some patients.

    Abstract Objective: To assess the value of serum lectin-like oxidized low-density lipoprotein receptor-1(LOX-1) in early prediction of acute coronary syndrome(ACS). Methods: Enzyme-linked immunosorbent assay was used to detect the level of serum LOX-1 in 75 patients who underwent coronary angiography, 22 normal control subjects, 28 with unstable angina(UA),25 with acute myocardial infarction(AMI). The SYNTAX score was calculated in all the 53 enrolled ACS patients, 19 patients with high SYNTAX score, 34 patients with low SYNTAX score. Results: The levels of LOX-1 was significantly higher in UA patients (86.08±29.93pg/ml )and AMI patients(103.93±36.53pg/ml) than in normal control subjects (66.78±23.49pg/ml ,P<0.01 or P<0.05).Furthermore, in AMI group, the levels of LOX-1 was significantly higher than in unstable angina group (P<0.05). Higher SYNTAX score was detected in AMI group (22.88±8.27) than in UA group(14.79±9.96, P<0.01). The levels of LOX-1 was significantly higher in patients with high SYNTAX score (107.43±35.18 pg/ml) than those with low SYNTAX score (86.90±31.27pg/ml,P<0.05 ). Conclusion: LOX-1 might be a promising biomarker for predicting ACS.

    Objective:To investigate the relation of HLA-B27 in the progression and prognosis of patients with juvenile-onsetspondyloarthropathies(JSpA),we compare the clinical and laboratory examination manifestation between the HLA-B27 antigen positive and negative groups。Methods:The clinical and laboratory manifestations of 94 JSpA patients with positive HLA-B27 were analyzed and compared with those of 80 patients with negative HLA-B27.Results:There was significance difference in the gender,age of disease onset,the involved of articulatio sacroiliaca,the imageology and the IgA,erythrocyte sedimentation rate(ESR),antinuclear antibody,protein electrophoresisβmanifestation between the two groups(p<0.05),in contrast to the peripheral joint involvement,the other immune index,C-reactive protein(CRP) and so on (p>0.05)。Conclusion: HLA-B27 (+) JSpA patients have an later onset of age,severer symptoms and are tend to develop irreversible lesions.

    [Objective] This study aimed to investigate the correlation of targets organs damage in patients with hypertensions which controlled well and Heart Rate Variability [Methods]We compared the Heart Rate Variability between 90 patients of hypertension with target organ damage group and 111 patients of hypertension group, patients are divided into subgroups according to diabetes.[Results] HRV decreases significantly both in hypertension with target organ damage and in diabetes(P<0.05) .[Conclusion] HRV is the early diagnosis of hypertension with target organ damage and autonomic nervous system damage in diabetes.

    Analysis Of Coma Patients With Cerebral Evoked potentials And Blink Reflex And Its Prognosis Evaluation

    Abstract Objective:to analyze the cerebral evoked potentials and blink reflex characteristics of coma patients, and combined with Glasgow (GCS) score to approach its prognostic value in patients with coma. Methods: 40 score of GCS 3-12 comatose in ICU ward were examined by brainstem auditory evoked potential (BAEP)、somatosensory evoked potential(SEP) and blink reflex(BR) in 24 hours after coma,at the same time,the GCS、clinical assessment of brainstem reflex was performed,motion detection and observe the patients’ prognosis.BAEP, SEP use Cant grading standards, BR grouped according to test results, the prognosis is divided into death, persistent coma state, improved. Results: of the 40cases, 23 patients died,13 cases in persistent coma state, 4 cases improved.BAEP examination unilateral or bilateral Ⅲ ~ Ⅴwaves are missing(Cant grade Ⅲ) in 15 cases, 14 died,the mortality rate was 93%. Unilateral or bilateral SEP N20disappeared (Cant grade Ⅲ) in 22 cases, 17died, the mortality rate was 77%. BR of all patients in the acute coma state were different degrees of abnormality, of which 30 cases were no response, and prognosis was not statistically significant(χ2=1.989,P=0.370).BAEP, SEPCant grade were grade Ⅲ in 10 cases, 9 cases GCS score of 3 points, 1 case GCS score of 4 points, all died within one month, the mortality rate was100%. Conclusions:BAEP, SEP can effectively predictthe prognosis of comatose patients at certain extent,their combination can significantly improve the affect of prognosis assessment, BR in the acute state of coma were different degrees of abnormality,can not accurately predict the prognosis.

    Rt-PA within 3 hours of intravenous thrombolytic therapy of acute cerebral infarction

    Objective : to evaluate the efficacy and safty of thrombolysis by intravenous rt-PA in acute cerebral infarction. Methods: 22 cases of acute ischemic stroke patients with intravenous thrombolysis; Onset time from thrombolysis within 3h, program for the 0.9 mg/kg, maximum dose 90mg. The total dose of 10% mixing in syringes,In 1 min injection. The remaining 90% liquid is added to an infusion pump, the static point, sustained over 1 h. A control group of 21 patients treated with conventional therapy. Outcome measures:Two groups before and after treatment with NHISS score and Barthel index ( BI)。A modified Rankin scale 90d integrated living ability。Safety index:Death, bleeding, and other Adverse events .Results: After thrombolytic therapy in patients with 24h, 14d, 90d NHISS score decreased significantly compared with the control group, the difference was statistically significant ( P < 0.05). After treatment, 24h, 14d, 90d thrombolysis group BI scores were higher than that of the control group. There were statistically significant ( P < 0.05 ), especially in the thrombolysis after 24hBI score than the control group improved more obviously ( P < 0.01 ). 90 d follow-up, the BI score and mRS score significantly benefit from thrombolysis group than in the control group, the difference was statistically significant ( P < 0.05 ) Two groups of overall adverse events and serious adverse events have no obvious difference, without statistical significance ( P > 0.05 ). Conclusion: Early rt-PA intravenous thrombolysis in treating acute cerebral infarction is effective and relatively safe method.

    Clinical characteristics of 39 autoimmune hepatitis patients

    Abstract Object: To summarize the clinical,pathological, immunnological and biochemical features of autoimmune hepatitis(AIH), to improve the rate of diagnosis and decrease the rate of missed diagnosis. Methods: Adopted the descriptive diagnosis standard and scoring system proposed by International Autoimmune Hepatitis Group. 39 cases with AIH were retrospectively analyzed and the clinical manifestations and laboratory examinations were investigated. 40 in-hospital patients of acute hepatitis B patients in the same period were selected as a comparison. Results: Most of AIH patients were the female (97.4%), mainly ranged in age from 45 to 59 (71.8%). Most of acute hepatitis B were male (72.5%) and lessed than 45 years old (85%). In AIH patients, 15 cases (38.5%) were accompanied by other autoimmune diseases. The first-degree relatives of 10 (25.6%) AIH patients were associated with autoimmune diseases. The level of γ-glutamyl transpeptidase (γ-GT) in AIH patients was higher than acute hepatitis B(216.4±158.5 vs 83.4±30.5, P <0.05). The level of γ-GT were greater than 10 times of upper limited normal (ULN) in 15 cases(38.5%)of AIH, and Continuously increased in 8 (20.5%)AIH cases in late stage when ALT has already decreased. The autoantibody positive rate of ANA was high in AIH group, but there are still 4 (10.3%) patients were autoantibodies negative. The main histologic change was interface hepatitis(65.6%). Conclusion: Carefully questioning the history of autoimmune diseases in the first-degree relatives and observing the change of γ-GT can help the diagnosis of AIH.

    Objective To study the relationship HP infection and plasma G-17, cyclooxygenase-2 (COX-2) expression of colon polyps, colon cancer. Methods To select colon polyps(n=60), colon cancer(n=60) and patients with negative colonoscopy examination (n=60). Through C13 breath test to detect HP infection,enzyme-linked immunosorbent assay to plasma G-17 and immunohistochemistry to COX-2 expression.Results H.P infection in patients with colon polyps,colon cancer was higher than that in normal group (P<0.05). In normal control group, colon polyps group and colon carcinoma group, plasma G-17 levels was higher in HP+ group than HP- group (all P<0.05). In colon polyps and cancer group, COX-2 expression in H.P+ group was higher than that in H.P- group (all P<0.05). In H.P- group, COX-2 expression in colon cancer higer than of colon polyps (P<0.05). In H.P+ group, COX-2 expression in colon cancer higer than that in colon polyps (P<0.05). HP infection and COX-2 expression was positively correlated (r = 0.538, P<0.01). Conclusions HP infection may be related to the development of colon polyps, colon cancer through affecting the expression of plasma G -17 and COX-2 .

    Clinical characteristic and diagnosis experience of acute superior mesenteric artery occlusive disease

    Objective: To explore the clinical characteristic , diagnosis and therapy of acute superior mesenteric artery occlusive disease. Methods: Retrospectively analyse the clinical data of 36 cases of inpatients with acute superior mesenteric artery occlusive disease in Tianjin Medical University General Hospital from Nov. 2003 to March. 2011. Results: Among the 36 cases, acute superior mesenteric artery embolism 26 cases, acute superior mesenteric artery thrombosis based on chronic superior mesenteric artery occlusion 10 cases. 27 cases by surgical therapy: simple embolectomy 4 cases; simple resection of necrotic intestine 10 cases; resection of intestine and embolectomy 11 cases; resection of intestion and artery bypass operation 1 case ;resection of intestine, embolectomy and artery bypass operation 1 case. 13 cases healed , 14 cases died. Expectant treatment 9 cases, 7 cases got better, 2 cases died, general mortality rate 44.4%. During the follow-up period of 20 cases, 1 post- operation case recurred, and cured by resection of intestine and embolectomy, there were no evidence of recurrence through B ultrosound in other cases. Conclusion: The patients of disease highly-suspected should be given the CTA or DSA as soon as possible, improving artery blood early and reconstructing superior mesenteric artery plays a crucial part in improving survival rate and cure rate.

    The Research of Clinical Significance for Expression of TLR4 and IFN-γ In RSV Pneumonia Children

    【Abstract】Object:First,to observe the expression of TLR4 on monocytes in peripheral blood after RSV infection and the relationship between TLR4 expression and disease severity or atopic disposition;Second,,to evaluate the relationgship between IFN-γ concentration and disease severity or atopic disposition in acute infection phase.Method:group 1:test group:30 children who got pneumonia,then the group was devided into two subgroup:22 children are the mild and moderate ones,8 children are severe ones.On the basis of atopy(eczema,for example)and family history(asthma,allergic rhinitis,allergic conjunctivitis,atopic dermatitis and so on),we devide the group into two groups:one group with atopic disposition concludes 19 children and the other without atopic disposition concludes 11 children.Group 2:27 children admitted into hospital for surgical reasons without respiratory infection in the last week.Results:1.The expression of TLR4 in the test group is 22.29±9.46%,and in control group is 12.67±9.46%,there is statistical significance(P =0.001<0.05)by U test.TLR4 in mild and moderate group of test group is 21.44±16.33%,while in severe group is 24.61±17.28%,there is no statistical significance(P =0.872>0.05).TLR4 in test group with atopic disposition is 21.99±16.14%,and in group without atopic disposition is 22.76±17.48%,there is no statistical significance(P =0.966>0.05).2.The concentration of IFN-γ in test group is 254±504.48ng/L,and in control group is 259.89±829.91ng/L,and between the two groups there is no statistical significance(P =0.64>0.05).The concentration in mild and moderate group of test group is 324.8±568.19ng/L,while in severe grouop is 62.36±167.6ng/L,obviousely there is statistical significance(P =0.045<0.05).The concentration in test group with atopic disposition is 350.19±586.57ng/L,and in test group without atopic disposition is 90.09±267.56ng/L,and there is no statistical significance(P =0.25>0.05).Conclusion:The expression of TLR4 on peripheral blood monocyte up-regulates during RSV infection.But it does not obviously relate to the severity of disease,and is not influced by atopic disposition during acute RSV infection.The concentration of IFN-γ in test group is not significantly different from that of control test.It negatively relates to disease severity,that the concentration of severe children is lower than that of mild children.It is rational to use IFN-γfor severe RSV pneumonia.During RSV infection ,the concentration of IFN-γ has not distinct relation with atopic disposition.
